Development of community-based intensive health care program for the community dwelling elderly (재가노인을 위한 지역사회 중심의 집중건강관리프로그램의 개발과 적용)

  • The purpose of the present study was to develop a community-based intensive health care program for the community dwelling elderly to strength their functional status and to verify the effect on their geriatric syndrome. A one-group pretest-posttest design was used for the study. A total of 69 frail elderly, who lived in the area within 20 minutes by car, were committed themselves to the day care center(Sangikjae), and had the ability of verbal communication were selected from G city in Kyunggi province. The participants completed a set of questionnaires to measure the sub-score of frailty, fall, urinary incontinence, malnutrition, and mild cognitive disorder domain, using the Otasha-Kensin through the physical examinations and interviews. After 4 weeks of intervention, the outcome was measured to evaluate the effects of the program, and the data obtained were analyzed using descriptive statistics, paired t-test and McNemar test. The results showed that the sub-score of frailty, fall, urinary incontinence, and malnutrition domain were statistically significantly decreased after intervention except those of urinary incontinence and mild cognitive disorder domain, implying that the risk of frailty, fall, and malnutrition was decreased. These findings indicated that community-based the intensive health care program is effective for relieving geriatric syndrome of the community dwelling elderly.

Effect of Community Based Child Safety Education Program for Child Care Teachers (보육교사에 대한 보건소 중심의 영유아 안전관리 교육 프로그램의 효과)

  • Purpose: The purpose of this study was to determine the effect of an education program promoting attitude, knowledge, and practice of safety for teachers in child care centers. Method: This education was provided at one public health center in Kyunggi Province. One group pretest-post test design was used, and knowledge on safety and emergency care, practice of safety, health belief and self confidence on safety practice were assessed from 74 teachers. Theoretical framework for this program was Pender's health promotion model. Result: After two hours group education session on safety management, knowledge on safety and emergency care was significantly increased and perception on the main cause of injuries was significantly changed. Practice on safety was significantly related to the knowledge, health belief and confidence on safety, and social support. Conclusion: The education program for teachers in child care centers regarding the child safety and emergency care was effective in promoting knowledge and perception on the main cause of injuries of infants.

Current Situation and Perspectives for Home Care Nursing Research In Korea (한국 가정간호 연구의 현황과 전망)

  • This study was conducted to identify the current situation of home care nursing research and to propose future research in the area of home care nursing in Korea. The design was a retrospective descriptive study based on 171 studies. The studies were collected from nursing academic magazines, the national library web site and dissertations on home care. The data were classified according to the independent and dependent variables which were represented in the research title. The final category classification was defined by considering the research objectives and content as found in the 171 studies. Eight categories were created to describe the results of home care nursing research in Korea. They included: home care needs. home care services. home care costs. development of home care programs and equipment. management of home care services. effectiveness of home care services, development of an educational curriculum for home care, and recognition of home care services. Based on our research we have identified other future research areas that need to be developed such as community needs assessment, standards and guidelines for home care nursing, quality assurance and quality improvement for controlling home care quality, home care informatics. and a system of home care cost and ethics.

A Need Analysis for Medical Supporting Service as a Part of Community-based Hospice Palliative Care (지역사회기반 호스피스완화케어에서의 의료지원서비스 요구분석)

  • Objectives : This study was conducted to investigate the need of medical supporting service (MSS) as a part of community-based hospice palliative care from the view point of beneficiaries and providers. Methods : This study adopted a methodological triangulation design. A questionnaire regarding intention to use MSS was completed by 175 patients under home-based cancer patient management program. And three focus groups consisted of hospice nurses, public health physicians, and public officials were interviewed to obtain the perceived needs, obstacles, and solutions of MSS. Results : Mean age of home-based cancer patient was 70.18 year old, 48.0% of them were living alone. Only 53.7% of them were treated pain and 93.7% intend to take pain medication prescribed by public health physician. All participants of focus group interviews agreed necessity and importance of MSS. Physicians' lack of confidence and unwillingness to prescribe opioid to terminal patients was the biggest obstacle to provide MSS in the public health center. Conclusions : The necessity and demand of MSS for community-dwelling cancer patients were verified. MSS is urgent issue to meet their needs.

  • This paper studies the activities characteristics of the elderly aged 60-75 years old who can take care of themselves in the community public space, and puts forward the design principles of the community public space in the aging society. Using the methodology of experience design, through naturalistic observation, questionnaires, interviews and shadowing, the spatial and temporal characteristics of daily activities of the elderly are analyzed. In the community public space, the behavior of the elderly is essentially a social activity to get more attention and communication. This determines the needs, time, frequency and duration of activities of the elderly in community public space. In the community public space, the behavior of the elderly has the characteristics of long-term and regularity, and its behavior changes slightly with the space, the objective factors that affect the elderly are gender, age and climate. The subjective factors are lifestyle, family members and income level.

