Cost-Utility Analysis of the Cochlear Implant (인공와우 이식의 비용효용 분석)

  • Objective : To determine the quality of life and cost consequences for deaf adults who received a cochlear implant. Methods : The data from 11 patients, post-lingual deaf adults who received cochlear implants from 1990 to 2002, underwent cost-utility analysis. The average age of the participants was 49.6 years. The main outcomes were direct cost per quality-adjusted life-year (QALY) using the visual analog scale (VAS), health utility index (HUI), EuroQol (EQ-5D), and quality well-being (QWB), with costs and utilities being discounted 3% annually. Results : Recipients had an average of 5.6 years of implant use. Mean VAS scores increased by 0.33, from 0.27 before implantation to 0.60 at survey. HUI scores increased by 0.36, from 0.29 to 0.65, EQ-5D scores increased by 0.26, from 0.52 to 0.78, and QWB scores increased by 0.16, from 0.45 to 0.61. Discounted direct costs were $22,320, yielding $19,223/QALY using VAS, $17,387/QALY using HUI, $24,604/QALY using EQ-5D, and $40,474/QALY using QWB. Cost-utility ratios using VAS, HUI, and EQ-5D were all below $25,000 per QALY, except using QWB. Conclusion : Cochlear implants in post-lingual deaf adult have a positive effect on quality of life at reasonable direct costs and appear to produce a net saving to society.

  • When, Software is developed, Applying development methods considering security, it is generated the problem of additional cost. These additional costs are caused not consider security in many developing organization. Even though, proceeding the developments, considering security, lack of ways to get the cost of handling the vulnerability throughput within the given cost. In this paper, propose a method for calculating the vulnerability throughput for using a security vulnerability processed cost-effectively. In the proposed method focuses on the implementation phase of the software development phase, leveraging static analysis tools to find security vulnerabilities in CWE TOP25. The found vulnerabilities are define risk, transaction costs, risk costs and defines the processing priority. utilizing the information in the CWE, Calculating a consumed cost in a detected vulnerability processed through a defined priority, and controls the vulnerability throughput in the input cost. When applying the method, it is expected to handle the maximum risk of vulnerability in the input cost.

Cost-benefit Analysis of Nutrition Management Program for Children Aged Under 5 Years in DR Congo (콩고민주공화국 5세 미만 아동대상 영양관리 프로그램의 비용편익 분석)

  • Objectives: This study aims to evaluate the economic efficiency of a nutrition management program for children under 5 years of age in Kenge, Kwango District, Democratic Republic of the Congo (DR Congo) from 2014 to 2016. Methods: The economic efficiency of a nutrition management program for children under 5 years of age who have recovered from malnutrition status was evaluated using a cost-benefit analysis. The costs were analyzed according to the executed budget incurred during the project period. The benefits were estimated as the monetary value of the saved lives of children under 5 years of age. The economic efficiency of the program was determined by the Benefit-Cost Ratio (BCR). The BCR was calculated by dividing the total discounted benefit by the total discounted costs. The project is economically efficient when the BCR is greater than 1. Results: The costs of the nutrition management program were calculated as 1,677,609,648 Korean Won (KRW). A total of 2,466 children survived with improved malnutrition status through this program. The benefit for the reduction of mortality for children under 5 years of age was estimated to be 6,814,354,467 KRW, the estimated value of life for 2,466 children. The BCR was 4.06. Conclusions: The nutrition management program for children under 5 years of age in DR Congo was found to be a cost-effective project. Successful and efficient Official Development Assistance (ODA) for a health project requires integrated and comprehensive strategies and specialized international development consulting to improve efficiency. Future nutrition management programs should take into account the national health program to maintain the sustainability of the project.

  • Data Envelopment Analysis(DEA) is a promising methodology to evaluate the relative efficiency of the decision-making units. We have compared the efficiency of six electric power generation companies in Korea using DEA. The analysis results by input-oriented CCR and BCC models are summarized as follows: first, different results were acquired between using input factors as total capacity of generators and as sub-totals of generator capacity based on primary energy sources. It is the result influenced by input factors which are characterized by the proportion of fixed costs(generating facilities) and variable costs(generation costs for primary energy), Second, the efficiency will be increased if the input factors selected, according to primary energy sources discussed in this research, are used during long-term expansion of electric power capacity plans. It is expected that this approach can give a feedback for management of electric power generation companies.

A Study on Obstacle Factors at the Mutual Access of Towed Trailer of Car-Ferry between Korea and China using Fuzzy-AHP (Fuzzy-AHP를 활용한 한·중 카페리 피견인 트레일러 상호주행 시 장애요인에 관한 연구)

  • The purpose of this study was to analyze the weights of obstacle factors against mutual access of towed trailers of car-ferry between South Korea and China using Fuzzy-AHP. To this end, factors were first selected through literature research and finally selected through in-depth interviews with a group of experts in cargoes who are currently in car-ferry companies. Selected high rank obstacle factors included unstable service provision, limited policy support, and additional costs being incurred and among them, unstable service provision was identified as the most serious obstacle factor through analysis. Eleven measurement variables selected include insufficient supply of container yard, trailer insurance and management issues, insufficient special equipment, insufficient specialized manpower, limited input service courses, insufficient marketing and public relations strategies, and increasing transportation costs and among them, increasing equipment returning costs due to unbalanced cargo gathering were identified as the most serious obstacle factor through analysis followed by limited input service courses, insufficient special equipment, delays in procedures, and occurrence of additional procedures in order of precedence.

