A Study on the Police Organizational Health Diagnosis Index Development (경찰 조직건강 진단지표에 관한 시론적 논의)

  • This study explored the police organizational health diagnosis index development. and the ultimate purpose of this study is to suggest the ways for the police to develop the police organizational health diagnosis index and then to raise the organizational effectiveness. A police organizational health diagnosis index comprises three categories; Organizational Behavior, Group Behavior, and Individual Behavior. A Organizational Behavior questionnaire comprises five categories; Organizational Structure, Organizational atmosphere, Environmental Compliance, Transformational Leadership, and Communication & decision-making. Secondly, A Group Behavior questionnaire comprises five categories; How to manage the personnel, Conflict management style, Group cohesion, Vision and Strategy, and Community-oriented & shared responsibility. Thirdly, An Individual Behavior questionnaire comprises five categories; Job Motivation, work-related factors, Trust, Vitality, and Organizational Cynicism. This author expects this study to contribute to the development of an adequate measuring instrument of the police organizational health diagnosis index.

Adherence to Capecitabine Treatment and Contributing Factors among Cancer Patients in Malaysia

  • Ensuring adherence to chemotherapy is important to prevent disease progression, prolong survival and sustain good quality of life. Capecitabine is a complex chemotherapeutic agent with many side effects that might affect patient adherence to treatment. This cross sectional study aimed to determine adherence to capecitabine and its contributing factors among cancer outpatients in Malaysia. One hundred and thirteen patients on single regime capecitabine were recruited from Hospital Sultan Ismail and Hospital Kuala Lumpur from October 2013 to March 2014. Adherence was determined based on adherence score using validated Medication Compliance Questionnaire. Patient socio-demographics, disease, and treatment characteristics were obtained from medical records. Satisfaction score was measured using the validated Patient Satisfaction with Healthcare questionnaire. The mean adherence score was 96.1% (standard deviation: 3.29%). The significant contributing factors of adherence to capecitabine were Malay ethnicity [${\beta}=1.3$; 95% confidence interval (CI): 0.21, 2.43; p value=0.020], being female [${\beta}=1.8$; 95%CI: 0.61, 2.99; p value=0.003]), satisfaction score [${\beta}=0.08$; 95%CI: 0.06, 1.46; p value=0.035], presence of nausea or vomiting [${\beta}=2.3$; 95%CI: 1.12, 3.48; p value <0.001] and other side effects [${\beta}=1.45$; 95%CI: 0.24, 2.65; p value=0.019]. Adherence to capecitabine was generally high in our local population. Attention should be given to non-Malay males and patients having nausea, vomiting or other side effects. Sufficient information, proactive assessment and appropriate management of side effects would improve patient satisfaction and thus create motivation to adhere to treatment plans.

  • Diet attrition and failure of long term treatment are very frequent in obese patients. This study aimed to identify pre-treatment variables determining dropout and to customise the characteristics of those most likely to abandon the program before treatment, thus making it possible to modify the therapy to increase compliance. A total of 146 outpatients were consecutively enrolled; 73 patients followed a prescriptive diet while 73 followed a novel brief group Cognitive Behavioural Treatment (CBT) in addition to prescriptive diet. The two interventions lasted for six months. Anthropometric, demographic, psychological parameters and feeding behaviour were assessed, the last two with the Italian instrument VCAO Ansisa; than, a semi-structured interview was performed on motivation to lose weight. To identify the baseline dropout risk factors among these parameters, univariate and multivariate logistic models were used. Comparison of the results in the two different treatments showed a higher attrition rate in CBT group, despite no statistically significant difference between the two treatment arms (P = 0.127). Dropout patients did not differ significantly from those who did not dropout with regards to sex, age, Body Mass Index (BMI), history of cycling, education, work and marriage. Regardless of weight loss, the most important factor that determines the dropout appears to be a high level of stress revealed by General Health Questionnaire-28 items (GHQ-28) score within VCAO test. The identification of hindering factors during the assessment is fundamental to reduce the dropout risk. For subjects at risk, it would be useful to dedicate a stress management program before beginning a dietary restriction.

