  • The purpose of this thesis is to analyze the error happening in the process of solving the simultaneous inequality with two unknown qualities and to propose the correct teaching method. We first introduce a problem about the simultaneous inequality with two unknown qualities. And we will see the solution which a student offers. Finally we propose the correct teaching method by analyzing the error happening in the process of solving the simultaneous inequality with two unknown qualities. The cause of the error are a wrong conception which started with the process of solving the simultaneous equality with two unknown qualities and an insufficient curriculum in connection with the simultaneous inequality with two unknown qualities. Especially we can find out the problem that the students don't look the interrelation between two valuables when they solve the simultaneous inequality with two unknown qualities. Therefore we insist that we must teach students looking the interrelation between two valuables when they solve the simultaneous inequality with two unknown qualities.

Pulmonary Arterial Growth Pattern after Shunt Operation in Patients of Pulmonary Atresia with Ventricular Septal Defect Associated with Juxtaductal Stenosis (Juxtaductal stenosis가 동반된 PA/VSD환자에서 체폐단락술 부위에 따른 폐동맥 크기의 변화)

  • Background: Pulmonary atresia (PA) with ventricular septal defect has various morphology of pulmonary arteries and pulmonary blood flow sources, so pulmonary arterial hypoplasia and arborization abnormality make this anomaly difficult to manage surgically. In cases associated with juxtaductal stenosis, we evaluated the change of the pulmonary arterial and juxtaductal stenotic site after shunt operations, and would like to find useful information in surgical planning and methodology of these patients. Material and Method: Among 59 cases diagnosed as PA with ventricular septal defect associated with juxtaductal stenosis, 29 cases who had cardiac catheterization before and after shunt operation were selected from July, 1991 to July, 1996. In 10 cases of right shunt operation(Group I) and 19 cases of left shunt operation (Group II), the diameters of the descending aorta, both pulmonary arteries, and the juxtaductal stenosis site were measured before and after the shunt operation. Result: In both Group I and II, the pre- and postoperative ratio of diameters of the ipsilateral pulmonary artery to the descending aorta was from 0.78${\pm}$0.31 units to 1.01${\pm}$0.26 units and from 0.67${\pm}$0.18 units to 0.84${\pm}$0.27 units respectively, showing a signigicant increase. The contralateral pulmonary artery index was increased from 0.92${\pm}$0.28 units to 1.05${\pm}$0.15 units and from 0.94${\pm}$0.27 units to 1.08${\pm}$0.37 units respectively, but could not be confirmed statistically. In both groups, the change of juxtaductal stenosis showed an aggravating tendency but of no statistical significance from 0.43${\pm}$0.27 units to 0.39${\pm}$0.25 units and from 0.32${\pm}$0.10 units to 0.30${\pm}$0.16 units respectively, and we experienced 2 total obstruction in Group II. Because the increased pulmonary blood flow by shunt operation has a favorable effect to the pulmonary arterial growth, the shunt operation is a recommended treatment in patients with hypoplastic pulmonary arteries. But in PA with ventricular septal defects, the change of juxtaductal stenosis is very important. In conclusion, the growth of ipsilateral (shunt site) pulmonary artery was promoted by shunt operation, but there is a tendency for the juxtaductal stenosis to be aggravated. And we experienced 2 total obstruction in Group II. Conclusion: Thus, in cases operated with shunt method, much careful postoperative follow up study including angiographic evaluation is needed, and after the shunt operation on the side of pulmonary artery associated with juxtaductal stenosis, early precise planning for total correction is recommended.

Determination of Representative Renal Depth for Accurate Attenuation Correction in Measurement of Glomerular Filtration Rate in Transplanted Kidney (이식 신의 사구체 여과율 측정에서 정확한 감쇄 보정을 위한 신장 깊이 대표값 설정)

