Objective : The purpose of this study is to help korean medical doctors write case reports based on Shanghanlun six clinical patterns and provisions accurately and effectively. Methods : To achieve the purpose of this study, we reviewed Shanghanlun six clinical patterns and provisions. After this, we formed a case report form according to the importance of Korean medical terms in Shanghanlun. Results : Through this study, we got the consistency and unity of case reports based on Shanghanlun six clinical patterns and provisions. Conclusions : This case report form is designed for gathering clinical data which are organized clearly according to the diagnostic system based on Shanghanlun six clinical patterns and provisions.

With the help of information technology, it is the most important to collect clinical data correctly in clinical trials. For this, recently, in many pharmaceutical companies and Clinical Research Organizations (CRO) have tried to adopt an Electronic Case Report Form (eCRF) system. The eCRF system is a computerized system designed for collecting and managing clinical data in clinical trials. Although the eCRF is recognized as a significant system for clinical trials, until now there are no widely useable, due to the huge effort and expense at the initiatory stage of adopting eCRF system. In this study, we inquired the use of eCRF system developed for clinical trial at Korea Institute of Oriental Medicine. Especially, we focused on the time required for entering clinical data into the eCRF, hours of use, and the number of queries according to the time flow.

This clinical report presents the clinical appearance and treatment approach in a case of excessive anterior teeth erosion resulted from swimming in a poorly-chlorinated swimming pool. Clinical findings revealed tooth sensitivity, severe enamel erosion resembling veneer preparations, and the presence of anterior open bite. A novel hybrid ceramic (Vita Enamic) was chosen for fabricating full-coverage crowns for this patient. After 6-months follow-up, the tooth sensitivity disappeared and the patient was satisfied with esthetic outcome. The hybrid ceramic restorations can be recommended with no complications.

Introduction The case report provides pivotal basis for clinical research and education, however systematically organized guideline for reporting case report was not properly applied and it tackled the development of traditional medicine in Korea. The purpose of this study was to implement guideline for reporting CAse REport(CARE guideline) incorporating unique characteristics of Sasang medicine with the help of examples and explanations. Methods and Results The guideline for reporting CAse REport for Sasang medicine(CARES guideline) including diagnosis of traditional medicine and treatment with acupuncture and medical herbs was suggested, and practical examples explaining details of each CARES items were provided. Discussion and Conclusion This study would provide foundation for consensus-based and common CARE guideline for traditional Korean medicine. And, the CARES guideline would provide clinical platform for developing refined Sasang type differentiation technique using reported biopsychosocial and clinical type-specific features and realistic cases for Problem Based Learning courses as evidence-based education.

Introduction: This case report addressed the effect of Korean medicine treatments on a patient with hemorrhagic anemia due to lower gastrointestinal bleeding. Case report: An 83-year-old male with lower gastrointestinal bleeding was treated with Korean medicine, including the herbal medication Subi-jeon. We evaluated the improvements of symptoms by hemoglobin levels and numbers of transfusions. From the 6^th day of taking the herbal medicine to discharge, a hemoglobin level of 7 g/dL or higher was maintained and the patient did not require blood transfusion. Conclusions: This clinical case study suggests that Subi-jeon might be effective in cases of hematosis of lower gastrointestinal bleeding.

We report an unusual case of respiratory depression and prolonged apnea after the second dose of 100-mg intravenous tramadol. Due to continuous pain at foley catheter keep site, intravenous tramadol was administered to the patient. Soon after the second dose of tramadol injection, the patient became apneic. The patient did not respond to verbal command and started exhibiting oxygen desaturation. The patient was quickly treated with 100% oxygen, and it took 4 hours for the spontaneous respiration to return to regular. This case report demonstrates that even two doses of tramadol administered intravenously may manifest as sudden and prolonged apnea. Respiratory depression with tramadol has been reported in patients with impaired renal functions and Cytochrome P(CYP) 2D6 gene duplication.

We report an unusual case of lupus flare induced by intravenous vancomycin in a patient with systemic lupus erythematosus. Due to methicillin resistant staphylococcus aureus in wound culture, intravenous vancomycin was administered to the patient. The patient had been on vancomycin for several days then she experienced fever, malar rash, and vomiting. Based on laboratory results, it was confirmed as lupus flare. Oral prednisolone was given to the patient for symptom control. However, when vancomycin was readministered, the patient had similar symptom to the previous one more intensively. Vancomycin was stopped then the patient became stable. This case report demonstrates that intravenous vancomycin may manifest as lupus flare in patients with systemic lupus erythematosus.

Transient global amnesia(TGA) is a clinical syndrome characterized by sudden, temporary dysfunction of anterograde and recent retrograde memory without other neurologic deficits. Different hypotheses have been considered for its etiology, but it still remains obscure. Recently the diffusion-weighted magnetic resonance imaging(DWI), has been considered as the sensitive tool to detect small punctate hyperintense lesions in the hippocampus in transient global amnesia(TGA). We report two TGA cases, all of them answers to TGA clinical criteria, and one of them showed two dot like high signal intense foci in Rt. hippocampus on DWI.

In clinical trials, it is important to have well designed case report forms (CRF) in order to obtain good quality of data. If CRF design at the first stage of your study goes wrong, after all efforts have been made, you may find practical difficulties in data analysis at the final stage of your study because of incomplete or wrong information. In this paper, the following rules fundamental to CRF design are introduced: rules in general, rules of format or style, categorized responses, forms with multiple records per subject, and international studies. Investigators are well aware of research goals, however sometimes they do not know how to express it on CRF. If they understand the rules fundamental to CRF design, time for CRF development will be saved and CRF completeness will be enhanced.

Objectives : The objective of this clinical case report is to describe an acupuncture treatment for dizziness after acoustic neuroma surgery. Methods : The patient was treated with acupuncture treatment for 4 months, 3 times per week. Duration for every treatment were 30 min by manual acupuncture with De Qi, which was designed to treat the dizziness and accompanied symptoms. Side-lying test was also used to evaluate dizziness and Hamilton Rating Scale for Depression(HDRS) to evaluate depression. We evaluated the clinical result of the treatment by observing the patients symptoms. Results : After acupuncture treatments for 4 months, patient's Side-lying test reduced to 10 second, and Hamilton Rating Scale for Depression reduced 4 points. Conclusions : The clinical case treatment report may suggest that the acupuncture treatment can be a meaningful option for the intractable dizziness after acoustic neuroma surgery, and also able to reduced depression scale.
