보건진료소의 관할 지역 건강조사 사례 (Case Report on a Community Health Practitioner's Health Survey)

  • Purpose: This case study was done to describe the health survey conducted by a community health practitioner. Methods: The community health survey was carried out from April 16 to May 31, 2018 with face to face interviews done by 48 trained senior nursing students. Results: Compared with other regions, rates for chronic disease prevalence, chewing discomfort, falls, and depression were higher than those of the relevant district/the relevant city, and the whole country. It is encouraging that the treatment rate for hypertension, diabetes, and hyperlipidemia, and walking practice rate were higher than those other regions. Those who participated in village events had low stress, and those who participated in health promotion programs had a higher quality of life. Conclusion: The community health practitioner in the public health center post needs to operate health promotion programs continuously. Programs including chronic disease management, fall prevention, depression control, and oral health management should be emphasized, and ways to increase social participation, including participation in village events should be developed.

  • The National Nutrition Survey in Japan (NNS-J) started in 1945 and has provided information on dietary intake and health status of Japanese citizens to the public and policymakers for more than half a century. We summarized several relevant issues on the survey in this report : the current framework of the NNS-J in accordance with the Nutrition Improvement Law, utilization of the survey for nutrition and health policy in Japan, the Health Promotion Law recently enacted in 2003, the national plan for health promotion and disease prevention (Health Japan 21), and possible measures to improve the survey systems under the new law. We also mentioned implementation structures of regional health and nutrition surveys, because the Health Promotion Law designates an active role of local governments on promoting health for their citizens, which will enhance the needs for appropriate assessment of health and nutrition conditions in each community as well as the monitoring at the national level. 보건진료원의 직무만족에 영향을 미치는 요인 (Job Satisfaction and Related Factors among Community Health Practitioners)

  • Purpose: This study was done to identify factors related to job satisfaction according to socioeconomic status and job related factors among community health practitioners. Method: A total of 91 community health practitioners participated in the survey. The survey was performed in November, 2008. The survey instruments included socioeconomic factors, work related factors and job satisfaction. The statistical analysis included frequencies, percentages, means, standard deviation, t-test, ANOVA, Scheffe test and multiple regression. Results: The mean age was $44.03{\pm}5.21$ years, 35.1% graduated from baccalaureate program. Job satisfactions related to wage level and job description had relatively lower score than other items. Job satisfaction was significantly different by education level, marital status, and motivation for choosing to be a community health practitioner. Conclusion: Characteristics of community health practitioners are changing and the Korean government needs to up-date job descriptions and welfare policies according to the higher quality of public health practitioners.

지역보건의료계획 수립과정에서의 주민참여실태와 이에 미치는 영향 (The Impacts of Community Participation on Regional Health Planning Process)

  • The purpose of this study is to analyze the community participation in regional health planning by using the operational definition of participation channels, and to find out the significant variables of community participation channel which are highly related to the scores of regional health plan. The channels we adopted for the study are recommended in the guidebook for regional health planning. They are performing residents' helath survey, submitting written agreement of publics, holding public hearings, putting publicnotice, conducting community health committee meeting, submitting opinion of chambers and Mayors, and operating task-force for planning. We analyzed the selected data of 8 community participation channels, which were submitted by 141 rural health centers for the pursuit of governmental subsidy in 1997. The major findings of this study are as follows; 1. In the process of regional health planning, 88.7% of rural community health centers have performed the residents' health survey, 14.9% submitted written agreement of publics, 11.3% held public hearings, 39.0% put public notice, 46.8% conducting community health committee meeting, 48.9% submitted opinion of chambers, 61.7% submitted opinion of Mayors and 25.5% operated task-force for planning. The result shows that most of community participation channels have been utilized at very low rates, except the residents' health survey. 2. We have analyzed the impact of these community participation channels on the regional health plan scores. In the multiple regression model, we set the regional health plan score as the dependent variable., and the use of participation channels as the idependent variables(1 if the channels are used, 0 otherwise). Finally, the regression analyses show that two channel variables, opinion of chambers and public notice, were the significant positive channel variables on the score of community health plan.