  • Purpose: To measure reliable glomerular filtration rate by using the representative values of transplanted renal depths, which are measured with ultrasonography. Materials and Methods: We included 54 patients (26 men, 28 women), with having both renal scintigraphy and ultrasonography after renal transplantation. We measured GFR with Gates' method using the renal depth measured by ultrasonography, and median and mean ones in each patient. We compared GFR derived from ultrasonography-measured renal depth with GFR derived from median and mean renal depths. The correlation coefficients were obtained among GFR derived from ultrasonography-measured renal depths, median and mean renal depths under linear regression analysis. We determined whether GFR derived from median or mean renal depth could substitute GFR derived from ultrasonography-measured renal depth with Bland-Altman method. We analyze the expected errors of the GFR using representative renal depth in terms of age, sex, weight, height, creatinine value, and body surface. Results: The transplanted renal depths range from 3.20 cm to 5.96 cm. The mean value and standard deviation of renal depths measured by ultrasonography are $4.09{\pm}0.65cm$ in men, and $4.24{\pm}0.78cm$ in women. The median value of renal depths measured by ultrasonography is 4.36 cm in men and 4.14 cm in women. The GFR derived from median renal depth is more consistent with GFR derived from ultrasonography-measured renal depth than GFR derived from mean renal depth. Differences of GFR derived from median and ultrasonography-measured renal depth are not significantly different in the groups classified with creatinine value, age, sex, height, weight and body surface. Conclusion: When median value is adapted as a representative renal depth, we could obtain reliable GFR in transplanted kidney simply.

Principal component analysis in C[11]-PIB imaging (주성분분석을 이용한 C[11]-PIB imaging 영상분석)

  • Purpose Principal component analysis (PCA) is a method often used in the neuroimagre analysis as a multivariate analysis technique for describing the structure of high dimensional correlation as the structure of lower dimensional space. PCA is a statistical procedure that uses an orthogonal transformation to convert a set of observations of correlated variables into a set of values of linearly independent variables called principal components. In this study, in order to investigate the usefulness of PCA in the brain PET image analysis, we tried to analyze C[11]-PIB PET image as a representative case. Materials and Methods Nineteen subjects were included in this study (normal = 9, AD/MCI = 10). For C[11]-PIB, PET scan were acquired for 20 min starting 40 min after intravenous injection of 9.6 MBq/kg C[11]-PIB. All emission recordings were acquired with the Biograph 6 Hi-Rez (Siemens-CTI, Knoxville, TN) in three-dimensional acquisition mode. Transmission map for attenuation-correction was acquired using the CT emission scans (130 kVp, 240 mA). Standardized uptake values (SUVs) of C[11]-PIB calculated from PET/CT. In normal subjects, 3T MRI T1-weighted images were obtained to create a C[11]-PIB template. Spatial normalization and smoothing were conducted as a pre-processing for PCA using SPM8 and PCA was conducted using Matlab2012b. Results Through the PCA, we obtained linearly uncorrelated independent principal component images. Principal component images obtained through the PCA can simplify the variation of whole C[11]-PIB images into several principal components including the variation of neocortex and white matter and the variation of deep brain structure such as pons. Conclusion PCA is useful to analyze and extract the main pattern of C[11]-PIB image. PCA, as a method of multivariate analysis, might be useful for pattern recognition of neuroimages such as FDG-PET or fMRI as well as C[11]-PIB image.

Possibility Estimating of Unaccessible Area on 1/5,000 Digital Topographic Mapping Using PLEIADES Images (PLEIADES 영상을 활용한 비접근지역의 1/5,000 수치지형도 제작 가능성 평가)

  • This paper evaluated the possibility for 1/5,000 digital topographic mapping by using PLEIADES images of 0.5m GSD(Ground Sampling Distance) resolution that has recently launched. Those results of check points by applying the initial RPC(Rational Polynomial Coefficient) of PLEIADES images came out as; RMSE of those were $X={\pm}1.806m$, $Y={\pm}2.132m$, $Z={\pm}1.973m$. Also, if we corrected geometric correction using 16 GCP(Ground Control Point)s, the results of RMSE became $X={\pm}0.104m$, $Y={\pm}0.171m$, $Z={\pm}0.036m$, and t he RMSE of check points were $X={\pm}0.357m$, $Y={\pm}0.239m$, $Z={\pm}0.188m$; which of those results indicated the accuracy of standard adjustment complied in error tolerances of the 1/5,000 scale. Additionally, we converted coordinates of points, obtained by TerraSAR. for comparing with measurements from GPS(Global Positioning System) surveying. The RMSE of comparing converted and GPS points were $X={\pm}0.818m$, $Y={\pm}0.200m$, $Z={\pm}0.265m$, which confirmed the possibility for 1/5,000 digital topographic mapping with PLEIADES images and GCPs. As method of obtaining GCPs in unaccessible area, however, the outcome evaluation of GCPs extracted from TerraSAR images was not acceptable for 1/5,000 digital topographic mapping. Therefore, we considered that further researches are needed on applicability of GCPs extracted from TerraSAR images for future alternative method.

