The aims of the present study are two-fold: 1) to explore differences on speech perception between younger and older adults according to noise conditions; and 2) to investigate which cognitive domains are correlated with speech perception. Data were acquired from 15 younger adults and 15 older adults. Sentence recognition test was conducted in four noise conditions(i.e., in-quiet, +5 dB SNR, 0 dB SNR, -5 dB SNR). All participants completed auditory and cognitive assessment. Upon controlling for hearing thresholds, the older group revealed significantly poorer performance compared to the younger adults only under the high noise condition at -5 dB SNR. For older group, performance on Seoul Verbal Learning Test(immediate recall) was significantly correlated with speech perception performance, upon controlling for hearing thresholds. In older adults, working memory and verbal short-term memory are the best predictors of speech-in-noise perception. The current study suggests that consideration of cognitive function for older adults in speech perception assessment is necessary due to its adverse effect on speech perception under background noise.

The research has sought to examine the effect of cognition training using smart device application games on the cognition function of traumatic brain injury patients. The research was conducted to the seventeen traumatic brain injury patients with slight symptoms who were treated with occupational therapy. The patients were divided into the two groups, nine for the experimental group and eight for the control group. The experimental group was assigned to conduct cognition training using smart device application games and traditional cognition training for fifteen minutes each, and the control group has conducted the traditional cognition training for 30 minutes. All arbitrations were conducted for 30 minutes a day, five times a week and for four weeks. To assess the cognitive function, Korean Mini-Mental State Examination (K-MMSE), Lowenstein Occupational Therapy Cognitive Assessment (LOTCA) and visual memory 1 and 2 of Motor-free Visual Perception Test-3 (MVPT-3) were measured before and after the intervention. In the comparison of the change between the two groups, the experimental groups showed a significant improvement in the visual memory of Motor-free Visual Perception Test-3 and remembrance section of the Korean Mini-Mental State Examination (p<.05). The result of this research has confirmed that the cognition training using the smart device application game can make the positive change to the visual memory of the traumatic brain injury patients more than the traditional cognition training.
The purpose of this study was to investigate the frequency of falls, fear of falling, perceived health status, and number of disease according to cognitive function in community-dwelling the elderly women. This study utilized a descriptive survey design. Data were collected for six months with 311 elderly women who visited in public health center at S city. After obtaining Institutional Review Board(IRB) approval, a face-to-face, and private interview was conducted with each participant who was eligible and agreed to participate in this study by trained graduate-level nursing students. The questionnaires consisted of MoCA-K, K-MMSE, Fall Efficacy Scale, PHS scale. The collected data were analyzed with SPSS/WIN 17.0 program, which was used descriptive statistics, Chi-Square test and t-test, Correlation. The major findings of this study were as follows; 1) The average age of the subjects was 71.68±5.13 and cognitive function score was 22.14±4.32. approximately 35% of participants had fallen within one year. 2) there were significant differences in perceived health status according to cognitive function. 3) fear of fall and cognitive function, perceived health status and cognitive function were significant correlation. In conclusion, this study will contribute to establish strategies for preventing fall. Interventions for fall prevention and fear of fall enhancement should be developed with the consideration of the level of cognitive function in the elderly. Reducing the fear of falling through the comprehensive nursing intervention will ultimately improve the quality of life in the elderly.
Objective : This case study was to verify effects of cognitive rehabilitation and swallowing rehabilitation on Hypoxic-Ischemic Brain Injury patient by Suicidal Attempt. Methods : The subject was a 32-year old Hypoxic-ischemic brain injury patient by suicidal attempt. He received treatment once a day five times a week, for a half an hour for each session from September 8th to December 16th, 2016. Treatment were cognitive and swallowing rehabilitation. He was assessed based on Mini-Mental State Examination-Korean (MMSE-K), Korean-Modified Barthel Index (K-MBI), Computerized Neurocognitive Function Test (CNT), Videofluoroscopic Dysphagia Scale (VDS), American Speech-Language-Hearing Association National Outcomes Measurements System (ASHA-NOMS). Results : The patient's total MMSE-K score increased from 25 to 27. His K-MBI score increased from 74 to 88. His memory, attention span, and executive function (DST, VST, SWCT, WCST) by CNT scores were improved. VDS score has no changes to 34, 44.5 and 34. ASHA-NOMS score also has no change to 6, 2 and 6. Conclusion : The study showed that the application of the treatment of cognitive and swallowing in hypoxic-ischemic brain injury patient by suicidal attempt results has positive effects on cognitive functions, and swallowing function.
