Longitudinal Evaluation of Lung Function Associated with Emphysema in Healthy Smokers (건강한 흡연자에서 폐기종의 유무에 따른 폐기능 변화)

  • Background: Smoking reduces pulmonary function and induces various lung diseases. Recently, the rate of emphysema detection has increased due to lung cancer screening with low-dose chest computed tomography (CT). The purpose of this study was to evaluate changes in lung function associated with emphysema in healthy smokers. Methods: One hundred and ninety one healthy smokers, who had undergone a low-dose chest CT (LDCT) scan as part of lung cancer screening and had revisited the health center after a median 23.9 months' time, were recruited into this study. The severity of emphysema was calculated by the direct observation of a radiologist and a pulmonologist indipendently. Longitudinal changes in lung function according to emphysema based on LDCT and type of smoker was analyzed. Results: Of the participants in this study, 25% of healthy smokers had emphysema, which was mild in severity, in older patients (p=0.003) and in heavy smokers (p<0.001). $FEV_1/FVC$ and FEF25-75% were decreased in current smokers with emphysema (p=0.001 and p=0.009, respectively) and without emphysema (p=0.001 and p=0.042). Although lung function was not decreased in ex-smokers without emphysema, $FEV_1/FVC$ and FEF25-75% were decreased in ex-smoker with emphysema (p=0.020 and p=0.010). Conclusion: Upon examination with LDCT, the prevalence of emphysema was higher in healthy smokers was than in non-smokers. Lung function was diminished in smokers with emphysema, in spite of former smoker.

  • Imaging assessment of prostate cancer is one of the most difficult sections of oncology imaging. Detecting, localizing and staging of the primary prostate cancer by preoperative imaging are still challenging for the radiologist. Magnetic resonance (MR) imaging provides excellent soft tissue contrast and is widely used for solid organ imaging, but results of preoperative imaging of the prostate gland with conventional MR imaging is unsatisfactory. Positron emission tomography and computed tomography (PET/CT) is the cornerstone in oncology imaging, but some limitations prohibit the assessment of primary prostate cancer with PET or PET/CT. Recent studies to overcome these insufficient accuracies of imaging evaluation of primary prostate cancers with advanced MR techniques and PET and PET/CT are reported. In this article, we review the imaging findings of prostate cancer on variable modalities, focused on MR imaging and PET/CT.

  • Magnetic resonance imaging (MRI) is an essential modality for the diagnosis of musculoskeletal system defects because of its higher soft-tissue contrast and spatial resolution. With the recent development of MRI-related technology, faster imaging and various image plane reconstructions are possible, enabling better assessment of three-dimensional musculoskeletal anatomy and lesions. Furthermore, the image quality, diagnostic accuracy, and acquisition time depend on the MRI protocol used. Moreover, the protocol affects the efficiency of the MRI scanner. Therefore, it is important for a radiologist to optimize the MRI protocol. In this review, we will provide guidance on patient positioning; selection of the radiofrequency coil, pulse sequences, and imaging planes; and control of MRI parameters to help optimize the MRI protocol for the six major joints of the musculoskeletal system.

The Usefulness of Cone Beam Computed Tomography in Diagnosis of Temporomandibular Joint Osteoarthritis (측두하악관절 골관절염 환자의 진단에서 Cone Beam 전산화 단층촬영의 유용성)

  • This study is designed to assess Relationship between clinical diagnosis of Temporomandibular Joint Disorder and diagnostic finding of Cone Beam Computed Tomography(CBCT) The author performed clinical examination for TMD Patients who visited Orofacial pain clinic, Jin-ju ooo Dental office. CBCT(Cone beam computed tomography) was taken for 190 joints in 95 subjects. A Oral medicine and Oral radiologist evaluated CBCT each other. then we compared with that result, Condyle bony changes were classified by no bone change, flattening, erosion and osteophyte. The obtained results were as follow: 1. The Kappa index of the diagnosis between oral medicine and oral radiogist were high, the index of diagnosis by degenerative joint disease were more higher. 2. The Kappa index of panoramic view and CBCT was low, more condylar bone chages were observed by CBCT diagnosis 3. Condylar bone changes of the 54.2% of non-DJD group clinicaly was observed by CBCT diagnosis and no bone changes of the 15.3% of DJD group.was observed by CBCT 4. TMJ pain was associated with erosion of condyle bone change of TMJ. Crepitation and longest duration of TMD were associated with osteophytic bone change.

