  • This study is designed to evaluate the effects of an oriental herbal medication namely Danggwihwahyeolsan-gamibang and Gami-Jeonssibaekchulsan, on ulcerative colitis. Subject: A patient diagnosed ulcerative colitis who're not improved through western medication. Methods: The Clinical data was analyzed on a pastient with ulcerative clitis due to Changpung(腸風) resulted from Hanyeolchakjab(寒熱錯雜), whose symptoms were combined diarrhea, abdominal pain et al. The patient visited at the internal medicine department of Daegu Haany University affiliated Daegu Oriental Hospital on May 18. 2007 to June 25. 2007. The improvement of symptoms were checked by Seo Colitis Activity Index(AI).

Protective Effect of Atractylodes macrocephala and Taraxacum spp. Combination Treatment in Balb/c Mice with Dextran Sulfate Sodium-Induced Ulcerative Colitis

  • Objectives : This study aimed to investigate the protective effects of an herbal mixture of Atractylodes macrocephala and Taraxacum spp. (ATC) on ulcerative colitis. We have previously screened traditional medicinal herbs to discover the effective candidate by the animal model. A. macrocephala and T. spp were identified as one of the effective herbs in the screening process. Methods : Experimental colitis was induced in male Balb/c mice by administering drinking water containing dextran sulfate sodium, which mimics the clinical and histological features of ulcerative colitis in human. ATC at doses of 30, 100 or 300 mg/kg were orally administered to mice twice per day for 10 consecutive days. To evaluate the damage from experimentla ulcerative colitis, body weight, colon length, disease activity index, myeloperoxidase and histological changes were measured and analyzed. Results : The administration of dextran sulfate sodium with drinking water resulted in markedly reduced colon length, severe body weight loss, increased levels of myeloperoxidase activity and histological damages in mice. ATC treatment significantly ameliorated the colon shortening, histological damage, body weight loss and disease activity index score in a dose-dependent manner. ATC also attenuated the colonic myeloperoxidase activity which reflects the severity and extent of inflammatory damage of colon. Conclusions : ATC exerts protective effects against inflammatory colonic structural damage induced by epithelial barrier integrity impairment. ATC also inhibits weight loss and related symptoms of UC which can be considered as the functional recovery of colon.

Changes of colonic endocrine cells in trinitrobenzene sulfonic acid (TNBS)-induced rat colitis

  • In this study, immunohistochemistry was used to examine the changes in the density of colonic endocrine cells - argyrophil and argentaffin cells, chromogranin A (CGA), serotonin, somatostatin and glucagon-containing cells in trinitrobenzene sulfonic acid (TNBS)-induced rat colitis. Ulcerative colitis was induced by the instillation of 10 mg of TNBS into the colonic lumen through the anus. To confirm the inducement of ulcerative colitis, the macroscopic and microscopic scores as well as the colonic myeloperoxidase (MPO) activities were monitored for 8 days after TNBS instillation in the colonic lumens. In addition, the number of argyrophil and argentaffin cells, CGA, serotonin, somatostatin and glucagon-immunoreactive cells were counted in the colonic mucosa, respectively. After TNBS instillation into the lumen of the colon from the anus in rats, increases in macroscopic and microscopic scores in the colon tissues were observed along with increases in the colonic MPO activities. Therefore, ulcerative colitis was relatively well induced by the TNBS instillations. Marked decreases in the number of colonic endocrine cells were detected in the TNBS-treated animal compared to the sham control. These results suggest that colonic endocrine cells were also disrupted by TNBS-induced ulcerative colitis.