Objective : Perinatal hypoxic-ischemic encephalopathy (HIE) and prolonged febrile seizures (pFS) are common neurologic problems that occur during childhood. However, there is insufficient evidence from experimental studies to conclude that pFS directly induces hippocampal injury. We studied cognitive function and histological changes in a rat model and investigated which among pFS, HIE, or a dual pathologic effect is most detrimental to the health of children. Methods : A rat model of HIE at postnatal day (PD) 7 and a pFS model at PD10 were used. Behavioral and cognitive functions were investigated by means of weekly open field tests from postnatal week (PW) 3 to PW7, and by daily testing with the Morris water maze test at PW8. Pathological changes in the hippocampus were observed in the control, pFS, HIE, and HIE+pFS groups at PW9. Results : The HIE priming group showed a seizure-prone state. The Morris water maze test revealed a decline in cognitive function in the HIE and HIE+pFS groups compared with the pFS and control groups. Additionally, the HIE and HIE+pFS groups showed significant hippocampal neuronal damage, astrogliosis, and volume loss, after maturation. The pFS alone induced minimal hippocampal neuronal damage without astrogliosis or volume loss. Conclusion : Our findings suggest that pFS alone causes no considerable memory or behavioral impairment, or cellular change. In contrast, HIE results in lasting memory impairment and neuronal damage, gliosis, and tissue loss. These findings may contribute to the understanding of the developing brain concerning conditions caused by HIE or pFS.
Background: Cognitive impairment has been reported to be closely associated with poor oral health, and the relationship is bidirectional, as older adults with poor oral health and chewing function are at a higher risk of cognitive decline (CD). This cross-sectional study aimed to determine whether masticatory discomfort in Korean elderly increases the risk of daily living difficulty (DLD) related to CD and whether there is a difference in risk according to gender and age. Methods: The data used were obtained from the Korean Community Health Survey (2019). The final analysis included 22,154 people aged 65 years and older who completed the survey and responded to all items on the variables used in the study. Chi-square test, trend test, and complex sample logistic regression were performed for statistical analysis, to clarify the purpose of this study. Results: As masticatory discomfort increased, the elderly's DLD tended to increase (p-for trend, p<0.001). For logistic regression, adjusting for all covariates, participants who reported masticatory discomfort as "discomfort" (adjusted odds ratio [AOR]=2.45, 95% confidence interval [CI]=2.11~3.50) and "severe discomfort" (AOR=2.95, 95% CI=2.49~3.50) had a more than a two-fold increased risk of CD-related DLD compared to participants who reported "no discomfort at all." In age-stratified analyses, elderly men aged 75~84 years and elderly women aged 65~74 years had the highest risk of developing CD-related DLD. Conclusion: Oral care interventions to improve masticatory function in older adults may slow CD and improve CD-related DLD. We hope that this study will raise awareness among caregivers and clinical professionals regarding the importance of oral care for older adults with CD.
