REBT 독서치료 프로그램이 초등 저학년 아동의 인지•정서•행동 및 또래 관계에 미치는 영향 (The Effect of REBT Reading Therapy Program on Peer Relationships in Lower Elementary School Students)

  • 본 연구의 목적은 독서치료 프로그램(수업)이 초등학교 저학년 아동의 인지•정서•행동 및 또래 관계에 미치는 영향을 파악하는 데 있다. 이를 위해 8명의 아동을 대상으로 총 12회에 걸쳐 공공도서관에서 합리적 정서행동치료(REBT) 독서치료 프로그램을 진행하였으며 독서치료 프로그램의 효과를 분석하였다. 사전 연구를 거쳐 독서치료 프로그램을 구안하였으며 회기별로 아동들의 인지•정서•행동 및 또래 관계를 관찰하였다. 프로그램 진행 후 아동이 사용한 언어와 정서 표현, 행동 관찰일지와 사전•사후 사람•그림을 분석하였다. 분석 결과, 회기가 늘어날수록 초등학교 저학년 아동의 인지적 능력과 정서적 공감 능력이 향상되었으며 사소한 장난말, 놀림, 따돌림 등의 부정적 행동이 긍정적 행동으로 변화되었다. 이를 통해 REBT 독서치료 프로그램이 초등학교 저학년 아동의 인지, 정서, 행동 및 또래 관계에 긍정적인 영향을 미치고 있음을 확인하였다. 또한, 공공도서관에서 수행된 독서치료 프로그램은 소속된 학교가 다른 아동들의 또래 관계 형성에도 긍정적인 것으로 나타났다. 연구 결과는 공공도서관을 포함한 어린이 청소년 기관이 다양한 집단을 대상으로 한 독서치료 프로그램을 개발하고 운영할 필요성을 보여준다.

노인의 인지기능과 낙상유발 행동요인과의 상관분석 (Correlation of Cognitive Function and Fall-risk Related Behavioral Factors)

  • 목적: 본 연구는 노인의 인지수준이 낙상을 유발하는 위험 행동요인과 어떤 상관관계를 갖는지 알아보기 위하여 진행되었다. 연구방법: 2017년 10월부터 12월까지 3개월 간 4개의 도시에 거주하고 있는 65세 이상 인지장애가 있는 노인 및 인지장애가 없는 노인 43명을 대상으로 인지기능 평가(K-MoCA) 및 낙상행동요인척도(FaB)를 실시하여 자료를 수집하였다. 두 평가의 총점 결과간의 상관관계를 SPSS 22를 사용하여 분석하였고, 각각의 낙상행동요인과 인지수준과의 상관관계를 분석하였다. 결과: 노인의 인지수준과 낙상행동요인 간에 통계적으로 유의미한 양의 상관관계가 있는 것으로 나타났다(p<.01). 노인 낙상행동요인 척도의 총 30개의 문항 중 9개의 문항이 K-MoCA의 총점과 통계적으로 유의미한 상관관계를 보였다. 결론 : 노인의 인지수준이 높을수록 낙상위험 행동요인이 적고, 인지수준이 낮을수록 낙상위험 행동요인을 인식하지 못하고 낙상관련 위험행동을 하는 양상을 보인다. 특히 일부 낙상위험 행동요인은 인지수준과 상관관계가 있어 관련 중재를 제공할 시 고려해주는 것이 필요하다.

소아기 불안장애의 인지행동치료 효과 : 예비 연구 (Effect of Cognitive-Behavioral Treatment in Children with Anxiety Disorder: A Preliminary Study)

  • Objectives: This study aimed to evaluate preliminarily the clinical effects of cognitive-behavioral treatment in children with anxiety disorders. Methods: Subjects were 11 children between 2nd and 6th grade with anxiety disorder. All subjects were diagnosed through Kiddie-Schedule for Affective Disorder and Schizophrenia Present and Lifetime Version (K-SADS-PL) interview. The CBT program consisted of sessions once a week (60min/session) for 14 weeks with parent education. Results: Children and parents reported significantly improved social skills, social competence and decreased anxiety. However, there were no significant changes in children's negative thoughts and subjective depressive symptoms. Conclusion: Cognitive-behavioral treatment is expected to be effective in children with anxiety disorders such as generalized anxiety disorder, phobia, separation anxiety disorder, and obsessive-compulsive disorder.

