The Effect of Geukhachukeutang on Intravascula Coagulation and Contusion Blood Stasis in the Rat (혈전(血栓)과 타박성어혈(打撲性瘀血)에 대(對)한 격하축어탕(膈下逐瘀湯)이 미치는 영향(影響))

  • Geukhachukeutang is used to cure blood stasis in Oriental Medicine. In order to investigate medical value on intravascula coagulation and contusion blood stasis, the effect of Platelet number, Fibrinogen quantity, Prothrombin time, FOP density, Paw edema increasing rate after prescribing Geukhachukeutang concentration liquid orally were observed. The obtained results are summerized as follows ; 1. Platelet number and Fibrinogen quantity are significantly increased, and Prothrombin time and FOP density are significantly decreased by Geukhachukeutang after inducing intravascula coagulation. 2. Platelet number and Fibrinogen quantity are not significantly increased, and Prothrombin time is not significantly decreased either, by Geukhachukeutang after inducing contusion blood stasis but Paw edema increasing rate is remarkablely decreased in three or four hours of the groups of prescribing. According to the above results, Geukhachukeutang will be used for contusion blood stasis and Intravascula coagulation effectively owing to being accepted as a favorable operation on Platelet number, Fibrinogen quantity, Prothrombin time, FDP density and Paw edema increasing rate.

Influences of Cold and Thermal Therapy on ILs and VEGF Expression after Muscle Contusion in Rats (온냉치료가 근타박 유발 흰쥐의와 발현에 미치는 ILs와 VEGF 발현에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate the effects of cold therapy and thermal therapy, and immunoreactivity of vascular endothelial growth factor(VEGF), Interleukin-1(IL-1) and Interleukin-6(IL-6) on angiogenesis after muscle contusion injury. Methods : Muscle contusion injury was induced in the gastronemius muscle by dropping a metal bead(22.8g). Cold and thermal theraphy was applied immediately and directly to the skin of injured muscle daily for three days. (experimental group-1 : $5^{\circ}$ cold pack, experimental group-2 : $50^{\circ}$ hot pack, control group non applied, treatment time : 10minutes) Results : The experimental group-1 and 2 showed higher immunoreactivity of VEGF, IL-1, IL-6 than control group during 3 days(P<0.05). And the experimental group-2 showed higher than the experimental group-1 especially VEGF(P<0.05). Conclusion : There data thermal therapy was more effective than cold therapy in the acute muscle contusion injury.

Effects of Scutellariae Radix on Neuronal Apoptosis of Spinal Cord Contusion Injury in Rats (황금(黃芩)이 척수압박손상 흰쥐의 척수신경세포 자연사에 미치는 영향)

  • Objectives : This Study was performed to evaluate the effects of Scutellariae Radix(SR) water-extract on the tissue and neuronal apoptosis of the spinal cord injury(SCI). Methods: SCI was induced by mechanical contusion following laminectomy of 10th thoracic vertebra in Sprague-Dawley rats. SR was orally given once a day for 7 days after SCI. Neuronal apoptosis was examined with terminal deoxynucleotidyl transferase-mediated dUTPnick-end labeling(TUNEL) assay. Bax (Bcl-2-asociated X protein), Bcl-2(B-cell blastoma 2), c-Fos(FBJ osteosarcoma oncogene) expressions were examined using immuno-histochemistry. Individual TUNEL and immuno-labeled cells expressing Bax, Bcl-2 and c-Fos were counted on the same level in peri-damaged region and in ventral horn. Results: 1. SR significantly reduced number of TUNEL labeled apoptotic cells induced by the spinal cord contusion injury. 2. SR significantly reduced Bax positive cells expression on the motor neuron in the ventral horn induced by the spinal cord contusion injury. 3. SR strengthened Bcl-2 expression on the motor neuron in the ventral horn induced by the spinal cord contusion injury. 4. SR reduced c-Fos expression on the motor neuron in the ventral horn induced by the spinal cord contusion injury. Conclusions : These results suggest that SR plays an inhibitory role against neuronal apoptosis and has significant effects for locomotor disfunction induced by SCI.

