간호사를 위한 혈액투석교육프로그램의 개발과 효과 (Development of a Hemodialysis Nurse Educational Program and its Effects)

  • 본 연구는 간호사를 대상으로 혈액투석교육프로그램을 개발하고 적용한 후 간호사의 혈액투석관련 지식에 미치는 효과를 검증한 연구이다. 연구대상은 본 교육프로그램을 수강한 간호사 총 25명이며, 교육프로그램은 2011년 4월부터 9월까지 이론교육 10주와 실습교육 10주 총 20주간 진행되었다. 연구결과 교육프로그램을 적용하기 전 대상자의 지식점수는 13.96점에서 프로그램 후 17.80점으로 유의하게 증가하여(t=-7.748, p=<.001) 본 연구에서 개발된 혈액투석교육프로그램이 대상자들의 혈액투석관련 지식정도를 증가시키는데 효과가 있음이 확인되었다. 또한 혈액투석교육프로그램 후 대상자의 자기효능감은 $3.90{\pm}0.42$점, 교육만족도는 $4.02{\pm}0.77$점으로 높게 나타났다. 본 연구결과를 토대로 현재 체계적인 교육과정을 제공하는 혈액투석전문 교육기관이 없는 현실에서, 이러한 교육과정을 혈액투석실에서 근무하거나 근무할 예정인 간호사들을 대상으로 확대 적용한다면 혈액투석간호사들의 실무지식을 증가시키며 자기효능감 및 현장적응능력을 높일 수 있을 것으로 사료된다.

스티커 침을 이용한 경혈지압이 척추수술 후 배변에 미치는 영향 (The Effectiveness of Meridian Acupressure Intervention Using Sticker Needles to Bowel Movement on Post Spinal Operative Patients)

  • This study was conducted to identify the effect of meridian acupressure on defecation of patients with post spinal operation. The nonequivalent control group posttest only design was used. The data were obtained from 77 post spinal operative patients, 34 in the experimental 43 in control group in Y Hospital, Seoul, Korea. The neurosurgical unit A and B ward, assigned by matched sample by the name of operation such as laminectomy and posterior lumbar interbody fusion are performed. Meridian acupressure meant the method that an examiner presses response points distributed in the pass of energy vessel. In this study, meridian acupressure program was performed on as points in order of Hegue (LI-4), Zhigou (TE-6), Zusanli (S-36), Shangjuxu (S-37), Xiajuxu (S-39), Tianshu (S-25), Taichong (L-3) which was known to be related to large intestine. Data were collected from 1, July 2003 to 1, September 2003. Meridian acupressure program was carried out for 20 minutes 4 hours after operation twice daily. In order to evaluate the effect of meridian acupressure intervention, they were asked time of bowel recovery, gas passing, and defecation though questionnaire method. Data were analyzed by the SPSS/ WIN 11.0 program. The results of this study were summarized as follows; 1. Homogeneity tests of general characteristics and operation related characteristics of the experimental group and the control group were performed. General characteristics included age, sex, defecation habit, eating pattern, fluid intake, life style, activity, usage of laxative and etc. 2. Hypotheses were verification as follows; 1) Recovery of bowel sound of the experimental group who received meridian acupressure intervention was faster than that of the control group after spinal operation (t=-6.770,P=.000). 2) Time of gas passing of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-8.003, P=.000). 3) Time to defecation of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-9.026, P=.000). 4) Abdominal discomfort due to defecation of the experimental group who received meridian acupressure program was lesser than that of the control group after spinal operation (t=-3.431, P=.001). From these results, meridian acupressure intervention was effective for recovery of bowel sound, reduce time to gas passing, time to defecation and lessen abdominal discomfort due to defecation on post spinal operative patients. And therefore this intervention can probably considered on clinical practice.

소아혈액 종양환자의 요추천자 후 침상안정시간이 천자 후 두통에 미치는 영향 (The Effect of the Periods of Bed Rest on the Postlumbar Puncture Headache in Pediatric Oncology Patients)

  • Bed rest is recommended to prevent postlumbar puncture headaches(PLPHA), but the period of bed rest varies in the literature from 6 hours to 24 hours. In clinical practice the period of bed rest varies but nursing methods for adults and children have little difference. In Seoul National University Hospital, children have been given at least 6 hours bed rest after a lumbar puncture. Pediatric oncology patients require a lumbar puncture for an initial diagnosis, follow up treatment or administration of chemotherapeutic agent. But it is difficult for young children to lie supine or to refrain from their usual activities in any way, and unpleasant problems related to a shortage of beds often occurs during discharge or in an outpatient setting. The purpose of this study is to substantiate the preventive effect of PLPHA by the period of bed rest, to identify the other factors that influence PLPHA, and to use the nursing methods proper to children. The subjects were 65 children, ages 1-17, undergoing treatment in the children's cancer center at SNUCH during the period June 1, 1995, to Aug. 31, 1995. The team nurses asked questions about PLPHA of the parents and children in order to fill out a questionnaire. The data were evaluated by percent, t-test, Chi-square test and Mann-Whitney U test. Result; 1. There was no significant difference relating the bed rest time spent to the occurrence of postspinal headaches (t-test). 2. There was a significant risk of PLPHA in the children who were irritable before procedure and/or had experienced previous PLPHA(p<0.05, ${x^2}-test$). 3. The following factors were not found to be associated with increased risk of PLPHA: previous puncture experience, giving analgesics, the choice of puncturist, inpatient/outpatient status, gauge of needle, purpose, the amount of CSF removed, gender, diagnosis, the number of peripheral WBCs, previous lumbago experience after LP, position after bed rest, age, the number of aural puncture at the time. A longer period of bed rest is unlikely to be more effective to prevent PLPHA and seems impractical. A shorter period will save time and effort. Perhaps it will also allay some of the fears which surround LP. So 1 hour bed rest after LP is suggested and nursing methods for emotional support should be investigated to reduce PLPH.

