Effects of Wheelchair Back Support and Ischial Pad on Neck, Trunk Angle and Chest Expansion in Stroke Patients (휠체어 허리 지지대와 궁둥 패드가 뇌졸중 환자의 목, 몸통 각도 및 가슴우리 확장에 미치는 영향)

  • The purpose of this study was to investigate the immediate effect of lumbar support and ischial pad on neck, trunk angle and chest expansion for stroke patients using wheelchair. Fifteen stroke patients were measured repeatedly when a lumbar support using(L support), a Ischial pad using(I Pad), a Lumbar support with ischial pad using(L With I), and non using it(Non using). The measurement of the neck and trunk angle was confirmed using a mobile phone camera, and chest expansion was performed using a tapeline. L With I increased significantly in neck and trunk angle and lower chest expansion than non using. This study shows that simultaneous use of lumbar support and ischial pad for stroke patients using wheelchair can increase the neck and trunk angle, chest expansion immediately. Future studies will need to identify more long-term changes by continuing intervention with more subjects.

A Study on Home Care and Home Visiting Nursing in Japan (일본의 재가간호 및 방문간호 -새로운 개호보험제도의 실시를 앞두고-)

  • Japan has been prepared for aging society from 1970. In 1970, the percentage of distribution of population of 65 years old and over was 7.1%. It is similar to present percentile of the elderly in Korea. Therefore, it will be needed to study about home care and home visiting nursing in Japan at present. This study was aimed to prepare the fundamental documents for home care nursing in Korea and to know the background of new health care system of Long-term Care Insurance in Japan, by studying home care and home visiting nursing in Japan. With the continuing aging of the population, especially the increase in the number of latter stage elderly, it is predicted that there will be an increase in the number of the elderly who are bedridden and suffering senile dementia. To ensure that these people will be able to continue living in the communities and homes they are accustomed to, surrounded by their families and neighbors, Japan substantially improve and expand in-home services. There were also long-term effort to reach the level of services outlined in the Gold Plan and the New Gold Plan within the decade between FY 1991 and FY 1999 in the field of health care and welfare. Under this plan, the most noticeable change was occurred in home care, home was permitted as the field of care and visiting nursing was established in law. Through this 10- Year Strategy for Promotion of Health and Welfare Services for the Aged, many problems have been improved and solved, but some problems remained such as inadequate service supply and consumption of medical insurance for the elderly. Japan will be a society composed 25% of elderly people of total population in 2020, and it will be soon faced with a shortage of welfare and medical facilities and manpower. As for equalizing the benefits and cost burdens, and other future arrangements for health care and welfare, Long-term Care insurance system was established in 1994. This system will be enforced from April 2000 and use present facilities and services. To know home care and home visiting nursing in Japan, we need to consider present conditions well and to take notice of changes and measures to cope with an aging society continuously.

Review of Communal Housing for the Elderly in the UK (영국의 노인공동생활주택에 대한 검토)

  • The purpose of this study was 1) to review communal housing in the UK, 2) to consider the policy implications for elderly communal housing in Korea. The research methods used were 1) literature review about communal housing and related policy in the UK 2) field survey in the UK 3) interpretative suggestion for the proper policy implication to develope communal housing for the elderly in Korea. Sheltered housing in the UK had been developed as communal housing for the elderly with special needs since the 1970s. The type of sheltered housing were category 1 and category 2. Very sheltered housing with more facilities and meal services was added in 1980s. Sheltered housing was evaluated as the most humanistic solution for older people in the UK in 1980s. Because of the policy of moving institutional care to community care, sheltered housing became less in demand because of more options for older people including being able to stay in their own home. So new completion of sheltered housing by registered social landlords reduced saliently. Sheltered housing already totalled over half million units in which 5% of all elderly over 65 still lived and a small quantity of private sector for sale schemes emerged in the 1990s. The reason why the residents moved to sheltered housing was for sociable, secure, and manageable living arrangements. In general the residents were satisfied with these characteristics but dissatisfied with the service charge and quality of meals, especially in category 2.5 schemes. The degree of utilisation of communal spaces and facilities depended on the wardens ability and enthusiasm. Evaluation of sheltered housing indicated several problems such as wardens duty as a \"good neighbour\" ; difficult-to-let problems with poor location or individual units of bedsittiing type with shared bathroom ; and the under use of communal spaces and facilities. Some ideas to solve these problems were suggested by researchers through expanding wardens duty as a professional, opening the scheme to the public, improving interior standards, and accepting non-elderly applicants who need support. Some researchers insisted continuing development of sheltered housing, but higher standards must be considered for the minority who want to live in communal living arrangement. Recently, enhanced sheltered housing with greater involvement of relatives and with tied up policy in registration and funding suggested as an alternative for residential care. In conclusion, the rights of choice for older people should be policy support for special needs housing. Elderly communal housing, especially a model similar to sheltered housing category 2 with at least 1 meal a day might be recommended for a Korean Model. For special needs housing development either for rent or for sale, participation of the public sector and long term and low interest financial support for the private sector must be developed in Korea. Providing a system for scheme managers to train and retrain must be encouraged. The professional ability of the scheme manager to plan and to deliver services might be the most important factor for the success of elderly communal housing projects in Korea. In addition the expansion of a public health care service, the development of leisure programs in Senior Citizens Centre, home helper both for the elderly in communal housing and the elderly in mainstream housing of the community as well. Providing of elderly communal housing through the modified general Construction Act rather than the present Elderly Welfare Act might be more helpful to encourage the access of general people in Korea. in Korea.