Investigating the Effect of Value Characteristics of SNS Users on SNS Usage Motivation, Social Capital, and Usage Behavior (SNS 이용자의 가치체계의 특징이 SNS 이용동기, 사회적 자본, 이용행위 등에 미치는 영향 분석)

  • This study investigated how Schwartz' value system(1992) would interfere with SNS users' motivations and behavioral responses. The study result shows that values are characterized in terms of openness, mutual reciprocity, self enhancement, normative compliance, and security. Each of them exerted differential impact on SNS usage motives, social capital, ad response, and word-of-mouth, among others. The five values were used as an input for segmenting SNS users and clustering method produced four value segments; experience seeker, interdependent sympathizer, self enhancer, and norm-bound. Each value group not only influenced SNS perceptions and behavioral responses differently, but also showed a systematic relationship with SNS service types. The study findings demonstrate that Schwartz's value system provides a very useful theoretical basis for understanding the psychological mechanism underlying SNS usage.

Implementation of ISO45001 Considering Strengthened Demands for OHSMS in South Korea: Based on Comparing Surveys Conducted in 2004 and 2018

  • Background: According to the previous studies, the work-related accident rate decreased in Korea after the introduction of occupational health and safety management system (OHSMS), but there were several disasters in Korea such as subway worker's death at Guui station in 2016 and the Taean thermal power plant accident in 2018, which escalated the social demand for safety. In 2018, OHSMS became an international standard, as ISO45001 was announced. Methods: A survey was conducted to research the implementation status of OHSMS and changes in people's perception, and the results were compared with those of a past survey. Results: Enhanced social demand and various stakeholders' (not only buyer) needs, and social responsibility are perceived as the motivation for the introduction of OHSMS rather than legal compliance or customer demand. In the questionnaire about problems with the implementation of OHSMS, the factors with higher response rate in 2018 than 2004 were "excessive cost" and "complicated documentation management." In the questionnaire about how to promote OHSMS in organizations, most people answered "reduction of workers' compensation insurance rate" in 2004, but most people answered "exemption from health and safety supervision" in 2018. Conclusion: For the effective implementation of ISO45001, emphasis is placed on social demand, training to recognize health and safety as a part of management, and the reduction of certification and consulting costs to promote the introduction of OHSMS. Incentives such as insurance premium cuts and exemptions from health and safety supervision are needed.

Analysis on Determination of Punishment in Sentencing for Cases Involving Child Maltreatment Fatalities (아동학대 사망사건 판결의 양형 분석)

  • In this study, we analyzed determination of punishment of victimizers presented in sentencing(81 plaintiffs, 95 sentencing) for 55 cases involving child maltreatment fatalities between 2001 and 2015. The results showed that about 40% of main victimizers were imposed relatively minor punishment such as probation or imprisonment for less than 3 years. Judgments that deviated from lower limit comprised large proportion in connection with compliance with standards for statutory punishment and punishment determination. The elements of punishment determination, such as earnest rearing of child, psychological pain arising from death of child, sense of guilt, motivation that can be considered, were found to have a significant influence on mitigation of punishment although such elements could be judged differently, depending on perception or subjective tendency of judges towards child abuse. Even abetters in a position to prevent death of child were imposed minor punishment mostly by probation or monetary penalty. This study presented the need to reconsider the circumstances of punishment determination which has been conventionally mentioned in cases involving child maltreatment fatalities, such as first offender, accidental crime, person with parental rights, fosters, agreement with family of the deceased, etc. Moreover, this study suggested the need to reinforce child abuse prevention training for law enforcement officers and to revamp standards for determining punishment unique to the cases of child abuse in the future.

CONSERVATIVE TREATMENT OF A UPPER CENTRAL INCISOR WITH POOR PROGNOSIS (예후가 불량한 상악 중절치의 유지)

  • The incisors function as instruments for biting and cutting food during mastication. They also support the lips and face and maintain vertical dimension. In addition, they contribute to overall normal arch appearance. They play important role during the articulation of speech and assist in guiding jaw closure. Extraction and space maintenance are the most common treatment for a tooth with poor prognosis. However, in the mixed dentition, extraction of the upper permanent incisors results in many complications, such as resorption of alveolar bone, poor esthetics, pronunciation, and mastication. Considering these various roles of incisors in oral cavity, approach for traumatized incisors, even the ones with poor prognosis, should be considered first prior to simple extraction. The dentist must take into account the age of the patient, growth potential, occlusion, oral hygiene status, economic status and motivation towards dental health in addition to patient compliance. In this case, although the prognosis was predicted to be unfavorable due to short root and mobility, we could save the central incisor using conservative treatment, reposition by orthodontic appliance instead of extraction.

