Damage Types of Levee and its Maintenance and Repair (제방의 손상 유형 및 보수보강)

  • In 2002, property loss caused by failure or leakage of existing river levee structures was about 1.8 trillion in Korean Won, and furthermore in which damages of river structures are getting more severe due to characteristics of extremely extraordinary rain such as torrential rain in the locality or guerrilla heavy rain. In this regards, this paper collects and analyzes those damage records and costs for repair by statistic method, and moreover categorizes the causes of failure, erosion and overtopping of levee structures in large and small scale rivers threatened frequently by typhoon and heavy rainfall. It is believed that the results from the analyses can be used as a basic source in developing criteria of standards for design, construction, maintenance and inspection(or diagnosis) of hydraulic structures such as levee and drain conduit.

  • Despite low mortality and excellent long-term results after repair of Tetralogy of Fallot, certain associated anomaly such as single pulmonary artery continues to be told to have a high operative mortality and morbidity, and there is still some debate on appropriate surgical intervention. During the 4 year period from 1981 to 1984, surgical repair was performed on 5 patients with tetralogy of Fallot and congenital [4 cases] or acquired [1 case] absence of left pulmonary artery. Previous left pneumonectomy had been performed in the patient with acquired absence of a pulmonary artery. Transannular patch or RVOT patch alone with or without pulmonic valvotomy was used with some modifications to reduce pulmonic insufficiency in individual patient. In contrast with previous reports, all patients survived operation and have exhibited marked symptomatic improvement without pulmonic valve insertion nor valved conduit.

Truncus Arteriosus, Type I one case report (총동맥간증 [1 형] 의 치험례)

  • Truncus arteriosus is one of the rare cyanotic congenital cardiac anomalies and thought to be result from complete or partial failure of trunco-conal septum. A single arterial trunk receiving blood from both ventricles supplies the coronary, pulmonary and systemic circulation. The symptoms were usually related to the degree of the pulmonary blood flow and functional status of truncal valve, and mostly appeared within the first two months of life. The prognosis is generally considered to be poor in spite of successful surgical correction. This report is a case of 13 years old female with type I truncus arteriosus, which was successfully corrected using a intracardiac Dacron tunnel graft[semilunar, 18mm] from VSD to the truncal valve, and a extra-cardiac lonescu-Shiley valved[20mm] Dacron conduit [21mm] from RV to the pulmonary artery[Rastelli operation].

Surgical Treatment of Annuloaortic Ectasia Report of One Case (Annuloaortic Ectasia 수술치험)

  • A case of Annuloaortic Ectasia associated with Marfan syndrome was treated by replacement of aorta and aortic valve with a valved conduit. A 26 years old man had suffered from palpitation and precordial pain. He had stigmata of Marfan`s syndrome. Aortogram and 2-D echocardiogram confirmed aneurysm of the ascending aorta with aortic insufficiency. Surgery was performed under the moderate hypothermia to 28oC. There was marked dilatation of the aortic annulus as well as sinus of Valsalva, with displacement of the coronary ostia. Aortic valve and aneurysm was replacement with 25mm, woven Dacron tubular graft, to which a 25mm, S.T. Jude Medical valve had been previously sutured. Right & left coronary ostia were anastomosed to the graft with the use of 3O Nylon pledget suture. The patient had a satisfactory post operation period with out specific complication.

A Case Report of Pulmonary Atresia with Pulmonary Artery, PDA and Ventricular Septal Defect (동맥관개존증, 심실중격결손 및 폐동맥이 동반된 폐동맥 폐쇄증: 1예보고)

  • A surgical correction was successfully performed in a adult female who had pulmonary atresia with almost non-confluent pulmonary artery, PDA and ventricular septal defect. Initially as a first stage of corrective surgery isolation of patent ductus arteriosus, ligation of aortopulmonary collaterals and identification of the pathologic anatomy of left pulmonary artery through left posterolateral approach were performed. At the second stage, a week after the initial operation, total correction was done making both pulmonary arteries confluent with albumin coated woven-dacron graft, external valved conduit and closure of large VSD of subarterial and perimembranous type. The PDA was ligated with previously encircled heavy stitch soon after partial bypass was started. Although massive bleeding from anastomotic site of dilated left pulmonary artery to the graft occurred preoperatively, postoperative functional improvement was excellent in terms of disappearance of cyanosis and normal exercise tolerance.

