노인의 성별에 따른 인지기능과 영양관리상태의 차이 (Gender Differences in the Cognitive Function and Nutritional Status in Older Age: A Representative Nationwide Data of Korean Elders)

  • Purpose: The purpose of this study was to identify the gender differences in the cognitive function and nutritional status among elderly people living in the community. Methods: This study used data from the 2014 National Survey on the Elderly and focused on 10,054 respondents who answered that they did not have dementia. The Korean version of Mini-Mental State Examination for Dementia Screening and the Nutrition Screening Initiative were used. IBM SPSS Statistics 23.0 was used to conduct statistical analyses. Results: The cognitive function score was 25.30 for men, which was higher than for women. The percentage of subjects who showed cognitive decline was 25.5% for males and 50.2% for females, which was statistically significant (p<.001). There were statistically significant differences in the nutritional status between males and females (p<.001). There was a statistically significant negative correlation between the cognitive function and nutritional status in both men and women (p<.001, p<.001). Conclusion: These results showed that the intervention to maintain cognitive functioning should be provided to elderly women with a low education level or high number of chronic diseases and medication, and an intervention to prevent the cognitive decline of the elderly should include nutritional management for health and function maintenance.

Job Characteristics in Nursing and Cognitive Failure at Work

  • Objectives: Stressors in nursing put high demands on cognitive control and, therefore, may increase the risk of cognitive failures that put patients at risk. Task-related stressors were expected to be positively associated with cognitive failure at work and job control was expected to be negatively associated with cognitive failure at work. Methods: Ninety-six registered nurses from 11 Swiss hospitals were investigated (89 women, 7 men, mean age = 36 years, standard deviation = 12 years, 80% supervisors, response rate 48%). A new German version of the Workplace Cognitive Failure Scale (WCFS) was employed to assess failure in memory function, failure in attention regulation, and failure in action exertion. In linear regression analyses, WCFS was related to work characteristics, neuroticism, and conscientiousness. Results: The German WCFS was valid and reliable. The factorial structure of the original WCF could be replicated. Multilevel regression task-related stressors and conscientiousness were significantly related to attention control and action exertion. Conclusion: The study sheds light on the association between job characteristics and work-related cognitive failure. These associations were unique, i.e. associations were shown even when individual differences in conscientiousness and neuroticism were controlled for. A job redesign in nursing should address task stressors.

노인의 건강증진행위가 삶의 만족도에 미치는 영향 - 인지기능의 매개효과 중심 - (Effects of Health Promotion Behavior on Life Satisfaction of the Elderly - Focused on the Mediation Effect of the cognitive function)

  • 본 연구는 노인의 건강증진행위가 인지기능을 매개로 하였을 때 삶의 만족도에 미치는 영향을 확인하기 위하여 고령화 연구패널 2018년 7차 자료에서 만 65세 이상 노인 4,284명을 SPSS 22.0으로 분석하였다. 연구대상자의 인지기능 수준은 치매의심 13.8%, 인지기능저하 23.8%, 정상 62.4%이며 삶의 만족도는 59.304점으로 파악되었다. 연구결과는 첫째, 인구학적 특성에 따른 인지기능의 차이는 성별, 연령, 학력, 종교, 혼인 요인에서 유의미하였다. 둘째, 인구학적 특성에 따른 노인의 삶의 만족도는 성별, 연령, 학력, 종교, 혼인 요인에서 유의미한 차이가 확인되었다. 셋째, 노인의 삶의 만족도는 인지기능을 매개로 하였을 때 독립변수 건강증진행위 중 음주, 규칙적 운동 요인에서 정적인 효과가 유의미하게 확인되었다. 반면에 일상생활활동제한은 유의미한 부적인 효과가 확인되었다. 이와 같이 건강증진행위가 노인 삶의 만족도에 미치는 영향에서 인지기능의 매개효과를 검증하였다.

