Is the Utilization of MID Services affected by the Implementation of Insurance Coverage?: Based on Claim Data of a General Hospital (MRI 보험급여 적용이 진료이용량에 미치는 영향 : 한 종합병원의 청구자료를 중심으로)

  • As medical insurance had been implemented for Magnetic Resonance Imaging (MRI) from January 1, 2005, this study investigated whether there had been any change in the amount of the medical care utilization of patients who undertook MRI before and after the insurance coverage, and was to examine factors affecting the amount of medical care utilization of MRI. Data were collected from patients who undertook MRI before and after the insurance coverage for a year at a general hospital in Kyeanggi-do. $X^2$ and t-test were used for the analysis of their general characteristics, the number of MRI, and its medical costs before and after the insurance coverage, and hierarchical multiple regression analysis for the factors affecting the amount of the medical care utilization of MRI. The results of this study were as follows. First, the number of MRI after the insurance coverage was significantly decreased. Second, there was no significant difference in the total medical costs of MRI after the insurance coverage, but a significant difference was found in patient's share of medical costs. Third, six variables were found to be affecting the amount of the medical care utilization of MRI, and the variables showed to lead the number of MRI decrease after the insurance coverage. These six factors explained 21.4% of the total number of MRI. As MRI had been covered by insurance, the use of MRI and patient's share of the costs were deceased, but the total medical costs were not affected. Reasons for that could be found in that MRI insurance, different from the case of CT insurance coverage, was allowed not to cover some items and the kinds of diseases subjected to the insurance coverage were extremely limited, lowering insurance prescription rate. In addition to that, the average medical cost of MRI was not changed after the insurance coverage. Therefore, as future measures for the MRI insurance, coverage, it should be considered to allow insurance coverage to no coverage items and to expand the scope of benefit coverage, or to lower patient's share of the costs. Furthermore, researches should be done to explore how recipients will act and how suppliers will react if the coverage is expanded, including expanding the scope of coverage and reducing patient's share of the costs, as well as to conduct research on its economic analysis according to case mix.

  • Occupational safety and health management expense in construction industry means the monetary amount that the purchaser appropriates to prevent various disaster from happening during the construction period by setting aside a certain amount from subcontract based on the construction type and scale. The current appropriation standard of the Ministry of Employment and Labor has been enforced since its stipulation in 1988 although various policy changes for the safety in construction industry, e.g. the development of occupational safety management techniques in construction industry, changes in construction field site environment, cost depreciation, etc., are happening in Korea and overseas. Therefore, it is required at this point to examine whether the current occupational safety and health management expense appropriation is proper or not on the basis of time trend and the changes in the construction industry environment. Accordingly, this study aims at investigating the appropriation of occupational safety and health management expense for construction industry by the construction type and target expense in response to current construction industry environment and proposing the proper appropriation. The above research result proposes the calibration of the appropriation as specified and stipulated by the Ministry of Employment and Labor by the construction type and target expense, which is necessary to reflect the actual condition of the current construction industry environment.

  • Since the year 2000, lung cancer has become the leading cause of cancer death in South Korea as in many other parts of the world. The current multidisciplinary approach for lung cancer includes a wide range of modalities, not only surgery, radiotherapy, medical drug therapy but also pain control, as well as social and psychological support. Therefore, thoracic surgeons, radiologists, nuclear medicine specialists, anesthetists, psychologist, nurses and social workers as well as medical doctors care for lung cancer patients. Sharing a common treatment protocol and optimal communication are vital aspects of shared care both from a medical and cost-effectiveness point of view. We developed a shared electronic medical record (SEMR) for treating patients with lung cancer in a university hospital to facilitate the sharing protocols and communications between doctors involved in a lung cancer clinic. A SEMR system was developed within a order communication system(OCS) for a lung cancer clinic. The records of radiological, laboratory and pathological studies as well as the records of surgery, chemotherapy, and radiotherapy were stored and presented to all doctors who treat the same patient. Every doctor was allowed to change his/her own records. They could review other doctor s records but could not alter them. With the SEMR, it was expected that the time to complete the medical records for one patient could be reduced because it was easy to review all the data from the other doctors who share the same patient. In addition, the confidence of the doctors who share a common treatment protocol would be higher. Therefore, a shared electronic medical record is expected to improve the quality of patient care.

