Relation of Compassionate Competence to Burnout, Job Stress, Turnover Intention, Job Satisfaction and Organizational Commitment for Oncology Nurses in Korea

  • Background: Nursing focuses on the development of an empathic relationship between the nurse and the patients. Compassionate competence, in particular, is a very important trait for oncology nurses. The current study sought to determine the degree of compassionate competence in oncology nurses, as well as to determine the relationships between compassionate competence, burnout, job stress, turnover intention, degrees of job satisfaction, and organizational commitment in oncology nurses. Materials and Methods: A descriptive correlational study evaluating the relationships between compassionate competence, burnout, job stress, turnover intention, degrees of job satisfaction, and organizational commitment in 419 oncology nurses was conducted between January 30 and February 20, 2015. Results: The average score of compassionate competence for oncology nurses in the current study was higher than for clinical nurses. Conclusions: The correlational analysis between compassionate competence and organizational commitment, burnout, job stress, turnover intention, and degree of job satisfaction revealed a high correlation between compassionate competence and positive job satisfaction and organizational commitment. Conclusions: Compassionate competence was higher in oncology nurses than in nurses investigated in previous studies and positively correlated with work experience. Job satisfaction and organizational commitment in nurses may be improved through compassionate competence enhancement programs that employ a variety of experiences.

  • Purpose: The purpose of this study was to describe end-of-life care stress, compassionate competence, and job satisfaction among nurses caring for cancer patients and to identify the relationship between variables. Methods: The participants comprised 141 nurses at the D cancer center in Busan metropolitan city. Data were collected from October 14 to the 31, 2022. The data were analyzed using the SPSS/WIN 29.0 program. Results: There was a significantly positive correlation between a participnat's job satisfaction and compassionate competence (r=.45, p<.001), and end-of-life care stress (r=.42, p<.001). Conclusion: Compassionate competence and end-of-life care experience among nurses caring for cancer patients are factors that influence job satisfaction. A program that can improve compassionate competence should be developed to improve job satisfaction.

아주 연민사랑척도 개발: Sprecher와 Fehr의 Compassionate Love Scale의 한국판 단축형 (The Development of the Ajou Compassionate Love Scale: A Korean Abbreviation of Sprecher and Fehr's Compassionate Love Scale)

  • 본 연구에서는 21문항짜리 연민적 사랑척도(CLS: Compassionate Love Scale, Sprecher & Fehr, 2005)에 대한 비판적 검토를 토대로 두 개의 연구를 통해 12문항으로 단축한 한국판 척도를 친지판과 일반인판으로 제시하였다. 연구1에서는 CLS의 일반인판 21문항을 번안하여 207명의 대학생에게 실시한 자료를 통해 각 문항의 변별력, 내적합치도에 미치는 영향 등을 토대로 21문항 척도의 요인구조와 신뢰도를 거의 손상시키지 않는 12문항을 선별하였다. 연구2에서는 269명의 대학생 자료를 수집하여, 12문항 척도의 요인구조의 안정성과 신뢰도를 재검토하였고, 관련된 다른 척도들과의 상관계수를 검토하였다. 그 결과, 본 연구에서 제시한 한국판 아주 연민사랑척도(ACLS; Ajou Compassionate Love Scale)는 원척도에 비해 문항수가 12개로 훨씬 짧으면서도 구조적으로 동일하고, 신뢰도가 .90을 넘으며, 다른 척도들과의 상관계수도 이론적 예상과 일치하는 효율적이며 타당한 척도로 나타났다. 연구의 한계와 함의에 대해 논의하였다.

일 종합병원 간호사의 영성과 임종간호태도와의 관계에서 공감역량의 매개효과 (The Mediating Effect of Compassionate Competence on the Relationship between Nurses' Spirituality and Attitude toward Care of Dying Patients)

  • Purpose: The purpose of this study was to identify the mediating effect of compassionate competence on the relationship between nurses' spirituality and attitude toward care of dying patients. Methods: A cross-sectional correlational research design was conducted. The total subjects were 172 nurses at one general hospital in Gangwon-do. Data were collected using self-report questionnaires and analyzed using t-test, one-way ANOVA, Scheffe's test, Pearson's correlation coefficients, and multiple regression analysis. Results: Compassionate competence showed the mediating effect (${\beta}=.19$, p=.025) on the relationship between nurses' vertical spirituality and attitudes toward care of dying patients. However, no mediating effect of compassionate competence on the relationship between nurses' horizontal spirituality and attitudes toward care of dying patients was found. Conclusions: Based on the findings of this study, development of training programs with a focus on spirituality and compassionate competence was highly recommended to improve and maintain nurses' positive attitudes toward caring of the dying patients.

