Korean Medicine for Women Infertility (여성 난임과 한의학 (다낭성난소증후군으로 인한 난임의 한양방 치료))

  • Korean medicine has traditionally provided a lot of medical care for the treatment of infertility. The combination treatment of Korean medicine and Western medicine can be effective for infertility to improve the problems caused by the extremely low fertility rate and the aging of pregnant women. In relation to female infertility, we reviewed the evidence for Korean medicine treatment for polycystic ovary syndrome, which has recently been increasing interest in female infertility, and the evidence for the collaboration between Korean and Western medicine in the treatment of polycystic ovary syndrome and infertility. Because polycystic ovary syndrome is closely related to metabolic diseases related to endocrine abnormalities, hormone treatment alone has limited therapeutic effects and has side effects. In domestic and international research, herbal medicine and acupuncture in the treatment of infertility have shown good clinical effects based on the hypothesis of various mechanisms of effect, and many cases of successful pregnancy have been reported. Although continuous research through large-scale randomized clinical studies is needed to prove efficacy, the importance of combined treatment of Korean medicine and Western medicine in infertility treatment is increasing in order to solve the extremely low fertility rate. Combining artificial insemination or in vitro fertilization with Korean medicine treatment is highly likely to be effective in increasing pregnancy success and fertility rates. In women with polycystic ovary syndrome, which is recognized as a metabolic disease due to endocrine abnormalities among the causes of female infertility, Korean medicine treatment can increase the pregnancy rate with herbal medicines and acupuncture, etc. Although well-designed large-scale clinical studies are currently lacking and meta-analysis has not provided sufficient evidence, the combination treatment of Korean medicine and Western medicine is more active to increase the pregnancy rate for solving the problem of low fertility.

  • A female hemihypertrophy patient with hypomelanosis of Ito is presented as a rare case combining classical features of both the syndrome. Chromosomal profile has been based on longitudinal study of repeated lymphocyte cultures during 1984-1992. The propositus has exhibited chromosomal mosaicism both hypoploid ($42{\pm}1$) and hyperploid ($48{\pm}2$ chromosome) counts, but the major stem line presented 46XX chromosomes. Ring chromosome with simple and complex translocations with marker dots appear to be the major cytogenetic assemblage of this child to posses unequal left and right halves of the body. Each and every organ from toe to the head has grown up unequally and lately the patient had been exhibiting different dark and light shapes of melanin on the skin. We believe that the patient had inherited, through her male parent, "a few" mutated loci on some chromosomes so as to generate different cell lines within the developing child. All sibs and the mother showed normal karyotype with no apparent aberration.

Serotonin Syndrome After an Overdose of Dextromethorphan and Chlorpeniramine: Two Case Reports (덱스트로메토판과 클로르페니라민 과용으로 인한 세로토닌 증후군 2례)

  • Dextromethorphan and chlorpeniramine are common ingredients of over-the-counter (OTC) cough pills. They are known to be safe when used alone, however, combination with other serotonergic drugs or use of an overdose can cause serotonergic toxicity. We report on a 43-year-old male and a 57-year-old female who ingested an overdose of antitussive drugs containing dextromethorphan and chlorpeniramine. They commonly presented with altered mentality and hyperreflexia on both upper and lower extremities. After conservative therapies, they were discharged with alert mentality. These cases are meaningful in that there are few cases of serotonin syndrome with an overdose of a combination of dextromethorphan and chlorpeniramine. Careful use with medication counseling for OTC cough pills is needed in order to prevent overdose of these ingredients.

  • BACKGROUND. In order to restore severely attrited teeth properly, surgical intervention in the form of a crown-lengthening procedure may be required. And also, proper diagnosis and treatment sequencing is critical to obtain a successful results. Adequate diagnostic wax-up ensures good esthetics and healthy periodontal tissue. CASE DESCRIPTION. This clinical case report describes a diagnostically based protocol for restoration on mandibular anterior teeth with crown lengthening procedure and the treatment of partially edentulous mandible combined with an edentulous maxilla. In addition, the effort to prevent the combination syndrome was described. CLINICAL IMPLICATION. An interdisciplinary diagnosis and examination through visualization of the desired results ensure conservative and more predictable outcome.

