A Comparison of the Opened Versus Closed-System of Suctioning -In Oxygen Saturation, Vital Signs and Suction Time- (개방형 흡인술과 폐쇄형 흡인술이 인공호흡기 사용 중환자의 산소포화도와 활력징후에 미치는 효과)

  • Purpose: The purpose of this study was to compare the difference of change in oxygen saturation, vital signs and suction time taken for the suctioning during endotracheal suctioning performed with closed suction system and with opened suction system. Methods: Data were collected from 31 adult patients with ventilator treatment who were admitted to a university hospital in Seoul and the collection period was from July 1 to November 15, 2005. Oxygen saturation, heart rate, respiration rate and mean arterial pressure were collected immediately prior to the suctioning intervention, during and 1 and 5 minutes after the suctioning from opened suction system and closed suction system. Results: 1) The difference in oxygen saturation was statistically significant in recovery time for oxygen saturation to return to baseline values after suctioning was significantly rapid on closed suction system (p<.05). 2) The difference in heart rate, respiration rate and mean arterial pressure was statistically insignificant 3) The suction time was shorter in closed suction system. Conclusion: Closed suction system is more efficient, as compared with the open suction system in the ventilator treatment.

  • Background: Tracheobronchial suctioning using the closed suctioning system has physiological benefits for critically ill patients. Despite these benefits, there are concerns about increased colonization of tracheobronchial tree by pathogenic organisms. The cost is another hinder to the introduction of closed suction system. The aim of this study was to evaluate the incidence of colonization and ventilator associated pneumonia and the cost-effectiveness of closed suction compared with open suction. Methods: During separated one month period, patients admitted MICU were cared by multiple-use, open suction, single-use, open suction and multiple-use, closed suction method, consecutively. Costs, colonization of tracheobronchial tree by MRSA and the incidence of ventilator-associated pneumonia (VAP) were analyzed. Results: One-hundred and six patients were enrolled. Twenty patients were treated with multiple-use, open suction, while 42 and 44 patients were cared with single-use, open catheter and multiple-use, closed catheter, respectively. Colonization by MRSA and the incidence of VAP were not different among three ways of suctioning. The overall costs per patient per day for suctioning were $10.58 for multiple-use, open suction, $28.27 for single-use, open suction and $23.76 for multiple-use, closed suction. Conclusion: Multiple-use, closed suctioning, when suction catheters were changed every 48 hrs, has the similar incidence of colonization of MRSA and occurrence of VAP and is a cost-efficient way of endotracheal suction.

  • Purpose: The purpose of this study was to examine the effects of a closed endotracheal suction system(CES) on oxygen saturation, ventilator associated pneumonia(VAP), and nursing efficacy in mechanically ventilated patients. Method: This study was conducted in the intensive care unit of a University Hospital in Gwangju City. Data was collected from July to October, 2003. Seventy mechanically ventilated patients were randomly divided into two groups; 32 for CES and 38 for open endotracheal suction system(OES) protocol. Twenty one nurses were also involved to examine the nurses' attitude of usefulness about CES. Result: $SaO_2$ was significantly different between CES and OES. The incidence of VAP in CES was lower than that of OES. Nursing efficacy was related to time, cost, and usefulness of the suction system. Time of suctioning in CES was shorter than that of OES. CES also contributed significantly to lower the cost of treatment than OES. The usefulness score of CES increased after 6 months of use. Conclusion: CES prevented VAP, was cost effective, and a safe suctioning system. CES can be used with patients with sensitivity to hypoxygenation and with a high risk of VAP.

Endotracheal Colonization and Ventilator-associated Pneumonia in Mechanically Ventilated Patients according to Type of Endotracheal Suction System (기관 흡인술 유형에 따른 인공호흡기 적용 환자의 기관 내 균집락화와 폐렴 발생률)

  • Purpose: This study was conducted to identify endotracheal colonization and the incidence of ventilator-associated pneumonia related to the type of endotracheal suction system. Methods: The participants in this study were ICU patients hospitalized between October 2009 to March 2010 who used ventilators for over 48 hr with closed (CSS, n=30) or open (OSS, n=32) suction systems. To standardize the pre-intervention suction system, a suctioning protocol was taught to the ICU nurses. Collected data were analyzed using ${\chi}^2$-test, Fisher's exact test, Wilcoxon rank sums test, Wilcoxon test, Log-rank test and Poisson regression. Results: Endotracheal colonization was higher in OSS than CSS from day 1 to day 8 while using a ventilator and there was a significant difference between the two groups. The CSS reached 50% of endotracheal colonization by the 4th day, whereas for the OSS, it was the 2nd day (p=.04). The incidence of ventilator-associated pneumonia showed no significant difference. Conclusion: For patients with a high risk of pneumonia, CSS must be used to lower endotracheal colonization.

