Roles of Health Technology Assessment for Better Health and Universal Health Coverage in Korea (우리나라 보건의료 발전을 위한 의료기술평가의 역할)

  • Health technology assessment (HTA) is defined as multidisciplinary policy analysis to look into the medical, economic, social, and ethical implications of the development, distribution, and use of health technology. Following the recent changes in the social environment, there are increasing needs to improve Korea's healthcare environment by, inter alia, assessing health technologies in an organized, timely manner in accordance with the government's strategies to ensure that citizens' medical expenses are kept at a stable level. Dedicated to HTA and research, the National Evidence-based Healthcare Collaborating Agency (NECA) analyzes and provides grounds on the clinical safety, efficacy, and economic feasibility of health technologies. HTA offers the most suitable grounds for decision making not only by healthcare professionals but also by policy makers and citizens as seen in a case in 2009 where research revealed that glucosamine lacked preventive and treatment effects for osteoarthritis and glucosamine was subsequently excluded from the National Health Insurance's benefit list to stop the insurance scheme from suffering financial losses and citizens from paying unnecessary medical expenses. For the development of HTA in Korea, the NECA will continue exerting itself to accomplish its mission of providing policy support by health technology reassessment, promoting the establishment and use of big data and HTA platforms for public interest, and developing a new value-based HTA system.

Economic Evaluation of Eastern, Western and Collaborative Treatments for Patients with Frozen Shoulder Pain (견비통의 한${\cdot}$양방 진료 및 협진의 경제성 평가)

  • Objectives : The purpose of this study was to evaluate and compare the cost-effectiveness of Eastern, Western, and collaborative treatments and suggest a cost-effective approach for patients with frozen shoulder pain. Methods : Using the data of fifty-two patients, treatment effectiveness was measured by CSA, SPABI, and ROM scales and changes from the baseline score were evaluated. Data source for cost estimation was based on the national health insurance (NHI) payment system. Because the price in NHI was differentiated by health care institutions, five collaborative types were considered in assessing costs. Cost-effective ratios were computed for economic evaluation. Results : Compared with Eastern treatment, collaborative and Western treatments showed better effects on CSA scale after 4 weeks' treatment. The collaborative approach was also the most effective treatment on SPADI and ROM scales. The direct cost per patient receiving Eastern treatment was less than other treatments. In general, collaborative treatment dominated Eastern and Western treatments in cost-effectiveness an analysis. However, the cost-effectiveness ratio of Eastern treatment resulted in \9,000 compared to \29,000 of collaborative treatment on SPADI. Four different indicators of ROM scales resulted in different approaches as the cost-effective treatment. Conclusions : Considering cost-effectiveness ratios, collaborative treatment was the best treatment on CSh and SPADI scales after 4 weeks' treatment. As for ROM scales, the recommended alternatives were Eastern treatment for patients with abduction and adduction disabilities, Western treatment for those with flexion disability, and collaborative approach fir those with extension disabiliry.

  • As e-commerce developing rapidly, it is becoming a research focus about how to find customer's behavior patterns and realize commerce intelligence by use of Web mining technology. One of the most successful and widely used technologies for building personalization and goods recommendation system is collaborating filtering. However, collaborative filtering have serious data sparsity problem. Traditional association rule does not consider user's interests or preferences to provide a user with specific personalized service.In this paper, we propose an goods recommendation system, which is integrated an collaborative filtering algorithm with item-to-item corelation and an improved Apriori algorithm. This system has user's interests or preferences ro provide a user with specific personalized service.

Data-Linking Infrastructure for the Health Technology Assessment (의료기술평가 기반으로서의 데이터 연계)

  • With the recent change of healthcare environment including rapid technological development, evidences are more and more important and necessary to support relevant policies in health technology assessment to provide safe and effective health services, utilizing medical resources efficiently. Despite of the emphasis on the importance of real world data and real world evidence in health care research, current infrastructure supporting clinical research is considerably weak due to absence of legal and institutional basis. However, in accordance with the Article 26 of the Health and Medical Technology Promotion Act, there is a limited legal apparatus that can be used only in public data with other dataset for the purpose of healthcare technology assessment at the National Evidence-based Collaborating Agency. Although the use of linked data from various sources was often required in the field of clinical research, it was not yet working well due to insufficient environmental conditions. In order to support the decision-making of medical practice and health care policies, data-linking platform for clinical research is needed. If the legal system that can link up to the data of the private institutions without violating the significant value such as the protection of private informations is established, it will be a decisive foundation reinforcing the researches and policy making processes for the improvement of the national health care system.

