Infestation Status of Clonorchis sinensis Metacercariae in Cyprinid Fish from four Artificial Lakes in South Korea (소양호, 남양호, 안동호 및 춘천호에 서식하는 잉어과 어류의 간흡충 Clonorchis sinensis 감염 조사)

  • Clonorchis sinensis is a main parasite that infects humans by making freshwater fish as an intermediate host in South Korea. There are so many reports about the infestation status of Clonorchis sinensis metacercariae (CsMc) in freshwater fish living in the river, but there are a few studies of fish in the lake. In this study, we examined CsMc in Cyprinid fish sampled from Soyang Lake, Namyang Lake, Andong Lake and Chunchun Lake from 2016 to 2017. Metacercaria of trematodes were found from Hemiculter eigenmanni and Carassius auratus in Namyang Lake, and Zacco platypus and Opsarichthys uncirostris in Soyang Lake. As a result of PCR using Clonorchis sinensis specific primer sets, it was confirmed that the metacercariae from Hemiculter eigenmanni in Namyang Lake was CsMc. This study provides information on CsMc infestation status of Cyprinid fish in four lakes and it is the first report of CsMc infestation in Namyang Lake.

  • Parasitic diseases remain an unarguable public health problem worldwide. Liver fluke Clonorchis sinensis is a high risk pathogenic parasitic helminth which is endemic predominantly in Asian countries, including Korea, China, Taiwan, Vietnam, and the far eastern parts of Russia, and is still actively transmitted. According to the earlier $8^{th}$ National Survey on the Prevalence of Intestinal Parasitic Infections in 2012, C. sinensis was revealed as the parasite with highest prevalence of 1.86% in general population among all parasite species surveyed in Korea. This fluke is now classified under one of the definite Group 1 human biological agents (carcinogens) by International Agency of Research on Cancer (IARC) along with two other parasites, Opisthorchis viverrini and Schistosoma haematobium. C. sinensis infestation is mainly linked to liver and biliary disorders, especially cholangiocarcinoma (CCA). For the purposes of this mini-review, we will only focus on C. sinensis and review pathogenesis and carcinogenesis of clonorchiasis, disease condition by C. sinensis infestation, and association between C. sinensis infestation and CCA. In this presentation, we briefly consider the current scientific status for progression of CCA by heavy C. sinensis infestation from the food-borne trematode and development of CCA.

Experimental Studies on the Second Intermediate Host of Clonorchis sinensis I.   • The present studies were undertaken to determine accurately whether Korean native fresh-water shrimps can serve as the second intermediate host of Clonorchis sinensis. The experimental results obtained were as follows; 1. Metacercaria of Clonorchis sinensis was not discovered in 36 Macrobranchium nipponensis (De Hann), 1992 Palaemon paucidens (De Hann) and Palaemon madestus Heller, and 860 Caridina leucastica Stimpson, collected in Kimhae district, which were digested in artificial gastric juice, using binocular dissecting microscope. 2. Cercariae of the Chinese liver fluke could not approach the body wall of young shrimps due to streaming caused by their motion. 3. Young Palaemon spp. of 247 were divided into two groups and placed in two different room aquariums, one is at high temperature, the other is normal. Then they were contacted with the cercariae. Only 11 degenerated metacercariae were found in 9 Palaemon spp. It was suggested that they had died immediately or within a few days after penetration. Furthermore the temperature did not affect the infective ratio. 4. At the field experimental aquarium, no metacercaria of Clonorchis sinensis was discovered tn 100 Palaemon spp. which had been brought into contact with 5000 Parafossarulus manchouricus two trials. 5. Specific changes in the cercariae mixed with the extract of shrimps meat were not observed through the dissecting microscope. 6. These results suggest that the Korean native shrimps cannot serve as the intermediate host of Clonorchis sinensis.

Cell Death and Proliferation after Treatment and Reinfection of Clonorchis sinensis in the Sprague-Dawley Rat Bile Duct

  • The structural change and distribution of mitochondrial enzyme (ATPase, cytochrome-c-oxidase), cell proliferation (proliferating cell nuclear antigen, PCNA), cell death (caspase-3) and cell growth factor (fibroblast growth factor 8, FGF-8) in the Sprague-Dawley rat bile duct during Clonorchis sinensis infection was investigated. Experimental groups were divided into C. sinensis infection, superinfection and reinfection of C. sinensis after 'praziquantel' treatment group. As a result, C. sinensis infected rat bile ducts showed the features of chronic clonorchiasis, i.e., connective tissue thickening, ductal fibrosis and epithelial tissue dilatation. PCNA for cell proliferation increased in the infection group, and decreased after praziquantel treatment. Caspase-3 was distributed in reinfection group only. FGF-8 was distributed in the rat bile duct after praziquantel treatment but not distributed in infection and reinfection group. Overall, C. sinensis infection causes physical and chemical irritations and then brings on the abnormalities of intracellular energy metabolism and cellular growth factors, which hinders bile duct tissue from functioning properly, and resultingly, fibrosis occurs and epithelial cells dilated abnormally. More intense infection makes tissue fibrosis chronical and activates apoptosis factors.