Long-Term Results of Surgical Treatment for the Idiopathic Clubfoot (선천성 만곡족의 장기 수술적 치료 결과)

  • Purpose: This study evaluated the results of surgical treatment for residual or recurrent deformity after the conservative treatment of idiopathic clubfoot. Materials and Methods: Fifty-one cases (32 patients), who were followed up to skeletal maturity, were reviewed retrospectively. The mean age at the last follow-up was 18.7 years. The surgical options included selective or comprehensive soft tissue release, tendon lengthening and transfer, and various types of osteotomy. The radiology measurements included the talocalcaneal angle and talo-first metatarsal angle in the anteroposterior (AP) view, and the talocalcaneal angle and calcaneal pitch in the lateral view. The radiology measurements were compared with the normal values for adults. The clinical evaluations were made using the ankle-hindfoot score and the midfoot score of the American Orthopaedic Foot and Ankle Society (AOFAS): excellent (>85), good (71-85), pair (56-70), and poor (<56). Results: At the last follow-up, the percentages of the 51 cases, whose parameter values fell within the normal ranges were as follows: in the AP view, 41.2% (talocalcaneal angle); and 90.2% (talo-first metatarsal angle). In the lateral view, the percentage was 84.3% (talocalcaneal angle). For the calcaneal pitch, the percentages were 61%. The mean AOFAS score was 88.1±10.7 on the ankle-hindfoot score and 86.7±11.5 on the midfoot score. Conclusion: The long-term outcome of patients with idiopathic clubfoot, who underwent surgical treatment after conservative treatment, was found to be 43%-90% of the normal range of radiographic indices. Clinically, the mean AOFAS scores were "excellent". Therefore, a satisfactory result can be obtained by analyzing the elements of deformity more accurately and then using the selective operation method, even if the non-surgical correction method fails.

Building Change Detection Methodology in Urban Area from Single Satellite Image (단일위성영상 기반 도심지 건물변화탐지 방안)

  • Urban is an area where small-scale changes to individual buildings occur frequently. An existing urban building database requires periodic updating to increase its usability. However, there are limitations in data collection for building changes over a wide urban. In this study, we check the possibility of detecting building changes and updating a building database by using satellite images that can capture a wide urban region by a single image. For this purpose, building areas in a satellite image are first extracted by projecting 3D coordinates of building corners available in a building database onto the image. Building areas are then divided into roof and facade areas. By comparing textures of the roof areas projected, building changes such as height change or building removal can be detected. New height values are estimated by adjusting building heights until projected roofs align to actual roofs observed in the image. If the projected image appeared in the image while no building is observed, it corresponds to a demolished building. By checking buildings in the original image whose roofs and facades areas are not projected, new buildings are identified. Based on these results, the building database is updated by the three categories of height update, building deletion, or new building creation. This method was tested with a KOMPSAT-3A image over Incheon Metropolitan City and Incheon building database available in public. Building change detection and building database update was carried out. Updated building corners were then projected to another KOMPSAT-3 image. It was confirmed that building areas projected by updated building information agreed with actual buildings in the image very well. Through this study, the possibility of semi-automatic building change detection and building database update based on single satellite image was confirmed. In the future, follow-up research is needed on technology to enhance computational automation of the proposed method.