  • The developed in-flex system completed the wiring work with the plug-in connection. To maintain electrical and mechanical stability, and an insulation between the conductors was strengthened by forming a partition. Moreover, the error according to a bad connection was prevented by separating the inlet from the outlet of the electric trace and thus the quick construction become possible. The metal reinforcing material was added outside the upper case and lower case. The fire-resistance efficiency was maximized in order to minimize a damage by the fire. As to the developed system, we found that it takes shorter time to complete installation than the rigid steel conduit wiring work, and that about 25 % of construction cost was saved because the labor costs decrease due to the shorter construction period of time.

  • 초고압케이블을 관로식으로 포설할 경우 케이블 포설장력이 증가함에 따라 측압은 커지게 된다. 측압은 지중관로 내 케이블 포설시 도르래 및 굴곡부에서 발생하는 관로 내면과 케이블 사이의 압력을 의미하며, 단심배열의 경우 곡률반경과 장력에 의존한다. 케이블의 측압을 정확하게 알고 안전성에 기초하여 장경간 포설을 한다면, 케이블 포설 시 긴 케이블 조장에 따라 접속함과 굴착개소의 감소로 이어지기 때문에 원가절감 뿐 아니라 공기 단축 차원에서도 큰 역할을 할 수 있다. 그러나 현재 케이블 측압은 포설 장력을 이용하여 계산되기 때문에 실제 측압과 오차가 발생할 수 있으며, 정확한 장력을 케이블에 인가하는것 또한 어려운 일이다. 따라서 본 논문에서는 케이블 포설시 발생하는 측압을 작용시키며, 장력을 케이블에 정확히 인가할 수 있는 측정기기 개발에 관한 내용을 정리하였다.

  • 본 연구는 컴팩트한 열교환기의 설계를 위하여 열교환기 내의 원형 단면 도관의 유변 유체의 압력 강하 및 대류 열전달률을 수치해석적으로 수행하였다. 유변 유체의 구성방정식은 기존의 비뉴톤 유체 멱법칙을 보완한 수정 멱법칙 모델을 채택하였다. 도관 내의 압력강하를 의미하는 마찰계수와 수정 레이놀즈 수의 곱은 기존 문헌치와 비교할 때 뉴톤 유체 영역과 유변 멱법칙 영역에서 각각 0.01% 및 0.004% 내에서 일치함을 보였고 유변 수정멱법칙 유체 모델의 형태를 띠는 유변 유체를 열교환기 내의 원형 단면 도관 내에서 사용하면 뉴톤 유체보다 최대 58%의 압력강하를 감소시켰고 최대 9%의 대류 열전달 증가을 발생시킬 수 있었다.

An operation for the correction of truncus arteriosus (동맥간의 교정수술 1례)

  • Truncus Arteriosus is an uncommon congenital anomaly which is now correctable surgically in patients with favorable anatomy. A case of a 9 month old male with truncus arteriosus, type II, is reported operation was done on cardiopulmonary bypass with deep hypothermia and circulatory arrest. The pulmonary arteries were not disconnected from the truncus, and the pulmonary common orifice was closed with a Dacron patch through anterior truncotomy and, for the distal anastomosis, the left pulmonary artery was opened near the common orifice. Continuity between the right ventricle and the left pulmonary artery was established with a valved conduit [Ionescu-Shiley, 14mm]. The postoperative course was excellent and uneventful.

  • Complete TGA is a common congenital cardiovacular anomaly, and without palliative or corrective surgery, the infant born with TGA rarely survives the first year of life. Hemodynamically, recirculated blood flow in the systemic and pulmonary circuit has a key role in systemic arterial oxygen saturation and the status of the pulmonary vascular bed. Recently a d-TGA with VSD and PS, in a 12 year old male patient had been tried for inversion of the ventricular flow with Rastelli operation. An intracardiac tunnel was constructed between the VSD and the aortic orifice to connect the ventricle to the aorta. The right ventricle was connected with the pulmonary circulation by anastomosis of an valved conduit between the right ventricle and the distal end of the pulmonary artery. During the postop, period, the irreversible renal failure, accompanied by metabolic acidosis and pulmonary edema, occured under relative stabilized cardiac performance state. The autopsy was done, which revealed diffuse infarcted area in both kidney and preserved intra & extracardiac graft constructed.
