Intensity of Aerobic Exercise and Level of Cognitive Task on Computerized Neurobehavioral System

  • Aerobic exercise affects cerebral circulation, action of neurotransmitters, glucose, oxygen, and energetic substances and influence on the central nervous system for cognition. This study suggests that both the intensity of exercise and the level of cognitive task need to be considered. Computerized neurobehavioral testing is a more effective method, compared to conventional methods, of neuropsychological testing when measuring cognition objectively, in cases that we found. The intensity of 80% max HR had effect on more complex tasks such as 3 Digit Addition and Digit Span Backward, and the intensity of 65% max HR had an effect on more simple tasks such as Color Word Vigilance and Digit Span Forward. We can assume that different intensity of aerobic exercise might involve specific areas of the brain as they could have different sensitivities, so further studies measuring regional cerebral blood flow or electroencephalogram are needed to confirm the results.

경도 인지장애 선별에 적용된 이중과제 평가에 대한 체계적 고찰 (A Systematic Review of the Application Dual Task Assessment for Screening Mild Cognitive Impairment)

  • Purpose: This study aims to systematically review the dual-task evaluation applied to the screening of mild cognitive impairment. It also aims to present various evaluation items and results analysis methods for dual tasks applied to patients with mild cognitive impairment. Methods: We conducted a systematic search of published studies in PubMed databases and KISS from January 2000 to August 2020 using the main keywords such as "Dual task," "Mild Cognitive impairment," "Elderly," and "Screening." We selected a total of 10 studies for the analysis from 1314 searched articles. Results: We analyzed the qualitative level of 10 studies that were nonrandomized two-group studies with evidence level II (100.0%). These results suggest that the evidence level of the studies was high. We analyzed 10 studies and identified 12 motor tasks and 19 cognitive tasks. Walking was the most commonly used evaluation motor task and counting backward by ones and naming animals were the most commonly used evaluation cognitive tasks. Moreover, the velocity speed was the most used result analysis method. The results indicate that there were significant differences in dual-task performance between patients with normal and mild cognitive impairment. Conclusion: The results of this study can be used as a basis for the selection of dual-task evaluation items and methods of analyzing the results for screening mild cognitive impairment. Furthermore, they are expected to be used for research on the development of dual-task evaluation tools. It is necessary to compare and analyze the usage trends of dual-task evaluation by cultural differences in future studies.

스노젤렌과 전산화 인지재활 프로그램(Rehacom)이 인지기능 향상에 미치는 영향 (The Effect of Snoezelen and Computerized Cognitive Rehabilitation(Rehacom) on Improvement of Cognitive Function)

  • Purpose : This study aims to investigate the effect of the Snoezelen and Rehacom programs on improvement of attention and memory, and the effect of the Snoezelen program on stress reduction. Method : This study was targeted at 11 subjects in the Snoezelen experimental group, 11 subjects in the Rehacom group and 11 subjects in the non-experimental group. As the initial evaluation, all the subjects took electroencephalography. Then, the Snoezelen group and Rehacom group did Snoezelen training and Rehacom training, respectively total 12 times(for 20 minutes twice per week for six weeks), but no training was applied to the control group. Three weeks after the training, the interim was carried out, and four weeks after the training, the final evaluation was carried out. Results : Subjects' attention increased to $58.15{\pm}4.96$ from $43.75{\pm}4.69$ during the Snoezelen training, and increased to $49.85{\pm}1.91$ from $43.28{\pm}2.71$ during the Rehacom training, which means the Snoezelen training was more effective in improving attention(P<0.05). Subjects' memory increased to $56.14{\pm}1.26$ from $43.19{\pm}3.46$ during the Snoezelen training, and increased to $50.94{\pm}4.0$ from $43.07{\pm}2.58$ during the Rehacom training. This also implies that the Snoezelen training was more effective in improving memory(P<0.05). Conclusion : Though both of the Snoezelen training and Rehacom training improved attention and memory, the Snoezelen program was more effective, and it also influenced stress resistance and physical arousal.

국내 장기요양보험서비스를 제공 받는 치매 환자 대상 작업 중심 회상치료 효과에 대한 체계적 고찰 (A systematic Review on the Effects of Occupational Reminiscence Therapy in Dementia Offered National Long Term Care Insurance)

