The Effects of Complex Exercise Program for Upper Extremity Function and Balance Proficiency in Person With Stroke: A Case-Study (복합운동프로그램이 뇌졸중 환자의 상지 기능 및 균형 능력에 미치는 영향 : 사례연구)

  • The purpose of this research was to investigate the effects of complex exercise program for upper extremity function and balance proficiency in person with stroke. The subject in this study was a 68 year old woman with left hemiplegia after stroke, and the study period was from April 16th, 2005 to August 30, 2007. Complex exercise program was compromised of range of motion exercises, strengthening exercises, and repetitive task-oriented activities and so on, and the subject performed exercises three times a week, one hour a day. The upper extremity function of the subject was evaluated by the Brunnstrom Hand Recovery Stage, the Brunnstrom Upper Extremity Recovery Stage, the Fugl-Meyer Assessment of Motor Function, and the Manual Function Test, and the balance proficiency was measured by the Tinetti Gait & Balance Scale, and the One Leg Standing Test. The results indicated that the upper extremity test scores were all improved, the balance proficiency test marks were maintained and improved, therefore complex exercise program was effective method to forward upper extremity and balance proficiency in person with stroke.

The Effects of the Ten-week Complex Exercise Program for Obese Postpartum Women on Metabolic Syndrome Related Risk Factors and Adipocytokines (10주간의 복합운동프로그램 참여가 산후비만여성들의 대사증후군 관련인자 및 아디포사이토카인에 미치는 영향)

  • The purpose of this study is to analyze how the ten-week complex exercise program for obese postpartum women affects their metabolic syndrome related risk factors (HDL-C, TG, Glucose, blood pressure, and waist circumference) and adipocytokines (Leptin and Adiponectin). The study subjects were 14 women who passed less than 40 months after their child birth, among whom seven were assigned to complex exercise group and seven to control group. The complex exercise program consists of rhythm dance aerobic exercise and resistance exercise. It has been performed twice a week for 10 weeks, and 80 minutes for each session. For data analysis, Mann Whitney U-test, Wilcoxon test, Spearman correlation analysis were conducted. As a result, the groups had no difference in all of the metabolic syndrome related risk factors before and after the program. Regarding intra-group difference, the complex exercise group significantly increased HDL-C, and significantly decreased TG, Glucose, and waist circumference, and the control group signfiicantly increased TG and Glucose. According to the analysis on adipocytokines, in terms of intra-group difference, the complex exercise group significantly increased Adiponectin after exercise. With regard to the correlations between metabolic syndrome related risk factors and adipocytokines, HDL-C and Glucose had a correlation with Adiponectin.

Effects of a Complex Exercise Program using Dynamic Taping on Patients with Tension-Type Headache and Chronic Neck Pain with Forward Head Posture (다이나믹 테이핑을 적용한 복합 운동프로그램이 긴장성 두통과 전방머리자세를 동반한 만성 목 통증 환자에게 미치는 효과)

  • Purpose : This study examined the effects of pain, neck dysfunction, psychosocial level, headache impact test (HIT), postural alignment, and trapezius muscle tone of the complex exercise program using dynamic taping in patients with tension headache and chronic neck pain with forward head posture. Methods : The design of this is a randomized controlled trial (RCT). Thirty-four patients with chronic neck pain were screened using a randomized assignment program and assigned to experimental group (n=17) and control group (n=17). Both groups underwent a complex exercise program. In addition, the experimental group dynamic taping was applied to the upper trapezius muscle. All interventions were applied three times per week for four weeks. The visual analogue scale (VAS), the neck disability index (NDI), short form-12 health survey questionnaire (SF-12), Headache impact test-6 (HIT-6), Craniovertebral angle (CVA), Cranial rotation angle (CRA), upper trapezius muscle tone were compared to evaluate the effect on intervention. Results : Both groups showed significant differences before and after the intervention in VAS, NDI, SF-12, HIT-6, and CVA, CRA (p<.05). In addition, significant differences in NDI and upper trapezius muscle tone were observed between the experimental group and control group (p<.05). Conclusion : A complex exercise program using dynamic taping for patients with tension headache and chronic neck pain with forward head posture are effective method with clinical significance in improving the function and reducing upper trapezius muscle tone.