건강증진을 위한 지역사회 참여와 지역사회 공동체의식: 대도시 지역을 중심으로 (Sense of community and community participation for health promotion in urban areas of Korea)

  • Objectives: This study aims at examining the association of sense of community with community participation for health promotion in urban areas of Korea. Methods: We analyzed data from 'Community Capacity for Healthy Gangdong Communites' Survey' in 2007. The survey was based on self-reported questionnaires, which were distributed to 1,800 community residents over the age of nineteen in five administrative communities of Gangdong-gu, Seoul, in Korea by using proportionate probability sampling method. We measured 'Sense of community' with four indicators including 'Good neighborhoods', 'Perceived possibility of cooperation', 'Pride of community' and 'Possibility of development' by using 5-point Likert scales. Community participation was measured with the experience rate or the extent of participation by 5-point Likert scales in seven community actions or activities including voting, community program planning, social actions, etc. We examined the association of sense of community with community participation by using regression analyses. Results: This study has shown that sense of community was associated with and made positive impacts on community participation in diverse community actions or activities in urban communities. Conclusions: For promoting community health in urban areas, we can increase community participation more effectively with the efforts of enhancing sense of community.

지역사회 기반 조직을 이용한 지역사회역량의 측정과 건강증진 기획 -서울시 S구를 중심으로- (The Measurement of Community Capacity Using Community-based Organizations Network and the Development of Health Promotion Plans)

  • Objectives: The community network is a foundation laid by the local community which has been formed historically and geographically to develop itself. This paper as a Korean way of healthy network survey for the community capacity building is an exploratory research to grasp the character of Korean society and then to organize an resident-governing partnership for that. Methods: Research objectives are CBOs(Community-Based Organizations) in S-district of Seoul. This region is a new town, however the solidarity and volunteerism of this resident is lively. The survey tool is Cho's CBOs Evaluation Questionnaire which was originated to measure the community capacity. The period of survey is from the January 2009 to the March 2009 and the study organizations which were collected by snowball sampling were 80. Results: The result shows two main networks: one is the civil society and craft union cluster, the other is welfare organization cluster. Groups of high centrality were organizations whose members are mainly women and craft organizations which were organized before 1990's. The group of high betweenness was the Association of Women's Organizations(0.188). Bi-components were six and they could be divided by organization's aim. In terms of the determinants of the participation to the health center enterprise, only the number of link(B=-0.60, p<0.04) was statistically significant. It means that when organization variables are controlled a tendency appears: the more the voluntary associations network, the less participation in the health enterprise. Conclusion: CBHOs(Community-Based Health Organizations) enhances residents' spontaneity and cohesion to increase the capability of the local community. If the surveyed healthy network, together the community health project, are used for various community development projects, the existing CBOs including CBHOs will be reorganized and furthered newly. For this it will be needed to construct an effective partnership of healthy network by restructuring the existing networks of voluntary associations.

농어촌 개발과 의료보장 개혁에 따른 보건진료원의 책임 (The role and responsibility of community health practitioner based on the rural community development and the reform of health care system)

  • This study was conducted by community survey of 215 community health practitiner's posts and literature review of official documents. The result was as follows ; 1. The role and responsibility of community health practitioner's post must be studied by the community health practitioner and the community health leader, and on the basement of community health needs, community development plan, and reforom of health care system. 2. Comprehensive health care of community is very important role and responsibility of community health practitioners. However, it was supervised by the senior community health practitioner in provincial government. 3. The community health practitioner must be trained by formal inservice educational program focused on comprehensive health care. 4. The community health practitoner must be the health guider and health leader as the member of community.

농촌노인의 여가생활 실태와 개선방안에 대한 인식 -성별 마을회관 활용을 중심으로- (A Research on Improving Leisure Activities for Elders in Rural Areas: Based on Using Village Community Centers)

