A successful conservative management of traumatic thoracic esophageal rupture (흉부둔상에 의한 식도파열의 성공적인 보존적 치료)

  • Thoracic esophageal rupture caused by blunt trauma is often not recognized until late because of the vague symptoms in the initial state as well as its rare incidence, which can easily lead to fulminant mediastinitis with frequent fatal outcome. Once extensive mediastinitis occurs, the primary surgical repair of the esophageal tear is considered to be practically impossible. Various methods have been proposed for the management of these desperately ill patients, but no one provides an acceptable good result yet. The purpose of this article is to report the successful result obtained in the treatment of a patient with fulminant mediastinitis from traumatic esophageal rupture by continuous transesophageal irrigation. A 27 year-old male patient was brought to the emergency room of our hospital complaining of dyspnea and chest pain after blunt trauma. The diagnosis of esophageal rupture in the thorax was made late, about 46 hours after the initial injury, when mediastinitis had already progressed. The transesophageal irrigation method was immediately instituted which consisted of profuse transesophageal irrigation of the mediastinum with orally ingested fluid and/or by Levin tube, positioned proximal to the site of the rupture, and drainage of the irrigation fluid by thoracoscopically accurately positioned chest tubes connected to a well suctioning system. With subsiding inflammatory signs and symptoms, the esophagogram, obtained 54 days after the treatment, showed no evidence of the mediastinal leakage of contrast material which contrasted previous esophagograms with definitive dye collections in the mediastinum. Additional endoscopic finding confirmed complete healing of the esophageal mucosa, previously ruptured. He has been followed up without any problem until recently, 6 months after discharge.

Temporal and Spatial Analysis of Hydrology and Water Quality in Small Rural Streams for Stream Depletion Investigation (건천화된 농촌소하천의 시·공간적 수문 수질 특성분석)

  • The purpose of this study was to analyze the temporal and spatial characteristics of the stream flow of small rural streams for investigating the status of stream depletion located downstream of irrigation reservoir. Bonghyun and Hai reservoirs and each downstream were selected for this study. Streamflow was measured for 8 stations downstream from two reservoirs from 2010 to 2012. The water quality samples were collected monthly from the 8 stream stations and 2 reservoir stations from 2011 to 2012. The stream depletion was found in most of the downstream of reservoirs for the non-irrigation period and even in the irrigation period when there were a lot of antecedent precipitation. We found that the stream segments where there were few streamflow, vegetation covers the stream and block the streamflow which makes the stream lost its original function as a stream. Water quality monitoring results of Bonghyun stream indicated that the concentration of SS, Turbidity, TOC, COD were decreased as the stream flows from the reservoir to downstream while the TN and TP were increased. The correlation analysis for water quality data indicated that the correlation between T-N and T-P was high for Bonghyeon and Sukji streams, respectively. Continuous monitoring for rural streams located in downstream of reservoirs are required to quantify the status of stream flow depletion and determine the amount of environmental flows.

Outcomes of the GentleWave system on root canal treatment: a narrative review

  • This study aimed to describe the outcomes of the GentleWave system (GW) (Sonendo) on root canal treatment. Published articles were collected from scientific databases (MEDLINE/PubMed platform, Web of Science, Scopus, Science Direct and Embase). A total of 24 studies were collected from August/2014 to July/2021, 20 in vitro and 4 clinical. GW System was not associated with extrusion of the irrigant, promoted faster organic dissolution than conventional syringe irrigation (CSI), passive ultrasonic irrigation (PUI) continuous ultrasonic irrigation (CUI) and EndoVac, reduced more bacterial DNA and biofilm than PUI and CUI, promoted higher penetration of sodium hypochlorite into dentinal tubules than PUI and CUI in vitro, and removed more intracanal medication than CSI and PUI. GW was able to remove pulp tissue and calcifications. Moreover, its ability to remove hard-tissue debris and smear layer was better than that of CSI, and its ability to remove root canal obturation residues was lower or similar to that of PUI, and similar to that of CSI and EndoVac. Regarding root canal obturation of minimally instrumented molar canals, GW was associated with high-quality obturation. Clinically, the success rate of endodontic treatment using GW was 97.3%, and the short-term postoperative pain in the GW group was not different from CSI. Further research, mainly clinical, is needed to establish whether GW has any advantages over other available irrigation methods.

