  • Though there might exist not a few differences between cyclic works and atypical works, many researchers have applied the same assessment techniques that used for repetitive works, which may result introduce bias in their conclusions. This research aimed to verify whether there exist non-negligible work characteristics and/or dissimilarity among works with different work nature and whether one of the most prevalent assessment techniques for assessing ergonomic hazards of musculoskeletal disorders, REBA, can be applied to atypical works. For a general hospital, an automobile repair shop, and two auto-part assembly plants which manufactures quite different parts, a questionnaire survey and field investigation and ergonomic assessment were carried out and analyzed statistically with reference to the 3rd Quantification technique. The results showed that there exist remarkable difference between physical factors in cyclic works and atypical non-cyclic works. As for repetitive work, body posture was significant factors affecting on musculoskeletal disorders while atypical works seemed to have none which implied that the necessity of taking psychosocial factors into account for assessment of hazards. Complain rate in repetitive works was highest shoulder, back, and neck or wrist in sequence. However, there existed no consistent trend in complain rate in atypical works. And, though weight of manufacturing objects was a common factor that can partly explain musculoskeletal complain, time duration was significant in atypical work whereas repeatability and body posture were significant in repetitive works. As being the results, to summarize, it could be said that application of conventional ergonomic assessment techniques regardless of repetitiveness would be fruitless, and that the necessity of a unique methodology focused on atypical non-cyclic works should not be neglected.

  • Duodenal perforation has a high incidence of complications and mortality if not detected and treated early. Delayed diagnosis of duodenal perforation is frequent because patients rarely complain of any symptoms. We report a case of duodenal perforation that appeared after trauma and was cured by using conservative treatment.

  • Purpose: This study was conducted to examine and compare clinical manifestations and predicting factors for treatment-seeking delay among patients <65 and ${\geq}65\;yr$ with first-time acute coronary syndrome (ACS). Methods: A total of 288 patients who were diagnosed with ACS were individually interviewed at C university hospital in G-city from November 2007 to December 2008. Results: Median pre-hospital delays for younger and older patients were 5 and 12 hr, respectively. Younger patients were more likely to be current smokers, heavy drinkers, obese, stressed, and have an unhealthy diet and family history, and to complain of chest pain, left shoulder and arm pain, perspiration, and nausea. Older patients were more likely to have hypertension and diabetes, and to complain syncope and dyspnea. Logistic regression analyses showed that after adjustment for age, gender and education, progressive onset of symptom and no attribution to cardiac problem significantly predicted pre-hospital delay >3 hr in both younger and older patients. Low perceived health status was a significant independent predictor in older patients only. Conclusion: Health care providers should be concerned with different manifestations between younger and older adults, and educate people at risk for heart attack about symptoms and actions to get immediate help.

A Study on Relationship between Hot Flush and Pulse Energy Measured by 3D Blood Pressure Pulse Analyzer in Women Who Complain of Perimenopausal and Postmenopausal Syndrome (갱년기증후군 환자의 안면홍조 증상과 맥 에너지와의 상관성 연구)

  • Objectives: The purpose of this study is to analyze the correlation between the pulse energy and hot flush in women who complain of climacteric syndrome. Methods: We analyzed the values of pulse energy on 78 of perimenopausal and postmenopausal women. The symptoms of patients were assessed by Kupperman’s index (KI) and Menopause Rating Scale (MRS). Statistical analysis was performed by Spearman correlation coefficient and student T-test, using SPSS 18.0 for window program. Results: The results were as follows. 1. There were no statistically significant relation between pulse energy of left Cheok subtracted from left Chon and KI, MRS hot flush score. 2. When divided depending on MRS hot flush score, There were no statistically significant differences on pulse energy. Conclusions: The results suggest that there is no correlation between hot flush score and pulse energy.