Comparative analysis of the amount of postoperative drainage after intraoral vertical ramus osteotomy and sagittal split ramus osteotomy

  • Objectives: The purpose of this retrospective study was to compare the amount of postoperative drainage via closed suction drainage system after intraoral vertical ramus osteotomy (IVRO) and sagittal split ramus osteotomy (SSRO). Materials and Methods: We planned a retrospective cohort study of 40 patients selected from a larger group who underwent orthognathic surgery from 2007 to 2013. Mean age (range) was 23.95 (16 to 35) years. Patients who underwent bilateral IVRO or SSRO were categorized into group I or group II, respectively, and each group consisted of 20 patients. Closed suction drainage system was inserted in mandibular osteotomy sites to decrease swelling and dead space, and records of drainage amount were collected. The data were compared and analyzed with independent t-test. Results: The closed suction drainage system was removed at 32 hours postoperatively, and the amount of drainage was recorded every 8 hours. In group I, the mean amount of drainage was 79.42 mL in total, with 31.20 mL, 19.90 mL, 13.90 mL, 9.47 mL, and 4.95 mL measured at 0, 8, 16, 24, and 32 hours postoperatively, respectively. In group II, the mean total amount of drainage was 90.11 mL, with 30.25 mL, 25.75 mL, 19.70 mL, 8.50 mL, and 5.91 mL measured at 0, 8, 16, 24, and 32 hours postoperatively, respectively. Total amount of drainage from group I was less than group II, but there was no statistically significant difference between the two groups (P=0.338). There was a significant difference in drainage between group I and group II only at 16 hours postoperatively (P=0.029). Conclusion: IVRO and SSRO have different osteotomy design and different extent of medullary exposure; however, our results reveal that there is no remarkable difference in postoperative drainage of blood and exudate.

Improvement of Pump Performance and Suppression of Cavitation in a Centrifugal Pump (원심펌프의 성능개선과 캐비테이션 억제에 관한 연구)

  • Recent trends of a centrifugal pump are high speed in rotation and high pressure in head with high efficiency to meet the demands of industries. However, the newly developed pumps make trouble of pressure pulsation in the pumping system by performance instability of the pump. Moreover, cavitation, which is a main obstacle of high rotational speed in the pump, occurring in an impeller gives serious damages to the impeller and casing wall. The purpose of present study is not only to develop a simple method to improve pump performance but also to suppress the occurrence of cavitation in the centrifugal pump by use of J-Groove. J-Groove is a shallow groove installed on the casing wall in the meridional direction. The application of J-Groove to a centrifugal pump with a new type impeller of "semi-closed impeller" has proved its effectiveness as a useful countermeasure of the unstable pump performance and cavitation. The results show that the combination of semi-closed impeller and J-Groove can be applied successfully and improves both the pump performance and suction performance.

  • The objective of this paper is to know the characteristics of pressure through a simplified typical carburetor used in small engines at the different throttle opening conditions. The carburetor is the device responsible for creating the right air-fuel mixture according to the different engine operating conditions. It is activated by the static or the dynamic pressure. The carburetor dummy is geometrically similar of LPG brush-cutter engine's diaphragm carburetor and is made of acrylic. Suction system gives body to crankcase vacuum using the vacuum pump and throttle opening conditions are controled by transfer device. Carburetor venturi throat and fuel charging tube diameter is each 20mm, 4.1mm. The result of the work presents an unprecedented phenomenon of suction pressure variation inside the carburetor venturi. It is predicted that these unprecedented pressure variation be caused by minor losses; sudden contraction or expansion, open or partially closed and so on.

Effect of Shape of Discharge Port on Hydraulic Performance of Automotive Closed Type Water Pump (자동차 밀폐형 워터펌프의 토출구 형상이 수력성능에 미치는 영향)

  • Recent trend in pursuit of high performance and effectiveness for automotive cooling system has changed the application of material for impeller of automotive water pump from metal to high ability engineering resin, which can achieve optimization of design of impeller geometry and realize lightweight high efficiency water pump. Closed type water pump improves hydraulic loss of fluid through the clearance between volute casing and impeller compared with that of the existing open type water pump(Although closed type is heavier than open type for the same size and same material, adoption of plastics can solve the problem.). In the present study, the characteristics of hydraulic performance of closed type water pump were investigated with respect to the angle between shroud and hub of impeller and the shape of discharge port of volute casing. Performance tests were carried out for 4 cases, that is, for 2 impellers and 2 casings. The modification of shape of only discharge port can enhance the hydraulic performance by 10 percent and the pump efficiency by 4-6 percent.

  • Automobiles are very important as modern society is developed. Increase of the number of the automobiles cause environmental problem, that is, air pollution. So, many countries are adopting a environmental law. Automobile manufacturing companies have developing methods to prevent air pollution with increase of the efficiency of automotive engines. PCV(Positive Crankcase Ventilation) system which is one of them is made by the closed loop that consists of combustion chamber, crankcase, manifold suction tube and manifold. PCV valve is attached on manifold tube to control the flowrate of blowby gas. PCV valve is an important part in this system but it is difficult to design PCV valve which satisfies the required flowrate of blowby gas. In this study, our purpose is to help a PCV valve designer with the development of a design program. We used 4th order Runge-Kutta method and Bernoulli's equation to analyze the spool dynamic motion. By the comparison between our program and experiment, we think that a PCV designer can use our program in their work place.

  • In this paper, the air flow induced by the rotating flat disk is numerically investigated in a hope to better understand the air flow structures inside the wax spin coater for a silicon wafer polishing station. Due to the complex inner geometry of actual spin coater such as the casing around the rotating ceramic block and servo motor, recirculation of air flow is inevitably found on the coating target if the internal space of spin coater is closed at the bottom and it could be the possible source of contamination on the wax coating. By numerical flow simulation, we found that it is necessary to install the air vent at the bottom and to apply the sufficient air suction in order to control the path of air flow and to eliminate the air recirculation zone above the spinning surface of coating target.