Usefulness of Non-coplanar Helical Tomotherapy Using Variable Axis Baseplate (Variable Axis Baseplate를 이용한 Non-coplanar 토모테라피의 유용성)

  • Purpose: Helical Tomotherapy allows only coplanar beam delivery because it does not allow couch rotation. We investigated a method to introduce non-coplanar beam by tilting a patient's head for Tomotherapy. The aim of this study was to compare intrafractional movement during Tomotherapy between coplanar and non-coplanar patient's setup. Materials and Methods: Helical Tomotherapy was used for treating eight patients with intracranial tumor. The subjects were divided into three groups: one group (coplanar) of 2 patients who lay on S-plate with supine position and wore thermoplastic mask for immobilizing the head, second group (non-coplanar) of 3 patients who lay on S-plate with supine position and whose head was tilted with Variable Axis Baseplate and wore thermoplastic mask, and third group (non-coplanar plus mouthpiece) of 3 patients whose head was tilted and wore a mouthpiece immobilization device and thermoplastic mask. The patients were treated with Tomotherapy after treatment planning with Tomotherapy Planning System. Megavoltage computed tomography (MVCT) was performed before and after treatment, and the intrafractional error was measured with lateral(X), longitudinal(Y), vertical(Z) direction movements and vector ($\sqrt{x^2+y^2+z^2}$) value for assessing overall movement. Results: Intrafractional error was compared among three groups by taking the error of MVCT taken after the treatment. As the correction values (X, Y, Z) between MVCT image taken after treatment and CT-simulation image are close to zero, the patient movement is small. When the mean values of movement of each direction for non-coplanar setup were compared with coplanar setup group, X-axis movement was decreased by 13%, but Y-axis and Z-axis movement were increased by 109% and 88%, respectively. Movements of Y-axis and Z-axis with non-coplanar setup were relatively greater than that of X-axis since a tilted head tended to slip down. The mean of X-axis movement of the group who used a mouthpiece was greater by 9.4% than the group who did not use, but the mean of Y-axis movement was lower by at least 64%, and the mean of Z-axis was lower by at least 67%, and the mean of Z-axis was lower by at least 67%, and the vector was lower by at least 59% with the use of a mouthpiece. Among these 8 patients, one patient whose tumor was located on left frontal lobe and left basal ganglia received reduced radiation dose of 38% in right eye, 23% in left eye, 30% in optic chiasm, 27% in brain stem, and 8% in normal brain with non-coplanar method. Conclusion: Tomotherapy only allows coplanar delivery of IMRT treatment. To complement this shortcoming, Tomotherapy can be used with non-coplanar method by artificially tilting the patient's head and using an oral immobilization instrument to minimize the movement of patient, when intracranial tumor locates near critical organs or has to be treated with high dose radiation.

Treat four group of Korean medicine, Constitution medicine, Western medicine, cooperative consultation of Korean medicine and Western medicine. 2. The Judgement of the patient The Result From 8/1/1997 to 7/31/1998, We have the following result by clinical analysis intended for CVA 86 patients who had been treated in the Oriental Medical Hospital at Dong Eui Medical Center from 1997. 8. 1 to 1998. 7. 31 in 1. Analysis according to Age The first stage of thirties, forties, seventies is heavier than forties, fifties in improvement and Index of improvement of symptom 2. Analysis according to sex We have no special relation in an average of symptom and improvement, Index improvement 3. Analysis according to Family History We have the better result in first stage and improvement, index improvement when no family history. 4. Analysis according to Past History We have no special relation in past history like hypertension, DM, heart problem 5. Analysis devided two group, above group and under group on the basis of the average in first stage of all patient. We have the better result when the first stage is light, that the first score of barthel index and CNS is high. 6. Analysis of the effect of treatment about Korean medical treatment, Western medical treatment, cooperative treatment. In this study, the highest group of rate of treatment at four contrast groups (Korean medicine, Constitution medicine, Western medicine, cooperative treatment according to dyagnosis and range of treatment was the patient group of doing dyagnosis and method of treatment based on constitution medicine theory. This is that of doing demostation, A-Tx, po-herb-medication according to dyagnosis and treat method of constitution of Lee Je-ma In case of left, the case of dyagnosis any disease according to doctor view but, normal in TCDwas 22-beginning of attack, 20- two weeks later, 11 case-four weeks later in case of right, 15-beginning of attack, 12-two weeks later, 9 case four weeks later. So left vessel compares to right vessel is more interference, in fact more than a 1/2 of the patients of MCA disease can't do dyagnosis. In rate of imparement, the state of pacient improved but there isn't the improved case of result in TCD. 7. In TCD dyagnosis, between the case of inconsus the doctor view specially MCA in brain blood vessel is in large numbers and in total 86's patient, impossible case of dyagnosis according to interferiance of temporal is 21 case. 7. Result study about application of Kreaan medical treatment 1) The impossible patient of observation MCA blood vescular for interference temporal bone happened in large numbers. 2) There is the case having difference result to CT,MRI, MRA result. 3) Because individual difference is large, excluding to ananalogy of symptom. This is normal numerical value that has possibility of being checked as abnormal numerical value 4) there are a lot of cases that the speed of normal part is as similarly measured as that of abnormal part. It means that we cannot judge the disease by this measure 5) It is rare that this measure represent degree of improvement in patient's condition of disease. When we observe patient's condition become better, but we have no case that the result of TCD test better. 6) The result could be appear differently by the technique of the tester or by the experience of the tester 7) In the TCD test, abnormal symptoms is checked at 0 week, but at 2th week, normal symptoms is checked, again at 4th week abnormal is checked. According to the above result, CVA diagnosis is difficult only with TCD, as it appear in diagnosis error check which is suggested in the problem connected to project, for the aged persons who have the worst hardening of part of the cranium (1998. 5. 26 77 of 83 patients is 50s) there is a lot of cases that the measurement is impossible by TCD and the correction of measurement numerical value is decreased, as the age of cerebral infarction is high, TCD is inappropriate to diagnosis equipment through this study.