Read Range Reduction in Passive UHF RFID Tag by Smart Device Signal Interference (스마트 기기 신호 간섭에 의한 수동형 UHF 대역 RFID 태그의 인식 거리 감소에 관한 연구)

  • The passive UHF RFID tags for item-level tagging are now explosively used in the supply chain and retail applications as they have several advantages, the two most relevant are cost and a longer read range. However, the signal interference problem between RFID tags and smart devices in real world is expected according to the smart-phone and tablet market growth together. The performance of RFID tags can be significantly less. The popular examples are the read-success rates and read range reduction. Especially, KT Corp. recently emphasized the serious signal interference at 900 MHz of LTE and old RFID frequencies through their public demonstration. By popular demands, this paper suggests the interference tolerance measurement method between the passive UHF RFID tag and the transmitted signal from a smart device. In addition, we selected three passive UHF RFID tags(Inlay) available on the market and quantitatively evaluated read range reduction results by interference signals using the PCR(Performance Change Rates) index. As a result, the LTE system is about three times as effective as the WCDMA system in terms of interference effects, and the read range performance of two RFID tags about 60 % drop.

An Exploratory Study on the Relationships among the Future Work-Family Compatibility, Gender Attitude of Couples, and Reasons for Low Birth Rate (미혼 남녀의 향후 일.가정 양립 방안, 부부 성역할 태도, 저출산 문제에 대한 견해 간 관계 탐색 연구)

  • This study explored the relationships among such variables as work-family compatibility, gender attitude, reasons for low birth rate, and education level, using a sample of 2,678 from the '2009 National Survey for Marriage and Birth Trend'. The purpose of this study was to identify the ideas about how to support married couples' work-family balance, and how to change gender attitude to promote childbirth. The results are as follows. The type of ideal work-family balance was different by education level, but not by gender. Low level of education was associated with choice of the full time work without a child. Male singles, compared to female singles, preferred the role of a man as a bread winner, and a woman as a house worker. Female singles had the higher level of hope for husband's capability of caring children. This trend was intensified by choice of the full time work with 2 children. Singles who prefer working as a full time worker with 2 children worried more about low birth rate in Korea than singles who prefer working as a part time worker with one or two children. The most frequently cited reason for low birth rate was 'high cost of both child care and education'. The next was 'difficulty in buying a residence, followed by 'increase in the number of career woman'. Female singles who want to work full time with 2 or more children worried more about the burden of child caring and education than male singles. Implications and suggestions were provided based on the results.

  • Terrorist organizations that take advantage of the occasion of international events are changing from targeting specified people to attacking general public, raising the anxiety of the audience and participating countries. Therefore, the host country intends to improve its status as the host by ensuring safety and crisis management at any cost. As a country with a wealth of experience of hosting international events, Korea takes the burden of the security at Pyeongchang Winter Olympics. Consequently, to successfully host this winter Olympics, the organizing committee should bear in mind that Korea is no longer a safe country regarding terrorist attacks and take countermeasures against various crimes and terrorism that may arise during the Winter Olympic Games. This study overviews various measures as follows. First, the change of counter-terrorism strategy is necessary. Second, the safety entity and safety measures for Pyeongchang Olympic Games should be established. Third, private security companies should be actively utilized. Fourth, the safety management training for private security and safety personnel should be strengthened. Lastly, it is necessary to establish a rapid and organic preemptive safety management system to cope with safety and crisis effectively at Pyeongchang Winter Olympics and other international events that will be held in the future.