  • 목적 : 본 연구는 국내 장기요양보험서비스를 제공받는 치매환자를 대상으로 다양한 분야에서 이루어진 작업 중심 회상치료 프로그램을 중재 적용한 연구 효과에 대해 검토해보고자 체계적 고찰을 시행하였다. 연구방법 : 2009년부터 2013년 까지 한국교원학술정보원을 통해서 확인되는 논문을 검색하였고 주요 검색용어로는 치매, 시설 서비스, 주간 보호 센터, 회상치료, 작업 중심 프로그램 등의 단어를 사용하였다. 연구결과 : 분석 결과 총 6편의 논문이 선정되었으며 선정된 연구에서의 회상치료는 모두 작업 중심 프로그램을 병행한 집단 회상치료이다. 세부 중재 형태는 의사소통 중심 회상치료, 작업 중심 회상치료로 크게 2가지 형태로 나눌 수 있었다. 치매 환자 대상의 회상치료의 효과는 인지기능 유지 및 회상기능 향상, 우울감소, 문제행동 감소, 의사소통 및 사회적 상호작용 향상, 삶의 질 향상 등 총 5가지 효과가 보고되었으며, 연구의 질적인 측면에서는 적은 대상군, 짧은 연구기간, 추적조사 미시행으로 인해 전반적으로 낮은 수준의 내적, 외적 타당도를 보였다. 결론 : 추후의 연구에서는 보다 많은 수의 대상군의 확보와 추적조사를 통한 지속적인 치료적 효과에 대한 연구가 이루어져야 할 것이며 치매환자 대상의 다양한 형태의 작업 중심 회상치료의 연구가 이루어져야 할 것으로 사료된다.

조현병 환자의 자아존중감을 위한 국내 프로그램 체계적 고찰 (A Systemic Review of Self-Esteem Programs for Schizophrenic Individuals in South Korea)

  • Purpose : This study systematically assessed programs self-esteem programs for schizophrenic individuals that have been implemented over the past 10 years in South Korea, with the objective of providing clinical grounds and data for further programs that can be implemented in the Korean environment. Methods : For this study, a data search was conducted from 2012 to 2021, using search terms 'schizophrenia' AND 'self-esteem' among the studies registered in the RISS, KISS, Google Scholar, and National Library of Korea databases. A total of 988 studies were searched, and among them, 40 studies meeting the selection criteria had been taken by reviewing the titles and abstracts. After, 33 studies were excluded according to the exclusion criteria, and finally, the 7 studies were selected. Results : As a result, research into self-esteem programs for schizophrenic individuals has progressed steady, and most studies tend to be non-randomized controlled studies. All studies used the Self-Esteem Scale to evaluate self-esteem. All studies used the Self-Esteem Scale to evaluate self-esteem. The interventions of the selected studies were classified as were social skill programs, cognitive behavioral therapy programs, art therapy programs, and emotional management programs. And among them, the cognitive behavioral therapy programs were the most frequently used (42.8 %). Typically, the majority of the intervention sessions consisted of 11~15 sessions, the frequency of intervention was twice a week, and each session lasted 40~80 minutes were the most (each 57.1 %). In the programs for self-esteem, significant effects were reported for cognitive behavioral and art therapy programs, and there is no significant effect on social skill programs and emotional management programs. Conclusion : Future studies should expand the research scope, both domestically and internationally, and suggest more comprehensive conditions for a literature search by widening the research subject coverage.

전산화인지치료와 고유수용성 감각운동이 뇌졸중 환자의 뇌파변화에 미치는 영향 (The Effect of EEG through Proprioceptive Exercise and Computerized Cognitive Therapy on Stroke)

  • PURPOSE: The purpose of this study applied computerized cognitive therapy and proprioception exercise to stroke patients and analyzed improvement in their changes in Electrocephalogram(EEG). METHODS: The subjects were 30 patients who were diagnosed with stroke and they were randomly selected to a proprioceptive training group(n=15), a cognitive training group(n=15). The experiment was performed for three times per week for 6 weeks and EEG was measured before and after the experiment. RESULT: Before and after the experiment in each group of experiments, the ${\alpha}$-and ${\beta}$-wave study showed significant changes but, there was no significant difference in the change between groups before and after the experiment. CONCLUSION: From the above results, the cognitive training and the proprioception training have a positive impact in stroke patients EEG changes, but it is difficult to suggest a better therapeutic interventions. However, as compared with that the cognitive training that directly involved in the cognitive and brain activation, the proprioception training have changes on brain activation. Therefore, of Clinical therapeutic interventions, the proprioception training can be presented effectively to the changes in brain activation in stroke patients.

인지상태와 연령에 따른 손 기능의 비교 (Comparison of Hand Functions According to Cognitive Status and Age)

  • Purpose: The purpose of this study is to analyze the hand functions of elderly persons according to their cognitive status and age. Methods: A total of 65 persons voluntarily participated in the study. The subjects were divided into three groups: impairment cognitive group, normal cognitive group, adult group (persons in their twenties). Assessment of cognitive status was performed using a mini-mental state examination for Koreans (MMSE-K). Hand function was assessed using the Purdue pegboard test. The collected data were analyzed using a one-way ANOVA and Pearsonn Acorrelation. Results: There were significant differences in hand functions in the three groups. Post-hoc test results showed significant differences between each group. There were statistically significant differences in the correlation among hand functions, cognitive status, and age. The findings of this study suggest that hand functions have a positive correlation with cognitive status. However, a negative correlation was found between hand function and age. Conclusion: According to the study's results, hand functions are correlated with age and cognitive functions in elderly persons. This study suggests that hand rehabilitation with cognitive intervention increases hand functions in elderly persons.

