상지의 압박 신경병증 (Compressive Neuropathy in Upper Extremity)

  • 상지의 말초신경 압박 병증의 진단은 환자의 병력 및 이학적 검사로 이환된 신경 및 부위를 임상적으로 짐작할 수 있지만 신경 압박 부위를 정확히 확인하기 위해서는 전기진단적 검사가 큰 도움이 된다. 대부분 증상의 초기에는 비 수술적 치료가 선행되지만 보존적 치료의 실패로 수술적 치료가 필요할 경우는 각 신경의 가능한 압박 부위 중 정확한 위치를 추정하는 것이 성공적인 수술의 시작이 된다. 또한 전기진단적 검사와 더불어 시행되는 근골격계 초음파 검사는 전기진단적 검사만으로는 진단이 어려운 공간 점유 병소(space occupying lesion; SOL)에 의한 말초신경 압박병증의 진단 및 신경의 해부학적 변화, 신경 내부 병변의 진단에 유용한 것으로 보고되고 있다. 성공적인 말초신경 압박병증의 치료를 위해서는 이러한 다양한 진단 방법을 동원하여 정확한 부위를 확인 후 효과적인 수술을 시행하는 것이 중요하다.

당뇨 신경병증 (Diabetic Neuropathy)

  • Neuropathy is a common complication of diabetes. It is characterized by a progressive loss of peripheral nerve fibers. The development of the neuropathy is linked to poor glycemic control, age, and the duration of diabetes. Peripheral sensory polyneuropathy is the most common type in neuropathy. Diabetic neuropathy is the most significant etiologic factor of the foot ulcer that may leads to amputation. Current treatments in diabetic neuropathy have no definitive effects on repair or reverse the damaged nerve but only to relieve of symptoms, especially on pain. When the focal compressive neuropathy is combined with diabetic neuropathy, the nerve would be more vulnerable and symptoms might get worse. Surgery is indicated for decompression of an entrapped nerve, like posterior tibial nerve in tarsal tunnel, after failure of the initial conservative treatments.

  • Acupuncture has been widely used to treat a variety of disease and symptoms. But various complications have been reported. Among them, peripheral nerve injuries have been reported less frequently than other complications. The purpose of this report is to describe what we believe to be the first case of delayed superficial and deep peroneal nerve compressive neuropathy caused by fibrotic mass formed by neglected broken acupuncture needle.

  • Objectives : The purpose of this study is to report the patients with compressive neuropathy of the radial nerve(Saturday Night Palsy), who was improved by acupuncture and Sweet Bee Venom(SBV) treatment. Methods & Results : The patients were hospitalized or outpatient in Dept. of Acupuncture & Moxibustion, traditional korean medical hospital, Sangji University from 1st, Dec. 2008 to 30th June 2009, and treated with acupuncture, SBV, electrical stimulation therapy and herbal medication therapy. To evaluate the wrist drop and numbness of fingers, coding result(Arbitrary values used to evaluate result) and digital infrared thermal image, which was well defined diagnostic tool estimating skin surface temperature difference, were used. As a result, patient symptoms were improved remarkably. Conclusions : Patients were treated during 6 weeks in average. 2 cases were excellent and 1 case was evaluated good result by the acupuncture, Sweet Bee Venom, electrical stimulation therapy.

Clinical Features of Wrist Drop Caused by Compressive Radial Neuropathy and Its Anatomical Considerations

  • Objective : Posture-induced radial neuropathy, known as Saturday night palsy, occurs because of compression of the radial nerve. The clinical symptoms of radial neuropathy are similar to stroke or a herniated cervical disk, which makes it difficult to diagnose and sometimes leads to inappropriate evaluations. The purpose of our study was to establish the clinical characteristics and diagnostic assessment of compressive radial neuropathy. Methods : Retrospectively, we reviewed neurophysiologic studies on 25 patients diagnosed with radial nerve palsy, who experienced wrist drop after maintaining a certain posture for an extended period. The neurologic presentations, clinical prognosis, and electrophysiology of the patients were obtained from medical records. Results : Subjects were 19 males and 6 females. The median age at diagnosis was 46 years. The right arm was affected in 13 patients and the left arm in 12 patients. The condition was induced by sleeping with the arms hanging over the armrest of a chair because of drunkenness, sleeping while bending the arm under the pillow, during drinking, and unknown. The most common clinical presentation was a wrist drop and paresthesia on the dorsum of the 1st to 3rd fingers. Improvement began after a mean of 2.4 weeks. Electrophysiologic evaluation was performed after 2 weeks that revealed delayed nerve conduction velocity in all patients. Conclusion : Wrist drop is an entrapment syndrome that has a good prognosis within several weeks. Awareness of its clinical characteristics and diagnostic assessment methods may help clinicians make diagnosis of radial neuropathy and exclude irrelevant evaluations.

