무맥증 수술치험 2례 (Pulseless Disease: Report of Two Cases)

  • A patient was 29 year old house wife who was admitted to the Yonsei University Medical Center on 6 th of Nov., in 1970, with chief complaints of complete loss of bilateral visual acuity, generalized weakness, frequent palpitations, claudication of masseter muscles and intermittent fainting. These symptoms were developed 5 years prior to admission. Physical finding on admission revealed complete loss of bilateral visual acuity, absence of both radial and carotid pulse, but there was good femoral and popliteal pulse. She couldn't open her mouth as she desired and had weakness of mastication. Radiological findings of plane chest PA were not significant. Aortogram(Cineangiogram) showed non-visualization of both carotid and subclavian arteries. It showed only innominate and interal mammary artery preoperatively. The operative findings were as follows: There were complete obliterative changes in both common carotid and subclavian arteries, and periarteritis in the innominate artery. Tube Dacron Prosthesis Bypass with V-arm between innominate artery and both common carotid arteries was performed after thromboendarterectomy. Histopathological finding of the thromboend arterectomy specimen was compatible with pulseless disease, which showed marked fibrous thickening of intima and a diffuse inflammatory cell infiltration of the whole layers. Her postopererative course was uneventful. Follow up aortogram(Cineangiogram) was taken on 11th postoperative day, which revealed both common carotid arteries patent. Her preoperative Subjective symtoms disappeared remarkably, such as her visual acuity improved much, fainting and vertigo disappeared completely and she can go to bath room without difficulty and help. Another patient was 34 year old house wife who was admitted on August, 1964 with chief complaints of frequent fainting and progresive visual loss. She was operated only thromboendarterectomy of both common carotid arteries. Postoperative course was smooth and subjective symptoms were disappeared.

치과보철기공물 제작실태에 관한 조사연구 (A SURVER OF THE ACTUAL CONDITIONS ON THE PRODUCTION OF DENTAL PROSTHESIS)

  • The author studied the actual conditions on the production of dental prosthesis made in laboratories, and also studied interrelationships between dentists and laboratory technicians in both personal and technical aspects. Two hundred-eighty four technicians, work in dental laboratories presently, were surveyed via mail and direct contact during the period from June 1 to June 30 and August 27 to August 28 in 1994 respectively. The obtained results were as follows : 1. Among the respondents, 90.5% we re working in commercial dental laboratories and their laboratories were mainly located in the Seoul area(40.9%, P<0.05). The numbers of employees in these laboratories were less than 10 persons(70.0%, P<0.01), and 75.9% of these laboratories have been in operation less than 15 years. 2. Most laboratory procedures were accomplished according to established disciplines. However, procedures such as die trimming in fixed restorations and the qualifications of the people designing removable partial dentures were not. Other problem areas were boxing of the working cast, the person determining the posterior palatal seal area, selection and arrangement of artificial teeth, occlusion rim correction and laboratory remounting of the processed denture in complete denture restorations. 3. Only half of the requesting dentists could send work authorizations to the laboratories with their work and even so, its contents were quite lacking. Consequently, there must be some standards in writing work authorization. 4. Technicians most desired clean and accurate impressions in fixed and removable dentures, and enough tooth reduction in porcelain fused to metal restorations. 5. For the establishment of better relationships between dentist and dental technician, the respondents desired the establishment of equal footing first(33.5%), and frequent conversations and muture understanding second(25.9%).

부분 혀 절제술을 받은 완전 무치악 환자에서 구개증대보철물을 이용한 수복 증례 (Complete denture rehabilitation of partially glossectomized patient using palatal augmentation prosthesis: A case report)

  • 혀는 대표적인 구강암 호발 부위 중 하나로, 현재 설암 치료의 표준은 외과적 절제술로 알려져 있다. 그러나 설암으로 인해 혀 절제술을 받은 환자의 경우 혀의 가동성이 저하되어 연하와 발음 같은 정상적인 구강기능을 수행하는 데 어려움을 겪을 수 있다. 따라서 이러한 환자들의 보철 수복 시에는 손상된 혀의 기능을 보완하기 위한 추가적인 고려가 필요하다. 구개증대보철물은 구개 연마면의 위치를 낮추어 혀-구개 접촉을 도움으로써 혀의 움직임이 제한된 환자의 구강기능을 개선할 수 있다. 본 증례에서는 설암으로 인한 혀의 좌측 부분 절제술 및 피판 재건술 이후 혀의 가동성이 감소된 상하악 완전 무치악 환자를 대상으로 구개증대보철물로써의 상악 총의치와 전통적인 하악 총의치를 제작하였으며, 최종 보철물을 통해 저작과 연하 및 발음 기능이 개선되어 환자와 술자 모두 만족하는 결과를 얻었기에 이를 보고하고자 한다.