Infection status of Clonorchis sinensis in residents of Hamyang-gun, Gyeongsangnam- do, Korea

  • Oriental liver fluke (Clonorchis sinensis) infection was surveyed among residents of Hamyang-gun, Gyeongsangnam-do, Korea during the period of January 2001 to March 2002. Total 1,041 stool samples were collected from residents who visited Public Health Center and its branches in Hamyang-gun and examined using formalin-ether sedimentation method. The overall egg positive rate was 16%, male showing higher positive rate (21%) than female (10%). The age group of 30 to 50 years had the highest egg positive rate of C. sinensis from 20% to 22%. The positive examinees were treated with praziquantel and educated individually to prevent reinfection . Egg positive rate in this area was decreased when compared with results recorded in the past, however, still remained more than 10%. This study suggests that periodic examination, treatment as well as education of residents should be continued and systematized.

Variation of nuclear and mitochondrial DNAs in Korean and Chinese isolates of Clonorchis sinensis

  • We compared the DNA sequence difference of isolates of Clonorchis sinensis from one Korean (Kimhae) and two Chinese areas (Guangxi and Shenyang), The sequences of nuclear rDNA (18S, internal transcribed spacer 1 and 2: ITS1 and ITS2) and mitochondrial DNA (cytochrome c oxidase subunit 1: cox1) were compared. A very few intraspecific nucleotide substitution of the 18S, ITS1, ITS2 and cox1 was found among three isolates of C. sinensis and a few nucleotide insertion and deletion of ITS1 were detected. The 18S, ITS1, ITS2 and cox1 sequences were highly conserved among three isolates. These findings indicated that the Korean and two Chinese isolates are similar at the DNA sequence level.

A Study of Clonorchis Sinensis Infection among Factory Workers in Pohang area. (포항지역 공장근로자의 간흡충감염 의식 조사)

  • Clonorchiasis should be realized as an important public health problems in Korea by their wide distribution, high prevalence rate and heavy infection intensity. This study was designed to obtain the infection rate, contributing factors as well as the behavior of infected persons among the factory workers in Pohang area where that parasite still remains as a problem. Study was undertaken from September '86 to October '87. Clonorchiasis skin test was performed for 3180 factory workers, and stools of skin test positive reactors were examined. Praziquantel was prescribed to stool examination positive persons for clonorchis sinensis ova. After one year. stool examination was repeated for them who received medicine. At the same time. a structured questionnaire was given and the results were analyzed and compared with control group. The results of this study are summarized as follows; 1. The infection rate of Clonorchis sinensis. 1) Clonorchiassis skin test positive rate was $26.2\%$ (834/3180). 2) Stool(Clonorchis sinensis ova) positive rate among skin test positive reactors was $21.6\%$ (129/598) All 129 infected persons were male. 2. Contributing factors of Clonorchis sinensis infection. 1) The chance of infection was higher in low-educated peple(p<0.01) 2) Stool(Clonorchis sinensis ova)positive rate was higher among the persons who reside near the riverside now(p<0.01), or among the persons who long history of riverside residence(p<0.01) 3) The infection rate was higher in thoes who like sliced raw fresh water fishes than in those who like cooked fishes(p<0.05). 4) Correct answering rate for questions about the cause and route of Clonorchis sinensis infection was low in infected persons. 5) The infection rate was lower among those who had experience of previous clonorchiasis test(p<0.01). 6) Family members of infected persons show higher rates of infection(p <0.01). 3. Treatment effect and compliance of infected person to treatment. 1) After one year, negative stool conversion rate was $85.4\%$. 2) $70.8\%$ of infected persons took the medicine; Only $3.1\%$ of them consulted to physician to know the medication effect; As many as $29.4\%$ of them continued to consume raw fresh water fishes. As a conclusion, City of Pohang area, Hyung San River in the center, is an endemic area of clonorchiasis. Current clonorchiasis control system seemed to be ineffective; many of the skin test positive group did not submitted their stool for examination; only $70.8\%$ of infected persons took the medicine even though the drugs were given free of charge; $29.4\%$ continued to consume raw fresh water fishes after taking medicine. Therefore. proper education program should be provided to improve the compliance to treatment. Follow-up for infected cases is mandatory to prevent waste of restricted budget.