The Relationships of End-of-life Care Stress with Compassionate Competence and Attitudes toward End-of-life Care among Pediatric Nurses

  • Purpose: This study aimed to provide basic data for developing interventions to relieve the end-of-life care stress experienced by pediatric nurses by examining the relationships of end-of-life care stress with compassionate competence and attitudes toward end-of-life care. Methods: Data were collected via a survey that was conducted from September 10 to September 30, 2018 and administered to 113 nurses who had worked for more than 6 months in a pediatric unit at a tertiary hospital in Seoul, South Korea. The data were analyzed for frequency, percentage, mean, and standard deviation, and the independent t-test, one-way analysis of variance, and Pearson correlation analysis were conducted using SPSS version 25.0. Results: End-of-life care stress among pediatric nurses had a weak positive correlation (r=0.216, P<0.05) with compassionate competence and had no significant correlation with attitudes toward end-of-life care. Among the sub-factors of end-of-life care stress, psychological difficulties had a weak positive correlation with sensitivity (r=0.309, P<0.01) and communication (r=0.230, P<0.05), which are aspects of compassionate competence. Lack of knowledge about end-of-life care had a weak positive correlation with communication (r=0.209, P<0.05) as an aspect of compassionate competence. Conclusion: To improve the quality of end-of-life care provided by pediatric nurses, it is necessary to improve their compassionate competence and reduce their end-of-life care stress by developing education and support programs tailored to the characteristics of children and specific communication methods.

치매대상자를 돌보는 간호사의 의사소통행위 영향요인: 공감역량과 소진을 중심으로 (Factors Influencing the Communication Behavior of nurses caring for people with Dementia: Focused on the compassionate competence and Burnout)

  • 본 연구의 목적은 치매대상자를 돌보는 간호사의 공감역량과 소진이 의사소통 행위에 미치는 영향을 파악하는 것이다. 연구 설계는 서술적 조사연구를 사용하였다. 연구대상은 B시, U시 및 Y시에 소재하고 있는 9개 요양병원에서 직접 치매 대상자를 돌보는 간호사 190명이다. 자료수집기간은 2018년 6월 20일에서 7월 1일까지였으며, 자기보고식 설문지로 조사하였다. 수집된 자료는 SPSS 22.0 프로그램을 이용하여, 서술통계, t-test, ANOVA, pearson correlation coefficient, multiple regression으로 분석하였다. 본 연구결과에서 치매 대상자를 돌보는 간호사의 의사소통 행위는 공감역량 및 소진과 통계적으로 유의한 상관관계가 있는 것으로 나타났다. 다중회귀분석 결과, 치매 대상자를 돌보는 간호사의 공감역량과 소진은 의사소통 행위에 통계적으로 유의한 영향을 미치는 것으로 나타났으며, 이들 변수의 총 설명력은 31%이었다. 따라서, 추후 본 연구결과를 토대로 치매 대상자를 돌보는 간호사의 의사소통행위를 증진시키기 위해 공감역량을 증진시키고 소진을 감소시킬 수 있는 교육프로그램의 개발과 평가를 위한 지속적인 연구를 제언한다.

  • Domestic SMEs play a key role in production and employment, accounting for 98.9% of total enterprises, 74.4% of employment, 50.8% of production, and 49.0% of value added (as of 2016). However, they are faced with difficulties of survival because of continuous decrease in profitability, shortage of funds and manpower, insufficient accumulation of technology, minimum wage increase, and 52-hour workweek. In order for SMEs to continue to develop in such a difficult environment, we must constantly innovate our organization by making full use of our knowledge, information, experience and ability. To do this, CEO (management) leadership is very important to ensure that the best people are focused on the organizational innovation and management performance without moving the company. In this study, we wanted to establish empirically how the newly emerging compassionate rationalism leadership of SMEs business managers affected the innovation activities and management performance of companies. Research has shown that management's compassionate rationalism leadership has a positive and significant causal relationship with the innovation activities and management performance of the enterprise. Therefore, SMEs managers need to learn the bottom eight strategic factors of compassionate rationalism leadership and use them strategically to make efforts to create positive emotions for the organization so that members can be immersed in innovative activities and management performance creation.

