Sleep Quality and Poor Sleep-related Factors Among Healthcare Workers During the COVID-19 Pandemic in Vietnam

  • Objectives: The coronavirus disease 2019 (COVID-19) pandemic has increased the workload of healthcare workers (HCWs), impacting their health. This study aimed to assess sleep quality using the Pittsburgh Sleep Quality Index (PSQI) and identify factors associated with poor sleep among HCWs in Vietnam during the COVID-19 pandemic. Methods: In this cross-sectional study, 1000 frontline HCWs were recruited from various healthcare facilities in Vietnam between October 2021 and November 2021. Data were collected using a 3-part self-administered questionnaire, which covered demographics, sleep quality, and factors related to poor sleep. Poor sleep quality was defined as a total PSQI score of 5 or higher. Results: Participants' mean age was 33.20±6.81 years (range, 20.0-61.0), and 63.0% were women. The median work experience was 8.54±6.30 years. Approximately 6.3% had chronic comorbidities, such as hypertension and diabetes mellitus. About 59.5% were directly responsible for patient care and treatment, while 7.1% worked in tracing and sampling. A total of 73.8% reported poor sleep quality. Multivariate logistic regression revealed significant associations between poor sleep quality and the presence of chronic comorbidities (odds ratio [OR], 2.34; 95% confidence interval [CI], 1.17 to 5.24), being a frontline HCW directly involved in patient care and treatment (OR, 1.59; 95% CI, 1.16 to 2.16), increased working hours (OR, 1.84; 95% CI,1.37 to 2.48), and a higher frequency of encountering critically ill and dying patients (OR, 1.42; 95% CI, 1.03 to 1.95). Conclusions: The high prevalence of poor sleep among HCWs in Vietnam during the COVID-19 pandemic was similar to that in other countries. Working conditions should be adjusted to improve sleep quality among this population.

COVID-19 유행 동안 대학생의 체중증가와 관련된 생활습관 및 식생활 변화 (Lifestyle and dietary changes related to weight gain in college students during the COVID-19 pandemic)

  • 본 연구는 COVID-19 유행 동안 대학생의 체중변화를 알아보고 체중증가와 관련된 생활습관 및 식생활 변화를 파악하기 위해 실시되었다. 대학생 270명을 대상으로 2021년 9월 22에서 10월 26일까지 온라인 설문조사를 실시하였다. 체중증가와 일반적 특성, 생활습관 및 식생활 변화와의 관련성이 로지스틱 회귀분석을 통해 평가되었다. 체중이 증가하였다고 응답한 비율은 남성이 42,9%, 여성이 44.7%이었다. 체중이 증가한 주요 이유로는 외부활동 제한으로 활동량 감소, 배달음식이나 인스턴트 위주의 식습관으로 조사되었다. COVID-19 유행 동안 체중증가와 일반적 특성 및 생활습관과의 관련성을 평가한 결과 체중증가는 주관적 건강상태가 나쁜 경우 (OR, 3.97, 95% CI, 1.98-7.96), 저체중 (OR, 0.19, 95% CI, 0.05-0.83)과 유의적 수준에서 관련성이 있었다. 체중증가와 식생활과의 관련성에 대하여 분석한 결과 체중증가가 아침 섭취빈도 증가(OR, 4.44, 95% CI, 1.76-11.21), 간식 섭취빈도 감소 (OR, 0.35, 95% CI, 0.16-0.77), 과일 섭취빈도 감소 (OR, 3.0, 95% CI, 1.32-6.80), 탄산음료 및 가당음료 섭취빈도 증가 (OR, 2.74, 95% CI, 1.26-5.99), 패스트푸드의 섭취빈도 증가 (OR, 2.32, 95% CI, 1.14-4.70)와 유의적 수준에서 관련이 있었다. COVID-19 유행은 체중증가와 생활습관 및 식생활 변화에 영향을 미쳤다. 향후 감염병을 대비하여 좀 더 많은 표본을 대상으로 한 조사결과를 바탕으로 구체적인 건강 및 영양관리 방안이 마련되어야 한다.

