Atypical Nodular Granulomatous Episclerokeratitis in a Dog (개에서 발생한 비정형의 결절성 육아종성 상공막염 1례)

  • A 5-year-old castrated male Cocker Spaniel was referred to Veterinary Medical Teaching Hospital of Seoul National University with a history of chronic conjunctival hyperemia and a fleshy corneal mass in the right eye. On ophthalmic examinations, it was observed that a well-vascularized fleshy mass at the dorsolateral limbus extended into the clear cornea. The lesion regressed by initial medications, including both topical and systemic corticosteroids, and topical cyclosporine A. However, the lesion relapsed and grossly infiltrated to cornea in a short period of time without improvement in spite of the immunosuppressive therapy, leading to the vision loss. The eye was enucleated and nodular granulomatous episclerokeratitis was confirmed on histopathological examination.

Efficacy of Twice a Week Selamectin Application as a Treatment for Mixed Canine Scabies and Ear Mite Infestation in a Dog (개에서 피부개선충(Canine Scabies)과 귀진드기(Ear Mite) 혼합감염에 대한 주 2회 selamectin 적용 일례)

  • A 6-month-old, male, American cocker spaniel was presented with severe pruritus, alopecia and head shaking. In physical examination generalized erythema, papules, scales and crust were observed. Otic discharge was dark-brown colored in both ear canals. After extensive dermatologic diagnostic evaluations, the infestation of scabies, ear mites, Malassezia spp, cocci were proved. Therefore, he was treated with selamectin 6 ㎎/㎏ which was applied to the skin in a single spot twice a week for 2 weeks in order to investigate the effect of selamectin on canine scabies and ear mites by this treatment protocol. When he was rechecked at 14th day after fourth being administered, scabies and ear mites wasn't seen. The dermatological signs, pruritus, papules were remarkably decreased and only slight scales were observed. After treatment of scabies and ear mites, the clavamox and koto-conazole was administered for cocci and Malassezia spp, respectively, for 1 week and topical therapy was initiated. with sebolytic and chlorhexidine shampoo once a week for 3 weeks. Two months later skin lesions were normal dermatologically and the hair was fully recovered. Despite of the expensiveness the selamectin is very effective in the dogs that infected by scabies and ear mites as this treatment protocol.

Diagnosis of Multiple Extrahepatic Portosystemic Shunt in Two Dogs (개의 다발성 간외성 간문맥전신단락증의 진단 2예)

  • Two dogs referred to Veterinary Medical Center, Chungbuk National University diagnosed as multiple extrahepatic portosystemic shunt were reported. The first dog was a 20-month-old, 8 kg, male Cocker spaniel with history of peritoneal effusion, diarrhea, anorexia and stunted growth. The second dog was a 3-year-old, 13.4 kg, male Jindo with a history of severe depression. Hematologic examination of first dog revealed mild microcytosis and nonregenerative anemia. All of 2 cases, serum chemical values showed increase of serum ammonia, ALP, r-GTP and glucose. In survey radiography, microhepatia was apparent. In the color Doppler ultrasonographic examination, the first dog revealed a dilated tortuous vein communicating with caudal vena cava was observed near the left kidney and the second dog revealed numerous shunting vessels ventral to L5 and L6. Transcolonic portal scintigraphy of the first dog confirmed the presence of portosystemic shunt. In intraoperative jejunoportography, the first dog showed single congenital extrahepatic portosystemic shunt and multiple acquired extrahepatic portosystemic shunts. The second dog showed multiple acquired extrahepatic portosystemic shunts. In these dogs, the presence of congenital and acquried portosystemic shunts and histopathologic findings were considered to represent a combination of multiple extrahepatic portosystemic shunts and noncirrhotic portal hypertension or portal vein hypoplasia.

Efficacy of itraconazole in 18 cases of Malassezia dermatitis in dogs (개에서 Malassezia 피부염에 대한 itraconazole 치료 18례)

  • Itraconazole was found to be an effective antifungal for the treatment of canine Malassezia dermatitis (MD). MD was diagnosed in 18 dogs, which were treated with itraconazole administered orally at 5 mg/kg of body weight, q12hrs, for 21 to 30 days. High prevalence breeds of MD were Maltese $(22\%)$, Cocker Spaniel $(17\%)$, Pekingese $(11\%)$, and Vizsla $(11\%)$. The dermatological signs of Malassezia dermatitis were crust $(31\%)$, alopecia $(25\%)$, hyperpigmentation $(25\%)$, scales$(19\%)$, erythema $(13\%)$, lichenification $(11\%)$, pustule $(11\%)$, ear swelling$(11\%)$, papules $(5\%)$, and offensive odor $(5\%)$. Commonly affected areas were ear canal $(41\%)$, axillae $(18\%)$, groin $(15\%)$, perianal $(12\%)$, ventral aspect of the neck $(9\%)$, interdigital spaces$(1\%)$, and muzzle $(1\%)$. Sixty seven percent of dogs with MD had cocci. Clinical responses of itraconazole were seen good, moderate, no responses of itraconazole, in $89\%,\;0\%,\;and\;11\%$, respectively, according to the owner's satisfaction to follow up call. Recurrence was detected on five good responsive dogs and adverse effects of the treatment were detected in only one dog. On the basis of this clinical study, itraconazole is a good choice in the treatment of canine Malassezia dermatitis. Efficacy, frequency of administration and veterinary approval are the major advantages.

Total Ear Canal Ablation and Lateral Bulla Osteotomy for Chronic Otitis Externa and Media in Dogs: Postoperative Recovery and Long-Term Follow-up (개에서 만성외이염과 중이염에 대한 전이도 적출술과 외측 고포절골술: 수술 후 회복과정과 장기간 예후)

  • The clinical results of 10 consecutive total ear canal ablation combined with lateral bulla osteotomy (TECA-LBO) in six dogs with chronic otitis externa and media were evaluated by the postoperative recovery and long-term follow-up. All dogs were selected for TECA-LBO on the basis of following five clinical symptoms. First, medical treatment couldnt improve clinical signs at least for eve. 2 months. Second, tympanic membrane was completely disappeared. Third, radiopacity was increased in tympanic cavity. Forth, petrous temporal bone was sclerosed. Fifth, ear canal calcification was progressed. And all cases were satisfied all five clinical symptoms. At 14. days after operation, the preoperative symptoms of chronic otitis externa and media which were scratching ear, pain, and hardening ear canal were resolved, and postoperative swelling, erythema, head tilt, and exudate from Penrose drainage were not existed In all cases. Loss of eye blink was happened in 4 cases, but these were disappeared between 14 days a(ter operation except one case on 3 months. All dogs were discharged form hospital at 14 days after operation. Between 3.5 and 6.5 months after discharging from hospital, para-aural abscessation was happened only in all Cocker spaniels. But this complication was solved by ventral bulla osteotomy (VBO). The dogs which didn't show para-aural abscessation after TECA-LBO didn't show scratching, pain, hardening of ear and hearing ability was improved, at 7.5 months after TECA-LBO. And the dogs which showed para-aural abscessation after TECA-LBO also didn't show scratching, pain, hardening of ear and hearing ability was also improved, at 7.5 months after VBO. In conclusion, After TECA-LBO, all dogs were recovered well without complication within 2 weeks except Cocker spaniel. And loss of eye blink can be cured naturally within 2 weeks after surgery. And para-aural abscessation can be happened between 4 and 7 months after TECA-LBO, so surgeon must follow-up until 8 months.