상급종합병원 간호사의 그릿과 간호업무환경이 간호업무성과와 재직의도에 미치는 영향: 온정적 합리주의 리더십의 매개효과 (The Effects of Grit and Nursing Work Environment of Nurses in Tertiary Hospitals on Nursing Performance and Retention Intention: Mediating Effect of Compassionate Rationalism Leadership)

  • Purpose: To identify the effect of a new leadership type by confirming the effect of clinical nurses' grit and nursing work environment on nursing performance and retention intention with the mediating effect of compassionate rationalism leadership in the process. Methods: A proportional stratified sampling method was used with 45 tertiary general hospitals nationwide as the sampling unit. An online survey targeting nurses who have been working continuously for more than one year was conducted from October 14 to November 9, 2022. The final analysis included 1,256 questionnaires. The correlation between the participants' variables was analyzed using Pearson's correlation coefficients. The mediating effect was confirmed using a regression analysis, and bootstrapping. Results: The compassionate rationalism leadership had a mediating effect on the relationship between grit and nursing performance (F=203.21, p<.001), nursing work environment and nursing performance (F=109.48, p<.001), grit and retention intention (F=149.90, p<.001), and nursing work environment and retention intention (F=25.14, p<.001). Conclusion: The compassionate rationalism leadership of nursing managers had a positive effect on nursing performance and retention intention. Therefore, with the development and application of educational programs of compassionate rationalism leadership, it can be used to improve nursing performance and operate an efficient nursing organization.

혼종모형을 이용한 정신 간호사의 공감적 의사소통역량 개념분석 (A Concet Analysis of Psychiatric Nurse's Compassionate Communication Competence: Hybrid Model)

  • 본 연구는 혼종 모형 방식을 사용하여 정신 간호사의 공감적 의사소통역량에 대한 개념을 명확히 하기 위해 분석되었다. 이론적 단계에서 2000년부터 2022년까지 출판된 문헌을 수집하여 최종 38편의 논문을 분석하였다. 현장 작업단계를 위하여 2022년 12월 1일부터 12월 28일까지 8명의 정신 간호사를 대상으로 심층 면담을 진행하였다. 최종 분석 단계를 통해 정신 간호사의 공감적 의사소통역량의 차원과 속성을 도출하여 개념을 분석하였다. 이론적 단계와 현장 작업단계에서 파악된 속성을 바탕으로 정신 간호사의 공감적 의사소통역량을 정의하면 5개의 차원과 12개의 속성으로 종합되었다. 이에 따라서 정신 간호사의 공감적 의사소통역량은 정신 간호사가 대상자에 대한 공감과 이해를 바탕으로 효과적인 의사소통을 위해 적극적 경청과 공감적 스킬을 사용하고, 대상자의 생각과 감정에 민감하게 반응하여 대상자가 표현하고자 하는 바를 정확하게 전달하고 대상자를 존중하며 대상자에게 힘을 실어주는 기술과 능력을 의미한다.

단기 자비명상이 자비심과 이타행동에 미치는 영향 (The Effects of Short-term Loving-Kindness & Compassion Meditation on Compassionate Love, Four Immeasurables, and Altruism)

  • 본 연구는 20분의 단기 자비명상이 대학생의 자비심과 이타행동에 어떤 영향을 미치는지 확인하고자 하였다. 대인관계 문제척도(KIIP-SC)에서 점수가 높은 순으로 대학생 35명을 선별하여 자비명상집단(n=19)과 심상화집단(n=16) 중 하나에 무선할당하였다. 자비명상집단에는 20분간의 자비명상을 실시하였고, 심상화집단에는 같은 시간동안 자비명상집단과 동일한 인물을 심상화하고 외모특징을 탐색하게 하였다. 연민적 사랑과 사무량심을 측정하여 자비심의 측정치로 삼았고, 곤경에 처한 인물에 관한 2개의 시나리오를 제작하고 주인공을 친구, 아는 사람, 낯선 사람으로 달리해서 기부와 도움의도를 측정하여 이타행동의 지표로 삼았다. 연구결과, 자비명상집단은 심상화집단에 비해 연민적 사랑이 더 많이 증가했으며 낯선 사람에 대한 기부행동, 친구와 아는 사람, 낯선 사람에 대한 도움행동도 유의하게 많이 증가하였다. 또한 두 집단 모두 수련 후에 긍정적 기분 수준이 증가하고, 부정적 기분은 감소하였다. 하지만, 사무량심과 대인관계 문제 수준에서는 두 집단의 증분점수에 유의한 차이가 없었다. 이러한 결과를 단기 자비명상이 대인관계 문제가 있는 사람들의 자비심과 이타행동을 증가시키는 데 긍정적인 영향을 미치며, 이런 효과를 기분의 효과로 설명할 수 없다는 것을 보여주는 것으로 해석하였다. 본 연구의 의의와 제한점 및 향후 연구의 방향에 대해 논의하였다.
