급성골수성백혈병환자의 항암화학요법 주기내의 호중구감소증과 영양상태 (Neutropenia & Nutritional status during Chemotherapeutic cycle in Acute Myeloid Leukemia)

  • 본 연구는 급성골수성백혈병의 우선적인 치료방법인 항암화학치료제 투여 후 발생되는 호중구 감소기간을 잘 관리하기 위하여 호중구감소증 정도와 영양상태와의 관련성을 규명한 서술적 조사연구이다. 본 연구는 B시 P대학병원에서 1차 강화항암화학치료까지 받고 회복한 성인환자 54명의 환자의 의무기록지를 열람하여 조사하였다. 연구결과, 항암화학요법 주기내의 호중구감소증이 지속되는 기간은 6일에서 28일(평균 14.78일)이었으며, 호중구감소증이 시작되는 시기는 5일째에서 15일째 사이(평균 9.54일)에 나타났으며, 최저백혈구수의 시기는 평균 18.41일째였으며, 27.8%에서 19일째 가장 적은 백혈구수를 보였다. 항암요법 주기내의 항암화학요법 시작시와 절대호중구수가 가장 낮은 시기의 총단백질량, 체중, 체질량지수와 같은 영양상태 변화는 거의 없었으며, 알부민, 콜레스테롤은 유의하게 감소하였다. 대상자의 일반적인 특성에 따른 호중구감소증은 성별, 나이, 동반질환유무, 체표면적에 따른 차이가 없었다. 호중구감소증과 영양상태간의 상관관계는 없었으나, 급성골수성백혈병 환자의 호중구감소 기간 동안의 영양상태에 대한 정보는 제공해 줄 수 있을 것으로 기대된다.

Short-course versus long-course neoadjuvant chemoradiotherapy in patients with rectal cancer: preliminary results of a randomized controlled trial

  • Purpose: Colorectal cancer is becoming an increasing concern in the middle-aged population of Iran. This study aimed to compare the preliminary results of short-course and long-course neoadjuvant chemoradiotherapy treatment for rectal cancer patients. Materials and Methods: In this clinical trial we recruited patients with rectal adenocarcinoma located from 5 cm to 15 cm above the anal verge. Patients in group I (short-course) received three-dimensional conformational radiotherapy with a dose of 25 Gy/5 fractions in 1 week plus concurrent XELOX regimen (capecitabine 625 mg/㎡ from day 1-5 twice daily and oxaliplatin 50 mg/㎡ on day 1 once daily). Patients in group II (long-course) received a total dose of 50-50.4 Gy/25-28 fractions for 5 to 5.5 weeks plus capecitabine 825 mg/㎡ twice daily. Both groups underwent consolidation chemotherapy followed by delayed surgery at least 8 weeks after radiotherapy completion. The pathological response was assessed with tumor regression grade. Results: In this preliminary report on complications and pathological response, 66 patients were randomized into two study groups. Mean duration of radiotherapy in the group II (long-course) was 5 ± 1 days (range, 5 to 8 days) and 38 ± 6 days (range, 30 to 58 days). The median follow-up was 18 months. Pathological complete response was achieved in 32.3% and 23.1% of patients in the shortcourse and long-course groups, respectively (p = 0.558). Overall, acute grade 3 or higher treatment-related toxicities occurred in 24.2% and 22.2% of patients in group I and II, respectively (p = 0.551). No acute grade 4 or 5 adverse events were observed in either group except one grade 4 hematologic toxicity that was seen in group II. Within one month of surgery, no significant difference was seen regarding grade ≥3 postoperative complications (p = 0.333). Conclusion: For patients with rectal cancer located at least 5 cm above the anal verge, short-course radiotherapy with concurrent and consolidation chemotherapy and delayed surgery is not different in terms of acute toxicity, postoperative morbidity, complete resection, and pathological response compared to long-course chemoradiotherapy.

Prognostic Factors in Stage III Non-Small-Cell Lung Cancer Patients

  • Aim: The objective of this study is to investigate prognostic factors affecting survival of patients undergoing concurrent or sequential chemoradiotherapy (CRT) for stage III non-small-cell lung cancer (NSCL). Methods and materials: We retrospectively reviewed the clinical records of 148 patients with advanced, inoperable stage III NSCLC, who were treated between 2007 and 2015. Results: The median survival was found to be 19 months and 3-year overall survival was 27%. Age (<65 vs ${\geq}65years$, p=0.026), stage (IIIA vs IIIB, p=0.033), dose of radiotherapy (RT) (<60 vs ${\geq}60Gy$, p=0.024) and treatment method (sequential chemotherapy+RT vs concurrent CRT, p=0.023) were found to be factors affecting survival in univariate analyses. Gender, histological subtype, weight loss during CRT, performance status, induction/consolidation chemotherapy and presence of comorbidities did not affect survival (p>0.050). Conclusion: Young age, stage IIIA, radiotherapy dose and concurrent chemoradiotherapy may positively affect survival in stage III NSCL cases.

