Purpose: The purpose of this study was to contribute to the activation of comprehensive nursing care services by comparing and analyzing the working environment and operational status of comprehensive nursing care wards with general wards. Methods: A questionnaire survey was conducted at hospitals operating comprehensive nursing care services. We collected data on patient characteristics, workforce, working environment, and work support facilities of 40 hospitals voluntarily participating in the survey. The collected data were analyzed by dividing it into an comprehensive nursing care ward and a general ward. Results: Compared with the general ward, the comprehensive nursing care ward had fewer hospitalized patients, but the severity of the patients and the need for nursing were high. The number of nurses, assistant nurses, and ward support workers per patient was higher in the comprehensive nursing care ward than in the general ward, and the implementation rate of night shift fixed system, the fixed shift system, and the preceptor system were higher. In terms of structural environment, comprehensive nursing care wards had more room for interviews. Conclusion: The comprehensive nursing care ward had more nursing staff and provided a better working environment than the general ward, but required additional support for the structural environment.
Purpose: The purpose of this study was to compare hospitalized patients in comprehensive nursing care units and general care units as to satisfaction with nursing care and factors influencing their intent to revisit the hospital. Methods: A cross-sectional study was conducted with 178 patients who had been hospitalized in a comprehensive nursing care unit and a general care unit in one hospital. Participants completed self-report questionnaires. Data were analyzed using SPSS 21.0. Results: There was a significant difference between the comprehensive nursing care unit and general care unit for intent to revisit the hospital (p=.036). Factors influencing intent to revisit the hospital for patients in the comprehensive nursing care unit were 'satisfaction with nursing care' (p<.001) and 'use of additional costs for comprehensive nursing care' (p=.041). The factor influencing intent to revisit hospital for patients in the general care unit was 'satisfaction with nursing care' (p<.001). Conclusion: Findings indicate that comprehensive nursing care service in which all care is provided by nursing staff only, without family or private caregivers, increases intent to revisit the hospital. These results can be used as the foundation of reviewing the operation and expansion of comprehensive nursing care service.
Purpose: The research looks at differences between the 'General ward' and the 'Comprehensive nursing ward' in Seoul Medical Center, regarding the facility improvements and changes in nursing services. It investigates and analyzes spatial problems and improvement needs through a survey and conduct investigation of staffs. It is to propose the primary data for the architectural planning of the future ward with the comprehensive nursing service. Methods: Targeting the comprehensive nursing ward, changes of the physical environment and spatial problems are analyzed through a field survey, behavior investigation, present-condition investigation, and floor plan analysis. Results: The workforce is increased by approximately twice the amount of the nursing staffs in the comprehensive nursing ward, compared to the pre-general ward. When utilizing the general ward, various spatial problems arise due to the restrictions of the facility condition. Because Sub N.S is an important facility as a part of the nursing work function in the ward of the comprehensive nursing service, the opinions of staffs must be considered when selecting a location and composing a space. Implications: It can be used as a primary data for the comprehensive nursing ward when architectural planning of a new hospital.
Purpose: This study aimed to improve nursing practice efficiency by analyzing and comparing the emotional labor, job stress, and performance of nurses in comprehensive nursing care wards and in general wards. Methods: The study surveyed 151 nurses in comprehensive nursing care and general wards. The results were analyzed by ${\chi}$ 2test, ANOVA, and Pearson's correlation coefficient. Results: There were no statistically significant differences in terms of emotional labor, job stress, and performance for the two different types of nurses. Statistically significant positive correlations were noted between job stress and emotional labor, nursing performance, emotional labor, and job stress in the comprehensive nursing care wards, and between job stress and emotional labor, and nursing performance in the general wards. Conclusion: A longer duration of direct nursing in comprehensive nursing care wards tended to increase, emotional labor and job stress. However, the quality of nursing provided to patients tended to improve. Therefore, it is necessary to establish an institutional plan for carrying out efficient nursing while reducing emotional labor and job stress among nurses in comprehensive nursing care wards.
Purpose: This study aimed to investigate whether the comprehensive nursing care service positively affected accidental falls and pressure injuries. Methods: This study was a retrospective study that analyzed the accidental falls and pressure injuries cases in an acute care hospital located in Seoul and compared the rates of accidental falls and pressure injuries before and after the comprehensive nursing care service was operated. Results: Comparing the accidental fall incidence rates per 100 person-months between a comprehensive nursing care ward and a general ward, it showed fewer accidental falls by 0.44 in comprehensive nursing care wards, but the result was not statistically significant. In the case of pressure ulcers, the incident rate per 100 person-month was 6.17 in general wards and 4.77 in comprehensive nursing care wards, which showed that the number of pressure ulcer patients was lower in comprehensive nursing care wards, however it was also not statistically significant. Conclusion: It is not confirmed that the operation of the comprehensive nursing care service contributes to the reduction of accidental hospital falls or pressure injuries. Follow-up studies are recommended to determine the effectiveness of comprehensive nursing services in quality indicators.