A Clinical Analysis of 24 cases of Cardiac Contusion and Cardiac Concussion (둔상에 의한 심타박상과 심좌상의 임상적 고찰)

  • Background: In the patients with thoracic injury, we suspect simultaneous cardiac contusion or concussion. We analyzed the patients with possible cardiac injury by electrocardiography, serum creatine kinase (CK), creatine kinase isoenzyme fraction (CK-MB) screening, followed by two dimentional echocardiogram (2-DE) to access the severity of injury. Material and Method: From January 1997 to April 1998, 15-month retrospective study of suspicious myocardial injury was undertaken in including 24 patients admitted for suspected cardiac injury. All patients with history or signs of blunt chest injury were checked serially and the serial CK, CK-MB fraction, electrocardiography (EKG) analysis screening were followed by 2-DE. Result: The age range was between 20-40 years and were predominant male patients in(M:F=3:1). Most common causes of injury were traffic accidents, 15 patients(62.5%). Associated injuries involved multiple rib fractures, sternal fracture and such. EKG findings on the cardiac concussion were within normal limits, EKG findings on the cardiac contusion were nonspecific ST and T wave abnormality. In cardiac contusion patients, CK-MB fraction did not increase significantly on admission but on 2nd, 3rd, 4th hospital days, it increased significantly (p=0.0080, 0.0130, 0.0130). The average admission days were 9.22 in concussion and 26.18 in contusion patients(p=0.0075). Most common complication was the adult respiratory distress syndrome(7 cases), 5 out of the patients with ARDS were mechanically ventilated. There were no deaths. Conclusion: We believe the serial checks of CK-MB, EKG and subsquent two-dementional echocardiographic sector scanning are presently the most sensitive indicators available for structural and functional cardiac injury.

Risk Factors for Pneumonia in Ventilated Trauma Patients with Multiple Rib Fractures

  • Background: Ventilator-associated pneumonia (VAP) is a common disease that may contribute to morbidity and mortality among trauma patients in the intensive care unit (ICU). This study evaluated the associations between trauma factors and the development of VAP in ventilated patients with multiple rib fractures. Methods: We retrospectively and consecutively evaluated 101 patients with multiple rib fractures who were ventilated and managed at our hospital between January 2010 and December 2015, analyzing the associations between VAP and trauma factors in these patients. Trauma factors included sternal fracture, flail chest, diaphragm injury, traumatic aortic dissection, combined cardiac injury, pulmonary contusion, pneumothorax, hemothorax, hemopneumothorax, abbreviated injury scale score, thoracic trauma severity score, and injury severity score. Results: Forty-six patients (45.5%) had at least 1 episode of VAP, 10 (21.7%) of whom died in the ICU. Of the 55 (54.5%) patients who did not have pneumonia, 9 (16.4%) died in the ICU. Using logistic regression analysis, we found that VAP was associated with severe lung contusion (odds ratio, 3.07; 95% confidence interval, 1.12 to 8.39; p=0.029). Conclusion: Severe pulmonary contusion (pulmonary lung contusion score 6-12) is an independent risk factor for VAP in ventilated trauma patients with multiple rib fractures.

The effects of Salviae Miltiorrhizae Radix(SR), Carthami Flos(CF) and Acori Graminei Rhizoma(AR) to cerebral contusion (뇌좌상(腦挫傷)에 단삼(丹蔘), 홍화(紅花), 석창포(石菖蒲)가 미치는 영향(影響))

  • This study was done to investigate effects of SR, CF and AR to angiogenesis of cerebral tissues, protecting damage of cerebral neurons and activating them in cerebral contusion-induced rats. I observed these conclusions as follows ; 1. Observation of VEGF-immunoreactive cells : Groups of administered AR were not meaningful in increasing VEGF-immunoreactive cells for 3 days and 7 days, groups of administered SR meaningfully increased them to control groups in all groups, and groups of administered CF meaningfully increased them to control groups in all groups, too, interestingly, increased double to control group for 7 days. 2. Observation on cerebral neurons by Cresyl violet stain : Dendrites and axons of groups of administered SR, AR for 3 days were clearly observed to control group. Cerebral neurons of groups of administered CF for 3days and 7 days were increased a little, but were not meaningful. In conclusion, AR will be careful of being used in cerebral contusion. CF and SR were effective to activating cerebral hemokinesis by inducing angiogenesis in trauma of tissue, but weakly to protecting trauma of cerebral neurons and activating them. I think more studies will be done in these facts.