삼킴장애가 있는 뇌졸중 환자의 특성에 따른 삼킴기능, 식이수준, 인지기능, 일상생활의 비교 및 상관관계 (Correlations and Comparison among Swallowing Function, Dietary Level, Cognitive Function, Daily Living according to Characteristic in Stroke Patients with Dysphagia)

  • 본 연구는 삼킴장애를 가진 뇌졸중 환자의 특성에 따른 삼킴기능, 식이수준, 인지기능, 일상생활의 비교와 상관관계를 알아보고자 하였다. 본 연구의 대상자들은 삼킴장애가 있는 뇌졸중 환자 56명이었다. 결과측정은 기능적 삼키장애 척도(FDS), 미국언어청각협회 삼킴척도(ASHA NOMS), 한국판 간이 정신상태 검사(K-MMSE), 한국판 수정바델지수(K-MBI)를 사용하였다. 수집한 자료는 대상자들의 일반적 특성에 따른 각 평가들의 차이를 비교하기 위하여 독립 t 검정을 이용하여 분석하였고 각 평가들의 상관관계를 알아보기 위하여 피어슨 상관분석을 수행하였다. 연구 결과, 대상자들의 손상위치에 따른 삼킴기능은 유의한 차이가 있었다. 발병기간에 따른 인지기능은 유의한 차이가 있었다. 대상자들의 나이는 식이수준, 인지기능과 유의한 상관이 있었다. 삼킴기능은 식이수준, 인지기능과 유의한 상관이 있었다. 인지기능은 식이수준, 일상생활과 유의한 상관이 있었다. 본 연구의 결과를 기반으로 삼킴장애가 있는 뇌졸중 환자에 대한 삼킴재활 치료는 환자들의 특성과 인지기능을 고려하여 수행해야 할 것으로 사료된다.

수공예활동이 자아존중감과 공감지수에 미치는 영향 (Effects of Handicraft Activities on Self-Esteem and Empathy Quotient)

  • Purpose : In this research, I tried to examine the effect of handwork activities through group activities on self-esteem and sympathy index. Method : In this research, for men and women enrolled in K university, we explain sufficiently the purpose and method of research and randomly assign to 20 training group and 20 control group for those who agreed with experiment we divided handwork activities twice a week from november 18, 2016 to December 6, six times in total for 30 minutes. Results : Compared with self-esteem among groups, group training exercises showed higher interpersonal relationships than control group, and self-accepted items and self-efficacy items decreased. empathy comparison that group fits according to arbitration period compared with group control training group, group of training exercise high emotional sympathy items and social skills items appear high, cognitive sympathy items are low. Conclusion : From the results of this study, it turned out that the activities of handwork by group improved interpersonal relationships, emotional sympathy and social skills. In light of the results of these studies, it can be considered that better results can be obtained when applied to children and young people who are forming self-esteem and cognitive sympathy. In future research, further studies based on age, sex and further research corresponding to each situation, student's self esteem and a broader material for developing program necessary for cognitive sympathy can be obtained.

국내 아동 작업치료사의 평가도구 사용 동향 (Current Trend in Use of Occupational Therapy Assessment Tool by Pediatric Occupational Therapist)

  • 목적 : 본 연구는 국내 아동 작업치료사들이 사용하고 있는 평가도구를 영역별로 구체화하여, 평가도구의 사용 동향을 파악하고자 하였다. 연구방법 : 본 연구는 아동 작업치료사를 대상으로 평가도구 사용에 대한 설문을 실시하였다. 평가도구의 하위영역은 작업수행, 일상생활활동, 교육, 놀이, 감각-지각, 운동 및 실행, 인지, 사회적 상호작용 기술, 발달, 이학적 검사로 구성하였다. 설문에 응답한 105명의 자료를 대상으로 기술통계 및 빈도분석을 사용하여 분석하였다. 결과 : 본 연구 결과는 초기 평가 시 소요되는 시간은 30~60분(41.9%)이 가장 많았으며, 재평가 주기는 3~6개월(41.0%)로 가장 많이 실시하는 것으로 나타났다. 영역 별 평가도구를 살펴보면, 작업 수행은 Canadian Occupation Performance Measure(55.2%)을, 일상생활활동은 Wee Functional Independence Measure(57.1%)을, 교육은 School Function Assessment(2.5%)를, 놀이 평가도구는 Revised Knox Preschool Play Scale(28.6%)을, 감각-지각은 Developmental Test of Visual Perception(94.3%)을, 운동 및 실행 평가도구는 Bruininks-Oseretsky Test of Motor Proficiency-2(42.9%)를, 인지 평가도구는 Lowenstein Occupational Therapy Cognitive Assessment(11.4%)를, 사회적 상호작용 기술은 Evaluation of Social Interaction(6.1%)을, 발달은 Denver Developmental Screening Test-II(92.4%)를, 이학적 검사는 임상관찰(89.5%)을 주로 사용하는 것으로 나타났다. 결론 : 본 연구를 통하여 국내 아동작업치료사들이 사용하고 있는 평가도구를 영역별로 구체적으로 파악할 수 있었으며, 이를 통해 추후 평가 교육 및 개발을 위한 기초적인 근거자료가 될 것으로 기대된다.