  • The objective of this paper is to analyze expenditure structures and impact factors of household's transportation cost in order to provide policy-maker with quantitative and economic information for reducing household's transportation costs, using 1995-2007 household survey data. For this purpose, this study performed various economic analysis, including econometric regression analysis and income re-distributional effects. The four models were separately estimated with dependent variable (personal and public costs, public transit and private car costs) and independent variables (family, employ, age, education, cars, household income, region dummy variable). In addition, We analyzes impacts of transport policy instruments on the transportation expenditure. These instruments include changes of oil price and public transit fare, reform of public transport system, and extension of subway facilities. Finally, income re-distributional effects before and after transportation expenditure, using Pechman-Okner (PO) index and Wolfson index.

Wind spectral characteristics on fatigue responses of towerbase and moorings of a floating offshore wind turbine

  • The tower-platform interface and mooring system of floating offshore wind turbines (FOWTs) are some of the most critical components with significant influences on overall project costs. In addition to satisfying strength requirements, it is typical and vital to meet fatigue criteria for a service life of 25 years or more. Wind spectra characteristics considered in analysis can penalize fatigue designs, leading to unnecessary costs. The International Electrotechnical Commission (IEC, 2009) recommends the use of site-specific wind data (spectrum, turbulence intensity, etc.) in design of FOWTs, but for offshore sites it is often the case that such data is unavailable and land-based data are used as surrogates in design. For such scenarios, it is worth investigating whether such alternative approach is suitable and accurate, and understanding the consequence of the selection of wind spectral characteristics on fatigue design. This paper addresses the impact of the subsequent selection on fatigue responses of towerbase and mooring system in a FOWT, as a sequel to the paper by Udoh and Zou (2018) which focused on impacts on strength design. The 5 MW semi-submersible FOWT platform with six mooring lines implemented in the preceding study is applied in analysis. Results indicate significant variations in resulting fatigue life with considered wind parameters. Thus, it is critical to apply proper wind spectra characteristics for analysis and design of FOWTs to avoid unnecessary conservatism and costs. Based on the findings of this study, more explicit guidance on the application of turbulence intensities for IEC-recommended models in offshore sites could lead to more accurate load estimates in design of FOWTs.

Analysis of accident types at small and medium-sized construction sites based on web scraping and text mining (웹 스크래핑 및 텍스트마이닝에 기반한 중소규모 건설현장 사고유형 분석)

  • The construction industry's fatality count stands at 402, comprising approximately 46% of total industrial accidents. Notably, construction costs less than 5 billion won account for about 69%, so strengthening safety management at small and medium-sized construction sites is required. In this study, 19,511 accident investigation data were collected using web scraping. Through statistical analysis of the collected structured data and text mining analysis of the unstructured data, accident types and causes of accidents were analyzed by construction costs at sites less than 5 billion won. As a result, it was confirmed that there were differences in accident types and causes depending on the construction costs. It is hoped that the results of this study will be used for customized safety management at small and medium-sized construction sites.

  • The quantitative evaluation method of the safety management cost was suggested to prevent a gas accident as a major industrial accident. In a gas governor station, process risk assessments such as the fault tree analysis(FTA) and the consequence analysis were performed. Based on process risk assessments, potential accident costs were estimated and the cost-benefit analysis(CBA) was performed. From the cost-benefit analysis for five classification items of safety management cost, the order of the cost/benefit ratio was estimated.

Changes in National Health Insurance Medical Expenses and Long-Term Care Costs between the Long-Term Care Group and General Older Adults Group before and after Long-Term Care Use (노인장기요양급여 이용 전후 장기요양군과 일반노인군 간 국민건강보험 및 노인장기요양보험 비용 추이)

  • Background: The Republic of Korea's aging population escalates medical and long-term care costs. While prior research has suggested that long-term care might reduce these costs, these studies had limitations in their subjects and duration, making it difficult to generalize the results. This study aims to evaluate cost changes between the long-term care group and the general older adults group after addressing these limitations. Methods: A cohort was derived from the 2015 national population using stratified sampling. Subsequently, 15,114 individuals (7,557 in each group) were identified through 1:1 propensity score matching. The study employed a difference-in-differences analysis to explore variances in medical costs and long-term care benefits post-utilization of long-term care services. Results: Compared to the general older adults group, the long-term care group experienced a reduction in monthly per capita total medical costs by 56,459 Korean won (KRW). Although costs at tertiary and general hospitals increased, those related to long-term care hospitals decreased by 90,687 KRW. Including long-term care benefits, overall expenditures increased by 948,038 KRW. Conclusion: The analysis reveals that the long-term care group faces higher medical costs in acute care than the general older adults group, emphasizing a greater need for medical services within this group. To meet the increasing medical demands of the long-term care group, a collaborative strategy linking community resources, healthcare, and long-term care facilities is imperative. Additionally, developing and implementing preventive health habit management strategies for middle-aged and older adults is essential to diminish the future requirement for long-term care.