Ultrastructural Change of the Bile Duct Fibroblast at Infected Rat with Clonorchis sinensis (간흡충에 감염된 실험쥐 담관 섬유모세포의 미세구조적 변화)

  • In this study, ultrastructural change of the bile duct fibroblast at infected rat with Clonorchis sinensis, and the distribution of lectin receptors and actin protein in cultured bile duct infected with Clonorchis sinensis. It explored using colloidal gold label complex with lectin WGA purified from wheat germ (Triticum vulgaris) and anti actin antibody purified actin (43 kDa) isolated from chicken back muscle. The lectin WGA with protein A gold complex labeled sections of the cultured fibroblast revealed gold particles specifically distributed on the multi vesicular form Golgi complex and cell surface of the fibroblast. The actin antibody with protein A gold complex labeled sections of the cultured fibroblast revealed gold particles specifically distributed on the cytoplasm of the fibroblast. Labeling of cultured fibroblast in rat bile duct infected with Clonorchis sinensis was then quantified and compared to that of cultured Fibroblast in Rat Bile duct. These results indicate that lectin WGA receptors are located in the multi vesicular form Golgi complex in the cytoplasm to the cytoplasmic process of the Rat bile duct fibroblast infected with Clonorchis sinensis. Therefore, the GlcNAc and NeuNac regions on the cell surface and cytoplasmic process appear to be functionally associated with cell-recognition and protection from other cell of the tissue, and linked with secretion and exocytosis of the fibroblst cytoplasm. GlcNAc and NeuNAc product in the multi vesicular form Golgi complex then it is transported to cell surface. Actin protein is many appears that infected fibroblast rather than normal fibroblast. The fibroblast of infected with Clonorchis sinensis are against of the physical and chemical stimulation. Then development of cytoplasmic process is relative some stimulation.

Experimental Infection of Clonorchis sinensis to Cyprinus curpio nudes (간흡충의 이스라엘잉어(향어)에 대한 감염실험)

  • As a part of observation on the suitability as a second intermediate host of Clonorchis sinensis, various infection experiments were tried to Cyprinus carpio nodus. The results obtained were summarized as follows: 1. The cercariae which attempted to contact with Cyprinus carpio nodus in the water were observed under the stereomicroscope. After contact, cercariae began to separate its tails from the bodies at 4 minutes increasingly, and then the numbers ranged to 80% at 13 minutes after the encounter. But very few cercariae could actually invade into the epidermis of the fish. 2. When Cyprinus carpio nudes were exposed to a number of cercariae in the beaker, only a few cercariae could invade through the epidermis. Most of the invaded cercariae were killed before forming the cyst. Also rare encysted cercariae were found to be dead within 48 hours. 3. Cyprinus carpio nudes were reared with Parafossarulus manchouricus shedding numerous cercariae in the indoor aquarium. When observed after 12 days, to determine the susceptibility, no metacercaria was found infected. 4. Cyprinus carpio nudum were reared with Parafossarulus manchouricus shedding numerous cercariae in Kimhae lake for 2 months in summer of 1982, in order to check the natural infection. No metacercaria of Clonorchis sinensis was found from the fish. 5. In the cases of control fish, Pseudoraibora larva, numerous cercariae were inspected on the course of invasion through epidermis of the fish under the stereomicroscope. And many metacercariae were also found from the Ssh whenever they were kept in beaker, indoor aquarium, or the lake for natural infection. The results suggested that Cyprinus carpio kudus might not be proper intermediate host of Clonorchis sinensis.

Excretory-secretory antigen is better than crude antigen for the serodiagnosis of clonorchiasis by ELISA

  • Although stool examination is the standard diagnostic method of clonorchiasis, serodiagnosis by ELISA using crude antigen is now widely used because of its convenience. However, ELISA diagnosis still suffers from cross-reactions, and therefore there is a need to improve the present conventional ELISA. The present study was undertaken to evaluate the diagnostic value of ELISA using excretory-secretory antigen (ESA) instead of crude antigen (CA) of Clonorchis sinensis. The diagnostic sensitivity of ELISA using excretory-secretory antigen was 92.5%, which was higher than that of ELISA using crude Clonorchis sinensis antigen (88.2%). In addition, the specificity of excretory-secretory antigen was found 93.1% while that of crude antigen was 87.8%. In summary, Clonorchis sinensis ESA was found to be a better serodiagnostic antigen than CA for ELISA.