조현병 환자를 위한 수용전념기반 인지행동 프로그램 개발 및 효과 (Development and Effects of an Acceptance Commitment-based Cognitive Behavioral Program for Patients with Schizophrenia)

  • Purpose: The purposes of this study were to develop an acceptance commitment-based cognitive behavioral program for patients with schizophrenia and to use it as an intervention tool for psychiatric mental health nursing and thereby confirm the program's effects on patients' psychological flexibility, psychological well-being, and treatment adherence including insight and attitude toward treatment. Methods: This study consisted of a pretest, posttest 1, and posttest 2 for a nonequivalent control group design. The participants were 42 patients with schizophrenia (experimental group: 21, control group: 21). The acceptance commitment-based cognitive behavioral program was conducted in 10 sessions for 5 weeks. Data collected from February to April 2018 were analyzed using the SPSS/WIN 25.0 for $x^2$ test, Fisher's exact test, independent t-test, and repeated measurement ANOVA. Results: The differences between experimental and control groups were statistically significant regarding psychological flexibility, psychological well-being, and insight. Conclusion: This study findings suggest that the acceptance commitment-based cognitive behavioral program can be used as an effective mental health nursing intervention for psychological flexibility, psychological well-being, and insight of patients with schizophrenia.

여자 청소년의 사회불안 감소를 위한 인지행동집단치료 프로그램의 개발과 효과 - 2년 추후연구 - (The Effects of a Cognitive-Behavioral Group Therapy Program for Social Anxiety in Female Adolescents)

  • The aim of the present study was to develop a cognitive-behavioral group therapy program for social anxiety in Korean adolescents and to examine its efficacy through pre-, post- and 2-year follow-up tests. The program included cognitive restructuring, exposure training and social skills training. KSAS-A and SASC-R were administered to 588 1st grade middle-school female students to assess their social anxiety levels. On the basis of double criteria procedure(top 10% scores on both scales), 59 students were selected initially. Individuals currently receiving clinical treatment were excluded. Finally, 40 students were selected and randomly assigned to either a treatment group or a wait-list control group. The program consisted of 12 weekly sessions, approximately 1.5-2 hours in duration. Whereas pre- and post-tests were administered to both groups, follow-up tests were administered to the treatment group only. The collected data were statistically analyzed through independent t-test and paired t-test. The results of the study were as follows: 1) At post-test students in the treatment group showed a very significant reduction in social anxiety and fear of negative evaluation by others in comparison with those in the wait-list group. 2) At post-test students in the treatment group showed a significant reduction in negative automatic thoughts in comparison with those in the wait-list group. 3) At post-test students in the treatment group showed a significant improvement in overall social skills in comparison with those in the wait-list group. 4) At post-test students in the wait-list group showed a significant increase in both social anxiety and negative automatic thoughts in comparison with those in the treatment group. 5) At 3-month, 1-year and 2-year follow-ups, the program's effectiveness was maintained.

The effects of a group cognitive behavioral therapy program using video communication for pregnant women with depressed mood in Korea: a pilot study

  • Purpose: This study presents the development of a group video communication-based cognitive behavioral therapy (CBT) program for depressed pregnant women. It also provides the results of a preliminary test of its effects on their depression, automatic thoughts, and dysfunctional attitudes. Methods: In this quasi-experimental single-group pre- and posttest design study, 13 pregnant women participated in a 4-week, eight-session group CBT program, based on Beck's cognitive theory and using video communications from November 2020 to January 2021. Pregnant women between 14 and 32 weeks who were members of an online maternity and parenting community and residing in the cities of Changwon and Gimhae, Korea, were invited to voluntarily participate. Trained nurses led CBT sessions of 3-4 participants per group via video communication. Participants were assessed pre- and postintervention with self-report questionnaires for measurement of depression, automatic thought, and dysfunctional attitude after normality test according to the Shapiro-Wilk test of the variables. The data were analyzed using paired t-test and Pearson correlation coefficients. Results: Depression (t=7.90, p<.001), automatic thoughts (t=4.89, p<.001), and dysfunctional attitudes (t=2.42, p=.032) significantly decreased after the 4-week online program. There were statistically significant correlations among the three variables. Conclusion: This program was found to be effective in reducing depression, automatic thoughts, and dysfunctional attitudes. Above findings suggest that a group CBT program using video communication can be an effective therapeutic modality that helps pregnant women at risk for depression alleviate their negative emotions related to depression.

