A Study on Community Perceptions of Common Cancers, Determinants of Community Behaviour and Program Implementation in New Delhi, India

  • Assessment of perceptions of the community, the determinants and development of their behavior regarding common malignancies, helps in establishing evidence-based activities for prevention and early diagnosis of cancer. However information on this important aspect is lacking in our country. Qualitative methods were here used to obtain information through in depth interviews and Focus Group Discussions (FGDs) with all categories of identified stakeholders. Most were unaware of the activities of the cancer control program. Even the providers were not fully conversant. All respondents mentioned lack of diagnostic and treatment facilities in government, primary and secondary level facilities. Perceptions of different categories of stakeholders regarding why many community members did not attend screening camps and other services reflect the determinants of community behavior, acting independently as well as in combination. The cancer control program was thought to be restricted only to some private facilities and overcrowded government tertiary care facilities where the visits were time consuming. Almost all the facilities were considered curative oriented. Issues of supervision, monitoring and feedback were inadequately addressed by providers who were inadequately trained in many program activities. The program lacked effective planning, coordination and appropriate implementation at the grass roots level in Delhi. Social mobilization was grossly inadequate, as most of the community members were unaware of the existence of the program. Misconceptions about the risk factors, signs and symptoms, diagnosis, and treatment were common amongst community members as well as many of the providers. Thus the national cancer control program in our country needs further community participation and social mobilization. Accessibility, availability, acceptability and affordability of various preventive, curative and rehabilitative activities, as well as intersectoral coordination, training, supervision and monitoring of program activities, all need to be addressed to ensure the success of this important public health program.

Ankle Sprain Affects Lower Leg Muscle Activation on Vertical Landing, Half Point, and Gait in Female Ballet Students

  • Purpose: This study aimed to investigate effect of ankle instability on peripheral muscle activation among female ballet dancers to provide information on the development of prevention programs for ankle injury rehabilitation. Methods: 32 female ballet dancers were randomly divided into two groups: experience ankle sprain group (n=16, age, $20.7{\pm}0.8years$, BMI $18.6{\pm}1.2kg/m2$) and non-experience ankle sprain group (n=16, $age=21.0{\pm}0.8years$, BMI $19.6{\pm}2.0kg/m2$). Activation of the peroneus longus, tibialis anterior muscle, and gastrocnemius during vertical landing, half pointe, and gait between the two groups were measured. Body composition analyzer was used to examine skeletal muscle mass and body fat mass. Results: A total of 32 patients were included. In the experience ankle sprain group (n=16: left sprain 14, right sprain 2), average ankle sprain injury occurred 7.5 months before the study. The average age of the dancers in the experience ankle sprain group and non-experience ankle sprain group was $20.7{\pm}0.8$ and $21.0{\pm}0.8years$, major period was $64.5{\pm}23.8$ and $71.6{\pm}25.8months$, BMI was $18.6{\pm}1.2$ and $19.5{\pm}2.0kg/m2$, respectively. No significant differences were found on body composition between the two groups (p>0.05). The experience ankle sprain group showed significantly lower tibialis anterior and peroneus longus muscle activation (p<0.5), while gastrocnemius muscle activation appeared to be significantly higher (p<0.05) during landing, half pointe, and normal gait. Conclusion: Ankle sprain can cause a decline in peripheral muscle activation and coordination, which increased the risk for repetitive ankle sprain in the future. Moreover, ankle peripheral muscle selective strength training, coordination program development, and application need to be considered to prevent ankle sprain.

지역기반 정신장애인 직업재활수행기관간 연계강화에 관한 연구 : 연계경험 및 예측요인 분석을 중심으로 (A Study on Promoting Inter-organizational Linkages for Vocational Rehabilitation of People with Psychiatric Disabilities : Focusing on Linkage Experiences and Predictors)