Preoperative short course radiotherapy with concurrent and consolidation chemotherapies followed by delayed surgery in locally advanced rectal cancer: preliminary results

  • Purpose: This study aimed to assess complications and outcomes of a new approach, that is, combining short course radiotherapy (SRT), concurrent and consolidative chemotherapies, and delayed surgery. Materials and Methods: In this single arm phase II prospective clinical trial, patients with T3-4 or N+ M0 rectal adenocarcinoma were enrolled. Patients who received induction chemotherapy or previous pelvic radiotherapy were excluded. Study protocol consisted of three-dimensional conformal SRT (25 Gy in 5 fractions in 1 week) with concurrent and consolidation chemotherapies including capecitabine and oxaliplatin. Total mesorectal excision was done at least 8 weeks after the last fraction of radiotherapy. Primary outcome was complete pathologic response and secondary outcomes were treatment related complications. Results: Thirty-three patients completed the planned preoperative chemoradiation and 26 of them underwent surgery (24 low anterior resection and 2 abdominoperineal resection). Acute proctitis grades 2 and 3 were seen in 11 (33.3%) and 7 (21.2%) patients, respectively. There were no grades 3 and 4 subacute hematologic and non-hematologic (genitourinary and peripheral neuropathy) toxicities and perioperative morbidities such as anastomose leakage. Grade 2 or higher late toxicities were observed among 29.6% of the patients. Complete pathologic response was achieved in 8 (30.8%) patients who underwent surgery. The 3-year overall survival and local control rates were 65% and 94%, respectively. Conclusion: This study showed that SRT combined with concurrent and consolidation chemotherapies followed by delayed surgery is not only feasible and tolerable without significant toxicity but also, associated with promising complete pathologic response rates.

  • A 60-year-old man was diagnosed with stage IV squamous cell carcinoma of lung and treated with weekly doses of docetaxel and cisplatin. Tumor mass and mediastinal lymphadenopathy disappeared after 4.5 cycles of chemotherapy. At one week post final chemotherapy, the patients developed sudden shortness of breath. New, multifocal infiltrations developed on both lungs without definitive evidence of infection. Despite administration of broad spectrum antibiotics, the lung lesion did not improve, so bronchoalveolar lavage and computed tomography-guided lung biopsy were performed. The proportion of lymphocytes was increased markedly and histopathology revealed squamous cell carcinoma combined with bronchiolitis obliterans organizing pneumonia. After high dose corticosteroid therapy, dyspnea and the newly developed consolidation had decreased slightly. However, dyspnea and hypoxemia increased again because of aggravated lung cancer since chemotherapy had stopped. Chemotherapy couldn't be restarted due to the poor performance status of the patient. Later, patient died of respiratory failure from poor general condition and progression of lung cancer.

Complete Remission of Unresectable Esophageal Cancer Achieved with Concurrent Chemoradiotherapy: A Case Report

  • A 41-year old woman with dysphagia visited, which was aggravated after eating. On physical examination, there was a palpable mass on the left supraclavicular area. Endoscopic examination revealed a mass on the distal esophagus with irregular mucosa, erythema and a whitish plaque with luminal narrowing. The patient was diagnosed with unresectable esophageal cancer (squamous cell carcinoma, T3N2M1, Stage IV). The patient received CCRT (total 63 Gy) with cisplatin and 5-fluorouracil (5-FU). After CCRT, the patient took an additionally 2 cycles of chemotherapy for consolidation (cisplatin and 5-FU every 4 weeks). After additional chemotherapy, endoscopic examination showed no residual tumor, a chest CT scan revealed that the mass in the distal esophagus had decreased and there was no enlargement of the lymph nodes around the left supraclavicular area. The patient has been in complete remission for 5 years.

Radiotherapy as an effective treatment modality for follicular lymphoma: a single institution experience

  • Purpose: Follicular lymphoma (FL) is an indolent non-Hodgkin's lymphoma that is highly sensitive to radiotherapy (RT). However, the effectiveness of RT has not been well established. We reviewed our experiences to assess the role of RT for FL and analyze treatment results. Materials and Methods: Retrospective analysis was done on 29 patients who received first RT between January 2003 and August 2013. Of 23 early stage (stage I, II) patients, 16 received RT alone, four received chemotherapy followed by RT, two received RT postoperatively, and one received salvage RT for relapse after resection. Six advanced-stage (stage III, IV) patients received RT after chemotherapy: two received consolidation RT, three received salvage RT for residual lesions, and one received RT for progressive sites. Median RT dose was 30.6 Gy (range, 21.6 to 48.6 Gy). Median follow-up duration was 62 months (range, 6 to 141 months). Results: All patients showed complete response in the radiation field. Eight outfield relapses were reported. Seven patients received salvage treatment (three chemotherapy, four RT). Four patients showed excellent responses, especially to RT. Estimated 5-year and 10-year relapse-free survivals were 72% and 60%. In the RT-alone group, 5-year relapse-free survival was 74.5%. All advanced-stage patients were disease-free with 100% 5-year overall survival. Disease-specific death was noted in only one patient; four others died of other unrelated causes. No significant toxicity was reported. Conclusion: RT resulted in excellent treatment outcomes for all FL stages when used as a primary treatment modality for early stage or salvage-treatment modality for advanced-stage disease.