강한 손바닥 압박 후 속발된 엄지두덩운동신경병증 1예 (A Case of Strong Palm Compression-Induced Thenar Motor Neuropathy)

  • Thenar motor neuropathy (TMN) is a compressive mononeuropathy of recurrent motor branch of median nerve. It is infrequent and may have different pathogenesis. It may be a unique entity of disease or considered a variant of carpal tunnel syndrome involving the motor branch only. We report a case of TMN induced by vigorous massage that applied strong digital pressure in the region of the base of palm and thenar muscles.

경추 추나치료를 적용한 압박성 요골신경마비 환자 치험 2례 (Two Cases of Radial Nerve Palsy with Chuna Treatment on Cervical Vertebrae)

  • Objective : The purpose of this study is to evaluate the korean medical treatment for compressive neuropathy of radial nerve, especially chuna treatment of cervical vertebrae. Methods : Two patients were treated with acupuncture and moxibustion, herbal medication, chuna treatment on cervical vertebrae. To evaluate the wrist drop and numbness of hand, Coding Result of arbitrary values and manual muscle test(MMT), visual analogue scale(VAS) were used. Results : After 2 weeks-treatment(in case 1) & 5 weeks-treatment(in case 2), the movement and power of wrist were restored to nearly normal range, also the numbness of hands was removed. All cases show the improvement in the movement of wrist and the numbness of hands. Conclusions : Korean medical treatment including cervical manual therapy as chuna treatment is remarkably effective for radial nerve palsy. But further studies are required to concretely prove the effectiveness of chuna treatment on central vertebrae for peripheral neuropathy.

의인성 신경손상을 모방한 손뒤침근내 결정종으로 인한 뒤뼈사이신경병증 증례 (A Case of Posterior Interosseus Neuroapathy due to Intrasupinator Ganglion Mimicking Iatrogenic Injection Nerve Injury)

  • Posterior interosseus (PI) nerve compression is a rare form of compressive neuropathy. It can cause pain along the radial nerve course and weakness of radial nerve innervating muscles without sensory symptoms. A 65-year-old man visited our institution complaining weakness of finger extension and thumb abduction after 2 times of injections at the right elbow in local clinic. The patient's clinical history and physical examination implied an iatrogenic radial nerve injury caused by the injection. The electrophysiologic study revealed of posterior interosseus neuropathy (PIN) with incomplete conduction block. However, the ultrasound study showed that the PI nerve was compressed by an anechoic cyst. The magnetic resonance imaging also confirmed of a ganglion cyst, not a hematoma. After repeated aspirations and a steroid injection, the electrophysiologic study showed recovery of motor weakness. Despite of the clue which implying an iatrogenic injury, clinician should consider other possibilities such as ganglion cysts and ultrasound guided aspiration and steroid injection could be an effective option for conservative management.

Endoscopic Decompression for Optic Neuropathy in McCune-Albright Syndrome

  • McCune-Albright syndrome (MAS) is characterized by a triad of poly/monostotic fibrous dysplasia, caf$\acute{e}$-au-lait macules and hyperfunctioning endocrinopathies including human growth hormone excess. Acromegaly as a manifestation of endocrine hyperfunction with MAS is uncommon. Surgical excision may be challenging due to the associated severe fibrous dysplasia of the skull base. Through the endoscopic procedures, we treated a case of MAS presenting with compressive optic neuropathy due to fibrous dysplasia and acromegaly caused by growth hormone secreting pituitary adenoma. We reviewed the literature on GH excess in MAS to highlight its surgical and medical challenges.
