일 가정간호 실습교육 평가 연구 (An Evaluation Study on Home Care Clinical Practicum)

  • From 1990 to 1997 Home Care Education Programs have been offered at 11 Home Care Education Institutes. But there have been no revisions in the program. Especially in the clinical practicum, the Ministry of Health and Welfare proposed 248 hours as 'Family Nursing and Practice'. But each of institutes has developed their own program, and the clinical practicum is very different. Institutions for home care practice have been very limited and even now only 40 hospitals started home care in the second demonstration project. A few community-based institutions have also been offering home care services. This study was conducted to analyze home care clinical practicum offered at Y university, and plan for a revised home care clinical practicum. Y university clinical practicum was revised to include 'visits to community institutions', 'laboratory practice', 'hospital practice', 'discharge planning and home care practice', and 'home care specialty practice'. The results of the evaluation and plan for a revision are as follows: 'Visits to community institutions' is a practice that helps the students understand community resources which are available to home care nurses, and as an orientation to institutions for practice. 'Laboratory practice' is to used to improve nursing skills that are applicable to home care. Problems that the students identified in the laboratory practice were 'lack of opportunity for individual practice', and 'inadequate theoretical preparation for practice'. To address these problems the basic nursing skills laboratory was open and could be used freely by the home care students, and practice could be done after the theoretical lectures. 'Hospital practice' is a practicum in which the students apply nursing skills to patients and to obtain assessment skills for discharge planning. Using a preceptorship, five days for hospital practice should be offered. 'Discharge planning and home care practice' was done at Wonju Christian Hospital. This institute is too far away that this practice should be provided at different institutions as soon as it is possible to contract with home care institutions. Patients groups in different home care institutions are very different, so the 'Home care specialty practice' should be done after analyzing patient groups and choosing specialty areas. These areas are' care of patients with respiratory dysfunction', 'care of patients with neurologic dysfunction', 'care of cancer patients', 'care of patients on dialysis', and 'wound care'. This practice should be offered with a preceptorship, so preceptors, clinical directors, and students should meet for home visit orientation.

Development of a protocol for guidance in the pediatric nursing practicum in South Korea: a methodology study

  • Purpose: Practical training in pediatric nursing gives students the opportunity to apply nursing knowledge to children in a realistic clinical context. Clinical practice faculty (CPF) and clinical nursing instructor (CNI) have played important roles in the pediatric nursing practicum. This study was conducted to develop a protocol to guide clinical practicum in pediatric nursing. Methods: A service design methodology was employed between August 2020 and May 2021 at four universities and four hospitals in South Korea. The participants were five CPFs, five CNIs, five nursing college graduates, and 60 nursing students. The service design process had four phases: discovery, definition, development, and delivery. Data were collected through self-report questionnaires, in-depth interviews, and observations. The data were analyzed using content analysis and descriptive statistics. Results: The participants reported needs for providing concrete guidance and explanation, nursing practice experience, and a link between school and the clinical field. A protocol was developed to fulfill the participants' needs. The protocol comprised detailed information, teaching methodology, and partnership to guide students in the pediatric nursing practicum. Conclusion: The protocol developed in this study can be used to provide guidance for students' clinical practice in the field of pediatric nursing.

임상실습과 교내실습 간의 간호과정 자신감, 비판적 사고 성향, 교육 만족도에 대한 비교 연구 (A comparison study of Nursing Process Competence, Critical Thinking Disposition and Education Satisfaction between Clinical Practicum and School Practicum)

  • 본 연구는 장기화되고 있는 COVID19 팬데믹 상황에서 임상실습과 교내실습 교육 간 간호과정 자신감, 비판적 사고 성향, 교육 만족도에 대한 차이를 확인하기 위해 진행되었다. 비동등성 대조군 사후설계 연구로 성인간호학실습을 수강한 간호대학생 3학년 중 연구 참여에 동의한 55명을 대상으로 교내실습에 참여한 학생을 실험군(N=29), 임상실습에 참여한 학생을 대조군(N=26명)으로 분류하여 자료를 수집하였다. 수집된 자료는 SPSS/WIN 22.0 프로그램을 이용하여 분석하였다. 연구 결과 두 집단 간 간호과정 자신감과 비판적 사고 성향은 유의한 차이가 없었으나 대조군에서 교육 전반에 대한 만족도(t=-9.82, p=.040) 및 타 학생에게 추천(t=-1.88, p<.001)점수가 유의하게 높은 것으로 확인되었다. 비록 본 연구의 결과로 임상실습과 교내실습 효과의 차이를 확인할 수는 없었지만 차이가 보이지 않았다는 점에서 연구의 의의를 찾을 수 있겠다. 포스트 코로나 시대에 맞추어 실습교육 목표를 달성하기 위한 다양한 콘텐츠와 교수법, 실습으로 구성된 실습교육 프로그램의 개발과 프로그램의 효과분석에 대한 추후 연구를 제언한다.

