We experienced an intraatrial baffle repair for unroofed coronary sinus syndrome in TOF. The patient was a 32-month old female, with complaints of cyanosis, exertional dyspnea and growth retardation. Physical examination showed cyanosis of lip, clubbing of finger, growth retardation and systolic murmur[0 /Gr VI] on left sternal border. With an aid of noninvasive and invasive diagnostic procedure, the patient was diagnosed as TOF combined with unroofed coronary sinus syndrome and LSVC connecting to left atrium. After patch closure of VSD, infundibulectomy and pulmonary valvotomy, the intraatrial baffle redirection of anomalous LSVC to right atrium was undertaken. The Dacron baffle was constructed along the roof of the left atrium to the plane of the atrial septum. The intraatrial septum was then reconstructed with Dacron which was sutured to residual septal tissue. After surgery, the systemic oxygen saturation was elevated to 95.5%, as compared with preoperative value 61%. The postoperative course was uneventful.

Intralobar pulmonary sequestration is rare congenital lung disease characterized by a cystic portion of the lung that derives its arterial blood supply though aberrant vessel directly of systemic circulation. Intralobar pulmonary sequestration is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. We experienced a case of pulmonary sequestration postoperatively confirmed. The patient was 48-year-old female whose complaints were cough and left chest pain. Chest film showed large homogenous opacity in left lower lung field. By operation, adult fist sized mass at the lower lobe were noted. An aberrant artery, measuring 1.0 cm. in diameter and 2.0 cm. in length, arose from the descending thoracic aorta just above the diaphragm. The anomalous systemic artery was ligatures and resection, and associated with left middle, lower bilobectomy was done. The postoperative course was uneventful, and 10 days later discharged.

Endodermal sinus tumor[EST] of the mediastinum is a rare germ cell neoplasm. It usually arises from the ovaries and testes but also arises from multiple extragonadal site including the mediastinum. Characteristically, alpha feto protein level is high and used for monitoring the clinical course. EST of the mediastinum is poor prognosis because of its direct invasion. The patient was 18 month old female with chief complaints of cough and fever. In the chest X-ray and CT, large encapsulated, 7x6cm sized, mass of anterior med-iastinum was found, and we could excise it completely because it was well encapsulated and not invaded but only adhered to aortic arch, pericardium and left upper lung. And confirmed it as EST by histopathology. Pre-operative alpha feto protein[AFP] level as 41,748ng/ml and decreased to 2, 663ng /ml at 14th postoperative day, 644ng /ml at 31th postoperative day. From 17th post-operative day, chemotherapy was started and keep going now.

From August 1990 to October 1991, we performed operation on 5 patients with cardiac myxoma, which was located in the left atrium, There was 1 male and 4 female patients ranging in age 45 years to 61 years. Chief complaints included exertional dyspnea, palpitation, cough, orthopnea, general weakness. M-mode and two dimensional echocardiography provided an accurate diagnosis in all cases which was utilized as safe, reliable and noninvasive imaging modalities. In all cases, myxoma was excised successfully. Follow up is current. No recurrent myxoma has been identified clinically or by echocardiography. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum.

Primary tumors of the heart are extremely rare, and about 25 per cent of primary cardiac tumors are malignant. Recently, We experienced a case of primary cardiac liposarcoma occurred on the right ventricular outflow tract and extended into the main pulmonary artery. The patient was 57 year-old man and the chief complaints was severe exertional dyspnea. Emergency operation to relieve the right ventricular outflow tract obstruction was performed under cardiopulmonary bypass. The tumor was 5$\times$4$\times$3 cm in size and infiltrated into the right ventricle and ventricular septum. The tumor in the main pulmonary artery was removed without any difficulty but the on the right ventricular outflow tract was difficult to remove completely. The patient was recovered without any specific problems. After successful surgical resection and postoperative radiotherapy the patient discharged with good condition. Herein, the clinical characteristics of cardiac liposarcoma, the problems in differential diagnosis, the prognosis and current therapy are reviewed.

An approximately 8-month-old, 2.61 kg, male Siamese kitten was referred with primary complaints of a 1-week history of respiratory distress, exercise intolerance and dyspnea. Diagnostic studies identified III/VI systolic murmur in the cardiac auscultation, right ventricular enlargement patterns in the electrocardiogram, pleural effusion and right-sided cardiomegaly in the thoracic radiography, and right marked ventricular dilatation, right atrial enlargement, atrial septal defect and abnormal left atrium divided by fibromuscular membrane. Based on these findings, the case was diagnosed as cor triatriatum sinistrum complicated with an ostium primum atrial septal defect. The cat was rescued with furosemide, nitroglycerine, oxygen supplement and fluid removal from pleural cavity.

Recently, data centers are being established because IT and telecommunication industries are growing. The data centers have to install emergency generators to prevent unexpected shutdown of the electrical power supply. When the data centers are located in the densely populated urban area, the operating noise of the emergency generators can be a cause of people's complaints. In this case, it is necessary to establish effective countermeasure by noise & vibration specialists. To achieve this, noise reduction measure using noise measurement data and 3D noise analysis method have been employed in this study.

Objective: This study reports on the effects of HongYi (red ant, or Formica rufa Linne) pharmacopuncture through three cases involving female patients with voiding difficulty.Method: We treated the three patients with HongYi pharmacopuncture and evaluated them using the international prostate symptom score (IPSS), which is used for measuring the severity of voiding difficulty.Result: After treatment, the IPSS showed improvement in each patient. The symptoms of voiding difficulty and other complaints also improved.Conclusion: Based on the three cases, HongYi pharmacopuncture may be a viable option for treating voiding difficulty among women.

The large transformers used in the electric power company become targets of public complaints because of those characteristics generating high-level noise. Reducing vibration level is the basis of noise control for the large transformers. For this reason, a vibrating reducing element is developed for the large transformers with a sub-element enduring seismic movement. Large attenuation of vibration is achieved by applying the element to the large transformers in a substation.

The purpose of this study is analysing the characteristics of vehicle fuel economy and greenhouse gase emissions according to driving pattern of drivers. Current fuel economy has not established on official test methods. The difference between actual fuel efficiency and specification fuel efficiency bring up consumer complaints and misunderstandings about fuel economy. Against this background, The country is progressing the study on influence of the fuel efficiency according to variety test conditions. This study analyze the driving pattern of the different drivers and influence of the fuel efficiency according to driving pattern of different drivers.