  • Skeletal muscle regeneration is a vital process for various muscle myopathies and muscular adaptation to physiological overload. Angiogenesis is the key event in the process of muscle regeneration, and vascular endothelial growth factor(VEGF) plays an important role in it. The purpose of this study was to evaluate the effect of GaAlAs(830nm) laser and immunoreactivity of VEGF on angiogenesis after muscle contusion injury. Muscle contusion injury was induced in the triceps surae muscle by dropping a metal bead(31.4g). GaAlAs laser irradiation(power 20 mW, frequency 2000 Hz, treatment time 15 min) was applied directly to the skin of injured muscle daily for seven days. The experimental group I was irradiated immediately by laser after injury, whereas the experimental group II was irradiated after 1 day of injury. The control group was non-irradiated. The results of this study were as follows. 1. In morphological observation, there were no significant changes in experimental and control groups for 7 days. At 3 days, however, the splited muscle fibers were observed in experimental groups, and the muscle atrophy and granular tissue viewed at 7 days in control group. 2. The VEGF was expressed in muscle fiber that located in the interspace between gastrocnemius and soleus muscles. As the time coursed, the immunoreactivity of VEGF also seemed to be strong in the individual muscle fibers. 3. The experimental group I & II showed higher immunoreactivity of VEGF than control group(p<0.05). Then, the experimental group I showed higher than group II especially(p<0.05). These data suggest GaAlAs semiconduct diode laser irradiation(830nm) enhanced angiogenesis in the skeletal muscle induced contusion injury, and immediate laser irradiation after injury promoted the angiogenesis greatly than after 1 day of injury.

  • Delirium is a acute syndrome of disorientation caused by disfunction of brain tissue and has a variety of symptomes. It is characterized by disturbance of consciousness and attention, cognition, and perception for a brief period of time and tends to fluctuate during the course of the day. We experienced a 56 year-old woman who had a hypertension as well as Delirium caused by cranial contusion and whose condition was improved by Oriental medical treatment. this case study illustrates what the manifestation of Delirium caused by cranial contusion. This study shows more objective mecical pregress by grading Korean version of Delirium Rating Scale(K-DRS) and Korean version of Mini-Mental State Examination(K-MMSE). After our treatments, Delirium and some other symptoms were improved.

  • Muscle contusion has a negative effect on muscle function. Although several studies showed that pain control and muscle recovery is facilitated by pulsed electromagnetic fields (PEMF), there has not been much research regarding the specific effects of PEMF on them. The aim of the present study is to investigate effects of PEMF on pain and muscle recovery following extensor digitorum longus (EDL) contusion injury through measuring the expression of the c-fos proto-oncogene and nerve growth factor (NGF). Significantly reduced c-fos expression in the spinal cord was shown in PEMF groups compared with control (CON) groups. There was no significant difference between PEMF1 and CON1, but significantly increased NGF expression was shown in PEMF3 and PEMF5 compared with in CON groups, where the numbers in the group names are the days from contusion. In conclusion, PEMF could be used to not only reduce pain in muscle injuries by down-regulating c-fos expression in the spinal cord, but it could also influence muscle healing through increasing NGF expression in the injured muscle.

A Study for Injuries due to Agricultural Machines in Kyeongsangnam Province (경상남도 농촌지역의 농기계손상에 관한 조사연구)

  • As compared before, agricultural machines are used more commonly instead of animal or manpower in rural areas and the injuries due to those are common. This study was conducted by questionnaire method in order to find out the current status of injuries due to agricultural machines for farmers who was selected from three Gun's in Kyeongsangnam Province. The study subjects were 385 persons in all(210 male persons, 175 female reasons) and the study period was from July through September 1993. The results were as follows; 1. The injury rate due to agricultural machines was high in male(p<0.05), and it was higher in younger age group and higher educated group. 2. The injury occurred high in summer and autumn seasons(77.6%), in the afternoon(60.6%), and during harvest(35.2%). 3. The major injuries were contusion, fracture, amputation in order and the injured sites were arms, legs, and chest in order. 31.7% of the injured farmers had been admitted, and they were treated at hospitals, home, drugstores and health centers in order. 4. The casualty damage was highest by cultivators, and agricultural instruments, threshing machine were followed. Among traumatic injuries concerned with cultivators contusions were most common, and fractures, amputations were followed. In case of agricultural instruments bruises were most common, and incisions, contusion were followed. In case of threshing machines fractures were most common and contusion, bruise were followed.