온라인 콘텐츠 협동학습과 개별학습의 Lab실-임상실습 혼합실습교육 연계가 학습성취에 미치는 효과비교 (Comparison of Blended Practicum Combined E-learning between Cooperative and Individual Learning on Learning Outcomes)

  • Purpose: This study was conducted to compare blended practicum with clinical and lab combined e-learning between cooperative and individual group on learning outcomes. Method: A total of 63 junior Nursing students were recruited from C University in G city from May, 2012 to June, 2012. Ten hours lab practicum for two weeks was provided for both two groups during the period of adult nursing practicum. Prior to blended practicum, e-learning was conducted. For cooperative group, two hours off line team learning with a tutor for eight weeks was provided, in other hands, for individual group, any off line team learning was not provided and self study on line was not evaluated by the tutor. Results: The result of ANCOVA showed that critical thinking and self directed learning were significantly improved in the individual group compared to the cooperative group (F=-18.15, p<.001; F=28.12, p<.001). In other hands, clinical competence was significantly higher in the cooperative group than in the individual group (F=16.61, p<.001). Conclusion: Through development of self-leaning facilitating online contents, the blended practicum combined e-learning could be effective in critical thinking, self-directed learning and clinical competence. Further studies about e-learning strategies of off-line learning are still needed.

간호대학생의 COVID-19에 대한 지식, 불안, 간호전문직관이 임상실습 스트레스에 미치는 영향 (The Influences of COVID-19 Knowledge, Anxiety, and Nursing Professionalism on Clinical Practicum Stress among Nursing Students)

  • Purpose: This study sought to identify the levels of coronavirus disease 2019 (COVID-19) knowledge, anxiety, nursing professionalism, and clinical practicum stress, and the factors influencing this stress in nursing students. Methods: A cross-sectional and descriptive study was conducted with the participation of 140 junior and senior nursing students recruited from three nursing colleges in Busan and Gyeongsangnam-do. Data analysis was performed using independent t-tests, Pearson's correlation coefficient, and multiple regression using the SPSS/WIN 26.0 program. Results: The study arrived at a mean score of 63.18±13.89 for clinical practicum stress, which showed a significant difference based on the perceived health status and reasons for admission to nursing school. Anxiety and professionalism exhibited a noteworthy correlation with clinical practicum stress. The significant predictors of clinical practicum stress among nursing students were perceived health status (β=.39, p<.001), anxiety (β=.25, p=.033), and the reasons for admission to nursing school (β=.17, p=.012). The total explanatory power of these variables was 23.0% (F=6.28, p<.001). Conclusion: The results of this study suggest that it is necessary to provide effective education to promote nursing professionalism during a pandemic. It is also necessary to develop and include various programs in the curriculum that strategically intervene to prevent anxiety in pre-service nurses. This would reduce the stress in the clinical practice of nursing students when faced with new infectious diseases in the future.

간호학생의 임상적 판단 경험에 관한 내용분석 (Content Analysis of Experience of Nursing Students in Clinical Judgment during Nursing Practicum)

  • Purpose: To describe the lived experience of nursing students when faced with clinical judgment in a nursing practicum at the hospital. Methods: A descriptive research design was utilized. Participants were 79 students in the clinical practicum. Participant consent was obtained for ethical protection. Data were collected from August to December 2007 using a semi-structured questionnaire. Content analysis was utilized for data analysis. Results: Two categories and 5 themes were extracted from the data for 'difficult' and 'easy' clinical judgments. For the student category, the two themes were 'knowledge' and 'skill', while the three themes for the clinical education environment category were, 'judgment of clinical symptoms and signs', 'differences between theory and practice' and 'human relationships'. For coping, 2 categories and 5 themes were extracted for the difficult clinical judgment situation, while one category and one theme were found for the easy clinical judgment situation. Conclusion: To develop students' clinical judgment, there is need to develop the method of clinical skills using simulation in clinical teaching. For future research, a study on factors affecting clinical judgment of nursing students in hospitals is needed.

임상실습과 임상실습대체 교내실습 간의 간호과정 수행능력, 비판적사고성향, SBAR 기술지식에 대한 비교 연구 (A Comparison study of Nursing process competency, Critical thinking disposition and SBAR technique knowledge between Clinical practice and Alternative clinical practicum)

  • 본 연구는 임상실습과 임상실습대체 교내실습 간의 간호대학생의 간호과정수행능력, 비판적사고성향, SBAR 기술지식의 차이를 비교하기 위해 시도되었다. 임상실습에 참여한 간호학과 3학년 학생 20명, 임상실습대체 교내실습에 참여한 학생 18명을 대상으로 자료를 수집하였다. 수집된 자료는 빈도와 백분율, 평균과 표준편차를 구하였고, t-test, ANOVA, 비모수 검정인 Mann-Whitney U-test, Kruskal Wallis test로 분석하였다. 연구결과 임상실습과 임상실습대체 교내실습 교육에 따른 간호대학생의 간호과정수행능력, 비판적사고성향, SBAR 기술지식 간에는 유의한 차이가 없었다. 임상실습에서 대학생활만족도가 높은 경우에 간호과정 수행능력과 SBAR 기술지식이 유의하게 높았고, 임상실습대체 교내실습에서는 간호학 만족도가 높은 경우에 비판적사고성향과 SBAR 기술지식이 높았고, 학점이 높을 때 비판적사고성향이 높았다. 향후 팬데믹 상황에 대비하여 간호과정 수행능력과 비판적사고성향 및 SBAR 기술지식 능력을 향상시킬 수 있는 체계화된 임상실습대체 실습교육 방안이 마련되어져야 할 것이다.