Knowledge and Organization: Perspectives of Knowledge Market and Knowledge Community (지식과 조직: 지식시장과 지식공동체 관점)

  • As the Knowledge Economy grows bigger, studies and practices of knowledge management flourish in almost every organizations. Yet, the studies of fundamental relationships between knowledge and organization have not been sufficiently developed. It is partly because current knowledge management mainly focus on the technical aspects of the theme. This paper tries to find the possible organizing principles and theoretical foundations of knowledge-intensive organizations through the perspective of knowledge market and knowledge community. Based on the differentiation of "knowledge" and "knowing", the author draws several propositions about knowledge creation and knowledge use. Then, the author relates these propositions to the design and implementation of two theoretical models of organizations, i. e. knowledge market and knowledge community. Major claims are: 1) Knowledge use and knowledge creation process are conceptually separable, so that different conceptual models (market & community) need to be applied. 2) Since knowledge use and creation should be integrated, organizations in the Knowledge Economy should be the efficient knowledge market and, at the same time, knowledge community with care and cooperations. 3) The network of independent small KIFs(knowledge-intensive firms) could be considered as the possible organizational model of the future.

  • The recent change of social environment has influence on the configuration of space for the community social welfare center. And so the relation of space and the service function required the change of spatial configuration. The social welfare center fills the role of family welfare, domiciliary care, community service etc. In such sense, this research explore the conditions of spatial configuration for the community welfare service unit of social welfare center in small city. For this study, it was investigated the space role and service function, the service program, the usage characteristics, the room satisfaction of social workers. In sum, the useful data were collected, analyzed, and colligated by case study. It could be used in the basic design criteria of community service unit for the social welfare center in small city.

The Effectiveness of a Cultural Competence Training Program for Public Health Nurses using Intervention Mapping

  • Purpose: This study evaluated the effects of a cultural competence training program for public health nurses (PHNs) using intervention mapping. Methods: An embedded mixed method design was used. Forty-one PHNs (experimental: 21, control: 20) and forty marriage migrant women (MMW) (20, in each group) who were provided nursing care by PHN participated in the study. The experimental group was provided with a four-week cultural competence program consisting of an eight hour offline and online course, e-mail newsletters and social networking services (BAND). Transcultural Self-efficacy (TSE) of the PHNs, client-nurse trust, and satisfaction with nursing care of MMW were measured. Ten PHNs in the experimental group were interviewed after the experimental study. Results: The experimental group showed a significantly greater improvement in TSE, client-nurse trust, and satisfaction with nursing care than did the control group. Six themes emerged from qualitative data: (a) Recognizing cultural differences, (b) Being interested in the multicultural policy, (c) Trying to communicate in MMW's own language, (d) Providing medical information using internet and smart phone, (e) Embracing culturally diverse people into society, and (f) Requiring ongoing cultural competence training. Conclusion: Cultural competence training enabled PHNs to provide culturally competent care and contribute to MMW's health outcomes.

The Effects of Community-Based Self-Help Management Program by Strengthening Self-Efficacy of Post Stroke Elderly Patients (재가 뇌졸중 노인환자에서 자기효능증진 전략을 이용한 자조관리프로그램의 효과)

  • The purpose of this study was to determine the effects of self-help management program which strengthens self-efficacy for post stroke elders who attend day care center in Chongju. One group pretest-posttest design was used, and the subjects were 7 post stroke elders with hemiparesis. The self-help management program consisted of 5 sessions and each session included group education on stroke, ROM exercise, recreation and self-care strategies. Those strategies for encouraging and reinforcing self-efficacy provided verbal persuasion, performance accomplishment and vicarious experience. All subjects were questioned before and after the program about ADLs, IADLs, self-efficacy, self-care behavior and depression. The analysis of data revealed that the program was effective to increase the IADLs, self-efficacy and self-care behavior. However, there was no significant difference between pre and post ADLs and depression. The results indicated that this program may be adopted to improve the physical and psychological function of post stroke elderly patients in community settings.

The Effect of Copayment on Medical Aid Beneficiaries in Korea

  • Purpose: This study was to ascertain whether there are differences in health care utilization and expenditure for Type I Medical Aid Beneficiaries before and after applying Copayment. Methods: This study was one-group pretest posttest design study using secondary data analysis. Data for pretest group were collected from claims data of the Korea National Health Insurance Corporation and data for posttest group were collected through door to-door interviews using a structured questionnaire. A total of 1,364 subjects were sampled systematically from medical aid beneficiaries who had applied for copayment during the period from December 12, 2007 to September 25, 2008. Results: There was no negative effect of copayment on accessibility to medical services, medication adherence (p=.94), and quality of life (p=.25). Some of the subjects' health behaviors even increased preferably after applying for copayment including flu prevention (p<.001), health care examination (p=.035), and cancer screening (p=.002). However, significant suppressive effects of copayment were found on outpatient hospital visiting days (p<.001) and outpatient medical expenditure (p<.001). Conclusion: Copayment does not seem to be a great influencing factor on beneficiaries' accessibility to medical services and their health behavior even though it has suppressive effects on outpatients' use of health care.