  • Purposes: Evaluate the level of mental health of administrative workers in medical institutions during the COVID-19 epidemic, and check the relationship. Methodology: It targeted 183 administrative workers at medical institutions located in Jeollabuk-do. The data survey method used a self-filled questionnaire, and the data collection period was 30 days from November 16, 2021 to December 16, 2021. A comparative analysis was conducted to find out the correlation between the subjects' biological factors, environmental factors, lifestyle factors, job related factors and depression, Post Traumatic Stress Disorder, and job stress. Findings: As an influencing factor of depression, it was found that the longer the working period, the higher the depression, and the higher the position, job satisfaction, and the total amount of smoking, the lower the depression. Due to the effect on Post Traumatic Stress Disorder, the higher the total amount of smoking, the higher the Post Traumatic Stress Disorder after effects, and the higher the organizational commitment and job satisfaction, the lower the Post Traumatic Stress Disorder aftereffects. It was found that organizational commitment had the highest influence on Post Traumatic Stress Disorder. As an influencing factor on job stress, it was found that the higher the job related factors was job satisfaction and organizational commitment, the lower the job stress. Practical Implications: Variables such as moderate physical activity, alcohol consumption, total smoking, working period, position, job satisfaction, and organizational commitment were identified as factors influencing depression, Post Traumatic Stress Disorder, and job stress during the COVID-19 pandemic.

코로나19 전후 남자청년의 우울, 건강 관련 습관, 식습관 및 영양소 섭취량의 변화 -2018년, 2020년 국민건강영양조사 자료 분석- (Depression, Health-Related Habits, Eating Habits, and Nutrient Intake of Male Youth Before and After the Outbreak of the COVID-19 Pandemic -Analysis of the 2018 and 2020 Korea National Health and Nutrition Examination Survey-)

  • This study analyzed the differences in the general characteristics, mental and health-related factors, eating habits, and nutrient intake of Korean male youths before and after the outbreak of the coronavirus disease 2019 (COVID-19). The data of a total of 1,231 subjects, from among the participants in the 2018 and 2020 Korea National Health and Nutrition Examination Survey (KNHANES), were analyzed. The 2020 group had fewer married members than the 2018 group. The subjects in the 2020 group weighed more (P<0.05), had a higher waist circumference (P<0.01), and showed greater weight gain (P<0.05) than the 2018 group subjects. The aerobic physical activity of the 2020 group was lower (P<0.01) compared to the 2018 group. The members of the 2020 group had higher depression scores (P<0.01), suicidal tendencies (P<0.01), and need for counseling for mental problems (P<0.05) than those of the 2018 group. The prevalence of high fasting blood sugar levels and diabetes was higher in the 2020 group compared to the 2018 group. The frequency of breakfast (P<0.01) and dining out (P<0.001) and the proportion of breakfast (P<0.01) and dinner (P<0.05) taken with the family or non-family members was lower in the 2020 group than in the 2018 group. The rate of dietary supplement intake of the 2020 group was higher than that of the 2018 group (P<0.01). There were significant differences in the protein (P<0.05), riboflavin (P<0.01), and carbohydrate (P<0.01) intakes between the two groups. These results suggest that male youths had a higher waist circumference and high levels of depression, and showed a decrease in frequency of breakfast intake and dining out after the out break COVID-19 pandemic. Therefore, nutrition and health promotion programs are urgently needed to improve mental health, other health-related factors and nutritional status of male adults that are associated with the COVID-19 pandemic.

Comparison of the Clinical and Laboratory Features of COVID-19 in Children During All Waves of the Epidemic: A Single Center Retrospective Study

  • 목적: 코로나19 판데믹이 시작된 이후, 다양한 주요 변이 바이러스가 출현했다. 코로나 19 판데믹 기간 동안 대표적인 주요 변이 바이러스 유행 시기를 네 가지로 나누고, 네 가지의 주요 변이 바이러스 시기로부터 임상적 그리고 혈액학적 검사의 특징을 파악하고자 하였다. 방법: 코로나19 확진으로 입원한 19세 이하 환자의 의무기록을 후향적으로 분석하였다. 변이전시기(2020년 2월 1일-2020년 9월 30일), 알파와 베타 변이 시기(2020년 10월 1일-2021년 5월 31일), 델타 변이 시기(2021년 6월 1일-2021년 10월 31일), 오미크론 변이 시기(2021년 11월 1일-2022년 5월 31일)를 비교하였다. 결과:대상환자 827명중에서 163명(19.7%)가무증상이었고, 발열과기침의빈도는각각 320명(38.7%), 399명(48.2%)이었다. 38.5℃ 이상의 발열이 있었던 경우는 12세 미만인 경우에 오미크론 변이 시기에 높게 관찰되었다. 혈액학적 검사에서 백혈구 감소증, 임파구 감소증 그리고 호중구 감소증은 각각 33%, 30.2%, 24.9%로 관찰되었다. 결론: 코로나 19의 주요 변이 바이러스 우세 시기에 다른 특징들이 있었다. 델타 변이 시기에 4일 이상의 발열이 지속되는 경우가 더 많았고, 오미크론 변이 시기에는 38.5℃ 이상의 발열을 가지는 경우가 많았다.