간호대학생의 손씻기 지식과 수행의 관계에서 자기효능감과 지각된 행위통제의 조절 및 매개효과 (Moderation and Mediation Effects of Self-efficacy and Perceived Behavioral Control on the Relationship between Knowledge of and Compliance with Handwashing in Nursing Students)

  • Purpose: The aim of this study was to examine the relationship between handwashing knowledge and handwashing compliance in nursing students and to identify the moderation and mediation effects of self-efficacy and perceived behavioral control on the relationship. Methods: The participants were 172 nursing college students who completed the self-report measures. Data were analyzed using t-test, ANOVA, and Pearson correlation coefficient, Multiple regression analysis was used to test moderation and mediation effects. Results: The score for knowledge of handwashing was higher in participants who had completed fundamentals of nursing practicum (t=-6.54, p<.001) and the score for compliance with handwashing was higher in participants who had completed clinical practicum (t=-3.81, p<.001) or fundamentals of nursing practicum (t=-5.97, p<.001). Self-efficacy showed a significant moderation effect (z=2.21, p=.027) but not a mediation effect while perceived behavioral control had a significant mediation effect but not a moderation effect on the relationship between knowledge of handwashing and compliance. Conclusion: Findings indicate that clinical practicum and fundamentals of nursing practicum were helpful to nursing students in acquiring knowledge and in practicing proper handwashing technique. These findings suggest that continuing and repetitive education programs on handwashing need to be enhanced and strategies to strengthen self-efficacy and perceived behavioral control need to be included in education programs.

사회복지과 학생의 현장실습을 통한 지역사회복지의 주체적 참여방안 연구 (A study on the subjective participation plan in community welfare by the medium of the field practicum for social work student in undergraduate courses)

  • In social welfare arena field practicum has been an essential instrument and method for social work students to get socialized to the social work professional. However, to whom involve in the actual field practicum-universities(or colleges), institutes - have some difficulties. On the base of community welfare is a clinical approach in this times. To strengthen community welfare policy, the beginning of community welfare committee is such a good opportunity to tackle the problems of the field practicum. This study suggest that the problem of the field training are 1) a guidance of an individual social work institute, 2) the lack of social work institute, 3) the absence of the trainee's sense of values, 4) the bad conditions of the field practicum for social work students in undergraduate courses at the university(or college), and 5) the lack of professionalism for supervisors. This study makes the following social work suggestions to vitalize field practicum in community welfare. It is more likely important to 1) make the law of the field practicum for undergraduate social work courses, 2) make the coordinated programs of the field practicum for undergraduate social work course in community welfare, 3) enlarge the opportunity that a field specialist take further education, 4) support and carry of supervisor reeducation, 5) make the program development of trainee's personal experiences in social work values, and 6) social work professors make a practice in the social work field.

간호대학생의 폭력경험, 대처양식 및 회복탄력성이 정서반응과 임상실습스트레스에 미치는 효과 (Effect of Verbal Abuse Experience, Coping Style and Resilience on Emotional Response and Stress During Clinical Practicum among Nursing Students)

  • 본 연구는 간호대학생의 임상실습 시 폭력경험, 대처양식 및 회복탄력성이 정서반응과 임상실습스트레스에 미치는 효과를 알아보고자 실시하였다. 대상자는 간호대학생 261명이 참여하였고 2014년 12월 1일부터 12월 20일까지 자가보고식 질문지를 사용하여 조사하였다. 상관관계 분석 결과 폭력경험은 정서중심대처, 임상실습스트레스와 유의한 정적 상관관계가 있었으며, 문제중심대처는 정서중심대처, 회복탄력성과 유의한 정적 상관관계가 있었다. 정서 중심대처는 정서반응, 임상실습스트레스와 유의한 정적 상관관계가 있었으며, 회복탄력성은 정서반응과 유의한 부적상관관계가 있었고, 정서반응은 임상실습스트레스와 유의한 정적 상관관계가 있었다. 정서반응에 영향을 미치는 가장 높은 영향요인은 정서중심대처(${\beta}=.422$)였다. 임상실습스트레스에 영향을 미치는 가장 높은 영향요인은 폭력경험(${\beta}=.283$)이였다. 간호대학생의 안전한 실습환경을 위해 대인관계훈련 프로그램과 효율적인 의사소통훈련 프로그램을 개발하는 것이 필요할 것으로 제언한다.