  • 본 연구에서는 우리나라에서 지역기반으로 정신장애인 직업재활서비스를 제공하는 기관을 조합하여 규정하고, 이들간의 연계경험과 연계실태 및 연계에 영향을 미치는 요인을 분석하였다. 연구방법은 질적조사와 양적조사를 병행하여 실시하는 삼각측정방식(triangulation)으로 진행되었다. 근거이론에 바탕을 둔 Strauss & Cobin의 절차에 따라 한 지역을 선정하여 담당자들의 연계경험을 분석하였으며, 설문조사를 통해 122개 기관의 연계실태와 연계시도요인, 연계강좌요인을 분석하였다. 조사결과, 연계경험은 '다른 기관에 관심돌리기', 즉 '협력추구'과정을 도출되었으며, 협력추구유형으로는 '주도형 협조형 권위형 반응형'으로 분류되었다. 연계실태는 전반적으로 Tobin(1986)의 연계긴밀도 5단계에 따라 정신장애인 직업재활수행기관들은 3단계 조정(Coordination)이 필요한 시기로 나타났으며, 정신보건기관과 전통적 재활기관사이에는 '협조(Cooperation)'가 정신보건기관간, 전통적 재활기관간에는'조정'이 더 필요한 것으로 나타났다. '연계시도'와 '연계강좌'에 영향을 미치는 요인에 대한 구성을 개인요인과 조직요인으로 구분하여 로지스틱 회귀분석과 중다귀회귀분석을 실시하였다. 그 결과, 개인의 '정신장애인 이해를 위한 교육연수경청'이 있을 경우, 조직의 '자원의존도'가 높을수록 연계시도를 할 확률이 증가하는 것으로 나타났다. 또한 개인의 '현 근무지 근무기간'이 길수록, '관련기관 사업인지도'가 높을수록, '정신장애인 이해를 위안 교육연수경험'이 있을 경우, 조직에 '직업재활전담인력'이 있을 경우 연계가 강화되는 것으로 분석되었다. 본 연구는 이러한 실증적 분석을 통하여 정신장애인 직업재활수행기관간 연계강화방안을 논의하였다.

상지 협응 움직임을 기반으로 한 과제중심적 접근 훈련이 발달성 협응 장애아동의 글씨쓰기 능력에 미치는 효과 (The Effects of Upper Limb Coordinated Movement Based Task Oriented Approach on Improving Handwriting Performance in Children With Developmental Coordination Disorder)

  • 목적 : 상지 협응 움직임을 기반으로 한 과제중심적 접근 훈련이 발달성 협응 장애아동의 한글 글씨쓰기 능력에 미치는 영향을 알아보며 각 접근이 한글 글씨 쓰기 수행의 질과 글씨쓰기 준비기술에 미치는 영향을 비교하고자 한다. 연구방법 : 본 연구는 5~10세의 발달성 협응장애 아동 40명을 대상으로 무작위 추출을 통해 두 집단으로 나뉘어 사전-사후 검사로 진행하였다. 두 집단은 과제중심접근 중재군과 과정중심접근 중재군으로 5주 동안 40분씩 주 2회 중재를 받았다. 글씨쓰기 수행은 한글 자모 쓰기 검사로 측정하고, 글씨쓰기 준비요소인 상지 운동 기능, 시 지각 기능과 시 운동기능은 각각 운동 유창성 평가(Bruininks-Oseretsky Test of Motor Proficiency-2: BOT-2)중 미세운동조절의 하위검사 일부, 한국판 시 지각 발달 검사(Korean-Developmental Test of Visual Perception-2: K-DTVP-2), 개정된 시 운동기술 평가(Test of Visual Motor Skills-Revised: TVMS-R)로 평가하였다. 고찰 : 본 연구는 과제중심접근법을 적용하여 발달성 협응 장애아동의 글씨쓰기 수행도를 향상시킨다는 임상적 근거를 제공하는데 의의가 있다. 또한 과정중심접근법과 과제중심접근법이 글씨쓰기 준비기술의 향상에 어떠한 영향을 끼치는 지에 대한 결과를 확인하여 보다 효과적인 글씨쓰기 중재방법을 제시하는 초석이 될 수 있을 것이라 생각한다.

