Cone beam computed tomography findings of ectopic mandibular third molar in the mandibular condyle: report of a case

  • Impaction of third molar is a common developmental abnormality. However, ectopic impaction of the mandibular third molar in condylar region is an extremely rare condition. This report describes a case of impacted tooth in the mandibular condyle without any associated pathologic condition. Also, this report presents the spatial relationship of the impacted mandibular third molar to the surrounding anatomic structures using cone beam computed tomography.

Foramen transversarium enlargement caused by vertebral artery tortuosity: Diagnosis with cone-beam computed tomography and magnetic resonance angiography

  • A markedly enlarged foramen transversarium was discovered incidentally on a cone-beam computed tomography scan of a 72-year-old male patient who was referred for dental implant placement. Further evaluation with magnetic resonance angiography revealed that the foramen enlargement was caused by a tortuosity in the course of the vertebral artery. This case report highlights the importance of recognizing significant incidental findings on diagnostic images and the potential need for additional imaging as part of the complete interpretative process.

  • The tympanic plate is a small part of the temporal bone that separates the mandibular condyle from the external auditory canal. Fracture of this small plate is rare and usually associated with other bony fractures, mainly temporal and mandibular bone. There is a limited amount of literature on this subject, which increases the chance of cases being overlooked by physicians and radiologists. This is further supported by purely isolated cases of tympanic plate fracture without evidence of other bony fractures. Cone-beam computed tomography is an investigative three-dimensional imaging modality that can be used to detect fine structures and fractures in maxillofacial trauma. This article presents four cases of isolated tympanic plate fracture diagnosed by cone-beam computed tomography with no evidence of fracture involving other bones and review of the literature.

Multiple intraosseous cervical pneumatocysts: A case report of a rare incidental finding on cone-beam computed tomography

  • This report presents a case of cervical pneumatocysts as an incidental finding on cone-beam computed tomography. Pneumatocysts are gas-containing lesions of unknown etiology. They usually present in the ilium or sacrum, adjacent to the sacroiliac joint. In the literature, 21 case reports have described cervical pneumatocysts. Cervical pneumatocysts should be differentiated from other lesions, such as osteomyelitis, osteonecrosis, and neoplasms, as well as post-traumatic and post-surgical cases. Computed tomography, cone-beam computed tomography, and magnetic resonance imaging are appropriate tools to diagnose cervical pneumatocysts.

  • The use of computed tomography for dental procedures has increased recently. Cone beam computed tomography (CBCT) systems have been designed for imaging hard tissues of the dentomaxillofacial region. CBCT is capable of providing high resolution in images of high diagnostic quality. This technology allows for 3-dimensional representation of the dentomaxillofacial skeleton with minimal distortion, but at lower equipment cost, simpler image acquisition and lower patient dose. Because this technology produces images with isotropic sub-millimeter spatial resolution, it is ideally suited for dedicated dentomaxillofacial imaging. In this paper, we provide a brief overview of cone beam scanning technology and compare it with the fan beam scanning used in conventional CT and the basic principles of currently available CBCT systems.

  • In general, the patients of the head and neck cancer are treated with 4MV photon beam up to prescribed dose, but spinal cord should be excluded in the treatment field. When its absorbed dose is limited at the tolerance dose. In case of the patients who has the positive posterior neck nodes need a boost electron beam treatment to the prescribed dose. In that case, the anatomical structure of the neck and the physical structure of the standard electron cone interrupt to allow proper access to the disease site. Therefore, we extended treatment SSD for the remove of the those hindrances. In this study, we evaluated the dosimetric variation of the standard electron cone for the extended SSD, from 100cm to 120cm, 5 cm increment, and compare to the custom-made electron cone. As a result, the $\%$ depth dose, the point of maximum dose and the range of maximum were changed within the $2\%$. The penumbra width was increased from 1.0cm to 2.0cm. However, the dosimetric characteristics of the custom-made electron cone was very similar to that of the 100cm SSD standard electron cone and due to its characteristic of physical structure, patients didn't need re-positioning after photon beam treatment, therefore accurate treatment was possible, we conclude that the custom-made electron cone was very useful for the clinical practice.