  • Complex regional pain syndrome Type II(CRPS) can be diagnosed by new IASP criteria in 1994. Sympathetically maintained pain may or may not be present in a patient with complex regional pain syndrome. We experienced a CRPS Type II patient who has sympathetically maintained pain as a major painful nature developed after right multiple iliac bone fracture, right femoral artery thrombosis and lumbosacral plexus injury. Combination treatment with L2, L3, L4 sympathetic ganglion block and continuous lower thoracic epidural block for 30 days were tried to get long term effect. The patient had signs of successful. sympathetic denervation of the right foot. After that pain relief was sustained until three month later.

Relationship between Metabolic Syndrome and Cataract in a Middle-Aged Men over the Age of 40's -Based on the 5th Korean National Health and Nutrition Examination Survey- (40세 이상 중장년의 대사증후군과 백내장과의 관련성 -제5기 국민건강영양조사 자료중심으로-)

  • Objectives: This study aimed to analyze the relationship between metabolic syndrome and cataract in a middle-aged men over the age of 40's by using the data from the 5th Korean National Health and Nutrition Examination Survey(2010~2012). Methods: The subjects for the analysis were comprised of 5,808 patients who were at least 40 years or more with metabolic syndrome and cataract. The comparison between the metabolic syndrome and ophthalmic diseases according to the health demographic characteristics, health behavior and health conditions was analyzed with ${\chi}^2-test$. The complex samples logistic regression analysis was used to calculate the odds ratio of cataract according to the health demographic characteristics, health behavior and health conditions and the combination of the comprising factors of the metabolic syndrome. Results: After controlling the confounding factors, the odds ratio of cataract in the metabolic syndrome group was OR 1.30, which was significantly high (p<0.01). Furthermore, the odds ratio of cataract was significantly high in the case of males, as the age increased, educational level decreased, with exercising habits of less than 3 days a week, sun exposure of at least 5 hours, with hypertension and diabetes (p<0.01). The results of analyzing the odds ratio for the occurrence of cataract with the components of metabolic syndrome were OR 1.36 (concurrent high fasting blood glucose, high triglycerides, low HDL cholesterol), OR 1.63 (concurrent high fasting blood glucose, high triglycerides, high waist circumference) and OR 1.33 (high triglycerides, low HDL cholesterol, high waist circumference), which showed significantly high odds ratio for cataract occurrence (p<0.05). Conclusions: We come to know that the relation between metabolic syndrome and components are associate with cataract. Therefore the thorough management of metabolic syndrome and components is needed to prevent cataract.

Literature Review on Syndrome Differentiation of Tremor, Focusing on Chinese Journals (진전 변증에 관한 문헌적 고찰 - 중국 논문 중심으로 -)

  • Background : It was not enough to apply three kinds of syndrome differentiation in our oriental medical textbook to tremor's treatment according to reports of Korea and traditional medical textbook of China. Objective : To investigate syndrome differentiations by types of diseases related to tremor through Chinese journals review and to suggest adding possible syndrome differentiations. Methods : Literature search was performed using China Academic Journal (CAJ), the search engine of China National Knowledge Infrastructure (CNKI) from January 1994 to December 2009. Searching key words were Chinese characters in combination meaning tremor, paralysis agitans, and syndrome differentiation. We included all types of articles that explained or referred to definite syndrome differentiations. The symptoms and oriental medications by syndrome differentiation in selected articles were extracted and summarized. Results : 56 Chinese journals were ultimately selected. 37 kinds of syndrome differentiations about tremor were investigated, which included dual deficiency of qi and blood (氣血兩虛) quoted 31 times, liver-kidney yin deficiency (肝腎陰虧) 23 times, liver-kidney deficiency (肝腎不足) 21 times, and phlegm-heat stirring wind (痰熱動風) 20 times. 37 kinds of syndrome differentiation could by group into eight types, such as liver-kidney yin deficiency (肝腎陰虧), dual deficiency of qi and blood (氣血兩虛), phlegm-heat stirring wind (痰熱動風), heart deficiency and spirit weakness (心虛神弱), blood stasis due to qi stagnation (氣滯血瘀), sea of marrow deficiency (髓海不足), liver qi depression (肝氣鬱結), and liver yang transforming into wind (肝陽化風). Conclusion : We suggest that the syndrome differentiations of tremor, such as heart deficiency and spirit weakness (心虛神弱), blood stasis due to qi stagnation (氣滯血瘀), sea of marrow deficiency (髓海不足), liver qi depression (肝氣鬱結), and liver yang transforming into wind (肝陽化風), can be added to liver-kidney deficiency (肝腎不足), dual deficiency of qi and blood (氣血兩虛), and phlegm-heat stirring wind (痰熱動風) of the textbook. Further systematic research will be needed on them.

