사상체질 처방 활용 현황 및 유효 질환 조사 (The Using Status and Effectiveness of Sasang Constitutional Herbal Prescription)

  • 1. Objectives: We aimed to derive using status of Sasang Constitutional herbal prescription and good effective one in specific disease. 2. Methods: We sent Questionnaire to 463 oriental medical doctor listed Korea Sasang Constitutional medicine from 2009 years. 10 to 11 and got back 191 questionnaire, which consisted of sex, age, clinical duration, working type, using status and effectiveness of Sasang Constitutional herbal prescription. 3. Results: - Digestive tract disease was especially most effective field, and circulatory, the endocrine, dermatology, psychopathy, urogenital disease went in order in western disease. - Spleen disease was especially most effective field, and the heart and circulatory, neuropsychiatry, lung, liver, psychopathy, gynecology disease went in order in oriental medicine pathology. - Chungshimyenja-tang was effective to neuropsychiatry and heart disease in Taeeumin, Hyangsayangwi-tang to disease related digestive system, Palmulgunja-tang to gynecology disease in Soeumin, Dockhalgiwhang-tang to diet disorder, Yanggeuksanwha-tang to heart and psychologic disease in Soyangin. 4. Conclusions: Constitutional prescription effected on special pathological symptom support weaken organ, and these result will help to make treatment standard and support clinical study of constitutional medicine.

  • 우리 헌법은, 국가로 하여금 국민의 건강을 보호할 의무를 지우고 있고, 이에 따라 구체화된 규범인 의료법은 의료기관 개설 등에 관한 사항을 상세히 규정하고 있는데, 그 내용 중 하나가 의료인의 의료기관 복수개설·운영 금지제도이다. 이에 대하여, 종래의 판례는 '다른 의사 명의로 추가 개설하는 의료기관에서 직접 의료행위 등을 하지 않는다면 여러 개의 의료기관을 개설·운영할 수 있다'라고 해석함으로써, 사실상 복수의 의료기관을 개설·운영할 수 있었다. 하지만 일부 의료인들이 다른 의료인의 면허로 의료기관을 여러 장소에 개설하고 이익을 극대화하기 위하여, 환자유인행위를 하거나 과잉진료 및 위임치료를 하는 등의 불법의료행위를 조장할 뿐만 아니라 국민의 건강권 등을 침해하는 현실적 문제가 발생하게 되었다. 이에 입법자는 의료법의 개정을 통해 의료인은 어떠한 명목으로도 둘 이상의 의료기관을 개설하거나 운영할 수 없도록 의료기관 개설제도를 정비하게 되었다. 이와 같은 개정 의료법 제33조 제8항이 위헌인지 여부에 대하여, 헌법소원과 위헌법률심판 제청이 되어 헌법재판소에서 오랜 기간 심층심리 끝에 최근 합헌결정이 선고되었다. 헌법재판소는, 보건의료는 상거래의 대상이 되어서는 아니 되고, 공공의료기관의 비중, 영리목적 환자유인, 과잉진료 등을 방지하기 위한 점을 감안하여 '목적의 정당성'을 인정하였다. 또한 의료인이 외부 자본에 종속될 우려가 있는 점, 의료기관 개설 명의인과 실제 운영자가 분리되는 것은 우려스러운 점, 인간의 신체와 생명이 수단이 되어서는 아니 된다는 점, 현재의 의료체계상 과잉진료 확인이 불가한 점 등을 고려해 볼 때 '침해의 최소성'도 인정하였다. 나아가 '법익 균형성' 등 기본권 제한의 원칙인 과잉금지원칙을 준수하여 합헌이라고 판단하였다. 이와 같이 헌법재판소가 우려하고 있는 영리추구, 과잉진료를 현실적으로 방지하기 위해서는 입법적 보완이 필요하다. 이와 관련하여 소비자단체는 입법의 필요성에 적극 찬성하고 있고, 보건의료 공급자 단체 또한 입법의 필요성에 공감하고 있다. 따라서 입법자는 헌법재판소의 이번 결정을 존중하고, 국민들의 입장을 반영하여 빠른 시일 내에 보완입법을 마련하길 기대한다.