과학교육 개혁 프로그램 '과학-기술-사회'의 국제적 동향-아이오와 프로그램을 중심으로- (The International Trend of Science Education Reform Program 'Science-Technology-Society'-The Case of Iowa Chautauqua Program-)

  • 과학-기술-사회 (Science-Technology-Society)와 구성주의(constructivism) 학습올 근간으로 한 아이오와 치타콰 프로그램과 Iowa-Scope, Sequence and Coordination 현직교육 프로그램은 80년대와 90년대의 과학 교육 연구 결과와 사회의 변화에 기초하고 있다. 초, 중,고등학교 과학 교과과정을 마친 학생들이 아주 피상적인 과학적 소양올 갖고 있음이 많은 연구들을 통해 확인 된 것이다. 반면 학생들의 관심과 호기싱을 바탕으로 학 생들의 삶과 유관한 교육과정과 교수는 의미있는 과학학습으로 인도될 수 있다. 아이오와 치타콰 프로그램과 Iowa-Scope, Sequence and Coordination 현직교육 프로그램은 교사교육을 통해 현실과 유관한 과학교육을 수행하는데 큰 영향을 미쳤다. 인간의 삶의 맥락에서 구성주의 전략을 사용하여 과학을 가르치는 기법은 교사들에게 교수와 학습의 의미를 되새기게 했으며, 참여 교사들의 교수 전반에 영향을 주었다. 특히 모듈 개발을 통해 현장에서의 경험과 아이디어를 구체화하고 다양한 평가 방법을 통해 확인 된 연구 결과는 교사와 학생 모두에게 바랍직한 영향을 준 것으로 확인되었다. 본 현직교육 프로그햄들은 1년 혹은 3년 동안 같은 교사들을 대상으로 지속적으로 이루어져 현장 교육에 직접적인 영향을 주었다. 이 프로그램에 대한 구체적 적용을 통해 우리 나라 과학교육의 방향을 지식 암기 중심에서 학생들의 삶에 기초한 탐구와 적용 중심의 학습으로 바꿀 수 있을 것이다. 또한, 현직교육 프로그램의 개선을 통해 이론과 현장이 만나는 장을 만들 수 있을 것으로 사료된다.

Qualitative Assessment of Breast Cancer Early Detection Services Provided through Well Woman Clinics in the District of Gampaha in Sri Lanka

  • Background: Breast cancer is the most common cancer diagnosed among women in Sri Lanka. Early detection can lead to reduction in morbidity and mortality. The objective here was to identify perceptions of public health midwives (PHMs) on the importance of early detection of breast cancer and deficiencies of and suggestions on improving existing breast cancer early detection services provided through Well Woman Clinics. Materials and Methods: A qualitative study using four focus group discussions (FGDs) were conducted among 38 PHMs in the Gampaha district in Sri Lanka and the meetings were audio-recorded, transcribed and analyzed using constant comparison and identifying themes and categories. Results: All the PHMs had a firm realization on the need of breast cancer early detection. The four FGDs among PHMs revealed non-availability of guidelines, inadequacy of training, lack of skills and material to provide health education, inability to provide privacy during clinical examination, shortage of stationery, lack of community awareness and motivation. The suggestions for the improvements of the programme identified in FGDs were capacity building of PHMs, making availability of guidelines, rescheduling clinics, improving the supervision, strengthening the monitoring, improving coordination between clinical and preventive sectors, and improving community awareness. Conclusions: Results of the FGDs can provide useful information on components to be improved in breast cancer early detection services. Study recommendations were training programmes at basic and post basic levels on a regular basis and supervision for the sustainance of the breast cancer early detection program.

패션 유통업체 위탁교육 활성화 방안 (The Methods to Activate the Consigned Education of Fashion Retailing Companies in the Dept. of Fashion Design in Junior Colleges)

  • This study was conducted to provide the basic data to activate the consigned education of fashion retailing company in the department of fashion design in a junior college. 36 consigned education students from fashion retailing company in Daegu were surveyed and interviewed. The result can be summarized as follows: 1. Their motives of study lie in ‘I wanted to study in a junior college’ (55.6%), ‘It would be some help for my job’. ‘My company recommended’ (16.7%), and ‘I wanted to get a higher degree’ (11.1%). 2. Their preferences in terms of major and subjects lie in ‘fashion marketing’ (30.6%), ‘fashion coordination’ (25.0%), and ‘fashion information’ (19.4%). 3. The occupations they wanted to get are ‘a displayer’ (38.9%), and ‘a shop master’ (30.6%), and ‘a merchandiser’ (8.3%). 4. The ratio of theories to practices they want is ‘40:60ㆍ30:70’ (27.8%), ‘50:50’ (22.2%), ‘20:80’ (11.1%) 5. With regard to their adaptability in school, they answered that ‘I'm doing quite well’ (44.4%), ‘I'm doing OK’. ‘It's a little hard to follow’ (22.2%) 6. About teaching methods and contents, they responded that they want ‘more emphasis on practices’ (41.7%), and they commented that ‘teachers are focused on the syllabus procedure rather than students’ practical improvement’ (19.4%), and ‘I feel loaded with too much assignment’ (13.9%) 7. They wanted to be taught by ‘professors’ (77.8%) and ‘professionals in the field’ (19.4%) In conclusion, it is required that both the college and the company need to develop a customized education program focusing on the field training specialized by their cooperation and constant researches. On the part of junior colleges, adjusting their educational directions to the trend of the 21st century, they need to focus on nurturing the consigned students to be highly qualified professionals required by the industry, using hi-tee equipment and facilities through academic and industrial cooperation.