The Effects of Home Visit Healthcare Using a Complex Program on Community-dwelling Frail Elders' Strength, Frailty, and Depression (복합 프로그램을 적용한 방문건강관리가 재가허약노인의 체력, 허약, 및 우울 개선에 미치는 영향)

  • Purpose: The purposes of this study were to examine the effects of home visit healthcare using a complex program on community-dwelling frail elders' physical strength, frailty, and depression and to compare the effects among three regions. Methods: This study was conducted as a quasi-experimental study using one-group pretest-posttest design. A total of 86 subjects were sampled from three regions (metropolis, small city, and rural area). The program was applied for 14 weeks. This program consisted of disease management by a visiting nurse, exercise with a visiting nurse, self exercise, telephone monitoring, health education, counseling, providing handbook, and incentive. Physical strength was measured by balance, upper-limb, and lower-limb muscle strength. Frailty was measured by the Korean Frail Scale and depression was measured by the Geriatric Depression Scale. Paired t-test and ANCOVA were used to analyze the effects. Results: Physical strength, frailty, and depression were significantly improved after applying the complex program. There were not differences among the regions. Conclusion: Home visit healthcare using a complex program was effective in improving frail elders' physical strength, frailty, and depression. There were not differences among the regions. Therefore, home visit healthcare using a complex program needs to be expanded to other regions.

Development of Preliminary Advanced Aquatic Exercise Program for Persons with Arthritis (관절염 환자를 위한 중급수중운동 프로그램 개발연구)

  • Purpose: This study was conducted to develop an advanced aquatic exercise program for the patients with arthritis. Method: This study subjects were six aquatic exercise program instructors of Korean Society Muscle and Joint Health. They withheld several meeting to develop advanced aquatic exercise program. Results: The basic aquatic exercise program which were used since 1996 in Korea consisted of 9 units of ROM exercise in water. The advanced aquatic exercise program for the patients with arthritis consist of 12 units with complex and resistive exercise. Conclusion: This preliminary advanced aquatic exercise program for the patients with arthritis should be investigate for the effectiveness to build a more concrete evidence and utilization.

Improvement of Postural Balance and Trunk Muscle Strength: Effect of Free Exercise, Elastic Band Exercise and Balance Exercise (맨손 운동, 탄력밴드 운동과 균형 운동이 고령자의 자세균형 기능 및 체간 근력 증진에 미치는 효과)

  • This study was to verify the effect of complex training programs on the postural balance and trunk muscle strength of the elderly. We recruited 40 elderly participants aged 60 to 75 years. Subjects were evaluated before, and 12 weeks after. The participants underwent complex training programs, including free exercise, elastic band and unstable plate. Exercise were performed as follows: 10 repeats in 50 minutes (0 to 4 weeks), 13 repeats in 50 minutes (5 to 8 weeks), and 15 repeats in 50 minutes (9 to 12 weeks). The training group underwent complex training, including warm up, thrice a week for twelve weeks. The control group did not perform any complex training. Results indicate that the postural balance and trunk muscle strength in the training group significantly increased. Data generated from this study could be applied to develop a complex training program to efficiently build whole body muscle strength.

Effects of Personalized Complex Aerobic Training Programs using Wearable Device on Cardiovascular and Respiratory Functions of Female Elderly

  • Objective: The purpose of this study was to examine the effect of personalized complex aerobic training programs using wearable device on cardiovascular and respiratory functions in community based female elderly. Design: One group pre-post intervention study. Methods: Twenty-one older female participants lived in 'D' city were included. The personalized complex aerobic training program using wearable devices was applied to all participants for 4 weeks, 3 times a week, 30 minutes for per session. The participants' blood pressure, heart rate, oxygen saturation, respiration rate, submaximal exercise stress test, pulmonary function test and respiratory muscle strength test were evaluated before and after the complex training program. Results: After intervention, resting diastolic blood pressure, resting systolic blood pressure and the systolic blood pressure after submaximal exercise stress test were significantly decreased over time (p<0.05), and the submaximal exercise stress test duration were significantly increased over time (p<0.05). The maximal inspiratory pressure (MIP) was significantly increased compare to before the intervention (p<0.05). Conclusions: This study showed that personalized complex training program using wearable device can provide personalized exercise intensity according to cardiopulmonary function that give feedback, and these interventions have a significant effect on improving the cardiovascular and respiratory system functions of the female elderly in the community dwelling.