성인 영양지수(Nutrition Quotient)를 이용한 사상체질에 따른 식습관 평가: 대전시민건강코호트 (Evaluation of Dietary Behaviors According to Sasang Constitution Using a Nutrition Quotient: A Korean Medicine Daejeon Citizen Cohort Study)

  • Objectives Meal and nutrition are important lifestyle factors that affect maintenance and promotion of health status and are also associated with chronic diseases. In Sasang constitutional medicine, dietary behavior according to the characteristics of Sasang constitution (SC) types is a key component in health management. The aim of this study was to evaluate the nutritional status and quality of meals according to the SC. Methods This was a cross-sectional study of 1,947 subjects surveyed in a community-based cohort called the Korean Medicine Daejeon Citizen Cohort study. Data were collected including demographic characteristics, Korea Sasang Constitutional Diagnostic Questionnaire, and nutrition quotient (NQ) for adults. We analyzed the differences in NQ grade and scores among three SC types: Taeeumin (TE), Soeumin (SE), and Soyangin (SY), by conducting chi-square and ANCOVA tests. Results The distribution of SC into TE, SE, and SY types was 43.1%, 25.2%, and 31.8%, respectively. The mean NQ score of all subjects was 53.4±8.6. There was a significant difference in the distribution of NQ grades and mean of NQ scores among SC types, including total and four sub-factors: balance, diversity, moderation, and dietary behavior. The TE type had the lowest score for the moderation factors in the crude model. The SE type showed lower NQ scores than the TE and SY types in all domains except moderation after adjusting for variables. Conclusions We found that dietary behaviors had to be distinct according to the SC types. These results elucidate why it is important to develop a customized dietary habit management program considering the SC types.

체질 의료 서비스 시장규모 및 현황 조사 -전국 한방의료기관을 중심으로- (The Survey on the State and Scale of Constitutional Medical Service Market in Korea)

  • Objectives The aim of this study is to survey the present scales of constitutional medical service market on the base of Korean medical service market following 2006, 2008 year survey. Methods This survey was conducted from Nov. 1 to Dec. 15, 2011 by offline mail. A questionnaire including personal information, general information, and management related to constitutional medicine of Korean medical institution was sent to each subject with a returned envelope. The subjects were asked to complete the questionnaire and 108 data were collected and analyzed. Results This study revealed that the 62% facilities of all Korean medical clinics in Korea are using constitutional medicine for patients. The proportion of constitutional medical service was 13.7% in Korean medical care insurance and 23.5% in total Korean medical service market. Conclusion It is assumed that the scales of total Korean medical service market seemed to be 3,974,425 million won and the scales of total constitutional medical service market seemed to be 929,404 million won.

사상체질과 음식 혈청 IgG 반응의 상관관계 분석 (Correlation between Sasang Institution and Reaction of Food IgG Serum)

  • Objectives The purpose of this study was to find correlation between food IgG serum and food classification for Sasangin match. Methods We recruited 10 Soeumin, 10 Soyangin, 10 Taeumin. We did a survey about food intake and obtained their blood samples. We detected IgG antigen reaction of 66 different foods using IgG exclusive Microarray assay. Results We found that IgG value of foods were correlated with Sasangin. Pork, shrimp and black tea has showed significant differences by constitution. The IgG response of the food according to the constitution and frequency of intake was found to be significantly correlated with those of banana, chestnut, citrus, milk, mushroom, cucumber, barley, corn, pineapple, honey and abalone. Conclusions The result of this study was partially matched to food classification for Sasangin.

대전시 여성과 베트남 이주여성의 단축형 사상체질진단 설문지(KS-15) 응답 비교 (A Comparative Study of Response of KS-15 Questionnaire between Migrant Vietnam and Daejeon Women)

  • Objectives The aim of this study was to reveal the difference of body shape, personality, physiological characteristics between migrant Vietnam and Daejeon women using propensity matching. Methods The number of 274 Vietnamese migrant women and Daejeon city women participate in this study. We surveyed Sasang Constitution (SC) expressive factor, such Body Mass Index (BMI) using Korea Sasang Constitutional Diagnostic Questionnaire (KS-15). A Chi-square test and a T-test were used. Significant p was .05. Results The height, weight and BMI of Daejeon women was bigger than those of Vietnam(p<.001). There was significant difference in personality characteristics in 'broad mind-narrow mind'(p<.001), 'Active-Passive'(p<.001), 'Masculine-Feminine'(p=.002). There was significant differentce in physio-pathological symptom in 'digestion'(p<.001), 'urine time'(p<.001), 'feeling cold/heat'(p=.006). There was significant differentce in distribution of SC between Vietnam and Daejeon women(p=.025). Conclusions This study reveals that there is differentce in body shape, personality, physiological characteristics between Vietnam and Daejeon women. These factors might influence on SC distribution between Vietnam and Dajeon women

사상체질에 따른 식습관 행태와 건강관련 삶의 질과의 연관성 (Association of Breakfast, Eating Frequency, and Energy Intake with Health-related Quality of Life According to Sasang Constitution: in Korean General Population)