Does Nasal Septal Deviation and Concha Bullosa Have Effect on Maxillary Sinus Volume and Maxillary Sinusitis?: A Retrospective Study (비중격 만곡증과 수포성 비갑개는 상악동 부피와 상악골 부비동염에 있어 관련성을 지니는가?: 후향적 연구)

  • Purpose This study aimed to determine whether nasal septal deviation and concha bullosa affect the maxillary sinus volume, and whether this effect is associated with the prevalence of chronic sinusitis. Materials and Methods This study retrospectively reviewed 209 paranasal sinus CT (PNS CT) images of patients with sinonasal symptoms from January 2017 to December 2018. The maxillary sinus volume was measured twice by a radiologist, and statistical analysis was performed using SAS 9.4. Results Intersex comparison of the maxillary sinus volume (on left and right sides) revealed that the volume was significantly larger on both the sides (p < 0.0001) in men compared with that in women. Concha bullosa was found to occur mainly in the concave cavity of the septal deviation (p < 0.0001). No significant association was found between nasal septal deviation and maxillary sinusitis (p = 0.8756) as well as between concha bullosa and maxillary sinusitis prevalence (p = 0.3401) or maxillary sinus volume (both: p = 0.6289, Rt.: p = 0.9522, Lt.: p = 0.9201). Conclusion Although nasal septal deviation and the location of concha bullosa may affect each other, maxillary sinus volume and maxillary sinusitis were neither associated with nasal septal deviation nor concha bullosa.

Radiologic Analysis of Congenital Origin Intestinal Obstruction in Neonate and Childhood (선천성 장관폐쇄의 방사선학적 분석)

  • Congenital origin intestinal obstruction are important disease due to required emergency operation. So accurate and rapid diagnosis is needed for decreased mortality and morbidity. Radiologist must detect to accurate obstruction site and also associated other congenital anomalies. And also embriological bases are very important role to the diagnosis of theses diseases. We were analysed radiologically and clinically 25 cases with congenital origin intestinal obstruction with review of literature. 1. Hypertrophic pyloric stenosis 6 cases, midgut malrotation 4 cases, congenital megacolon 8 case, imperforated anus 5 cases, ileal atresia 1 case and duodenal atresia 1 case. 2. Male and female radio were 16:9. Especially on hypertrophic pyloric stenosis, 5 cases were male infants. 3. All cases of hypertrophic pyloric stenosis represented string sign and also pyloric beak sign shoulder sign on UGI. 4. I case duodenal atresia showed double bubble sign on simple abdomen x-ray and ileal atresia showed mechanical small bowel obstruction sign with microcolon. 5. On midgut malrotation, cecum was located in right upper abdomen on 4 cases. And 2 cases were associated with Ladd's band. I case with volvulus and 1 case with mesenteric defect. 6. Involved site of all congenital megacolon were localized to rectosigmoid colon. 7. On 5 cases imperforated anus, 3 cases were low type and 2 case high type. Rectoperineal and rectourogenital fistula were demonstrated on 4 cases.

Morphological analysis of mandibular posterior edentulous bones using cross-sectional CT images (단면 CT 영상을 이용한 무치악 하악 대구치부 잔존골의 형태 분석)

  • Purpose: The purpose of this study was to evaluate the morphologic features of posterior edentulous mandible for Korean patients using cross-sectional CT images. Materials and Methods: Computed tomographic cross-sectional views taken in 2004 and 2005 at Seoul National University Dental Hospital were analyzed by an oral and maxillofacial radiologist. Four indices were measured to meet the purpose of this study: 1) the horizontal distance between the alveolar crest and mandibular canal (Type), 2) the angle of the mandibular long axis (Angle), 3) the bucco-lingual location of mandibular canal, and 4) the depth of the submandibular fossa. The averages and standard deviations of the measurements were compared according to the location (the first and second molar area) and sex of the patients. Results: Statistically significant difference was found in Type, Angle, and submandibular fossa depth between the first and second molar area (p<0.05). However, there was no significant difference between men and women in any of the measured indices. Most of the mandibular canals were located in lingual side of posterior mandible. Conclusion: More care should be taken when an implant is installed on the mandibular second molar area.