The Effects of Complex Exercise Program with Visual Feedback on Navicular Bone Height, Plantar Pressure and Low Extremity Alignment in Flat-Footed Patients (시각적 피드백을 병행한 복합운동프로그램이 편평발 환자의 발배뼈 높이, 족저압 및 다리 정렬에 미치는 영향)

  • Purpose : The most effective intervention for flat foot is strengthening exercises for the intrinsic and extrinsic of the foot. Additionally, visual feedback is necessary for movement accuracy. However, the effectiveness of the intervention when combined with visual feedback was not revealed. To confirm this, the research was to investigate the effect of visual feedback and a complex exercise program on navicular bone height, plantar pressure, and lower extremity alignment. Methods : The twenty eight adult men and women with flat foot were randomly assigned to group 1 (n=14) and group2 (n=14), group1 performed complex exercises with visual feedback, and group 2 performed only complex exercises. Both groups performed a 40 minute compound exercise program three times a week. Navicular drop test, plantar pressure test, and lower extremity alignment test were performed equally in both group. Results : As a result of comparing the change in navicular height within the group according to the intervention, both groups showed a significant difference before and after the exercise (p>.05). There was not significant difference comparing the difference between the groups in the navicular height (p>.05). Comparing the change in plantar pressure within groups, there was not significant difference in the change in plantar pressure in both groups (p>.05). Coparing the difference before and after exercise between groups, there was not significant plantar pressure (p>.05). Comparing the change in leg alignment within the group, there was a significant difference in the change in ankle before and after exercise in group 1 (p<.05), but there was not significant difference in group 2. There was not significant difference in pelvic tilt and knee tilt before and after exercise in both groups (p>.05). Comparing the before and after exercise difference between groups, there were not significant in all variables of leg alignment (p>.05). Conclusion : The results of this study showed that complex exercise applied to patients with flat foot were effective in increasing the height of the navicular bone and ankle angle, but there was no effect due to visual feedback.

The Effect of Cognitive Rehabilitation Program Combined with Physical Exercise on Cognitive Function, Depression, and Sleep in Chronic Stroke Patients

  • Objective: This study was conducted to verify the effectiveness of a cognitive rehabilitation program consisting of physical exercises and mental activities for patients with chronic stroke with mild cognitive impairment (MCI). We aimed to investigate how this cognitive rehabilitation program affects patients' cognitive ability, depression, and sleep quality. Design: One group pretest-posttest design Methods: The study was conductedon 12 patients who participated in thecognitive rehabilitation complex exercise program for 16 weeks. The Korean version of the Montreal Cognitive Assessment (MoCA-K), Hamilton Depression Rating Scale (K-HDRS), and Pittsburgh Sleep Quality Index (PSQI) were used to evaluate the measured variables before and after study participation. The cognitive rehabilitation complex exercise program included 30 minutes of cognitive exercise and 30 minutes of Brill Exercise. The Wilcoxon signed-rank test was used to compare the variables before and after program participation. Cronbach's ɑ was used to assess the reliability of the test variables. Results: The post-program assessment showed a statistically significant increase in the MoCA-K score, which measures cognitive function (Z=-2.628, P=0.009). For depression ratings, there was a statistically significant decrease in the K-HDRS score (Z=-2.041, P=0.041). For sleep quality, although there was a numerical increase in the PSQI score, the difference was not statistically significant (Z=-0.702, P=0.483). The reliability test confirmed that all the individual test variables exhibited high reliability (cognitive function, 0.859; depression, 0.872; sleep, 0.822). Conclusions: We found that cognitive rehabilitation program used in this study had a positive effect on the cognitive function and depression in patients with chronic stroke with MCI.

  • Objective : The study was to investigate the neurophysiological approach to the effect of complex exercise on memory, one of brain functions, and the degree of sleep disorder using experimental animals with sleep disorders. Methods : This study carried out a complex exercise that designed in an animal laboratory for 4 days to 16 sleep - disordered model rats. After the exercise, brain function was confirmed with the changes of BDNF in the hippocampus and the change of sleep level was confirmed with the concentration of melatonin in the blood. Results : First, the effect of the complex exercise program on brain function was significantly increased in the experimental group(p<0.01). Second, the effect of complex exercise program on sleep disturbance was significantly increased in the experimental group and control group(p<0.01)(p<0.05). Conclusion : The rate of increase of the elderly in the community is rapidly increasing, and the sleep disorder of the elderly can affect the quality of life of these elderly people. Secondary memory impairment due to sleep disturbances can also be a problem. Although there are many ways to improve sleep disturbance, it has been scientifically proven through experimental animals that sleep and memory can be improved with complex exercise that is not economically, spatially burdensome.