Protective Effect of Curcumin on Dextran Sulfate Sodium-Induced Ulcerative Colitis in Obese Mice (비만마우스에서 Dextran Sulfate Sodium 유도 궤양성 대장염에 대한 커큐민의 보호효과)

  • Ulcerative colitis is a disease that causes inflammation in the mucosal or submucosal layer of the colon. Previous studies have reported that obesity increases the prevalence of ulcerative colitis and aggravates the progression. This study was therefore undertaken to investigate whether curcumin inhibits the progression of ulcerative colitis caused by obesity. Mice were bred on a high-fat diet to induce obesity, and curcumin was administered with the high-fat diet to confirm the anti-inflammatory effect. To induce ulcerative colitis, dextran sulfate sodium (DSS) was administered orally, and clinical symptoms of colitis were subsequently observed. For histological evaluation of curcumin, the colon, liver and abdominal fat tissue samples were prepared and analyzed by hematoxylin and eosin (H&E) and Alcian blue-periodic acid-Schiff (PAS) staining. Our results confirm that consumption of curcumin resulted in decreasing the score of the disease activity index, and inhibited shortening of the colon length. In addition, inflammatory cell infiltration and mucosal damage were inhibited in the colon tissue of ulcerative colitis exacerbated by obesity. We further confirmed that exposure to curcumin significantly reduced the steatosis area of the liver and adipocytes of abdominal fat. In conclusion, we believe that curcumin can be applied as a therapeutic agent to treat ulcerative colitis, by inhibiting the progression of colitis caused by obesity.

The Anti-oxidative and Anti-inflammatory Effect of Lonicera Japonica on Ulcerative Colitis Induced by Dextran Sulfate Sodium in Mice (금은화 추출물의 항산화, 항염증 효과가 Dextran Sulfate Sodium으로 유도된 생쥐의 궤양성 대장염에 미치는 영향)

  • Objectives : This study was to investigate the anti-oxidative and anti-inflammatory effect of Lonicera japonica water extracts (LE) on Ulcerative Colitis Induced by DSS (Dextran Sulfate Sodium) in Mice. Methods : Colitis was induced by DSS in Balb/c mice. The sample group was divided into three. The mice in control group were not inflammation-induced. The pathological group was composed of untreated colitis elicited mice. The experimental group was administered Lonicera japonica water extracts (LE) after colitis elicitation. The effects on ulcerative colitis were evaluated the anti-oxidant effect, inhibition of COX-2 mRNA expression, the morphological change of colonic mucosa, decrease effect of HSP 70 and COX-2 in mucosa. Results : The SOD ability of LE was dose-dependently increased and the LPS-induced COX-2 mRNA expression of LE was dose-dependently decreased. LE showed the protective effects on DSS-induced experimental colitis. LE inhibited shortening of colon length, the hemorrhagic erosion in colonic mucosa. LE also showed the decrease effect for HSP70 and COX-2 in mucosa. Conclusions : The current results demonstrate the clinical utility of LE in traditional medicine and indicate the possible treatments for ulcerative colitis from natural products. Further investigations for exact mechanisms will be needed.

  • Ulcerative colitis (UC) is a chronic inflammatory disorder of the gastrointestinal tract that affects the large bowel. Its etiology remains controversial. However, an infectious or immunologic origin is considered the primary cause. The onset of UC is typically slow and insidious, but some patients may present acutely with symptoms mimicking infectious colitis. We report a case of ulcerative colitis mimicking acute hemorrhagic colitis at initial presentation. A 60-year-old man was referred to Yeungnam University Hospital for bloody diarrhea and abdominal pain. Sigmoidoscopy revealed mildly edematous mucosa in the rectum and hyperemic mucosa with petechiae in the sigmoid colon. The patient was treated with antibiotics for several days, and his symptoms improved. However, after one month, his bloody diarrhea relapsed. Follow-up sigmoidoscopy revealed mucosal friability in the rectum and sigmoid colon. He was diagnosed with ulcerative colitis, and his symptoms were improved with mesalazine and a steroid enema.