  • 무치악 환자에서 악간관계의 부조화는 자주 관찰되며, 의치에서 인공치의 교합접촉 시 안정의 결여는 환자의 불편감과 점막 외상, 나아가서는 근육과 신경의 변화나 감정적 장애를 일으킬 수 있다. 따라서 기능 시 의치에 가해지는 교합력을 고려하여 적절한 교합을 설정하는 것이 성공적인 의치 치료에 중요한 요소라고 할 수 있다. 본 증례에서는 하악 전돌로 인한 상하악 악궁간의 심한 부조화를 가진 완전무치악 환자를 양악총의치로 수복하였다. 비정상적인 악간 관계를 보상하기 위해 구치부 교차교합으로 인공치를 배열함으로써 의치의 안정과 유지를 확보하였고, 환자 또한 기능적, 심미적으로 만족하였다.

  • 심한 잔존치조제의 흡수와 의치지지조직의 손상을 동반한 총의치 난증례는 정확한 진단과 체계적인 치료계획 수립을 통해 보철물의 유지, 안정, 지지를 증진시켜야 한다. Provisional restoration은 총의치의 안정성을 다각도로 평가하고 환자와 치과의사의 피드백을 가능하게 하는 유용한 도구로서 사용될 수 있다. 본 증례에서는 진단용의치를 통해 인상면, 연마면, 교합면을 평가하여 의치의 안정성을 증대시키고 환자의 심미적 요구를 반영하여 최종의치로 이행하였으며 임상적으로 기능과 심미적인 측면에 있어서 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

A COMPARISON OF THE MASTICATORY FUNCTION BETWEEN TWO DIFFERENT TYPES OF IMPLANT SUPPORTED PROSTHESES AND COMPLETE DENTURE FOR FULLY EDENTULOUS PATIENTS

  • STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.

노인의 구강 건강 상태와 의치 필요도와의 관련성 (Relationship between oral health status and denture needs in elderly)

  • Purpose: The purpose of this study was to investigate the denture attachment status and the denture need of elderly Koreans and to provide basic data for improving the quality of life by developing methods and education programs for the oral health of the elderly. Methods: This study selected 4,340 persons who completed questionnaires and oral examinations among elderly people aged 65 or older only from the National Health and Nutrition Examination Survey data of the 6th period (2013-2015). Results: There was a significant relationship between age, residence, and education level in complete dentures. There was a significant relationship between gender, age, residence, and education level in the partial denture wearing state. In the complete denture need, there was a significant relationship with age. The need for partial denture was found to be significant in gender, age, and residence. Conclusion : Based on the results of this study, it can be used as basic data for oral health education and dental prosthesis related to denture which can improve oral health of the elderly in the future.

의료급여 대상 노인의 치과진료 수요에 관한 연구 (Study on the dental treatment needs of elderly people covered by Medical Aid Program)

  • The purpose of this study was to investigate the oral health condition and dental treatment need of low socioeconomic elderly people. A total of 116 elderly people who visited Boramae Hospital were evaluated with oral examination and panoramic radiogaph. The results showed that the most expecting treatment of low income aged people was removable partial denture. It was very clear in the age group of 70-84. In the age group under 70, most of the patients wanted prosthodontic tratement using fixed partial denture or crown. As the result of this study, elderly people with low socioeconomic status under age 85 needed removable partial denture or fixed prosthesis than complete denture. It is necessary to develop a more expanding and customized Senile prosthetic restoration program.

  • Correct occlusal relationships are part of the successful prosthetic treatment for edentulous patients. Fabrication of complete dentures comprises of clinical and laboratory procedures that should be executed accurately for achieving success with fabricated dentures. Errors occurring during the clinical and laboratory procedures of a denture may subsequently lead to the occlusal errors in the final prosthesis. These occlusal errors can be corrected in two ways: i) in patient's mouth ii) by recording new centric relation and remounting dentures on an articulator. The latter method is more feasible because the mobility of denture base on the mucosa in oral cavity does not permit the identification of premature contacts in centric occlusion and tooth guided eccentric excursions. This article describes a modest and effective clinical chair-side remount procedure using customized mounting platforms.

  • The evolution of breast reconstructive surgery keeps pace with the evolution of breast oncologic surgery. The reconstructive choice should always balance the optimal local control of disease and the best cosmetic result, reflecting an informed decision that is shared with the patient, who is always at the center of the decision-making process. Implant-based breast reconstruction following mastectomy represents a complex choice. In order to obtain optimal results, the following considerations are mandatory: thorough preoperative planning, a complete knowledge of the devices that are used, accurately performed surgery, and appropriate follow-up.