Development of Self-Managed Food Sanitation Check-List and On-Site Monitoring of Food Sanitation Management Practices in Restaurants for Control of Foodborne Illness Risk Factors (식중독 발생 위험요인 관리를 위한 외식업체 자가위생관리점검표 개발 및 현장모니터링)

  • Based on recent dramatic increases in foodborne outbreaks in restaurants, self-managed sanitation systems are now recommended to control contributing risk factors. This study aimed to improve sanitation management practices in restaurants and had two objectives. First, we tried to develop a self-managed sanitation check-list, including risk factors contributing to foodborne illness and Korean food hygiene regulation articles. We also tried to evaluate current sanitation management practices in restaurants according to operation and restaurant type. Thirty restaurants were evaluated by on-the-spot inspectors using an auditing tool consisting of four dimensions, seventeen categories, and forty-one items. Total compliance rate categorized by operation type significantly differed between chain restaurants and self-managed restaurants, with values of 85.5% and 51.6%, respectively. Therefore, self-managed restaurants, which showed the lowest compliance rate of below 30.0%, need more strict control to improve current unsanitary management practices, specifically relating to 'sterilization of knives, chopping boards, and wiping cloths', 'sanitation training', 'not allowing access into the kitchen to outsiders', 'handling of food or utensils on shelves at a 15 cm distance away from floor', 'prevention of cross-contamination of cooked foods or vegetables', and 'records of kitchen access or inspection'. Thus, an effective food sanitation system is essential and should be implemented to improve the existing sanitary conditions in restaurants. However, the most important factor to achieving food sanitation management objectives is food handlers' self-motivation.

Prevalence Rate of Hypertension and Cared Pattern in Rural Aged over Sixty Years Old (농촌지역 60세 이상 노인인구의 고혈압 유병율 및 관리형태)

  • The survey has for its object to detect prevalence rate of hypertension in target population, to find out the risk factor to hypertension, to detect the hypertensive patients cared pattern and therefore, to consider a effective counter plan for the long term about hypertension. The study, population of this survey was 894 out of 1013 target population from Feb. 1 1994 to March 31 1994 in Kyougsang-namdo Ulsan-gun Samnam-myoun. For these two month, check of blood pressure and direct measurement of height and weight was carried out by mass screening and home visiting and had an interview about risk factors for hypertension and cared pattern by questionnaire. The results of survey were as follows : 1. The prevalence rate of 894 study, population was 27.5% and 26.0% in man and 28.6% in women. 2. In male, the prevalence rate for age group, family history of hypertension, drinking, salt intake by risk factors were significant statistically. 3. In female, the prevalence rate for salt intake, body mass index by risk factors were significant statistically. 4. Motivation which was diagnosed as hypertension was that be examined for subjective symptoms of hypertension and routine check for health was only 25.0%, 9.1% and visiting to the hospital for other diseases, detect hypertension by chance was 65.9%. 5. The experience of treatment in prevalent cases was significant statistically in middle class of SES. And the place of treatment by risk factors could not be significant statistically in spite of the majority selected hospital generally. 6. The reasons of non-compliance in prevalent cases was restricting daily activities for its 45.5% most high and the interruption of treatment in prevalent cases was far from hospital geographically for its 47.6% most high 7. The preventive behavior about hypertensive by risk factors or general characteristics wasn't significant statistically. 8. Being treated or not in the near future about age group, SES, family history of the hypertension was significant statistically. And with regard to the place of treatment in the near future, in spite of the majority selected hospital generally, it wasn't significant statistically. 9. The reasons of non-compliance in incidence cases was restricting daily activities for its 46.8% most high. 10. The preventive behavior in the near future about age group, education level, SES, family history of hypertension was significant statistically.