Neuroimaging Findings in Patients with COVID-19: A Systematic Review and Meta-Analysis

  • Objective: Central nervous system involvement in coronavirus disease 2019 (COVID-19) has been increasingly reported. We performed a systematic review and meta-analysis to evaluate the incidence of radiologically demonstrated neurologic complications and detailed neuroimaging findings associated with COVID-19. Materials and Methods: A systematic literature search of MEDLINE/PubMed and EMBASE databases was performed up to September 17, 2020, and studies evaluating neuroimaging findings of COVID-19 using brain CT or MRI were included. Several cohort-based outcomes, including the proportion of patients with abnormal neuroimaging findings related to COVID-19 were evaluated. The proportion of patients showing specific neuroimaging findings was also assessed. Subgroup analyses were also conducted focusing on critically ill COVID-19 patients and results from studies that used MRI as the only imaging modality. Results: A total of 1394 COVID-19 patients who underwent neuroimaging from 17 studies were included; among them, 3.4% of the patients demonstrated COVID-19-related neuroimaging findings. Olfactory bulb abnormalities were the most commonly observed (23.1%). The predominant cerebral neuroimaging finding was white matter abnormality (17.6%), followed by acute/subacute ischemic infarction (16.0%), and encephalopathy (13.0%). Significantly more critically ill patients had COVID-19-related neuroimaging findings than other patients (9.1% vs. 1.6%; p = 0.029). The type of imaging modality used did not significantly affect the proportion of COVID-19-related neuroimaging findings. Conclusion: Abnormal neuroimaging findings were occasionally observed in COVID-19 patients. Olfactory bulb abnormalities were the most commonly observed finding. Critically ill patients showed abnormal neuroimaging findings more frequently than the other patient groups. White matter abnormalities, ischemic infarctions, and encephalopathies were the common cerebral neuroimaging findings.

COVID-19 Vaccination Alters NK Cell Dynamics and Transiently Reduces HBsAg Titers Among Patients With Chronic Hepatitis B

  • Coronavirus disease 2019 (COVID-19) vaccination may non-specifically alter the host immune system. This study aimed to evaluate the effect of COVID-19 vaccination on hepatitis B surface Ag (HBsAg) titer and host immunity in chronic hepatitis B (CHB) patients. Consecutive 2,797 CHB patients who had serial HBsAg measurements during antiviral treatment were included in this study. Changes in the HBsAg levels after COVID-19 vaccination were analyzed. The dynamics of NK cells following COVID-19 vaccination were also examined using serial blood samples collected prospectively from 25 healthy volunteers. Vaccinated CHB patients (n=2,329) had significantly lower HBsAg levels 1-30 days post-vaccination compared to baseline (median, -21.4 IU/ml from baseline), but the levels reverted to baseline by 91-180 days (median, -3.8 IU/ml). The velocity of the HBsAg decline was transiently accelerated within 30 days after vaccination (median velocity: -0.06, -0.39, and -0.04 log10 IU/ml/year in pre-vaccination period, days 1-30, and days 31-90, respectively). In contrast, unvaccinated patients (n=468) had no change in HBsAg levels. Flow cytometric analysis showed that the frequency of NK cells expressing NKG2A, an NK inhibitory receptor, significantly decreased within 7 days after the first dose of COVID-19 vaccine (median, -13.1% from baseline; p<0.001). The decrease in the frequency of NKG2A+ NK cells was observed in the CD56dimCD16+ NK cell population regardless of type of COVID-19 vaccine. COVID-19 vaccination leads to a rapid, transient decline in HBsAg titer and a decrease in the frequency of NKG2A+ NK cells.

