Diabetic Atherosclerosis and Glycation of LDL(Low Density Lipoprotein)

  • Diabetes carries an increased risk of atherosclerotic disease that is not fully explained by known car-diovascular risk factors. There is accumulating evidence that advanced glycation of structural proteins, and oxidation and glycation of circulating lipoproteins, are implicated in the pathogenesis of diabetic ather-osclerosis. Reactions involving glycation and oxidation of proteins and lipids are believed to contribute to atherogenesis. Glycation, the nonenzymatic binding of glucose to protein molecules, can increase the ather-ogenic potential of certain plasma constituents, including low density lipoptotein(LDL). Glycation of LDL is significant increased in diabetic patients compared with normal subjects, even in the presence of good glycemic control. Metabolic abnormalities associated with glycation of LDL include diminished recognition of LDL by the classic LDL receptor; increased covalent binding of LDL in vessel walls ; enhanced uptake of LDL by the macrophages, thus stimulating foam cell formation ; increased platelet aggregation; formation of LDL-immune complexes ; and generation of oxygen free radicals, resulting on oxidative damage to both the lipid and protein components of LDL and to any nearby macromolecules. Oxidized lipoproteins are characterzied by cytotoxicity, potent stimulation of foam cell formation by macrophages, and procoagulant effects. Combined glycation and oxidation, "glycoxidation" occurs when oxidative reactions affect the initial products of glycation, and results in irreversible structural alterations of proteins. Glycoxidation is of greatest significance in long lived proteins such as collagen. In these proteins, glycoxidation products, believed to be atherogenic, accumulate with advancing age : in diabetes, their rate of accumulate is accelerated. Inhibition of glycation, oxidation and glycoxidation may form the basis of future antiaterogenic strategies in both diabetic and nondiabetic individuals.dividuals.

Romosozumab의 효과 및 심혈관계 안전성: 체계적 문헌고찰 및 메타분석 (Efficacy and Cardiovascular Safety of Romosozumab: A Meta-analysis and Systematic Review)

  • Background: This systematic review and meta-analysis aimed to evaluate the efficacy and cardiovascular safety of romosozumab compared with placebo. Methods: Randomized controlled trials (RCTs) were searched from Medline, EMBASE, Cochrane Central, and Web of Science until July 2022. Primary outcomes included the change in bone mineral density (BMD) from baseline at month 6. The secondary outcomes were the change of bone turnover markers (N-terminal propeptide of type 1 procollagen (P1NP); C-terminal telopeptide of type 1 collagen (CTX)) from baseline at month 3, and the incidence of cardiovascular adverse events for the total follow-up period. Results: A total of 7 RCTs on 8,370patients were included. Romosozumab showed better effects in improving BMD in both lumbar spine and femoral neck at month 6 (standardized mean difference, SMD 2.20 [95% CI: 1.89-2.52], SMD 0.63 [95% CI: 0.41-0.86]). In contrast to placebo, romosozumab significantly increased PINP levels and reduced CTX levels at month 3 (SMD 0.93 [95% CI: 0.65-1.22], SMD -1.03 [95% CI: -1.23~ -0.82]. However, there was no significant difference in the composite incidence of cardiovascular adverse events and major adverse cardiovascular events (OR 1.16 [95% CI: 0.82-1.65], OR 1.08 [95% CI: 0.75-1.56]). Conclusion: This analysis showed that romosozumab significantly improved BMD compared to placebo and was beneficial for change in bone turnover markers. There is no significant difference in the incidence of cardiovascular adverse events compared to placebo.