  • A true three-dimensional cone-beam reconstruction (TTCR) algorithm for the complete sphere geometry is derived, which is applicable to the direct volume image reconstruction from 2-D cone-beam projections. The algorithm is based on the modified filtered backprojection technique which uses a set of 2-D space-invariant filters and is derived from the previously developed parallel-beam true three-dimensional reconstruction(TTR) algorithm. The proposed algorithm proved to be superior in spatial resolution compared with the parallel-beam TTR algorithm.

Cone beam CT와 일반 CT의 흡수선량 및 유효선량 비교평가 (Comparison of cone beam CT and conventional CT in absorbed and effective dose)

  • Purpose: This study provides comparative measurements of absorbed and effective doses for newly developed cone beam computed tomography (CT) in comparison with these doses for conventional CT. Materials and Methods: Thermoluminescent dosimeter rods (TLD rod: GR-200, Thermo Fisher Scientific Inc., Waltham, MA, USA) were placed at 25 sites throughout the layers of Male ART Head and Neck Phantom (Radiology Support Devices Inc., Long Beach, USA) for dosimetry. Implagraphy, DCT Pro (Vatech Co., Hwasung, Korea) units, SCT-6800TXL (Shimadzu Corp., Kyoto, Japan), and Crane x 3+(Soredex Orion Corp., Helsinki, Finland) were used for radiation exposures. Absorption doses were measured with Harshaw 3500TLD reader (Thermo Fisher Scientific Inc., Waltham, MA, USA). Radiation weighted doses and effective doses were measured and calculated by 2005 ICRP tissue weighting factors. Results: Absorbed doses in Rt. submandibular gland were 110.57 mGy for SCT 6800TXL (Implant), 24.56 mGy for SCT 6800TXL (3D), 22.39 mGy for Implagraphy 3, 7.19 mGy for DCT Pro, 5.96 mGy for Implagraphy 1, 0.70 mGy for Cranex 3+. Effective doses $(E_{2005draft)$ were 2.551 mSv for SCT 6800TXL (Implant), 1.272 mSv for SCT 6800TXL (3D), 0.598 mSv for Implagraphy 3, 0.428 mSv for DCT Pro and 0.146 mSv for Implagraphy 1. These are 108.6, 54.1, 25.5, 18.2 and 6.2 times greater than panoramic examination (Cranex 3+) doses (0.023mSv). Conclusion: Cone beam CT machines recently developed in Korea, showed lower effective doses than conventional CT. Cone beam CT provides a lower dose and cost alternative to conventional CT, promising to revolutionize the practice of oral and maxillofacial radiology.

Cone beam형 전산화단층영상을 이용한 하악과두 위치의 연구 (Mandibular condyle position in cone beam computed tomography)

  • Purpose : To evaluate position of the mandibular condyle within articular fossa in an asymptomatic population radiographically by a cone beam computed tomography. Materials and Methods : Cone beam computed tomography of 60 temporomandibular joints was performed on 15 males and 15 females with no history of any temporomandibular disorders, or any other orthodontic or prosthodontic treatments. Position of mandibular condyle within articular fossa at centric occlusion was evaluated. A statistical evaluation was done using a SPSS. Results : In the sagittal views, mandibular condyle within articular fossa was posteriorly located at medial and central sections. In the coronal views, mandibular condyle within articular fossa was laterally located at central section. Mandibular condyles in the right and left sides were showed asymmetric positional relationship at medial, central, and lateral sections. Conclusion : Mandibular condyle within articular fossa in an asymptomatic population was observed nonconcentric position in the sagittal and coronal views.

Al 원추경 초정밀가공 특성에 관한 연구 (A Study on the Characteristics of Ultra Precision Machining of a Al Cone Mirror)

  • In this work, diamond turning process is used to produce mirror surface on a Al cone. The Al cone as used as a mirror which can reflect a laser beam without scattering and, hence, it is critical to minimize the surface roughness of a Al cone. During diamond turning, feedrate and tool nose radius are changed to investigate characteristics of the ultra precision machined surface of a Al cone. A laser beam of 633 nm is applied to examine the effect of surface roughness on the characteristics of reflectivity. It is found that surface roughness is not significantly affected by feedrate. The main factor influencing surface roughness is tool nose radius. The line patterns of reflected laser beams show that the minimum surface roughness of 0.08 $\mu\textrm{m}$ (Ra) is required to avoid scattering phenomena of reflectivity.
