The Association of Hospital Volume of Percutaneous Coronary Intervention with Cardiac Mortality

  • Background: This study investigates the potential volume and outcome association of coronary heart disease (CHD) patients who have undergone percutaneous coronary intervention (PCI) using a large and representative sample. Methods: We used a National Health Insurance Service-Cohort Sample Database from 2002 to 2013 released by the Korean National Health Insurance Service. A total of 8,908 subjects were analyzed. The primary analysis was based on Cox proportional hazards models to examine our hypothesis. Results: After adjusting for confounders, the hazard ratio of thirty-day and 1-year mortality in hospitals with a low volume of CHD patients with PCI was 2.8 and 2.2 times higher (p=0.00) compared to hospitals with a high volume of CHD patients with PCI, respectively. Thirty-day and 1-year mortality of CHD patients with PCI in low-volume hospitals admitted through the emergency room were 3.101 (p=0.00) and 2.8 times higher (p=0.01) than those in high-volume hospitals, respectively. Only 30-day mortality in low-volume hospitals of angina pectoris and myocardial infarction patients with PCI was 5.3 and 2.4 times those in high-volume hospitals with PCI, respectively. Conclusion: Mortality was significantly lower when PCI was performed in a high-volume hospital than in a low-volume hospital. Among patients admitted through the emergency room and diagnosed with angina pectoris, total PCI volume (low vs. high) was associated with significantly greater cardiac mortality risk of CHD patients. Thus, There is a need for better strategic approaches from both clinical and health policy standpoints for treatment of CHD patients.

Biomass estimation of sailfin sandfish, Arctoscopus japonicus, in Korean waters (한국 연근해 도루묵, Arctoscopus japonicus의 자원량 추정)

  • Available ecological and fishery data of sailfin sandfish, Arctoscopus japonicus, was examined to ascertain its population dynamic parameters in Korean waters. The instantaneous coefficient of total mortality (Z) was estimated to be 1.361/yr, and annual survival rate (S) was 0.256. The estimated instantaneous coefficient of natural mortality (M) was 0.482/yr, and the instantaneous coefficient of fishing mortality (F) for recent years was calculated to be 0.879/yr from Z and M values. Age at first capture was estimated to be 1.958 years. These parameters, in conjunction with catch landings data between 1991 and 2008, were used to estimate annual biomass using a biomass-based cohort analysis. The biomass of A. japonicus was estimated at approximately 7,600 mt in 1991, but since 1994 decreased to below 4,000 mt by 2001. After 2002 they started to increase gradually, and showed the level of more than 5,000 mt in recent years.

  • The purpose of the study is to provide the basic data for preventing and caring cardiovascular disease. The study was performed with the data from the same 1,668 persons who had medical checkup three times from 2004 to 2008 at a health promotion center. The related factors were examined with Cohort Study. The result of the study is as followed, The incidence of metabolic syndrome for the subjects from 2004 to 2008 increased; 0% in 2004, 8.3% in 2006 and 12.0% in 2008. Multiple logistic regression analysis showed that the related factors in the occurrence of metabolic syndrome which changed from the non metabolic syndrome group in 2004 to metabolic syndrome were age, smoking, total cholesterol, ALT, and uric acid etc.

Systematic Review on Epstein-Barr Virus (EBV) DNA in Diagnosis of Nasopharyngeal Carcinoma in Asian Populations

  • Objective: To conduct a meta-analysis to investigate the value of EBV DNA in diagnosis of nasopharyngeal cancer (NPC) in Asian populations, and provide important evidence for screening. Methods: Prospective or respective case-control or cohort studies regarding the detection role of EBV DNA for NPC were included in our study. We conducted a comprehensive literature search in PubMed, EMBASE, and the Chinese Biomedical Database (CBM database between January 1980 and March 2012. Results: A total of 18 studies with 1492 NPC cases and 2641 health controls were included. Almost of the included studies were conducted in China, and only one other conducted in Thailand. The overall results demonstrated that the pooled sensitivity, specificity, positive likelihood (+LR) and negative likelihood (-LR) were 0.73 (0.71-0.75), 0.89 (0.88-0.90), 8.84 (5.65-13.84) and 0.19(0.11-0.32), respectively. The overall EBV DNA detection showed the largest area of 0.932 under the summary receiver operator curve (SROC). The accuracy of detection by plasma for NPC (0.86) was higher than in serum (0.81), with largest areas under the SROC of 0.97 and 0.91, respectively. Conclusion: Our results demonstrated the EBV DNA detection in plasma or serum has high sensitivity and specificity in diagnosis of NPC, especially in Chinese populations with a high risk of cancer.

Expression Profiles of Loneliness-associated Genes for Survival Prediction in Cancer Patients

  • Influence of loneliness on human survival has been established epidemiologically, but genomic research remains undeveloped. We identified 34 loneliness-associated genes which were statistically significant for high-lonely and low-lonely individuals. With the univariate Cox proportional hazards regression model, we obtained corresponding regression coefficients for loneliness-associated genes fo individual cancer patients. Furthermore, risk scores could be generated with the combination of gene expression level multiplied by corresponding regression coefficients of loneliness-associated genes. We verified that high-risk score cancer patients had shorter mean survival time than their low-risk score counterparts. Then we validated the loneliness-associated gene signature in three independent brain cancer cohorts with Kaplan-Meier survival curves (n=77, 85 and 191), significantly separable by log-rank test with hazard ratios (HR) >1 and p-values <0.0001 (HR=2.94, 3.82, and 1.78). Moreover, we validated the loneliness-associated gene signature in bone cancer (HR=5.10, p-value=4.69e-3), lung cancer (HR=2.86, p-value=4.71e-5), ovarian cancer (HR=1.97, p-value=3.11e-5), and leukemia (HR=2.06, p-value=1.79e-4) cohorts. The last lymphoma cohort proved to have an HR=3.50, p-value=1.15e-7. Loneliness-associated genes had good survival prediction for cancer patients, especially bone cancer patients. Our study provided the first indication that expression of loneliness-associated genes are related to survival time of cancer patients.