Anew formula CPC22 regulates bone loss, hot flashes, and dysregulated lipid metabolism in ovariectomized postmenopausal mice

  • Background and objective: A new formular CPC22 consists of Cynanchum wilfordii root, Pueraria thomsonii flower, and Citrus unshiu peel and has been developed to improve the postmenopausal symptoms. The research intended to evaluate whether CPC22 would regulate bone loss, hot flashes, and dysregulated lipid metabolism in ovariectomized (OVX) postmenopausal mice. Method: The OVX mice were orally administered with CPC22 daily for 7 weeks. Results: CPC22 regulated OVX-induced bon loss by enhancing serum osteoprotegerin, alkaline phosphatase, and osteocalcin levels and diminishing serum receptor-activator of the NF-κB ligand (RANKL), collagen type 1 cross-linked N-telopeptide, and tartrate-resistant acid phosphatase levels. As a result of CPC22 treatment, notable decreases in tail skin temperature and rectal temperature were observed, along with diminishment in hypothalamic RANKL and monoamine oxidase A levels and enhancement in hypothalamic serotonin (5-HT), norepinephrine, dopamine, 5-HT2A, and estrogen receptor-β levels. CPC22 enhanced levels of serum estrogen and diminished levels of serum follicle-stimulating hormone and luteinizing hormone. CPC22 regulated levels of serum lipid metabolites, including total cholesterol, triglycerides, low-density lipoprotein cholesterol, and high-density lipoprotein cholesterol. Furthermore, CPC22 diminished levels of serum blood urea nitrogen, creatine kinase, alanine transaminase, aspartate aminotransferase, and lactate dehydrogenase and restored vaginal dryness without affecting uterus atrophy index and vagina weights. Conclusion: Therefore, these results indicated that CPC22 improves OVX-induced bone loss, hot flashes, and dysregulated lipid metabolism by compensating for estrogen deficiency without side effects, suggesting that CPC22 may be used for the prevention and treatment of post menopause.

계지복령환(桂枝茯笭丸) 및 그 구성약물(構成藥物)의 혈소판응집억제(血小板凝集抑制)에 관(關)한 연구(硏究) (Effect of Geijibokryunghwan and each constituent herb on inhibition of platelet aggregation)

  • 동물성(動物性) 지방섭취량(脂肪攝取量)의 증가(增加), 운동부족(運動不足), 비만(肥滿), 스트레스의 가중(加重), 고령화(高齡化)의 증가(增加) 등(等)의 원인(原因)으로 순환기계질환(循環器系疾患)의 발병률(發病率)이 증가(增加)하고 있으며, 이러한 순환기계질환(循環器系疾患)의 위험인자(危險因子)로서 혈전증(血栓症)이 중요(重要)하게 대두되고 있다. 특히 최근 문제시되고 있는 협심증(狹心症)이나 심근경새(心筋梗塞)등의 허혈성(虛血性) 심질환(心疾患)은 혈소판응집(血小板凝集)에 의해 일어나는 혈전형성(血栓形成)에 기인(起因)하고 있다. 한의학(韓醫學)에서 혈전증(血栓症)은 어혈(瘀血)의 범주(範疇)에 속(屬)하며, 어혈(瘀血)은 각종 병리적(病理的) 원인(原因)에 의해 발생한 전신성(全身性) 또는 국소성(局所性)의 혈액순환(血液循環) 장애(障碍) 또는 혈류정체(血流停滯)와 그에 수반되는 일련의 증후(症候)를 나타내며, 경계정충, 고창(鼓脹), 적취(積聚), 미하, 전광(癲狂), 중풍등(中風等)의 발병원인(發病原因)이 된다. 또한 어혈(瘀血)에 의한 각종 증후(症候)에는 활혈거어제(活血祛瘀劑) 또는 구어혈제(驅瘀血劑)등이 사용되고 있다. 본(本) 연구(硏究)에서는 한의학(韓醫學)에서 어혈증(瘀血症)으로 야기(惹起)되는 여러 가지 증상(症狀)의 개선에 사용되는 구어혈제(驅瘀血劑)들의 혈소판응집(血小板凝集)에 미치는 영향을 검색하기 위하여 계지복령환(Geijibokryunghwan; GBH) 및 그 구성약물(構成藥物)을 사용(使用)하였다. 계지복령환은 "금궤요략" 에 있는 방(方)으로써 거사부상정(祛邪不傷正)하고 조기한열(調氣寒熱)하여 예로부터 구어혈제(驅瘀血劑)로 사용되어 왔다. 이에 계지복령환 및 그 구성약제(構成藥劑)의 ADP, AA 또는 collagen으로 유도되는 혈소판응집(血小板凝集)에 대하여 억제효과(抑制效果)를 탐색(探索)한 결과(結果), 계지복령환 및 개별(個別) 구성약물(構成藥物)의 혈소판응집억제작용(血小板凝集抑制作用)을 확인하였고, 혈소판응집(血小板凝集)으로 야기(惹起)되는 혈전증(血栓症)등에 계지복령환 및 개별(個別) 구성약물(構成藥物)은 매우 임상실험적(臨床實驗的) 응용가치(應用價値)가 있는 것으로 생각되었다.