Objectives It was the aim to examine the association of the thyroid-related hormones with cognitive function, depression, and subjective memory impairment in community-dwelling elders with questionable dementia. Methods The sample consisted of 399 community residents with 'questionable dementia' aged 60 or over in whom serum thyroid-related hormones [thyroid stimulating hormone (TSH) and thyroxine] had been assayed. Cognitive impairment was defined using the Korean version of the Consortium Establish a Registry for Alzheimer's Disease. Depression was diagnosed using the Korean version of Geriatric Depression Scale and subjective memory complaint (SMC) was checked using the subjective memory complaints questionnaire (SMCQ). Age, gender, education, and the presence of apolipoprotein E {\varepsilon}4 were included as covariates. Results There was a significant positive association between verbal fluency test (VFT) score and serum TSH levels (p = 0.01). There was a significant negative association between SMCQ total score and word list memory test (WLMT)(p = 0.002) or word list recall test (WLRT) score (p = 0.013). Conclusions Lower serum TSH levels were associated with semantic memory (VFT), and we found that SMC was associated with episodic memory (WLMT and WLRT) in this sample.
PURPOSE. This pilot study was to find the influence of complete denture on the brain activity and cognitive function of edentulous patients measured through Electroencephalogram (EEG) signals. MATERIALS AND METHODS. The study recruited 20 patients aged from 50 to 60 years requiring complete dentures with inclusion and exclusion criteria. The brain function and cognitive function were analyzed with a mental state questionnaire and a 15-minute analysis of power spectral density of EEG alpha waves. The analysis included edentulous phase and post denture insertion adaptive phase, each done before and after chewing. The results obtained were statistically evaluated. RESULTS. Power Spectral Density (PSD) values increased from edentulous phase to post denture insertion adaption phase. The data were grouped as edentulous phase before chewing (EEG p1-0.0064), edentulous phase after chewing (EEG p2-0.0073), post denture insertion adaptive phase before chewing (EEG p3-0.0077), and post denture insertion adaptive phase after chewing (EEG p4-0.0096). The acquired values were statistically analyzed using paired t-test, which showed statistically significant results (P<.05). CONCLUSION. This pilot study showed functional improvement in brain function of edentulous patients with complete dentures rehabilitation.

The aim of this study was to assess the effectiveness of group therapeutic exercise programs on the cognitive function, Activities of Daily Living (ADL), and balance-performing ability in older adults. Fifteen community-dwelling subjects (mean age 73.7 yrs, standard deviation 2.4) participated in this study. An 8-week group therapeutic exercise program, including strengthening and balance training, breathing and gait exercise, and recreation, consisted of activities related to daily living. The Mini-Mental State Examination-Korean (MMSE-K), Modified Barthel Index (MBI), Berg Balance Scale (BBS), Functional Reaching Test (FRT), and Timed Up & Go (TUG) test were measured during pre-exercise and post-exercise points. The results of this study were as follows: 1. After eight weeks, the MMSE-K and MBI total score was more significantly increased for post-exercise tests than pre-exercise tests (respectively, p<.05, p<.01). 2. Of the MBI contents, personal hygiene, dressing, ambulation, and chair/bed transfers scores were significant increased for post-exercise tests. 3. Post exercise BBS, FRT, and TUG scores were higher than the pre-exercise scores. The difference was statistically significant (respectively, p<.05, p<.05, p<.01). These findings suggest that group therapeutic exercise can be used to improve the cognitive function, ADL, and balance-performing ability in elderly persons.
The purpose of this study was to analyze factors affecting malnutritional status of elders with dementia in facilities. The subjects were 140 elders with dementia residing in three nursing facilities in Chungnam, South Korea. Data were collected from May 30 to September 30 in 2016, using questionnaires. The collected data were tested by t-test, ANOVA, correlation, regression analysis. The malnutritional status of elders with dementia in facilities was high in the high-risk group (84, 60.0%), and the nutritional status was appeared to be related to sex, long-term care grade, Korean version-Activities of Daily Living and feeding difficulty and cognitive function. As a result of multiple regression analysis, significant influence variables on malnutritional status were Activities of Daily Living(${\beta}=0.379$, p=.001), feeding difficulty(${\beta}=0.264$, p=.001), cognitive function(${\beta}=-0.187$, p=.014) and these variables showed an explanatory power of 35.9% on malnutritional status. The results of this study are expected to be used as basic data for the development of malnutritional status improvement program for elders in facilities with dementia.