Increase in the Th1-Cell-Based Immune Response in Healthy Workers Exposed to Low-Dose Radiation - Immune System Status of Radiology Staff

  • Objectives: Radiation is one of the most important sources of free radical (such as reactive oxygen species) production, which plays an essential role in the etiology of over hundred diseases. The aim of the study was to investigate some immune parameters and hematological indices in healthy workers of the Radiology Department, University Hospital of Mashhad, Iran. Methods: The study was performed on 50 healthy workers: 30 radiology staff as the case group and 20 laboratory workers as the control group. The radiation dose received by the radiology staff participating in the study was less than the annual maximum permissible level, 50 millisievert. Hematological parameters, lymphocyte proliferation and cytokine production were studied in both groups. Results: Among healthy radiology workers, the hematological indices did not differ statistically; however, their proliferation indices and $IFN-{\gamma}$ levels showed significant increases in parallel with decreases in the IL-4 levels as compared to controls. The immune system of workers exposed to low-dose ionizing radiation was found to be shifted from a Type 2 to a Type 1 response to promote cellular immunity. Conclusion: Based on our data, exposure to low-dose ionizing radiation may decrease the prevalence, frequency, and recurrence of various cancers and infectious diseases because of an increase in Th1-cell-based response, thus leading to more protection of the human body against tumor cells and foreign agents and possibly increased longevity. However, due to high rate of fluoroscopy use for interventional radiology, we suggest continuing research projects on radiation protection and hazards to prevent irreversible damage. As a recommendation, in future studies, radiology staff with a weakened immunity due to high radiation exposure should be considered as good choices to be treated using acupuncture techniques because acupuncture has been demonstrated to enhance the function and the number of immune cells.

Comparison of the Estimated Result of Ecosystem Service Value Using Pixel-based and Object-based Analysis (화소 및 객체기반 분석기법을 활용한 생태계서비스 가치 추정 결과 비교)

  • Despite the continuing effort to estimate the value of function and services of ecosystem, most of the researches has used low and medium resolution satellite imagery such as MODIS or Landsat. It means that the researches to measure the ecosystem service value using VHR (Very High Resolution) satellite imagery have not been performed much, while the source of available VHR imagery is increasing. Thus, the aim of this study is to estimate and compare the result of ecosystem service value over Sejong city, S. Korea, which is one of the rapidly changed city, through the pixel-based and object-based classification analysis using VHR KOMPSAT-3 images, for more specific and precise information. In the result of the classification, forest and grassland were underestimated while agriculture and urban were overestimated in the pixel-based result compared to the object-based result. Furthermore, bare soil area was presented contrasting result that was increased in the pixel-based result, however, decreased in the object-based result. Using those results, ecosystem service values were estimated. The annual ecosystem service values in 2014 were $8.18 million USD(pixel-based) and $8.63 million USD(object-based), however, decreased to $7.80 million USD(pixel-based) and $8.62 million USD(object-based) in 2016. It is expected to use those results as a preliminary data when to make sustainable development plan and policy to improve the quality of life in the local level.