남원자생 허브의 혼합추출물이 창상치유에 미치는 영향 (The Effect of Extract from Several Herbs Grown Naturally in Namwon Province on Wound Treatment)

  • Purpose: This study was aim to examine the effect of extract from several herbs on wound treatment which are grown naturally in Namwon province. Methods: Adult male Sprague-Dawley rats were assigned to 6 of 2 group: control group (C, n=6), experimental group (E, n=6). Rats in E group had 3 sessions (everyday) for 4 days or 8 days of the extract treatment which was composed of German chemomile, Teatree and Lavender (CTL). Results: CTL extract on wound-induced mice showed significantly the increase of wound treatment rate (WTR), epidermal gap (EP) and collagen density (CD), compared with control. Conclusion: CTL extract may be a putative resources for cure or treatment of wound.

치아이동(齒牙移動)이 백서(白鼠) 치주조직(齒周組職)에 미치는 영향(影響)에 관(關)한 실험적(實驗的) 연구(硏究) (AN EXPERIMENTAL STUDY ON THE PERIODONTAL TISSUE REACTION TO TOOTH MOVEMENT IN THE RAT)

  • For the purpose of investigating the response of the periodontal tissue in relation to the experimental tooth movement, the rats were inhibited from collagen formation by adminstration of aminoacetonitrile. Findings were as follows: 1) In experimental group, the principal fibers of the periodontal ligament came to run parallel along the alveolar bone or root surface on the pressure side, while the decrease in density, irregular arrangement, and partial loss of principal fibers were observed on the tension side. 2) Sharpey's fibers at the alveolar bone decreased in number, and as the aminoacetonitrile administration continued, the capability of matrix formation decreased on the tension side, the narrowing of the alveolar septum and poor-bony trabeculation appeared on the pressure side. 3) In cementum, Sharpey's fibers were distributed irregularly. The formation of acellura cementum was decreased on the tension side, while the formative capability of cellular cementum was increased. 4) The degree of staining by Herovici technique decreased in the periodontal membrane. By PAS and ninhydrine -Schiff reaction it was appeared weakly positive in the region where the Sharpey's fibers existed.

당뇨유발 모델에서 맥동성초음파와 키토산의 창상치유효과 (The Effect of Wound Healing of Pulsed Ultrasound and Chitosan in Diabetic Induced Model)

  • The studies have been designed to evaluate the effect of wound healing of pulsed ultrasound and chitosan application in diabetic incisive wound of rats. Mild diabetes mellitus was induced in rat used 30 mg/kg streptozotocin. Full thickness skin incision was made on the backs of Sprague-Dawley rats. We used 72 Sprague-Dawley rats which were divided into 4 groups; the subjects were divided into group of 6 rats each 3, 6, and 15 days. The results were summarized as follows; The rate of wound length of pulsed ultrasound with chitosan application groups more decreased than only pulsed ultrasound treatment group. The density of inflammatory cells in the experimental groups was more significantly decreased than diabetic control group(p<0.05). Historically, in the ultrasound with chitosan application groups, reepithelized epithelium was thicker and the collagen fiber were organized in a liner manner and connective tissue was matured faster those of the diabetic control group(p<0.05). From the conclusions above, in this study application of pulsed ultrasound and chitosan can be an effective way of promotion of wound healing in diabetic model.