Invention of the Guide Catheter Irrigation Monitoring Device for Neuroendovascular Therapy

  • Objective : The thromboembolic events during neuroendovascular therapy (NET) are the major complications of concern that can be occasionally fatal. The thrombotic occlusion of the guide catheter for NET is thought to be the risk of the thromboembolic events. We have developed an idea for inventing the monitoring system of the continuous irrigation through the guide catheter. We herein present a unique invention of the guide catheter irrigation monitoring device. Methods : We have developed ideas for preventing the thrombotic occlusion of the guide catheter. In order to design a convenient device working in the practical use, we have consulted and shared the ideas with the electrical engineers about putting the invention. Results : The guide catheter irrigation monitoring device (GCIMD) consisted of three parts of optical sensor, main body and electric adapter. In brief, the basic principles of working of the GCIMD are as follows. The optical sensor is attached to the dripping chamber of the line to irrigation solution. The main body had the small light and speaker to make an alarm sounds. The sensor monitors the dripping of flush solution. If the dripping stops more than three seconds, a warning alarm has been activated. So, the operating physicians can concentrate and check the guide catheter irrigation. After the use of the GCIMD, there was no major thromboembolic complication in conjunction with the thrombotic occlusion of the guide catheter in our institute. Conclusion : We have developed a brilliant invention of the GCIMD for NET.

Sewage Treatment Using Natural Systems and Effluent Reuse for Crop Irrigation in Small Communities

  • A pilot study was performed from July 1998 to December 2002, including winter performance, to examine seasonal performance of a constructed wetland and subsequent pond system for treatment of sewage in small communities of Korea. Pond was operated as a intermittent-discharge pond during winter period, and continuous flow system during growing season; its effects was evaluated from December 2001 to April 2003. The subsurface flow (SSF) wetland was satisfactory for treating sewage with good removal efficiency even during the winter period. The wetland effluent concentrations of $BOD_5$ and TSS were often higher in winter than in the growing season, but this was explained by the higher loading rates, rather than lower removal efficiency. The relatively poor-quality wetland effluent was further polished during winter in the pond. The upper layer of the pond water column became remarkably clear immediately after ice melt. In the growing season, ponds could be operated as a continuous flow system to remove nutrients and pathogens, and the effluent of pond could be reused as a supplemental irrigation water without risk of infection by sewage-borne pathogens as well as causing adverse effect on growth and yield. Overall, the wetland system was found to be adequate for treating sewage with stable removal efficiency, and the intermittent-discharge pond was found to be effective for further polishing if necessary. Therefore, the combination of a wetland and subsequent pond system and reuse of effluent as crop irrigation water is recommended as a practical alternative to treat sewage in Korean small communities, and partial discharge of pond water in March is suggested.

Response of Nutrient Dynamics with Topography during the Rice Cultivation in Paddy Field

  • This study aimed to evaluate the nutrient load balance from rice paddy fields with different topographies, alluvial plain and local valley. Continuous monitoring from May to September, 2013 was conducted for water quantification and qualification from alluvial plain in Yeoju region (32 ha) and local valley in Jincheon region (24 ha). The discharge rates of T-N from the alluvial plain were 57.2, 5.84, 22.7, and $5.20kg\;ha^{-1}$ for irrigation, precipitation, drainage, and percolation, respectively. In case of local valley, T-N loads were 34.6, 4.73, 21.1, and $4.15kg\;ha^{-1}$ for irrigation, precipitation, drainage, and percolation, respectively. In contrary, the T-P loads from the alluvial plain were 2.23, 2.22, 2.54, and $0.41kg\;ha^{-1}$ for irrigation, precipitation, drainage, and percolation, respectively. In case of local valley, T-P loads were 1.44, 1.57, 1.82, and $0.34kg\;ha^{-1}$ for irrigation, precipitation, drainage, and percolation, respectively. The nutrient contents in drainage water were influenced by the amount of waters, rainfall, and surface drainage water. The Pearson correlation analysis showed that rainfall was significantly correlated with nutrient loads from July to August due to the amount of runoff in local valley paddy field, and irrigation was related with nutrient loads of drainage from July to August. This study showed that paddy rice farming in alluvial plain and local valley might be beneficial to water quality protection.