Breast Imaging Using Electrical Impedance Tomography: Correlation of Quantitative Assessment with Visual Interpretation

  • Background: Electrical impedance tomography (EIT) is a new non-invasive, mobile screening method which does not use ionizing radiation to the human breast; allows conducting quantitative assessment of the images besides the visual interpretation. The aim of this study was to correlate the quantitative assessment and visual interpretation of breast electrical impedance tomographs and associated factors. Materials and Methods: One hundred and fifty mammography patients above 40 years and undergoing EIT were chosen using convenient sampling. Visual interpretation of the images was carried out by a radiologist with minimum of three years experience using the breast imaging - electrical impedance (BI-EIM) classification for detection of abnormalities. A set of thirty blinded EIT images were reinterpreted to determine the intra-rater reliability using kappa. Quantitative assessment was by comparison of the breast average electric conductivity with the norm and correlations with visual interpretation of the images were determined using Chi-square. One-way ANOVA was used to compare the mean electrical conductivity between groups and t-test was used for comparisons with pre-existing Caucasians statistics. Independent t-tests were applied to compare the mean electrical conductivity of women with factors like exogenous hormone use and family history of breast cancer. Results: The mean electrical conductivity of Malaysian women was significantly lower than that of Caucasians (p<0.05). Quantitative assessment of electrical impedance tomography was significantly related with visual interpretation of images of the breast (p<0.05). Conclusions: Quantitative assessment of electrical impedance tomography images was significantly related with visual interpretation.

Evaluation of ROC Curve in High Kilovoltage Technique Using Simulated Nodules on Chest (고관전압 흉부촬영의 가상결절을 이용한 ROC평가)

  • With transmitted dose through chest which has the problem of wide variations in absorption, simple film/screen combination method makes it diffucult to image lung field, mediastinum and retrocardiac areas. In order to overcome this, it is very common to use the high kilovoltage technique in diminishing the differences between high and low contrast. We have been adopting this method at department of diagnostic radiology, Seoul National University Hospital. To compare the image of it with that of low kilovoltage technique, we did radiographic tests using beans on the skin. We marked off into three anatomical categories such as lungs, mediastinum and near diaphragm, then attached a bean on a marked area at random. In order to compare with high and low, we took a radiography of high($120{\sim}140\;kVp$) and low($70{\sim}90\;kVp$) kilovoltage tehchniques, respectively at the same time. We have done experiments 320 cases. We evaluated the results of test in response to sensitivity(true positive) and specificity(true negative). In evaluating, we gave them points from 1 to 5 according to true or false. With given points by a radiologist having much experiences, we could acquire the percentage of sensitivity and specificity. The percentage made us to get the schematic table of ROC curve of those two methods. Consequently, high kilovoltage technique appeared 18% better than low kilovoltage technique for detecting beans with our apparatus.

  • Medical imaging has increased rapidly in the increase of interest in health, with the development of computer technology, digitization of medical imaging is rapidly advancing, PACS has been introduced to the medical field. Increase in the production of medical images by these phenomena made increased the workload of radiologist who must read a medical image. in response to the need for secondary diagnosis using a computer, The term of CAD in medical radiology field was introduced. In this study, we have proposed a CAD algorithm for the interpretation of the image obtained by the digital X-ray mammography equipment. The experiments were performed by programmed in Visual C++ for the proposed algorithm. A result of the execution of the CAD algorithm seven sample images, the results of five samples was confirmed in breast cancer and benign tumors, both the images sample was error processing. If you use a program that implements this with the algorithm proposed in this study it is helpful to reading breast images, and it is considered to contribute significantly to the early detection of breast cancer.