A study on the current status and development of the new health technology assessment of Korean medicine field (한의 신의료기술 평가 활성화 방안 제언)

  • Objectives: The purpose of this study is to examine the current status of Korean medicine health technology assessment and explore realistic plans to activate it. Methods: We investigated all the applications for new health technology assessment related to Korean medicine from 2007 to 2016. The several expert meetings were held to draw out the barriers and improvement strategies of the new health technology assessment of Korean medicine field. Results: There were 31 cases in total except for duplications or reapplies falling into 3 main types. First, 19 of them were to try to enter a medical market and be covered by National Health Insurance. Eight cases were to apply western medicine technology as new health technology in Korean medicine area. The rest was 4 cases, which were totally not appropriate for the purpose of new health technology assessment system. According to the expert opinion, the obstacles of activation in new health technology assessment of Korean medicine were application of unstandardized technology, lack of understanding and experience, lack of clinical trial supporting system for Korean medicine, lack of committee members within the nHTA(new Health Technology Assessment) review board, ambiguous definition of medical practice and sharp conflict between western medicine and Korean medicine. Conclusions: Several suggestions were derived. First of all, to activate Korean medicine in the nHTA system, the existing system should be used sufficiently, and multifaceted efforts are needed to upgrade the system, if necessary. Also, self-help efforts, Korean medicine clinical trial supporting system and increasing R&D investment, establishing extra-committee for Korean medicine in nHTA could be needed. Finally, long-term strategy for improving collaboration between Korean medicine and western medicine should be considered.

Effects of amino acids on ethanol metabolism and oxidative stress in the ethanol-perfused rat liver

  • One mechanism of free-radical production by ethanol is suggested to be through the intracellular conversion of XDH to XO by increased ratio of NADH to NAD. The major mechanism for physiological compensation of cytosolic NADH/NAD balance is the malate/aspartate shutfie. Therefore, it is important to develop the method to improve the efficiency of malate/aspartate shuttle in ethanol metabolism. In the present study, various amino acids and organic acid involved in the shuttle were tested for their functional efficiency in modulating shuttle in the ethanol-perfused rat liver. The rate of ethanol oxidation in the liver perfused with aspartate alone or aspartate in combination with pyruvate, respectively, was increased by about 10% compared to control liver, but not in the tissues perfused with glummate, cysteine or pyruvate alone. Though glummate, cysteine and pyravate did not affect the ethanol oxidation significanfiy, they showed some suppresive effect on the ethanol-induced radical generation monitored by protein carbonylation analysis. Among the tested components, aspartate is confirmed to be the most efficient as a metabolic regulator for both ethanol oxidation and ethanol-induced oxidative stress in our perfusion system. These effects of aspartate would result from NAD recycling by its supplementation through the coupled aspartate aminotransferase/malate dehydrogenase reactions and the malate-aspartate shuttle.

  • Recently broadcasting production companies have imported a number of broadcast program formats to get ideas quickly and easily. The term format in broadcasting industry is defined as a paper form which includes principal elements throughout each episode of the series in a program to keep consistency. In case of the quiz program titled 1:100 which is being aired in Korea, the domestic company imported the format and produced to broadcast in Korea. In other words, a format includes components of the inherent creative ideas and proposals needed in production a program. The study proposed a web based collaborative system that many users create the format for the broadcast program together. the system is constructed to combine creative ideas to making them a format. Also, production experts will take advantage of the system.

  • Objectives : The warm needling technique is the method in combining the effects of acupuncture needle with the effects of moxibustion. We need to standardize the characteristics of the warm needling technique in order to get more systematic and objective result in operation mechanism and effects and then get more clinical abilities in these fields. Methods : In this study, using of labview system on the warm needling technique we studied about measurement and comparison with partial temperature changes according to the position of ignition. Results & conclusion : When we measured the warm needling's partial temperature according to the position of ignition, the bottom ignition method got the higher result on the peak temperature measured at 2cm below the head than the apex ignition method.

  • A knowledge portal is an integrated gateway for accessing relevant knowledge, collaborating and communicating with other users, and also linking internal applications which is becoming crucial in the age of information abundance. Research and development is a typical knowledge-intensive activity. However, knowledge management support in R&D has been minimal in most research organizations. In this paper, a knowledge portal is designed to support team-based researches in science and technology for searching and browsing knowledge, and also communicating with other team members, coordinating research project and collaborating with other researchers. Automating knowledge acquisition from various knowledge sources, knowledge categorization by applying text categorization method, and knowledge recommendation can help to relieve management effort and increase the efficiency of knowledge management processes. A prototype system based on the suggested model is also presented.

  • One of the most important prerequisites for a successful software process is the collaboration and software requirements negotiation of all stakeholders in the software development process. Remote users using computer networks can negotiate software requirements by computer supported system, and can share their informations. The goal of software requirements negotiation system is an integration of all win conditions and an agreement after resolution of conflicts. The existing systems need an exclusive system and must be dependent on specific platform and network. Users must have the knowledge of all stakeholder's status and use homogeneous collaborating applications. This paper presents the Web-based software requirements negotiation system for the purpose of resolution of the existing systems' problems. The Web-based software requirements negotiation system can be driven by WinWin Spiral model, and it is based on hybrid execution method. The proposed system is validated and tested on heterogeneous environments.