우울한 임신 여성의 화상 인지행동치료 프로그램 참여 경험: 탐색적 질적연구 (The experiences of depressed pregnant women participating in a cognitive behavioral therapy program via video communication: an exploratory qualitative study)

  • Purpose: This study explored the experiences of pregnant women with depressed mood participating in a group cognitive behavioral therapy (CBT) program using video communication, based on Beck's cognitive theory. Methods: The participants were six pregnant women out of 13 women who had participated in an 8-session group CBT program using video communication for women with depressed mood (Edinburgh Postnatal Depression score of ≥9). Data were collected from February 20 through March 25, 2021. In-depth individual interviews were conducted through a video conferencing platform at 1 month post-baseline. Thematic analysis was done. Results: Three themes, 10 subthemes, and 38 concepts were derived from experiences of participating in the 4-week group CBT program (twice a week). The first theme, entitled "continuing realization" had subthemes of "a negative and instable self," "a selfish judgment that excludes others," and "a strong belief in self-control." The second theme, entitled "attempt to change for restoration" had subthemes of "shift to rational thinking," "freedom from suppressed beliefs," "tolerance of other people," and "courage for self-expression." The third theme, entitled "departure for a positive life," had subthemes of "emotional healing," "faith in oneself," and "reestablishing the criteria for happiness." Conclusion: Pregnant women with depressed mood expressed that continuing realizations and attempts to change supported their transition toward a positive direction of healing. Thus, they were able to change their distorted thinking into rational thinking through CBT using video communication. These findings support the use of group CBT using video communication with pregnant women who have depressed mood.

인지행동치료 후 약물 중단 예측 요인에 관한 연구 (Predicting Factors of Discontinuation of Medication after Cognitive Behavioral Therapy for Panic Disorder)

  • Objective : The authors experienced that cognitive behavioral therapy(CBT) could replace medication for controlling panic attacks and anticipatory anxiety symptoms. The objective of this study was finding out predicting factors of discontinuation of medication after CBT for patients with panic disorder. Method : A hundred forty-eight patients who met DSM-IV criteria for panic disorder with or without agoraphobia for at least 3 months had completed 12 weekly sessions of Panic Control Therapy(PCT ; Barlow et al). Eighty-one patients who could discontinue medication and sixty-seven patients who could not discontinue medication were measured with several scales as the pre- and post-treatment aassessment. The scales were Beck Depression Inventory(BDI), Clinical Global Impression(CGI), Spielberger State Anxiety Inventory(STAI-state), Anxiety Sensitivity Index(ASI), Body Sensation Questionnaire (BSQ), Panic Belief Questionnaire(PBQ), Agoraphobic Cognition Questionnaire(ACQ), Fear Questionnaire(FQ), Toronto Alexithymia Scale(TAS). Results : At the pre-treatment assessment, the scores of BDI, CGI, STAI-state, ACQ, BSQ were higher in the patients who could discontinue medication than in the patients who could not discontinue medication(t=-2.68, t=-4.88, t=-3.07, t=-3.68, t=-3.35, p<0.01). Conclusion : Patients with panic disorder who were less depressed, less anxious, less agoraphobic and who had less negative cognitions for the bodily sensation and who had higher scores in the therapist's assessment could discontinue their medications.

  • Acceptance and Commitment Therapy (ACT) is a functional contextual intervention approach based on the behavioral theory on human language, which views human suffering as originating in psychological inflexibility fostered by cognitive fusion and experiential avoidance. Thus, the goal of ACT is to enhance psychological flexibility using six core processes including acceptance, cognitive defusion, self-as-context, contact with present moment, values clarification, and committed action. Recent clinical trials have suggested the efficacy for ACT in the treatment of various mental illness and psychological distress. The aim of this review is to offer more knowledge and better understanding of ACT by presenting its underlying principle and an overview of the research field.

마음챙김명상과 불면증 (Mindfulness-Based Therapy for Insomnia)

  • Sleep disturbance is a common health complaint affecting an estimated 10-25% of the general population. Pharmacotherapy is a traditional treatment and cognitive behavioral therapy is well demonstrated as an efficacious treatment for insomnia. There is a growing interest in the effectiveness of mindfulness meditation for sleep disturbed populations. Mindfulness involves focused, purposeful, nonjudgmental attention to a particular idea. In clinical application of mindfulness practice, mindfulness-based stress reduction and mindfulness-based cognitive therapy are used widely. Clinical trials support the effectiveness of meditation for the treatment of insomnia. The aim of this article is to review the effect of mindfulness meditation on insomnia.