  • Because of rapid expansion of third party logistics, fierce competition in the transportation industry, and the diversification and globalization of transportation channels, an effective transportation planning by means of multimodal transport is badly needed. Accordingly, this study aims to suggest an optimal transport algorithm for the multimodal transport in the international logistics. Cargoes and stopovers can be changed numerously according to the change of transportation modes, thus being a NP-hard problem. As a solution for this problem, first of all, we have applied a pruning algorithm to simplify it, suggesting a heuristic algorithm for constrained shortest path problem to find out a feasible area with an effective time range and effective cost range, which has been applied to the Label Setting Algorithm, consequently leading to multiple Pareto optimal solutions. Also, in order to test the efficiency of the algorithm for constrained shortest path problem, this paper has applied it to the actual transportation path from Busan port of Korea to Rotterdam port of Netherlands.

  • As all industries field has changed very quickly, the field of construction faces in many changed. It is the representatively change that focus on the growth of quality level than increase of the quantity in construction. Whether the project successes or not is related to the survival of the company and it is necessity that we need performance measurement indicators to complete the project successfully. The domestic construction company considers overbearingly the cost-performance indicator in executing a project so far. It is rare of studies that suggest the methodology of more deeply measurement and analysis about the each project itself. The performance measured methodology based on Lean Construction is linked with the each project performance measurement methodology. This methodology uses three indicators such as the reliability, effectiveness and productivity at the sane their measuring whether each task is completed at the point of work. The measurement on the three performance indicators measure and evaluate whether the task is completed on the focus of construction duration. The research suggest the work plan reliability and weight of the performance measurement methodology based on the Lean Construction and the three considerately in the scope of measurement. If the suggestion is considered fully, we can indicate more objective and accurate construction project performance measurement indicators based on Lena Construction.

  • As EU's enlargement toward Central and Eastern countries, it is expected to change of the economic activity space of EU. This paper aims to examine the changing of economic activity space of EU with a locational analysis of foreign direct investment in Central and Eastern countries. The foreign enterprises, particularly EU's firms are increasing the direct investment in the Central and Eastern countries from the middle 1990's for a raising of the efficiency of production and a prior occupation of the potential market. The foreign direct investment in Central and Eastern Europe has a tendency to concentrate in Poland, Hungary, Czech and Slovakia, which has relatively a large market, a cheap and plentiful labor, a stable political system and a positive inviting policy of foreign direct investment. And, the foreign direct investment shows a tendency to locate generally in a big city, which has a advantageous production factors by a well-developed infrastructure and a regional industrial organization, a skilled labors, a market, etc., and in a border area between East and West Europe because of a cheap and plentiful labor, a similar cultural environment and the low cost of transportation. In conclusion, as an integration of the Central and Eastern countries into EU, the capital territories of Hungary, Czech and Poland, and the border area between East and West Europe is becoming a new economic activity space of EU with a location of foreign direct investment. And. the Baltic zone is also expect to become a new economic activity space according to the augmentation of exchanges and direct investment inter near countries.

Private Procurement for Constructions and Operation of Buman Road in Daegu (도시 인프라 시설을 위한 민자 유치 -대구시 범안로 민자도로의 운영 실태와 대책-)

  • Recently private procurement projects for urban infrastructure have been emphasized and introduced by government(both central and local) as an important method to supplement public budget shortage and to improve creativity and efficiency in its construction and operation, and hence to promote regional development. But these private procurement projects seem to be implemented under the neoliberal condition of urban policy for private capital to extend its sphere of investment and to ensure its opportunity of profit. Such private procurement projects for urban infrastructure have been legislated and widely applied since 1994 in S. Korea, but in recent years they have triggered out a variety of serious problems. In results of this study on the private procurement for construction and operation of Burnan Road in Daegu, it can be pointed out, it has led serious problems such as overestimation of traffic volume in plan, over-appropriation of construction cost and appurtenant business, irYational operation and account of the operating firm, problem in refinancing and change in contents of practical agreement, and over-compensation of minimum operation revenue. Some measures that can be considered as alternatives to resolve those problems and to operate more rationally the Buman Road include those of increasing traffic revenues, of reducing supports of public financing, and withdrawal of (part or whole of) operational right from the private firm.