  • The concept of “precision agriculture” or “site-specific farming” is usually confined to the fields of soil science, crop science and agronomy. However, because plants grow in soil, animals eat plants, and humans eat animal products, it could be argued (perhaps with some poetic licence) that the fields of feed quality, animal nutrition and animal production should also be considered in this context. NIR spectroscopy has proved over the last 20 years that it can provide a firm foundation for quality measurement across all of these fields, and with the continuing developments in instrumentation, computer capacity and software, is now a major cog in the wheel of precision agriculture. There have been a few giant leaps and a lot of small steps in the impact of NIR on the animal world. These have not been confined to the amazing advances in hardware and software, although would not have occurred without them. Rapid testing of forages, grains and mixed feeds by NIR for nutritional value to livestock is now commonplace in commercial laboratories world-wide. This would never have been possible without the pioneering work done by the USDA NIR Forage Research Network in the 1980's, following the landmark paper of Norris et al. in 1976. The advent of calibration transfer between instruments, algorithms which utilize huge databases for calibration and prediction, and the ability to directly scan whole grains and fresh forages can also be considered as major steps, if not leaps. More adventurous NIR applications have emerged in animal nutrition, with emphasis on estimating the functional properties of feeds, such as in vivo digestibility, voluntary intake, protein degradability and in vitro assays to simulate starch digestion. The potential to monitor the diets of grazing animals by using faecal NIR spectra is also now being realized. NIR measurements on animal carcasses and even live animals have also been attempted, with varying degrees of success, The use of discriminant analysis in these fields is proving a useful tool. The latest giant leap is likely to be the advent of relatively low-cost, portable and ultra-fast diode array NIR instruments, which can be used “on-site” and also be fitted to forage or grain harvesters. The fodder and livestock industries are no longer satisfied with what we once thought was revolutionary: a 2-3 day laboratory turnaround for fred quality testing. This means that the instrument needs to be taken to the samples rather than vice versa. Considerable research is underway in this area, but the challenge of calibration transfer and maintenance of instrument networks of this type remains. The animal world is currently facing its biggest challenges ever; animal welfare, alleged effects of animal products on human health, environmental and economic issues are difficult enough, but the current calamities of BSE and foot and mouth disease are “the last straw” NIR will not of course solve all these problems, but is already proving useful in some of these areas and will continue to do so.

  • The use of composite restorative materials is established due to continuing improvements in the materials and restorative techniques. Composite resins are widely used for the restoration of cervical lesions because of esthetics, good physical properties and working time. There are several types of cavity design for class V composite resin filling, but inappropriate cavity form may affect bonding failure, microleakage and fracture during mastication. Cavity preparations for composite materials should be as conservative as possible. The extent of the preparation is usually determined by the size, shape, and location of the defect. The design of the cavity preparation to receive a composite restoration may vary depending on several factors. In this study, 5 types of class V cavity were prepared on each maxillary central incisor. The types are; 1) V-shape, 2) round(U) shape, 3) box form, 4) box form with incisal bevel and 5) box form with incisal bevel and grooves for axial line angles. After restoration, in order to observe the concentration of stress at bonding surfaces of teeth and restorations, developing a 2-dimensional finite element model of labiopalatal section in tooth, surrounding bone, periodontal ligament and gingiva, based on the measurements by Wheeler, loading force from direction of 45 degrees from lingual side near the incisal edge was applied. This study analysed Von Mises stress with SuperSap finite element analysis program(Algor Interactive System, Inc.). The results were as follows : 1. Stress concentration was prevalent at tooth-resin bonding surface of cervical side on each model. 2. In model 2 without line angle, stress was distributed evenly. 3. Preparing bevel eliminated stress concentration much or less at line angle. 4. Model with round-shape distributed stress concentration more evenly than box-type model with sharp line angle, therefore decreased possibility of fracture. 5. Adding grooves to line angles had no effect of decreasing stress concentration to the area.

A Non-Periodic Synchronization Algorithm using Address Field of Point-to-Point Protocol in CDMA Mobile Network (CDMA이동망에서 점대점 프로토콜의 주소영역을 이용한 비주기적 동기 알고리즘)

  • 동기식 스트림 암호통신 방식을 사용하는 암호통신에서는 암/복호화 과정 수행시 암호통신 과정에서 발생하는 사이클슬립으로 인해 키수열의 동기이탈 현상이 발생되고 이로 인해 오복호된 데이타를 얻게된다. 이러한 위험성을 감소하기 위한 방안으로 현재까지 암호문에 동기신호와 세션키를 주기적으로 삽입하여 동기를 이루는 주기적인 동기암호 통신방식을 사용하여 왔다. 본 논문에서는 CDMA(Cellular Division Multiple Access) 이동망에서 데이타서비스를 제공할 때 사용되는 점대점 프로토콜의 주소영역의 특성을 이용하여 단위 측정시간 동안 측정된 주소비트 정보와 플래그 패턴의 수신률을 이용하여 문턱 값보다 작은경우 동기신호와 세션키를 전송하는 비주기적인 동기방식을 사용하므로써 종래의 주기적인 동기방식으로 인한 전송효율성 저하와 주기적인 상이한 세션키 발생 및 다음 주기까지의 동기이탈 상태의 지속으로 인한 오류확산 등의 단점을 해결하였다. 제안된 알고리즘을 링크계층의 점대점 프로토콜(Point to Point Protocol)을 사용하는 CDMA 이동망에서 동기식 스트림 암호 통신방식에 적용시 동기이탈율 10-7의 환경에서 주기가 1sec인 주기적인 동기방식에서 요구되는 6.45x107비트에 비해 3.84x105비트가 소요됨으로써 전송율측면에서의 성능향상과 오복호율과 오복호 데이타 비트측면에서 성능향상을 얻었다. Abstract In the cipher system using the synchronous stream cipher system, encryption / decryption cause the synchronization loss (of key arrangement) by cycle slip, then it makes incorrect decrypted data. To lessen the risk, we have used a periodic synchronous cipher system which achieve synchronization at fixed timesteps by inserting synchronization signal and session key. In this paper, we solved the problem(fault) like the transfer efficiency drops by a periodic synchronous method, the periodic generations of different session key, and the incorrectness increases by continuing synchronization loss in next time step. They are achieved by the transfer of a non-periodic synchronous signal which carries synchronous signal and session key when it is less than the threshold value, analyzing the address field of point-to-point protocol, using the receiving rate of address bits information and flag patterns in the decision duration, in providing data services by CDMA mobile network. When the proposed algorithm is applied to the synchronous stream cipher system using point-to-point protocol, which is used data link level in CDMA mobile network, it has advanced the result in Rerror and Derror and in transmission rate, by the use of 3.84$\times$105bits, not 6.45$\times$107bits required in periodic synchronous method, having lsec time step, in slip rate 10-7.