($TGF-{\beta}$)이 Minocycline을 전처리한 사람 치주인대세포의 활성에 미치는 영향 (Effects of $TGF-{\beta}1$ on Cellular Activity of Minocycline-Pretreated Human Periodontal Ligament Cells)

  • The initial events required for periodontal regeneration is the attachment, spreading, and proliferation of appropriated cells at the healing sites. These have been reported that minocycline stimulates the attachment of periodontal ligament cells, and also $TGF-{\beta}1$ enhances the proliferation of periodontal ligament cells. The purpose of the present study was to evaluate the effects of $TGF-{\beta}1$ on the cellular activity of minocycline treated human periodontal ligament cells. Periodontal ligament cells were obtained from the explants of healthy periodontal ligaments of extracted 3rd molars or premolar teeth extracted from the patients for orthodontic treatment. The cells were cultured in minimal essential medium(${\alpha}-MEM$) supplemented with 10.000units/ml penicillin, $10,000{\mu}g/ml$ streptomycin and 10% FBS(fetal bovine serum) at $37^{\circ}C$ in a humidified atmosphere of 5% carbon dioxide and the 5th to the 8th passages of the cells were used. To evaluate the effect of minocycline on cell attachment, the cells were seeded at a cell density of $1.5{\times}10^4$ cells/well in 24-well culture plates and treated with $20{\mu}g/ml$ and $100{\mu}g/ml$ of minocycline for 1.5 h. After trypsinization, the cells were counted with hemocytometer and were taken photographs for observation of cellular morphology. To evaluate the effect of $TGF-{\beta}1$ on minocycline-pretreated periodontal ligament cells, the cells were seeded at a cell density of $1{\times}10^4$ cells/ well in 24-well culture plates and treated with $20{\mu}g/ml$ and $100{\mu}g/ml$ of minocycline for 1.5 h. After incubation, 1 and 10ng/ml of $rh-TGF-{\beta}1$ were also added to the each well and incubated for 1 and 2 days, respectively. Then, MTT assay, DNA synthesis($^3H-thymidine\;assay$), and protein and collagen assay(3H-proline assay) were carried out. In the MIT assay, after 200ul MTT solutionlconeentration of 5mg/ml) were added to the each well of the 24-well plates and incubated for 3 hours, and 200 ul DMSO were added so as to dissolve insoluble blue formazan crystals which was formed in incubated period. Then it read plates on a ELISA reader. For mitogenic assay, 1 uCi/ml $^3H-thymidine$ was added to each well for the final 2 hours of the incubation periods. After labeling, the wells were washed 3 times with ice cold PBS and 4 times with 5% TCA to remove unincorporated label and precipitate the cellular DNA. DNA, with the incorporated $^3H-thymidine$, was solubilized with 500 ul of 0.1% NaOH/0.1% SDS. A 250 ul aliquot was removed from each well and placed in a scintillation vial with 4ml of scintillation cocktail. Using an liguid scintillation counter, counts per minute(CPM) were determined for each samples. 3 uCi/ml $^3H-proline$ was added to each well for the final 4 hours of the incubation periods and total protein and percent collagen synthesis were carried out. The results indicate that minocycline treated group with $100{\mu}g/ml$ concentration for 1.5 hours significantly increased than that of control in cell attachment, and cell process is also evident compared with that of control in cell morphology, and the cellular activity and DNA synthesis rate of cells treated minocycline and $TGF-{\beta}1$ significantly increased than that of control values, but were below to values of the $TGF-{\beta}1$ only treated group in MIT assay and $^3H-thymidine\;assay$, and the total protein synthesis of minocycline and $TGF-{\beta}1$ treated group also significantly increased than that of control values, but the percent collagen synthesis of tested group significantly decreased to compared with control. On the above the findings, the tested group of minocycline and $TGF-{\beta}1$ did not increase the effect on the cell activity than $TGF-{\beta}1$ only tested group and the tested group of minocycline inhibited cell activity. This results indicate that minocycline was effective on cell attachment in early stage, but it is harmful to cell activity, that inhibitory effect of minocycline was compensated with stimulatory effect of $TGF-{\beta}1$.