  • A pot experiment was conducted to find out the effect of water management on the growth and uptake of arsenic and inorganic nutrients of rice plant at As added soil. The arsenic were added to soil at the levels of As 0, 10, 50, 100 and 150 ppm, respectively. Water management was done with two ways: intermittent irrigation from ten days after transplanting, and continuous submersion until harvest. Higher soil As levels increased As content in plant but reduced growth rate. Aresenic content in plant was considerably reduced with intermittent irrigation compared to continuous submersion. Rice growth showed also same trend. With increasing As levels in soil, N content in plant was increased but P, K, Ca, Mg, $SiO_2$, Fe and Mn content in plant were tend to be decreased. These inorganic nutrients in plant were also much absorbed in continuous submersion compared to intermittent irrigation. Soil pH was slightly increased with increasing As levels in soil while soil Eh has no relationship with soil As levels. On the other hand, soil pH was higher in the treatment of continuous submersion than that of intermittent irrigation but soil Eh showed reverse trend. With increasing As levels in soil, water soluble-As and Ca-As fractions in soil tend to be increased with continuous submersion, but these fractions has no tendency with intermittant irrigation.

Effects of Irrigation and Ginseng Root Residue on Root Rot Disease of 2-Years-Old Ginseng and Soil Microbial Community in the Continuous Cropping Soil of Ginseng (인삼 연작토양에서 관수 및 인삼뿌리 잔사물이 토양 미생물상 및 뿌리썩음병 발생에 미치는 영향)

  • Background: Some phenolics detected in the soil may inhibit the seed germination and seedling growth of ginseng (Panax ginseng). This study investigated the effect of irrigation and ginseng root residue addition on the soil microbial community and root rot disease in 2-year-old ginseng. Methods and Results: Each $20{\ell}$ pot was filled with soil infected with ginseng root rot pathogens, and irrigated daily with $2{\ell}$ of water for one month. After the irrigation treatment, ginseng fine root powder was mixed with the irrigated soil at a rate of 20 g per pot. In descending order, ${NO_3}^-$, electric conductivity (EC), exchangeable Na (Ex. Na) and K (Ex. K) decreased due to irrigation. In descending order, ${NO_3}^-$, EC, Ex. K, and available $P_2O_5$ increased with the additon of ginseng powder to the soil. The abundance of Trichoderma crassum decreased with irrigation, but increased again with the incorporation of ginseng powder. The abundance of Haematonectria haematococca increased with irrigation, but decreased with the incorporation of ginseng powder. The abundance of Cylindrocarpon spp. and Fusarium spp., which cause ginseng root rot, increased with the incorporation of ginseng powder. The abundance of Arthrobacter oryzae and Streptomyces lavendulae increased with irrigation. The abundance of Streptomyces lavendulae decreased, and that of Arthrobacter spp. increased, with the incorporation of ginseng powder. Aerial growth of ginseng was promoted by irrigation, and ginseng root rot increased with the incorporation of ginseng powder. Conclusions: Ginseng root residues in the soil affected soil nutrients and microorganisms, and promoted ginseng root rot, but did not affect the aerial growth of ginseng.

Stereotactic Endoscopic Treatment of Brain Abscess Ruptured into Ventricle - Case Report - (뇌실로 파열되어 있는 뇌농양에 대한 뇌정위적 내시경하 제거술 - 증례보고 -)

  • The major indication of endoscope in neurosurgical field is intraventricular procedures. Recently, it can be used selectively in the intraaxial mass lesion associated cavity. For example, cystic mass, with liquefied necrosis, and blood clot can be approached with this technique. The authors present its usage in brain abscess ruptured into lateral ventricle. The neuroendoscope was introduced into abscess cavity through stereotactic guidance, the pus was then removed through continuous irrigation and suction under direct video visualization. The intraventricular pus was also cleaned through gentle, direct endoscopic irrigation and suction. The postoperative clinical course was uneventful. Brief overview is given for this intraaxial neuroendoscopic procedure.

Successful Surgical Treatments of Scleral Rupture with Hyphema in a Young Cat (어린 고양이에서 발생한 안구내출혈을 동반한 공막 파열의 수술적 치료)

  • A 3-month-old, female Persian cat was presented with sclera mass. The mass was detected after cat's squeals during the playing with a dog before 2 days. On ophthalmic examination, a $2{\times}5mm$ dark purple colored raised mass was observed 1 mm behind the limbus in the right eye. Anterior chamber was filled with blood and epithelial defect was detected at 2 o'clock in the right central cornea. After general anesthesia, resection of the protruded iris was performed with iris scissors and bipolar cautery. Ruptured sclera and conjunctiva were closed with simple interrupted and simple continuous suture, respectively. Also, irrigation and aspiration (I/A) of the anterior chamber was performed to remove blood clot with bimanual I/A handpiece in the right eye. Four months after the surgery, the right eye was recovered completely and menace response was positive in spite of the retinal lesions.