The Interaction of Cognitive Interference, Standing Surface, and Fatigue on Lower Extremity Muscle Activity

  • Background: Performing cognitive tasks and muscular fatigue have been shown to increase muscle activity of the lower extremity during quiet standing. A common intervention to reduce muscular fatigue is to provide a softer shoe-surface interface. However, little is known regarding how muscle activity is affected by softer shoe-surface interfaces during static standing. The purpose of this study was to assess lower extremity muscular activity during erect standing on three different standing surfaces, before and after an acute workload and during cognitive tasks. Methods: Surface electromyography was collected on ankle dorsiflexors and plantarflexors, and knee flexors and extensors of fifteen male participants. Dependent electromyography variables of mean, peak, root mean square, and cocontraction index were calculated and analyzed with a $2{\times}2{\times}3$ within-subject repeated measures analysis of variance. Results: Pre-workload muscle activity did not differ between surfaces and cognitive task conditions. However, greater muscle activity during post-workload balance assessment was found, specifically during the cognitive task. Cognitive task errors did not differ between surface and workload. Conclusions: The cognitive task after workload increased lower extremity muscular activity compared to quite standing, irrespective of the surface condition, suggesting an increased demand was placed on the postural control system as the result of both fatigue and cognitive task.

주거지에 적용된 인지건강디자인 시범사업의 유지관리 실태 연구 (A study on the management of the cognitive health design pilot projects applied to residential areas)

  • Purpose: This study is aimed at the case of the cognitive health design pilot projects promoted by the Seoul Metropolitan Government since 2014 in terms of design to solve social problems in accordance with the aging population of our society. The purpose of this study is to analyze the maintenance and management of the projects and to suggest implications for the promotion and expansion of sustainable cognitive health design in the future. Method: It set the analysis frame by dividing management into maintenance-damage-demolition for the spatial types and spatial elements suggested in the Seoul Cognitive Health Guidelines. And it analyze the actual conditions of four pilot projects based on the field survey. Results: First, the ratio of damage and demolition was higher than maintenance. Second, designs and techniques with low durability were applied. Most of the cases where floor marks were applied to the external environment were lost or difficult to recognize, and their functions were limited due to storage of goods and parking of vehicles and motorcycles. Third, there was a large variation according to the type of residence. The project contents that can be applied to the low-rise residential area were also limited, and more elements were demolished than in the apartment type. Implications: First, it should limit project contents of space types and space elements that can be maintained even over time. Second, it should seek sustainable design and technical solutions. Third, it should seek alternatives to cognitive health design in low-rise residential areas where a large number of elderly people live.

여성 노인의 식이요인과 인지기능 감퇴의 관련성 (Association of Dietary Factors with Cognitive Impairment in Older Women)

  • Objective : To examine the association between dietary factors and cognitive impairment in older Korean women living in the community. Methods : Wave 2 data, from the Suwon Longitudinal Aging Study (SLAS), of 365 women aged 65 years or over, were used. The Korean version of the 30-point Mini-Mental State Examination (MMSE-K) was used to assess cognitive impairment (score$\geq$19). Dietary habits and frequencies of food group consumption were also examined. Results : A total of 67 women (18.4%) were found to be cognitively impaired. In bivariate analyses, nondietary factors, such as age, marital status, education, income, self-rated health, depression, emotional support, social activity, exercise, and dietary factors, such as self-rated nutritional status, frequency of beans and bean products and milk and dairy products consumption were associated with cognitive impairment. In the multivariate analysis, a higher frequency ($\geq$1 vs. <1 time/day) of beans and bean products (OR=0.48, 95% CI: 0.23-0.99) and milk and dairy products (OR=0.25, 95% CI: 0.10-0.61) consumption was inversely associated with cognitive impairment, after adjustment for non-dietary factors. Conclusion : These results suggest that dietary factors may play a significant role in cognitive impairment of older Korean women.

사회인지이론을 적용한 신체활동에 관한 문헌고찰 (A study on physical activities by applying a social cognitive theory)

  • This study attempted to extract a suggestive point to help the design of a program, which is used to promote physical activities, by applying a social cognitive theory based on literature review on the physical activity based on a social cognitive theory. This study considers 10 journal articles that used a social cognitive theory, physical activity, and exercise as the major variable using the EBSCOhost Academic Search Premier and Educator's Reference Desk (ERIC). The type of papers was analyzed using a certain criterion, which can be configured according to the number of each year's papers, characteristics of research subjects, application type of a social cognitive theory, and classification of the application of objects in a social cognitive theory. The characteristics of each year's papers presented no specific characteristics for each year's papers, but the study in 2004 especially presented a high level. The characteristics of research subjects presented four highest cases in the case of the college student, and there were zero cases for children. The application type of studies on physical activities using a social cognitive theory can be largely classified as three types. The results of the measurement using a sectional investigation for SCT objects were 2 cases, the application of SCT for promoting physical activities was 1 case, and the demonstration of evaluation for the effect of SCT objects presented 8 highest cases. Although the social cognitive theory in the characteristics of the classification of object applications can be classified as 10 objects, there were no cases that used 10 all objects, partial applications of the object were measured in 8 studies, and two cases presented no detailed considerations on the object. Most of studies used a part of the object where the application of self-efficacy were measured by 8 highest cases. In addition, there were no measurements on the situation, observation learning, answer and response, and self-management. The elements of attitude, cognitive activity, self-efficacy, and handicaps among the SCT object were commonly used, and studies that the self-efficacy largely affects on the promotion of physical activities presented the main current.