Validity Verification of ARCS Evaluation Models for Promoting University Students' Learning Motivation (대학생의 학습동기 촉진을 위한 ARCS 평가모형의 타당화 검증)

  • Due to the lack of learning motivation, there is a need to seek ways to facilitate learning incentives because it's causing drop out and maladjustment of university life. This study is to examine whether the ARCS evaluation model developed by Keller (1983) is a valid model for evaluating the motivation level of local university students sample 276(male 116, female 157) in the Republic of Korea. To analyze the data, average statistic, one-way ANOVA and confirmative factor analysis were used. The conclusion of this study is as follows. First, the level of motivation per ARCS factor has demonstrated the highest relevance factor. Second, the level of motivation by the ARCS for the 1st and 3rd graders was appropriate. Differences in 'attention' and 'relevance' have only been observed for each year, and the 1st grade group was found to be larger than the 2nd grade cohort. Third, construct validity and convergent validity were obtained for measuring the level of motivation. The results of the verification of the variables also showed that the AVE and CR were met, and the model fit well was satisfactory. Based on the finding results, discussion and implication for further research were suggested.

Risk of Death and Occurrence of Secondary Disease of Cancer and Cardiovascular Disease Patient by Income Level in Korea (암, 심뇌혈관 질환자의 소득수준에 따른 사망 및 이차 질환 발생 위험)

  • In this study, we analyzed the effect of the income level of cancer, stroke, and myocardial infarction on mortality by using National Health Insurance Service(NHIS) Cohort 2.0 DB. Patients who newly developed the disease in 2007 were observed till 2015. The analysis used the Cox probability proportional risk model and the competing risk model. The income level used information at the time of the onset of the disease in 2007, categorized into low / mid / high. The results showed that there were differences in the risks of death and secondary disease in patients with cancer, stroke, or myocardial infarction according to the income level. In addition to the need for a social safety net to lower the incidence of early deaths in low-income families, it seems necessary to continue to strengthen universal protection for serious diseases similar to the current policy.

Analysis of Longitudinal Relation of Children's Social Withdrawal and Peer Alienation (아동의 사회적 위축과 또래소외 간의 종단적 관계)

  • The purpose of this study is to verify the causal relationship between social withdrawal and peer alienation among Korean children. The survey study participants were 2,110 children in 2nd, 3rd, 4th, and 5th grades. The 5-year longitudinal data (Korean Child and Youth Panel Survey; KCYPS) of 1st grade elementary school student cohort was used. Latent Growth Modeling was applied to analyze the causality over time of social withdrawal and peer isolation. The results revealed that the trajectories of children's social withdrawal decreased over time. Children's social withdrawal showed significant individual differences in terms of initial value and rates of change. The initial value and slope of children's social withdrawal had an effect on the peer alienation in the 3rd grade; however, there was no significant effect on the peer alienation in the 5th grade.

The Risk of Breast Cancer in Women in Their 40s by Economic Activity (경제활동에 따른 40대 여성의 유방암 발생 위험도)

  • In South Korea, female individuals in their forties show a high rate of incidence, with approximately 13% of the patients being <40 years. This statistic is more than twice as high as that in Western countries. It is therefore necessary to identify the risk factors for breast cancer incidence by age and economic activity participation status. Women aged 30 to 59-whether breast cancer patients or those in the control group and having no breast cancer-were appraised from the sample cohort database. The data were analyzed using the statistical software R36.2. To identify the factors affecting breast cancer incidence, the degree of association was determined with HR and 95% CI by means of cox regression analysis. As for the socio-demographic variables, the older the individual, the higher the risk of breast cancer incidence becomes. As for the economic activity variables, those who were dependents (unemployed) and who had higher income (medium and high) were at higher risk of breast cancer incidence, which was statistically significant. The income-adjusted HR (model 1) for breast cancer development associated with the economic activity was 1.452 (95% CI, 1.19-1.77). The body mass index and alcohol intake-adjusted HR (model 2) was 1.431 (95% CI, 1.18-1.74). One needs to pay attention to policy plans regarding women's quality of life, as well as to the risk of breast cancer incidence by their economic activity. In other words, policies need to give post care, instead of focus on early detection and cancer treatment.

Three-dimensional analysis of dental decompensation for skeletal Class III malocclusion on the basis of vertical skeletal patterns obtained using cone-beam computed tomography

  • Objective: To evaluate the presurgical orthodontic tooth movement of mandibular teeth after dental decompensation for skeletal Class III deformities on the basis of vertical skeletal patterns. Methods: This cohort was comprised of 62 patients who received presurgical orthodontic treatment. These patients were divided into 3 groups according to their vertical skeletal patterns. Changes in the positions of the mandibular central incisor, canine, premolar, and 1st molar after presurgical orthodontic treatment were measured using a cone-beam computed tomography (CBCT) superimposition method. Results: The incisors moved forward after dental decompensation in all 3 groups. The canines in group I and the 1st premolars in groups I and III also moved forward. The incisors and canines were extruded in groups I and II. The 1st and 2nd premolars were also extruded in all groups. Vertical changes in the 1st premolars differed significantly between the groups. We also observed lateral movement of the canines in group III and of the 1st premolar, 2nd premolar, and 1st molar in all 3 groups (p < 0.05). Conclusions: Movement of the mandibular incisors and premolars resolved the dental compensation. The skeletal facial pattern did not affect the dental decompensation, except in the case of vertical changes of the 1st premolars.
