  • Meridian collateral and meridian points have been the base of acupuncture and moxibustion therapy. Also the theory have composed the main portion of Oriental Medicine. But the mechanism and scientific background has not been completely eatablished, and the research on the objectification of diagnosis of meridian collateral and meridian points, and acupuncture & moxibustion therapy has been necessary nowadays. A new understanding of value of Oriental Medicine has been increasing, the scientific understanding of meridian collateral and meridian points should have been examined. The system of meridian collateral and meridian points was very interesting topics between the scientists in the world. Especially, the elucidation of function and mechanism of Qi(氣) was very important in the scientific theme of 21th century. But there has been many difficulties in the study of meridian collateral and meridian points, since the system of meridian collateral and meridian points has the complexed function and vague structure in the organism. As the one of index of meridian points, the electric current has been used. In this report, the volume of electric current on the skin around the meridian points was investigated. The results of investigation showed the meridian points have higher electric current volume than non-meridian points.

A Clinical Analysis to Study Effectiveness of Korean Medicine for Medial Collateral Ligament Injury of the Knee (무릎 내측측부인대 손상의 한의학적 치료에 대한 국내외 임상연구 동향 분석)

  • Objectives This study aimed to review clinical studies on traditional Korean medicine treatment for medial collateral ligament injury of the knee. Methods Clinical studies on Korean traditional medicine treatment of medial collateral ligament injury were conducted. We used five Korean online databases (OASIS, KISS, RISS, DBPia, and ScienceOn) and three foreign databases (PubMed, Cochrane Library, and CNKI). Out of 99 studies that were found, we excluded repeated articles, studies that were not related to Korean medicine, and those not relevant to the topic of the study. Results Ten randomized controlled trials and 20 case studies were selected. Eight traditional Korean medicine treatments, including acupuncture, herbal medicine, chuna, and herbal ointment, were used in these studies. The most commonly used treatment was found to be acupuncture. Conclusions Our study showed that traditional Korean medicine for medial collateral ligament injuries was effective. However, there were some limitations. Further clinical studies and randomized controlled clinical trials are needed for more evidence on Korean traditional medicine.

Study on the Relationship Between 12Meridians Flow and Facial Expressions by Emotion (감정에 따른 얼굴 표정변화와 12경락(經絡) 흐름의 상관성 연구)

  • Facial expression was an important communication methods. In oriental medicine, according to the emotion the face has changed shape and difference occurs in physiology and pathology. To verify such a theory, we studied the correlation between emotional facial expressions and meridian and collateral flow. The facial region divided by meridian, outer brow was Gallbladder meridian, inner brow was Bladder meridian, medial canthus was Bladder meridian, lateral canthus was Gallbladder meridian, upper eyelid was Bladder meridian, lower eyelid was Stomach meridian, central cheeks was Stomach meridian, lateral cheeks was Small intestine meridian, upper and lower lips, lip corner, chin were Small and Large intestine meridian. Meridian and collateral associated with happiness was six. This proves happiness is a high importance on facial expression. Meridian and collateral associated with anger was five. Meridian and Collateral associated with fear and sadness was four. This shows fear and sadness are a low importance on facial expression than different emotion. Based on yang meridian which originally descending flow in the body, the ratio of anterograde and retrograde were happiness 3:4, angry 2:5, sadness 5:3, fear 4:1. Based on face of the meridian flow, the ratio of anterograde and retrograde were happiness 5:2, angry 3:4, sadness 3:5, fear 4:1. We found out that practical meridian and collateral flow change by emotion does not correspond to the expected meridian and collateral flow change by emotion.

A Case of Inferior Vena Cava Obstruction Accompanying Pericardiacophrenic Collateral Circulation (심막횡격정맥 측부순환을 동반한 하대정맥 폐쇄 1예)

  • The obstruction of inferior vena cava(IVC) is uncommon condition. The classification is based on the obstructive sites of major anatomic segments of IVC. The main collateral pathways of interruption of IVC were central channels through ascending lumbar veins, intervertebral veins and azygos-hemiazygos complex. However, the complete obstruction of mid-portion of IVC, accompanying collateral cirulation with pericardiacophrenic vein was rarely reported. We had experienced a case of complete obstruction of mid-portion of IVC with lobulated left cardiac border, which was unforgettable characteristic finding on chest radiograph. It was confirmed by venographic examination that the lobulated left cardiac shadow was a collateral circulation of pericardiacophrenic vein.

  • Complete occlusion of the main LCA is associated with a poor prognosis if the RCA becomes severely occluded. The functional role of collateral circulation is critical in this situation. This study lends support to the hypothesis that coronary collateral circulation can provide critically needed myocardial blood flow via right to left collaterals. Not surprisingly, left ventricular function was maintained in 4 of the 5 patients studied who had main LCA occlusion and well-developed collateral circulation.

  • The collateral diagnosis method is unique in Traditional Chinese Medicine diagnosis methods which has important clinical value. In my article, the contents of the "Hwangjenaegyeong(黃帝內經)" related to this method is discussed. According to the site of inspection in the diagnosis process, there are five types as following: inspection of the face and surface, the thenar, the orifices, abdominal collaterals and finally the index finger of children. This method can inspire clinical practitioners.

  • A patient underwent reconstruction of skin and soft tissue using V-Y advancement of a superior ulnar collateral artery perforator flap after resection of the scar tissue on the upper arm. Successful flap healing was observed without complications. The medial side of the upper arm is an ideal donor site because of its thin, elastic, and hairless skin, resulting in a well-hidden scar. The elasticity of the medial side of the upper arm allows primary closure after flap elevation. The superior ulnar collateral artery perforator flap is an option for reconstruction of the upper arm.

  • Not only morphology of pulmonary artery or pulmonary artery resistance, but major aortopulmonary collateral arteries are risk factors of Fontan procedure. We report a successful Fontan procedure after rehabilitation of pulmonary arteries by unifocalization and systemic to pulmonary shunt in a high risk Fontan candidate with functional single ventricle combined with hypoplastic pulmonary arteries and major aortopulmonary collateral arteries supplying most of the bilateral lung field.

  • Coronary arterial involvement in Takayasu arteritis (TA) is not uncommon. Herein, we describe a case of TA with celiac trunk and superior mesenteric artery occlusion combined with coronary artery disease. Bilateral huge internal thoracic arteries (ITAs) and the inferior mesenteric artery provided the major visceral collateral circulation. After percutaneous intervention to the right coronary artery, off-pump coronary artery bypass grafting for the left coronary territory was done using a right ITA graft and its large side branch because of its relatively minor contribution to the visceral collateral circulation.

  • Recurrent dislocation of elbow joint occurs relatively rarely by the injury of the collateral ligament which contributes elbow joint stability. Among them, posterolateral rotatory instability occurs by the injury to the lateral ulnar collateral ligament. We experienced a case of recurrent dislocation of elbow joint due to posterolateral rotatory instability. We treated operatively with lateral ulnar collateral ligament reconstruction using the palmaris longus tendon by technique of Nestor et al. We report it with literature analysis.