  • Purpose: The purpose of this study is to analyze the characteristics of the pulse energy in women who complain of climacteric and menopausal symptoms. Methods: We analyzed the values of pulse energy on 45 climacteric and menopausal women. The patients' symptoms were assessed by Kupperman's index and Menopause Rating Scale (MRS). Statistical analysis was performed using SPSS 18.0 for window program, one way ANOVA, pearson correlation coefficient. Results: The results were as follows. 1. The pulse energy of the Left Chon was significantly largest and Left Cheok was comparatively smallest. 2. There was no statistically significant correlation in the pulse energy of Left Chon minus Left Cheok, Kupperman's index, MRS total sum score. Conclusions: The results suggest that the pulse energy of the Left Chon tends to be increased and Left Cheok decreased in climacteric and menopausal women.

The Study About the Clinical Use of Gwanjung-tang (관중탕의 임상적 활용에 대한 연구)

  • 1. Objectives This paper was tried to find out the clinical use of Gwanjung-tang by comparing patients' chief complain and natural symptom. 2. Methods The study was researched as clinically with medical records of 88 patients who visited a certain oriental medicine clinic which is in Seoul during January, 2000 and June, 2006. 3. Results 1) The male patient who was treated with Gwanjung-tang(寬中湯) is rare. The rate of patients who visited clinic in her forties is 27.3%, in fifties is 18.2%, in thirties is 15.9%, in sixties is 12.5%, in twenties is 10.3%. 2) Chief complain which made patients to visit clinic is a chest pain(19.3%), back pain and shoulder pain (15.9%), edema (12.5%), headache (9.1%), irregular menstruation (8.0%), dyspepsia (8.0%), fatigue (8.0%), insomnia (5.6%), menopausal disorder (4.5%), lumbago (4.5%), abnormal condition of stools (2.3%), and abnormal condition of urine (2.3%). 3) Natural symptom is classified into appetite, digestion, stools, urine, sweat, and sleep. The rate of patients who complained abnormal condition of digestion is 71.6%, of sleep is 59%, of stools is 52.3%, of urine is 47.7%, if sweat is 34.1%, and of appetite is 30.7%. 4) The rate of patients who is treated with Gwanjung-tang between thirty and forty packs is 43.2%, twenty packs is 39.7%, between fifty and sixty packs is 5.7%, between seventy and eighty packs is 2.3%, and between ninety and hundred packs is 3.4%. 5) The rate of patients who is treated with Gwanjung-tang during 15 days is 45.5%, between 16 and 30 days is 26.2%, between 31 and 60 days 7.9%, between 61 and 90 days 3.4%, between 91 and 120 days is 4.5%, and more than 120 days is 12.5%. 6) There are not many cases that chief complain was abnormal condition of digestion (8.0%), sleep (5.6%) stools (2.3%), and urine (2.3%). But the rate of patients who complained discomfort of digestion(71.6%), sleep(59%) stools (52.3%), and urine (47.7%) is significantly high. 7) According to "Sasang Sin Pyeon", Gwanjung-tang was used in To-sa-gwak-ran (吐瀉藿亂), Hae-su (咳嗽), Jeok-chui (積聚), Pyo-han -bu-jong (表寒浮腫), Chang-man (脹滿), Chil-gi (七氣), Dam-um (痰飮), Dam-goe (痰塊), Seom-jwa-yo-tong (閃挫腰痛), Bi-tong (臂痛) of Soeumin, and it is corresponds with clinical cases for various components. 4. Conclusions Gwanjung-tang is a medicine of reconciliation, and it makes Stomach to be warm and take down Yin Qi. The most important effect of Gwanjung-tang is Sun-qi(順氣). So it is considerable that Gyejibanhasenggang-tang (少陰人 桂枝半夏生薑湯), Hyangsayangwi-tang (少陰人 香砂養胃湯), and Gwakhyangjunggi-san (少陰人 藿香正氣散) is also effective to symptoms above, because these are medicines of reconciliatio.

  • Headache is a common complaint in pediatric office practices as well as in children presenting to emergency departments. Children who complain of headache usually are brought to medical attention by their parents, who seek reassurance that the headaches are not a sign of a serious illness. The etiologies of headache range from school problems to brain tumors. A history taking, physical examination, and appropriate diagnostic testing will enable to distinguish primary headaches from those of a secondary etiology. The clinical, neuroimaging, and laboratory evaluation of the child with headache are reviewed here.