신생 흰쥐와 성숙 흰쥐 각막의 미세구조 변화에 관한 연구 (An Ultrastructral changes of the new-born rats and adult rat's cornea)

  • 본 연구는 발생과정에 따른 흰쥐 각막의 변화를 주사 및 투과형 전자현미경으로 관찰하였다. 생후 1일된 안검이 개검되지 않은 흰쥐의 각막 60안, 생후 10주된 성체 흰쥐의 각막 40안을 적출하여 각막의 변화를 비교 연구 하였다. 연령증가에 따라 각막의 미세구조의 변화가 나타났으며 각막의 상피는 오각형의 모양이며, 세포간 경계는 분명하였으나 성장할수록 탈락세포는 짙은 전자밀도를 나타냈으며, 새로 생성된 세포는 밝은 전자밀도를 나타내어 명암의 차이가 뚜렷하였다. 보우만층은 연령의 증가에 따라 콜라겐섬유가 불균질한 구조에서 균질한 구조로 변화하였다. 실질층의 각막세포는 세포 소기관의 밀도가 성체로 갈수록 감소하였고, 콜라겐층은 연령증가에 따라 종횡으로 평행배열을 나타내었다. 데스멧층은 성체로 갈수록 "banded layer"에서 "non-banded layer"의 형태로 변화하였으며 층의 두께가 현저히 두꺼워진것이 관찰되었다. 내피층은 단층으로 구성되어 있으며, 성체로 갈수록 내피 세포수는 감소하는 반면 감소된 세포수의 자리는 세포 크기가 확대됨으로써 보상되었다.

생쥐 자궁조직 내 비만세포의 연령별 밀도 및 분포 변화 (Age-dependent Changes in Density and Distribution of Mouse Uterine Mast Cells)

  • 비만세포는 세포질 내에 다양한 신호전달물질을 과립형태로 함유하고 있는 세포로써, 피부, 기도, 소화관 등의 점막과 결합조직에 주로 분포하고 있으며, 염증반응, 자기방어, 조직재생, 자가면역질환 등 다양한 생리적, 병리적 현상에 관여하고 있는 면역세포이다. 본 연구는 생쥐 연령별 자궁의 발달과 퇴행에 따른 자궁조직 내 비만세포의 분포와 밀도변화를 조사함으로써, 생쥐 자궁에서 비만세포의 기능을 알아보고자 실시하였다. 자궁조직 내 비만세포는 발정주기가 시작되는 생후 6주 이전에는 매우 적은 수가 관찰되었으나, 생후 7주부터 자궁의 조직형태적 발달과 더불어 급격히 증가하기 시작하여 32주에 이르기까지 지속적으로 증가하였다. 그러나 생후 38주부터는 자궁조직의 퇴행과 더불어 비만세포의 밀도도 감소하였다. 비만세포는 자궁의 근층조직에서 주로 관찰되었으며, 주요 세포외기질인 교원섬유도 자궁의 발달, 비만 세포의 밀도 증가와 더불어 그 함량이 증가하였다가 자궁의 퇴행과 함께 감소하였다. 전자현미경적 관찰에서 비만세포는 자궁 근층조직에서 평활근세포, 섬유모세포, 교원섬유와 근접하고 있는 형태로 관찰되었다. 이상의 연구 결과는 비만세포가 자궁에서의 면역기능에 중요한 역할을 할 뿐만 아니라, 분만, 생리주기에 따른 자궁조직의 재생 및 재구성, 그리고 평활 근조직의 수축 등에도 관여할 수 있음을 시사하고 있다.