A Study of Acupuncture Documentary Characteristics of "Chimgugapelgyeong(鍼灸甲乙經)" ("침구갑을경(鍼灸甲乙經)"의 침구문헌적(鍼灸文獻的) 특징(特徵)에 관한 연구(硏究))

  • The acupuncture documentary characteristics of the "Chimgugapeulgyeong" can be summarized into 7 parts such as the following. 1. After Imeok(林億)'s revised edition of the "Gapeulgyeong(甲乙經)" was printed during the Song dynasty, there were no reprints during the Southern Song, Geum(金) and Won(元) eras, and the first printed edition that remains today is the 'Uihakyukgyeong edition[醫學六經本]' published by Omyeonhak(吳勉學) during the Mallyeok(萬曆) era of the Myeong(明) dynasty. This publication was put into the "Uitongjeongmaek(醫統正脈)" collection in the 29th year of the Manlleok(萬曆) era(1601). Most of the remaining copies have been restored during the Cheong dynasty at bookstores, and we can see that much was restored because of damage and missing characters. Also, the 'Namgyeokcho edition[藍格抄本]' and 'Yukgyeong edition[六經本]' of the Myeong dynasty do not come from the same original document, which allows the correction of the former in many places. However, this edition was not copied well, so the order of contents is different, and there are many mistakes. The 'Sagojeonseo edition[四庫全書本]' and the 'Gajeong edition[嘉靖本]', which Yeounsu(余云岫) quoted from, coincide with each other, making them worth much reference. So, the "Gapeulgyeong" and 'Yukgyeong edition' should be seen as the original, with the 'Myeongcho edition[明抄本]' as the main revision, and the 'Sago edition[四庫本]' as a reference edition. The so-called 'Chojeongtong edition(鈔正統本)' has many problems and marks of forgery, so therefore cannot be used in revising the "Gapeulgyeong" through comparison. 2. The table of contents[序例] in the front of the current edition was in the original edition and was not added by Imeok. The structure of sentences quoted by medical books before the Song dynasty coincide with this 'table of contents'. The "Gapeulgyeong" of the Song dynasty also coincide with the 'table of contents' but the edition remaining differs much from this 'table of contents' so it was edited or erased by people from future generations, especially after the Song dynasty. 3. The remaining edition of "Gapeulgyeong" consists of at least 4 parts. The original edited by Hwangbomil(皇甫謐), annotations added by medicinal practitioners before the Song dynasty, Imeok's revisionary annotations during the Song dynasty, and annotations after the Song dynasty. 4. Expressions such as 'Somun says[素問曰]' 'Gugwon says[九卷曰]' and explanatory annotations like 'Hae says[解曰]' are old writings from the original text and were not added by someone later. 5. Almost all of the 'Double lined small letter annotations[雙行小字注文]' of the 'Yukgyoeng edition' was by people during the Song dynasty. 6. There are many omitted and wrong letters in the remaining edition and there are also many places where future generations edited and supplemented the text.