Background: The purpose of this study was to identify role ambiguity of comprehensive nursing care unit nurses. Methods: A concept analysis method by Walker and Avant was used to understand role ambiguity of comprehensive nursing care unit nurses. Results: The antecedents of role ambiguity of nurses at comprehensive nursing units were shortage of nurses, unclear admission criteria, and demands for customized nursing care according to severity. Attributes include ambiguity in role delegation, patient placement ambiguity, and professional ambiguity among nursing staff. The consequences were diminished job satisfaction due to excessive workload, difficulty in resolving role ambiguity due to the lack of work analysis studies, and poor outcome of nursing indicators. Conclusion: Improvement of nationwide awareness for comprehensive nursing care unit is required. Clear division at scope of practice for nursing staff in accordance of each medical institution's characteristics is essential. Nurses at comprehensive nursing care unit should understand nature of role ambiguity that occurs as they work in large groups. Nurses should promote communications between nursing staff and they must have volition to improve status quo. An additional research of comprehensive nursing care on the causes of role ambiguity in the practice of nursing care for ward nurses is needed, and management measures should be sought at the organizational level.
Purpose: This study was conducted to examine the experience of nurse in comprehensive nursing care. Methods: Experiential data collected from 9 nurses through in-depth interviews. Participants were selected from nurses working in the comprehensive nursing care unit at general hospital. The main question was "Can you describe your experience in the comprehensive nursing care unit?" All interviews were recorded and transcribed, then analyzed using Colaizzi's method. Results: Nine themes were derived from the analysis: 'Practice nursing care', 'Feel thankful of the client', 'Difficulty in nursing due to absence of patients' guardian', 'Tired of over-demanding patient and distrust of guardian', 'Confusion regarding one's identity as a nurse', 'Not enough to support system', 'Insufficient pre-training for nurse and client', 'Requirement of work establishment for nurse and nurse aid', 'Concerns about low rewards and high safety accidents'. Conclusion: As a comprehensive nursing service, the nurses provided total patient care, and patient satisfaction and expression of appreciation increased. However, disadvantages were identified, such as patients' excessive needs, communication difficulties, lack of support systems, low compensation, and a high number of safety accidents. Therefore, systematic comprehensive nursing will be achieved if these shortcomings are addressed.
Purpose: This study aims to examine the relationships between the self-leadership, role conflict, nursing work environment, and quality of nursing service in comprehensive nursing care service wards and identify the factors that affect the quality of nursing service. Methods: The data were collected from 158 nurses working in comprehensive nursing care service wards from three general hospitals with 200 beds or more in Seoul, Korea. The data were analyzed by descriptive statistics, t-test, ANOVA, scheffe test, Pearson's correlation coefficient, and multiple linear regression analysis of enter method using SPSS/WIN ver 22.0 program. Results: The factors of quality of nursing services were self-leadership (β=.44, p<.001), nursing work environment (β=.17, p=.014), and the work experience in comprehensive nursing care service wards (β=-.15, p=.035) explaining 32% of the total variance. Conclusions: The results indicated that self-leadership, work experience in the comprehensive nursing care service wards, and nursing work environment affect the quality of nursing services of nurses in the comprehensive nursing care service wards. It is necessary to make efforts for seeking various intervention strategies and improving nursing work environment.
Purpose: This is a retrospective secondary data analysis study based on real-world data to analyze the level of nursing care needs of patients in a comprehensive nursing care service ward, and identify factors influencing nursing needs. Methods: Study participants included patients admitted to two comprehensive nursing care service wards at a tertiary general hospital located in Gyeonggi-do, Korea. After obtaining permission from the health and medical information team of the target hospital, data were collected from their electronic medical records. Nursing care needs were measured using seven items on nursing activity and four items on daily living activities developed by the National Health Insurance Service (NHIS). The collected data were analyzed using the SPSS version 29.0 with frequency and percentage, mean and standard deviation, minimum and maximum values, t-test, ANOVA, Pearson's correlation coefficients, and multiple linear regression. Results: The level of nursing care needs of patients in comprehensive nursing care service wards was found to be higher for patients with pressure sores (β=.33), older patients (β=.26), patients who underwent procedures (β=.15), patients with present guardians (β=.15), and patients with more comorbidities (β=.10). The total explanatory power was 51.0%. Conclusion: It is necessary to accurately identify patients' nursing care needs and provide nursing care according to priority by considering the characteristics of patients in comprehensive nursing care service wards.
Purpose: The purpose of this study was to compare the differences in fatigue, sleep quality and interpersonal relationship stress levels between nurses in comprehensive nursing care wards and in general wards. Methods: Data were collected using self-reported structured questionnaire from 14 August to 17 September 2018. The participants were 220 nurses from 6 hospitals in Seoul and Gyeonggi-do. The data were analyzed using the IBM SPSS 23.0 program, descriptive statistics, t-test, ${\chi}^2-test$, Fisher's exact test, and Pearson's correlation coefficient. Results: The subjects' fatigue, sleep quality, and interpersonal relationship stress were not significantly different between the comprehensive nursing care services wards and the general wards. The interpersonal stress with a superior was statistically higher among general wards than comprehensive nursing care services wards. The relationship stress between patient and caregiver was higher in comprehensive nursing care services wards than in general wards. Conclusion: A follow-up study is needed to develop a strategic method to reduce interpersonal stress in nurses.