  • 5-Aminosalicylate agents are the main therapeutic agents for ulcerative colitis. Balsalazide is a prodrug of 5-aminosalicylate and has fewer side effects than the other 5-aminosalicylate agents. Pulmonary complications resembling granulomatosis with polyangiitis in ulcerative colitis are extremely rare. Here, we report a patient with ulcerative colitis on balsalazide presenting respiratory symptoms and multiple pulmonary nodules from a chest radiography that was pathologically diagnosed with a limited form of granulomatosis with polyangiitis with bronchiolitis obliterans organizing pneumonia-like variant. To our knowledge, this is the first report of a balsalazide-induced limited form of granulomatosis with polyangiitis with bronchiolitis obliterans organizing pneumonia-like variant.

Factors related to Quality of Life of Patients with Ulcerative Colitis (궤양성 대장염 환자의 삶의 질 영향요인)

  • Purpose: The purpose of this study was to identify factors related to the quality of life of patients with ulcerative colitis. Methods: A total 116 patients with ulcerative colitis were asked on general characteristics, illness-related characteristics, depression, and quality of life. The collected data were analyzed by t-test, ANOVA, and stepwise multiple regression. Results: As for depression the 81.03% were normal, the 8.62% were mild depression, the 6.90% were moderated depression, and the 3.45% were severe. As for quality of life, the social functions was highest, followed by intestine-related symptoms, systemic symptoms, and emotional functions. Quality of life was positively correlated to subjective health status, age, and body mass index, and negatively to depression. Main factors affecting the quality of life included depression, subjective health status, physician's global assessment, age, days of loss in social life, and present abdominal pain. Conclusion: Physical and psychosocial health problems related to ulcerative colitis affected the quality of life of the patients. Further research is warranted for developing educational programs and psychosocial strategies in order to efficiently handle the illness.

Two Cases of Ulcerative Colitis Diagnosed as Damp-Heat Dysentery Treated with Jakyaktang-gamibang (습열리(濕熱痢)로 변증한 궤양성 대장염 환자의 작약탕가미방(芍藥湯加味方) 치험 2례)

  • Ulcerative colitis, a diffuse inflammatory disease of the mucosal lining of the colon and rectum, is characterized by bloody diarrhea that waxes and wanes without apparent cause. In oriental medicine, Jakyaktang has been used to treat the disease, diagnosed as Damp-Heat dysentery. We prescribed Jakyaktang-gamibang to two patients who had suffered from ulcerative colitis diagnosed as Damp-Heat dysentery. The two patients were treated with Jakyaktang-gamibang and improved in consciousness of symptoms. We suggest that oriental medicine therapy is effective for treatment of ulcerative colitis.

The Effects of Zanthoxylum schinifolium Extract in a Dextran Sulfate Sodium-induced Rat Colitis Model (Dextran Sulfate Sodium 유도 흰쥐 궤양성 대장염에 대한 산초 추출물의 효과)

  • The aim of this study was to evaluate the anti-inlfammatory effect of the extract of Zanthoxylum schinifolium on ulcerative colitis induced by 3% dextran sulfate sodium in the rat. The experiment animals were divided into six groups: control (normal), DDS-induced colitis, 1 mg/kg, 15 mg/kg and 150 mg/kg of the extract of Z. schinifolium and 150 mg/kg of 5-aminosalicylic acid (5-ASA) as comparative drug. Anti-ulcerative colitis effect was evaluated pathologically by disease activity index (DAI), the change of weight and myeloperoxidase (MPO) in colon mucosa. Treatment with 15 mg/kg and 150 mg/kg of the extract of Z. schinifolium significantly improved the gain of body weight and DAI as clinical symptoms, and reduced MPO and $PGE_2$ level as biochemical index. Especially, 150 mg/kg of the extract of Z. schinifolium showed markedly more improvement than the same dose of 5-ASA in all kind of index such as MPO, $PGE_2$ and DAI. These results suggest that Z. schinifolium mediated anti-inflammatory action on colorectal sites may be a useful therapeutic approach to ulcerative colitis.