침윤성 방광암 환자에서 방광 보존 치료 (Combined Modality Therapy with Selective Bladder Preservation for Muscle Invading Bladder Cancer)

  • 목적 : 침윤성 방광암 환자를 대상으로 TURBT (Transurethral Resection of Bladder Tumor)과 유도 항암화학요법(neoadjuvant MCV chemotherapy) 후 방사선 항암화학 병용요법(cisplatin chemotherapy and radiotherapy)을 받은 환자에서 치료의 적응도, 국소 반응율, 방광 보존율과 생존율을 알아보고자 하였다. 대상 및 방법 : 1990년 10월부터 1998년 6월까지 TURBT 시행 후 병리 조직검사 상 침윤성 방광 이행상피암으로 진단 받은 병기 T2-T4, Grade II 환자 21명을 대상으로 하였다. 치료과정은 TURBT 후 4주 간격으로 2회의 유도 MCV(methotrexate, cisplatin, vinblastine) 복합 항암화학요법을 시행하고 그 후 cisplatin과 방사선 동시 병용요법을 시행하였다. 방사선치료는 $6\~15$ MV X-ray를 사용하여 하루 1.8 Gy씩 $39.6\~45\;Gy$를 전 골반에 $4\~5$주간에 걸쳐 조사하였다. 치료 종료 후 방광내시경을 통해 관해율을 판정하여 완전 관해인 경우 $1\~2$주 후 보강 방사선조사를 cisplatin과 병용하여 19.8 Gy를 시행하였다. 부분 관해인 경우 근치적 방광 절제술을 예정하였고 수술이 불가능한 경우에는 항암제 병용 강화 방사선치료를 받도록 하였다. 환자의 추적관찰기간은 $34\~67$개월로 중앙값이 49.5개월이었다. 결과 : 총 26명의 환자가 유도 항암화학요법을 위해 등록되었으며 유도 항암화학요법과 방사선 항암화학 병용요법을 완료한 환자는 21명$(81\%)$이었다. 유도 항암화학요법과 방사선 항암화학 병용요법 후 방광내시경을 통해 완전 관해 판정을 받은 환자는 21명 중 7명$(33\%)$이었고 부분 관해 환자는 14명$(67\%)$이었다. 완전 관해 환자 중 강화 항암제 병용 방사선 19.8 Gy를 받은 환자는 7명 중 6명$(85\%)$이었고, 이들 중 5명$(14\%)$은 재발 없이 방광을 보존하였다. 부분관해 환자중 1명이 근치적 방광적출술을 받았으며 3명이 TURBT와 2회의 MCV 항암요법을, 10명이 추가의 항암화학 방사선 병용요법을 받았다. 근치적 방광적출술을 받은 1명은 수술 소견상 방광 내 종양이 존재했으나 수술 후 재발이 없었다. 전체 21명의 환자중 12명$(58\%)$이 방광을 보존하면서 생존하고 있고 8명이 방광암으로 사망하였으며 1명이 다른 질병으로 사망하였다. 전체 환자의 중앙 생존기간은 27개월이며, 5년 생존율은 $55\%$ 였다. 유도 항암화학요법과 항암제 방사선치료 후 완전 관해 환자의 경우 5년 생존율은 $80\%$, 부분 관해 환자 $14\%$로 의미 있는 차이를 보였다(p=0.001). 유도 항암화학요법과 항암제 병용 방사선치료를 받은 환자에서 grade 3 이상의 혈액학적 부작용의 빈도는 각각 백혈구 감소증 6명$(29\%)$, 혈소판 감소증 1명$(4\%)$이었으며 이로 인한 화학요법의 지연은 1명$(4\%)$에서 관찰되었다. 결론 : 침윤성 방광암 환자에서 유도 항암화학요법 및 항암제 병용 방사선치료를 적용하여 방광 보존을 및 5년 생존율에 있어 만족할 만한 결과를 얻었다. 완전한 TURBT와 유도 MCV 항암요법과 cisplatin 방사선 병용요법에서 완전관해 여부가 방광보존가능성과 생존율에 영향을 미치는 요소로 생각되고 만약 완전관해를 이루지 못하면 근치적 방광적출술을 시행하는 것이 바람직할 것이다.

호중구 감소된 급성 림프구성 백혈병환아에서 발생한 Pneumocystis carinii 폐렴 1례 (A Case of Pneumocystis carinii Pneumonia with Febrile Neutropenia in Acute Lymphoblastic Leukemia)

  • Pneumocystis carinii pneumonia mainly occurs in immunocompromised patients and it is also known of major cause of death in children with acute lymphoblastic leukemia. After consolidation chemotherapy, acute lymphoblastic leukemia children is developed Pneumocystis carinii pneumonia frequently no an opportunistic infection but there were no controlled studies which have been performed to evaluate the usefulness of corticosteroid in Pneumocystis carinii pneumonia with acute lymphoblastic leukemia. We experienced a case of Pneumocystis carinii pneumonia in acute lymphoblastic leukemia with febrile neutropenic 6 years old girl. She was treated with trimethoprim-sulfamethoxazole and prednisone. We report this case with brief review of related literature.