  • Background According to the Sasang constitutional medicine, eating habits according to the characteristics of Sasang Constitution (SC) types affect management of chronic diseases and health conditions. The aim of this study is to identify the association between eating habits, such as breakfast, eating frequency, and energy intake, according to SC, and quality of life. Method This study is a cross-sectional study of people aged 33 to 55 years, sampled by stratification based on population distribution. The SC types were classified into Taeeumin (TE), Soeumin (SE), and Soyangin (SY) using a structured questionnaire (KS-15). Items examined regarding eating habits were whether the subjects had breakfast; the frequency of eating such as breakfast, lunch, dinner, and after-dinner snacks; and energy intake surveyed using a food frequency questionnaire. As for quality of life, SF-12, which is widely used worldwide to assess health conditions, was utilized. A regression analysis was conducted on the relations between eating habits according to SC and quality of life. Results The SC distribution of the 3,895 subjects was 47% for TE, 22% for SE, and 31% for SY. The scores indicating quality of life differed for TE and SY depending on whether they ate breakfast and eating frequency groups, and those of SE varied depending on levels of energy intake. A regression analysis having adjusted all covariates demonstrated for TE a positive relation between the eating frequency of three times per day or more (compared to 1.5 to 3 meals per day) and quality of life (B=1.365, p<0.001). For SE, there was a negative association between low energy intake (compared to adequate energy intake) and quality of life (B=-1.642, p=0.004). Meanwhile, no relation was found between eating habits and quality of life for SY. Conclusion This study identified the association between eating habits according to SC types and quality of life. For TE, it was found to be important to eat evenly dispersed adequate amounts as opposed to overeating or binging at once. For SE, adequate energy intake affected good quality of life. This study suggests that eating habits based on SC improve quality of life, and health management customized for each constitution should be performed in the future through proper eating habits.

우리나라 성인의 사상체질에 따른 식생활 상태와 신체구성의 상관성에 관한 단면분석연구 (A relationship of healthy eating status and body composition in Korean adults by their constitution type: A cross-sectional study)

  • Background Adherence to healthy diet acts as a key role to modify sedentary lifestyle in real life setting. Constitution type of traditional Korean medicine has been prediagnosed to risk factors of cardiometabolic diseases. This study aims to evaluate the associations between body composition and healthy eating status in Korean adults by their constitution type. Methods Of 4046 participants from Korean Medicine Daejeon Citizen Cohort study, Korean Medicine Daejeon Citizen Cohort (KDCC) study were included for analysis. Demographic, health-related behaviors and Korean Medicine (KM) type were surveyed based on a general health-related questionnaire. Anthropometric measurements and dietary factors by using Korean Healthy Eating Index (KHEI) were assessed only in the half of the original participants. Results 50.8% of Taeeum was observed from eligible 1967 participants (66.7%, women). The highest KHEI score was observed in soyang group (52.0±0.3, p=0.006) compared to other two groups. In taeeum group, lower appendicular skeletal muscle mass (ASM)(%)(Taeeum: 38.7±0.1 vs. Soeum/Soyang: 39.2±0.1, p < 0.05), and higher percent of body fat (PBF)(%) (Taeeum: 32.9±0.2 vs. Soeum/Soyang: 29.0±0.2, p < 0.05) by the lowest tertile (T1) of the KHEI score, respectively. When the KHEI score increased by 1 point in taeeum group, a positive relationship was observed, which increased by 0.015% of ASM, however, a negatively related to in which body fat mass (BFM) decreased by 0.022 kg and PBF decreased by 0.024%. Conclusion Customized nutritional management is required that could help maintaining physical health and diet by their constitution type.

  • The problem lies in 'Sasang Constitution Medicine' is a subjectiveness of diagnosis that the result of diagnosis varies according to the doctors. In order to supplement to this demerit, multiple approach has done to diagnose objectively. This study has focused on evaluating the effectiveness and accuracy of the questionnaire which considered as a basic data to identify constitution By analysing the tendency of the respondents who has defined constitution by clinical diagnosis and comparing of their answers, the result of their constitution analysis by our questionnaire were re-examined. The answer of each question to each constitution were tested how it is relevant to a scale of a constitution. As a result, we have found that there is limitation to find out constitution by the questionnaire we used, one we hope that suitable questionnaire will be developed for the research in the future

피구금자에 대한 권리보호적 측면에서의 도서관봉사

  • Persons who are waiting for decision of the court concerning whether they are guilty or not after they are involved in the criminal case and detained in the special institution isolated from society, will desire to de found innocent and acquited or to be slightly punished. Inmates are the suspected persons and the accused persons who are detained in the correctional institution. They have the right to de assisted by lawyes in order to receive the favorable verdict in the court. However, the right of the poor and the ignorant, in reality, cannot be perfectly protected due to the imperfection and defect of the defense counsel system itself and its application. Therefore, as a means to guarantee the so-called access to the court, the fundamental constitutional right, the law libraries are established and the legal information services are provided to the inmates within the correctional institution in the advanced country such as the United States. In addition, the judicial precedent and the various kinds of professional organizations provide the managerial guide-lines for such libraries to enoughly collect materials and to provide the effective information services to the inmates. In order to furnish the management of the correctional institution of Korea with useful information, the legal information services, materials collected, and information service personnel of the law libraries within the correctional institution are minutely examined in this paper.