The effect of hip joint strengthening exercise using proprioceptive neuromuscular facilitation on balance, sit to stand and walking ability in a person with traumatic brain injury: a case report

  • Objective: The purpose of this study was to investigate the effect of the hip joint strengthening exercises using proprioceptive neuromuscular facilitation (PNF) on the clinical symptoms and the treatment effects in balance, sit to stand, and gait abilities in patients with TBI. Design: A single case study. Methods: A 13-year-old adolescent with quadriplegia and hip joint control impairment participated in this four-week training intervention. The patient, diagnosed with TBI, wastreated with hip joint strengthening exercises using PNF. In the first week, we focused on strengthening the body, relaxing the hip flexors and activating the hip extensor muscles in order to solve the patient's physical function and body structure. From the 2nd and 4th week, we improved the motivation through the task-oriented method, and then weight-bearing training of the right lower extremity was proceeded by kicking a soccor ball with the left lower extremity. The exercises were performed for 4 weeks, 5 days a week, for 60 minutes with the exercise intensity gradually increased according to the subject's physical abilities. Results: As a result of the study, the patient demonstrated improvements in the physical examination, which were evaluated before and after intervention and included the manual muscle test, modified Ashworth scale, sensory assessment, coordination assessment, Berg balance scale, 5-time sit to stand test, and the 10 meters walk test. Conclusions: The results of this case suggest that a hip joint strengthening exercise program using PNF may improve hip control ability, balance, sit to stand and gait ability in a patient with TBI.

Yoga for children

  • Excessive stress is harmful to academic performance in children and may lead to dropping out of school. To meet the demands of a modern life-style which is full of speed, stress and tension, an all-round child health program is crucial. The use of yoga for children has diverse applications in maintaining and developing their physical, mental, intellectual, emotional and spiritual levels. Yoga, through its physical postures (asana), breathing practices (pranayama), cleansing techniques (kriya), meditation therapies (dhyana) and relaxation training (yoga nidra) yields a positive effect in the management of stress in children. Yoga practice benefited children by improving their eye-hand coordination, attention span, levels of concentration, competitive performance and relaxation. Visually impaired children showed a significant decrease in their abnormal anxiety levels when they practiced yoga for three weeks, while a program of physical activity had no such effect. Socially disadvantaged children in a remand home showed significant improvements in sleep, appetite and general well being, as well as a decrease in physiological arousal after yoga. In one study, it is found that a 4-week program of asana and meditation lowers the aggressive behavior of children. Meditation helped to reduce problems related to maladaptive behavior, increase emotional and physical health and psychological well-being in children. Finally, the possible role of yoga in improving the mental state and general well-being of children with cancer is being explored.

A Novel Scheme for detection of Parkinson’s disorder from Hand-eye Co-ordination behavior and DaTscan Images

  • With millions of people across the globe suffering from Parkinson's disease (PD), an objective, confirmatory test for the same is yet to be developed. This research aims to develop a system which can assist the doctor in objectively saying whether the patient is normal or under risk of PD. The proposed work combines the eye-hand co-ordination behaviour with the DaTscan images in order to determine the risk of this disorder. Initially, eye-hand coordination level of the patient is assessed through a hardware module. Then, the DaTscan image is analysed and used to extract certain geometrical parameters which shall indicate the presence of PD. These parameters are then finally fed into a Multi-Layer Perceptron Neural Network using Levenberg-Marquardt (LM) Back propagation training algorithm. Experimental results indicate that the proposed system exhibits an accuracy of around 93%.