Hyeongok's Formulas for Purging Five Viscera and their Ingredients Compatibility of the Properties and Tastes in Herbal Medicines (현곡(玄谷) 오장(五臟) 사방(瀉方)의 조성 약물과 그 기미배오(氣味配伍) 분석)

  • Objectives : The objectives of this study lie in theoretical establishment of formulas for purging five viscera for curing the sthenic syndrome of five viscera through analyzing the component medicines and combination principles of Hyeongok's formulas for purging five viscera, and furthermore, maximizing the clinical use of them. Methods : This study analyzed the component medicines and combination principles of Hyeongok's formulas for purging five viscera based on the theory for properties and tastes of herbal medicines from the "Yellow Emperor's Canon of Internal Medicine", the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine, and the five elements doctrine. Hyeongok's formulas for purging five viscera are Sagantang, Saseemtang, Sabeetang, Sapyeotang, and Pyeongshintang. Results : There are three methods for curing the sthenic syndrome of five viscera according to the five elements doctrine : in case of Sagantang purging the liver, purging the heart and invigorating the lung ; Saseemtang, purging the heart, purging the spleen and invigorating the kidney ; Sabeetang, purging the spleen, purging the lung and invigorating the liver ; Sapyeotang, purging the lung, purging the kidney and invigorating the heart ; Pyeongshintang, purging the kidney, purging the spleen and invigorating the spleen. Conclusions : First, to cure the sthenic syndrome of five viscera, the methods of purging five viscera, and invigorating five viscera should be used according to the five elements doctrine. Secondly, herbal medicines appropriate for those treatment methods should be chosen according to the theory for properties and tastes of herbal medicine and thirdly, the combination of those herbal medicines should be carried out according to the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine.   • The objectives of this study lie in theoretical establishment of Bobee-tang for curing the asthenic syndrome of spleen through analyzing the component medicines and combination principles of Hyeongok's Bobee-tang, and furthermore, maximizing the clinical use of Bobee-tang. This study analyzed the component medicines and combination principles of Hyeongok's Bobee-tang based on the theory for properties and tastes of herbal medicines from the ${\ulcorner}$Yellow Emperor's Canon of Internal Medicine${\lrcorner}$, the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine, and the five elements doctrine. Hyeongok's Bobee-tang is an odd prescription, composed of 7 kinds of ingredients : No.1 Radix Ginseng (2don;7.5 g), No.2 Rhizoma Atractylodis Macrocephalae (1don;3.75 g), No.3 Carapax Et Plastrumtestudinis (1don), No.4 Concha Ostreae (1don), No.5 Cortex Cinnamomi (1don), No.6 Rhizoma Zingiberis Praeparatae (1don), and No.7 Radix Glycyrrhizae (1don). First, to cure the asthenic syndrome of the spleen, the methods of invigorating the spleen and the heart, and purging the liver should be used according to the five elements doctrine. Secondly, herbal medicines appropriate for those treatment methods should be chosen according to the theory for properties and tastes of herbal medicine and thirdly, the combination of those herbal medicines should be carried out according to the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine. As a good example, Hyeongok's Bobee-tang is combined according to the above theories. In conclusion, this formula was created by applying to the theory for properties and tastes of herbal medicines.