Comparison of compost product quality with nature soil standard (국내생산퇴비의 부숙토 기준에 대한 적합성 검토)

  • As construction and expansion of wastewater treatment works is continuing, generation of sludge is increasing. Bur most sludge is not used appropriately but disposed by landfill dumping to sea. Furthermore direct landfilling of sludge cake containing moisture content over 75% has been legally prohibited since 2001 that's enforcement will be more strict in 2003. Such a situation means nowadays recycling of organic waste such as sludge and food waste is necessary. Composting is one of recycling methods commonly and used as an effective means of stabilizing organic waste and then compost can be used as fertilizer. However fertilizer law management which include standard of compost products and other fertilizers applied all sludge products indiscriminiately and was not flexible. So MOE has graded organic composts according to land applications to improve recycle of organic wastes. The classified organic compost which contains low contaminants has been possible to use as various purpose. This study enalyzed 30 samples which were raw materials for compost and compost products management well and to estimate the quality of compost products. Heavy metals were measured in Raw materials and OM/N, NaCl and VS were tested in compost products as well as heavy metals. As a result, approximate 10% of raw materials was not suitable to the grade A and 6.7% over the grade B of the regulation on raw materials for compost. In the case of 30 produced compost propducts approximate, 57% of composts was not compatible with the grade A and B of the regulation on composting product. The qualities of compost products were worse than raw materials, because the compost products have more regulation item raw materials have.

A Review of the Operation Community Health Practitioner System as a Reorientation of Primary Health Care (보건 진료원 제도 운영 평가에 관한 연구 -우리나라 1차 보건의료 제도 방향 재설정을 위하여 -)

  • In the changing social and economic conditions, reorientation of the health care system is a process of rearranging health care resources keeping in mind the appropriativeness, relevancy, and efficacy of health care programs. Also it has been recognized recently that the CHP program is in need of review for the same reasons, that is to say, the ease in which health care facilities are available, the high rate of coverage with insurance and the development of an effective transportation system. Therefore there is a social inclination to think that there are no remote areas and to question the roles of public health facilities, health centers, health sub centers and CHP posts. This paper was done to review problems and to propose new directions for the CHP system. The findings of this study are as follows ; 1) It is necessary that primary health care should be simplified into three parts, medical treatment, preventive care services and the organization of administration and logistics. Also each department should be supplemented with the appropriate professional personnel in order to develop a task oriented system. The reorientation of the CHP system should be managed in keeping with that of other public health care systems. Therefore it is necessary to look at the CHP system problems as one aspect of the reorientation process of public health care systems, and to work to find new ways to address these problems. 2) The location of the CHP post should be decided by the needs of the community in both the medical and preventive areas. If the people have a minimum need, the location of the CHP post should be altered and the existing roles of the CHP should be modified to allow for flexibility according to the community needs. 3) Use of the problem solving method in regular team meetings will prove to be as efficient as continuing education programs in improving job competancy. 4) The supervision of CHP's activities should be made by the same type professional personnel, that is, senior CHPs or charge nurses in the public health center at the county level. 5) The operational expensies of CHP post should be supported by the administrative department of the public health center and should create working conditions that will allow the CHP to concentrate on community health service programs. 6) The organizations for community participation, working committees, community health workers and a number of the local assembly, should be activated to provide for participation in finding solutions to health related problems in the com-munity.