Antibody response to COVID-19 vaccination in patients on chronic hemodialysis

  • Purpose: Since patients on hemodialysis (HD) are known to be vulnerable to coronavirus disease 2019 (COVID-19), many studies were conducted regarding the effectiveness of the COVID-19 vaccine in HD patients in Western countries. Here, we assessed antibody response of HD patients for 6 months post-vaccination to identify the duration and effectiveness of the COVID-19 vaccine in the Asian population. Materials and Methods: We compared antibody response of the COVID-19 vaccine in HD patients with healthy volunteers. Patient and control groups had two doses of ChAdOx1 nCoV-19 and mRNA-1273, respectively. Immunoglobulin G (IgG) was measured before vaccination, 2 weeks after the first dose, 2 and 4 weeks, 3 and 6 months after the second dose. Neutralizing antibody was measured before vaccination and at 2 weeks, 3 and 6 months after second dose. Since the third dose was started in the middle of the study, we analyzed the effect of the third dose as well. Results: Although antibody production was weaker than the control group (n=22), the patient group (n=39) showed an increase in IgG and neutralizing antibody after two doses. And, 21/39 patients and 14/22 participants had a third dose (BNT162b2 or mRNA-1273 in the patient group, mRNA-1273 in the control group), and it did not affect antibody response in both group. Trend analysis showed IgG and neutralizing antibody did not decrease over time. Age, sex, and HD vintage did not affect antibody production in HD patients. Patients with higher body mass index displayed better seroresponse, while those on immunosuppressants showed poor seroresponse. Conclusion: Two doses of vaccination led to significant antibody response in HD patients, and the antibody did not wane until 6 months.

Factors associated with parental intention to vaccinate their preschool children against COVID-19: a cross-sectional survey in urban area of Jakarta, Indonesia

  • Purpose: We reported a survey-based study assessing the parental intention to vaccinate children of 5 to 7 years old against coronavirus disease 2019 (COVID-19). The aim of this study is to assess factors influencing the parental intention to vaccinate their children against COVID-19. Materials and Methods: This study adopted a cross-sectional design, held at the public health center of Senen district, Jakarta, Indonesia from November 1-30, 2022. The off-line questionnaires were distributed via the school administrator to all eligible parents. Factors associated with intention to vaccinate were analyzed with the regression logistic models. Results: Of the 435 parents in this study, 215 had already vaccinated their children against COVID-19 (49.4%), and the overall intention of the participants to vaccinate was 69.7%. Factors associated with intention to vaccinate the children against COVID-19 were parental employment status, parental COVID-19 vaccine status and concern of contracting COVID-19. Parents who are employed, had completed vaccines with COVID-19 booster vaccine, and had concern of their children contracting COVID-19 were more likely to vaccinate their children (odds ratio [OR], 2.10; 95% confidence interval [CI], 1.22-3.69; p=0.011; OR, 2.15; 95% CI, 1.21-3.83; p=0.013; OR, 2.40; 95% CI, 1.34-4.30; p=0.004, respectively). Concern on the vaccine's side effects was negatively associated with the willingness to vaccinate. Conclusion: This study showed that childhood COVID-19 vaccine only covered half of the population, with parental intentions for childhood COVID-19 vaccination being high, reaching almost two-thirds of the study participants. Factors influencing parental intentions were employment status, parental COVID-19 vaccine status, concerns about COVID-19 and concerns about vaccine side effects.

Antibody Response Induced by Two Doses of ChAdOx1 nCoV-19, mRNA-1273, or BNT162b2 in Liver Transplant Recipients

  • Coronavirus disease 2019 (COVID-19) vaccination in immunocompromised, especially transplant recipients, may induce a weaker immune response. But there are limited data on the immune response after COVID-19 vaccination in liver transplant (LT) recipients, especially on the comparison of Ab responses after different vaccine platforms between mRNA and adenoviral vector vaccines. Thus, we conducted a prospective study on LT recipients who received two doses of the ChAdOx1 nCoV-19 (ChAdOx1), mRNA-1273, or BNT162b2 vaccines compared with healthy healthcare workers (HCWs). SARS-CoV-2 S1-specific IgG Ab titers were measured using ELISA. Overall, 89 LT recipients (ChAdOx1, n=16 [18%]) or mRNA vaccines (mRNA-1273 vaccine, n=23 [26%]; BNT162b2 vaccine, n=50 [56%]) received 3 different vaccines. Of them, 16 (18%) had a positive Ab response after one dose of COVID-19 vaccine and 62 (73%) after 2 doses. However, the median Ab titer after two doses of mRNA vaccines was significantly higher (44.6 IU/ml) than after two doses of ChAdOx1 (19.2 IU/ml, p=0.04). The longer time interval from transplantation was significantly associated with high Ab titers after two doses of vaccine (p=0.003). However, mycophenolic acid use was not associated with Ab titers (p=0.53). In conclusion, about 3-quarters of LT recipients had a positive Ab response after 2 doses of vaccine, and the mRNA vaccines induced higher Ab responses than the ChAdOx1 vaccine.