  • The purpose of this research is to examine how the village community centers are being used and find how to improve leisure activities of elders in rural areas by using them. In order to achieve the purpose of this research, we did convenience sampling from elders using 92 village community centers in one of the counties in Gyeong-gi province, and did field survey to 343 persons. The collected data was analyzed by SPSS 18 program, and the results are as follows: The survey on population sociological peculiarity of elders using village community centers shows that most of elders are over 70 years old. As for health conditions, there are more elders who are physically weaker than healthy elders. And the survey shows that most of the elders are suffering from illness such as hypertension, arthritis, neuralgia, and indigestion so such programs related to health are critically required. First, the survey on using village community centers by elders in rural areas shows that they think the village community centers are important for them and use them often. Most of respondents said that they use the village community center so as to meet friends, to enjoy leisure, and to spend time in rural areas where they have difficulty enjoying cultural facilities. Meanwhile, some people dislike to use the village community centers mainly because there is few programs for leisure or each one has a different personality or they don't have enough money to pay for the membership fee. Second, according to the survey on leisure for the elders in village community centers, there are few leisure programs. For men, they spend time by playing Baduk, Janggi, Hwatu and drinking. Meanwhile, women spend time by meeting with friends, listening to radio and TV watching. There are almost nothing to improve leisure activities such as hobby, recreation and health etc. Male elders using centers are suffering from health problems, economic difficulty, and lack of leisure activities, meanwhile, female elders are suffering from loneliness, health problems, and economic difficulty. Third, according to the survey on improving the leisure program of village community center, elders preferred health related programs to improve leisure activities. The most necessary leisure activities in village community center are hobby classrooms, health programs and group travelling. They preferred health related programs for leisure in village community center. Based upon the reality described above, improving the village community centers as leisure facilities is considered to be important for the welfare well of elders in rural areas.

지역사회 스케일링경험률에 영향을 미치는 요인: 지역사회건강조사 자료이용 (Factors associated with community scaling rate: Using community health survey data)

  • Objectives: The purpose of the study is to investigate the influencing factors of community scaling rate using community health survey data. Methods: The data were extracted from 2013 Community Health Survey, Ministry of education, Korea Dental Association, Statistics Korea, Health Insurance Review and Assessment Service, and Ministry of the Interior. The resource factors of independent variables were analysed by Geographical Information System(GIS) using Map Wizard for Excel 17.0. The data were analyzed by descriptive analysis, pearson correlation and multiple linear regression analysis(p<0.05). Results: Seocho-gu in Seoul had the highest annual scaling rate(55.5%) and Goheung-gun had the lowest rate(11%) showing 44.5 percent gap. The influencing factors of scaling included the number of dental hygienists(r=0.316), dentists(r=0.332), dental hospitals(r=0.470), high school graduation rate(r=0.757) and equivalence scales household income(r=0.764)(p<0.05). Multiple linear regression analysis showed that community scaling rate was closely associated with community education level and monthly income(p<0.05). Conclusions: Community scaling rate was closely related to the community education and income level. It is necessary to provide the equal distribution of the oral health service to the community society.

지역사회건강조사를 이용한 충청북도지역 당뇨병 관리 현황 및 적절성 (Current Status and Appropriateness of Diabetes Management in Chungbuk Region Using Community Health Survey)

  • Objectives: This study aims to assess the current status of diabetes management among residents in 11 administrative regions (Si and Gun) of Chungcheongbuk-do, using data from the 2023 Community Health Survey, and to analyze variations according to the types of local governments. Methods: A total of 12,033 residents of Chungcheongbuk-do, aged 19 and older, were selected as study participants through the 2023 Korean Community Health Survey by Korea Centers for Disease Control and Prevention. Diabetes management indicators, including blood glucose awareness, treatment adherence, and complication prevention, were analyzed using SAS Enterprise Guide version 8.3, focusing on regional differences and patterns according to local government classifications. Results: Less than half of the residents of Chungcheongbuk-do were aware of their blood glucose levels, with considerable variation observed across regions. The overall proportion of individuals receiving appropriate diabetes management in Chungcheongbuk-do was 9.6%, but the rates differed significantly between regions. For instance, Yeongdong had the highest rate of diabetes self-management education (57.5%), while Chungju had the lowest (4.3%). The frequency of regular diabetic complication screenings, such as eye and kidney exams, remained suboptimal in most regions, with many falling below 50%. Even among regions with similar local government characteristics, substantial disparities in diabetes management were identified. Conclusions: There is a pressing need for Chungcheongbuk-do and its local governments to enhance blood glucose awareness among residents and integrate comprehensive diabetes education into local health care strategies. Tailored health initiatives must be developed at the local level to improve diabetes management outcomes and reduce regional disparities, ultimately aiming to improve the quality of life for individuals with diabetes.