경도인지장애 노인의 인지기능, 도구적 일상생활수행능력, 우울 및 삶의 질 관계 (Cognitive Functions, Instrumental Activities of Daily Living, Depression and Quality of Life in the Elderly with Mild Cognitive Impairment)

  • Purpose: This research study not only examine the relationship between cognitive function, IADL, depression and quality of life, but also clarifies factors impacting quality of life for the elderly. The purpose of this study is to administer health promotion programs and to improve quality of life for the elderly. Methods: Data were collected from May 20, to June 10, 2014. The participants were 157 elderly persons with mild cognitive injury (MCI), who had under 23 points on MoCa-K from three different senior welfare centers in Daejeon. The questionnaires were conducted by four university students after receiving instruction from the researcher and consisted of general characteristics, cognitive function, depression and quality of life. SPSS WIN 19.0 program was used for data analysis. Results: Cognitive function showed positive correlation with quality of life (r=175, p=.028). IADL did not show correlation with quality of life (r=-.005, p=.947). However cognitive function(r=-.344, p<.001) and depression (r=-.623, p<.001) showed negative correlation with quality of life. In addition, the factors impacting quality of life were cognitive function (${\beta}=.14$, p=.29) and depression (${\beta}=-.61$, p<.001) which accounted for 39% of quality of life. Conclusion: These findings showed that the participants had depression and their cognitive function was low. As a result, the more participants' cognitive function decreased, the more their quality of life decreased. In addition, the more their depression decreased, the more their quality of life improved.

치매예방을 위한 활동 프로그램이 지역사회 노인의 우울, 인지기능 및 삶의 질에 미치는 효과 (The Effects of the Activity Program for Preventing Dementia Against Depression, Cognitive Function, and Quality of Life for the Elderly)

  • Objectives: This study is conducted to identify the effects of the activity program for preventing dementia against depression, cognitive function, and quality of life for the elderly. Methods: A non-equivalent control group with pretest-posttest design was used on 50 elderly who were assigned to the experimental and control group (25 for each group). Results: Cognitive function was significantly different between the two groups of the pretest and posttest (Z=4.217, p<.001). There were significant differences between the two groups of the pretest and posttest (Z=-2.324, p=.020) for the quality of life on the EQ-5D index. Conclusions: These findings confirmed that the activity program for preventing dementia was effective for improving cognitive functions and quality of life for the elderly. Therefore, it is further encouraged to perform the activity programs for preventing dementia within other community health posts and community centers. It is necessary to study the emotional supports from community health posts in the future.

치매노인을 대상으로 한 인지 자극 훈련의 효과 (The Effect of Cognitive Stimulation Training on Elderly Persons with Dementia)

  • This study was conducted to test the effect of cognitive stimulation training on elderly persons with dementia. The design of the research was one group in pre-test-post-test design. The subjects were nine demented persons over sixty years, with mild to moderate cognitive impairment. Training was administered by research assistants on a one to one basis for thirty to forty minutes, three times a week for eight weeks. In order to evaluate the effect of cognitive stimulation training, we measured cognitive function before and after three training sessions each. Data were analyzed using descriptive statistics and a paired t-test analysis using a spss pc package, The results are as follows: 1) The recipients of the training program showed improvement in overall cognitive functioning. The MMSK - K score, recall levels of concepts, daily tasks, personal past history and performance of word fluency were significantly increased after training. 2) There was a significant improvement in cognitive functioning over the training period: recall levels of concepts, daily tasks, past personal history and performance of word fluency significantly increased over the training period progessively, The results suggest that cognitive stimulation training is effective in improving and maintaining overall cognitive function of elderly persons with dementia.