일개 대학병원 간호사의 암성통증관리에 대한 지식과 태도 (A University Hospital Nurses' Knowledge and Attitude about Cancer Pain Management)

  • 본 연구는 일 대학병원 간호사 303명을 대상으로 암성통증관리에 대한 지식과 태도를 알아보고 통증 관리 교육을 위한 기초자료로 활용하기 위한 조사연구로 진행되었다. 연구 도구는 일반적 특성과 통증의 지식과 태도와 관련한 107문항으로 구성되었고 수집된 자료는 SPSS WIN 18.0을 이용하여 분석하였다. 연구결과 통증에 대한 일반적 지식의 정답률은 79.6%, 암성 통증 지식 점수의 정답률은 82.2%로 나타났고, 진통제 사용에 대한 지식의 정답률은 55.6%, 통증관리에 대한 태도의 정답률은 65.2%, 통증관리에 대한 지식과 태도의 전체 정답률은 70.7%로 나타났다. 일반적 특성에 따른 진통제 사용에 대한 지식은 연령, 교육정도, 직위, 총 근무경력, 현 근무지, 현 근무처 경력, 통증교육 경험에서 유의한 차이가 있었고 통증관리에 대한 태도는 성별, 교육정도, 현 근무지에서 유의한 차이가 있었다. 결론적으로 환자의 통증을 관리해야 하는 간호사의 통증과 관련한 항목별 지식과 태도의 평균점수가 높지 않음을 알 수 있고 대부분 통증 중재 방법으로는 진통제 투여에 의존하고 있는 것으로 나타났다. 따라서 간호사의 암성통증관리에 대한 지식과 태도를 향상시키기 위해 실무에 적용 가능한 교육 프로그램의 개발이 필요하고 개발된 교육 프로그램을 적용하여 임상간호사의 통증관리에 대한 지식과 태도의 변화를 반복 연구할 필요가 있다.

암화학요법을 받는 유방암 환자의 불안, 절망감 및 가족지지와의 관계 (The Relationship of Anxiety, Hopelessness, and Family Suppoort of Breast Cancer Patients Undergoing Chemotherapy)

  • This study was attempted to provide the basic data for nursing intervention to improve the psychosociological adaptation of patients receiving chemotherapy for breast cancer by examining relationship between anxiety and hopelessness that they are experiencing and family support, in order to help them successfully cope with various psychological problems. This study was carried out with 93 breast cancer patients who are receiving chemotherapy in the injection treatment room of K University Hospital located in the downtown of Taegu after having underwent mastectomy in the hospital between December 1995 and August 1996. This study used the systematized questionnaires which contain 7 questions about general characteristics, Spielberger's trait anxiety & state anxiety scale, the tool that WON(1987) modified the hopelessness scale which was developed by Beck et al.(1967) and the family support tool made by TAE(1985). By using the SPSS/PC program, this study obtained the real number and percentage for the general characteristics of the subjects, and mean and standard variation for the degrees of trait anxiety, state anxiety, hopelessness and family support. The correlation between each variables was identified on the basis of the Pearson Correlation, and the degrees of trait anxiety, state anxiety, hopelessness and family support in the general characteristics of the subjects were analyzed by using the t-test, ANOVA, and Duncan test. The results of this study were summarized as follows. In the general characteristics of the subjects, most of each group were 51 years old or more and the middle class in income, had educational background under elementary school, no job, Buddhism in religion and spouse, and were receiving chemotherapy using MTX and 5FU. It was shown that the degree of the subjects' trait anxiety is, on an average, 50. 29, state anxiety 49. 68, hopelessness 51. 46 and family support 34. 28. Both trait anxiety and hopelessness showed normal correlation ; the higher the degree of trait anxiety is, the higher the degree of hopelessness is, while trait anxiety and family support showed reverse correlation ; the higher the degree of trait anxiety, the lower the degree of family support that the subjects perceive is. State anxiety and hopelessness also showed normal correlation ; the higher the degree of state anxiety is, the higher the degree of hopelessness is. Family support and hopelessness showed reverse correlation ; the higher the degree of family support is, the lower the degree of hopelessness that the subjects perceive is. And family support and state anxiety showed reverse correlation but there was a statistically significant difference. The degree of trait anxiety in the general characteristics of the subjects showed a significant difference by age, job and religion, the degree of state anxiety a signigicant difference by job and religion, the degree of hopelessness a signigicant difference by age, educational background and existence or not of spouse. In conclusion, the breast cancer patients receiving chemotherapy perceive anxiety and hopelessness due to several causes such as diagnosis itself or side effects of chemotherapy, so that it is required not only to develop specific nursing interventions including family support to alleviate anxiety and hopelessness but also to apply such interventions to clinical practice.

임상 간호원을 위한 실무교육 과정으로서의 가족계획 (Family Planning as a Part of the Nursing-Staff In - Service Education Program)

  • When Korean family planning services began as a part of the National Policy in 1962, the annual population growth was 3.0%. This growth rate has been decreased to 2.0% during last ten year period. And it seems imperative that all hospitals, as well as related organizations, should participate in family planning in order to contribute to achieving the National goal of 1.5% population growth by 1976, the end of the Third Five Year Economic Development Plan. Nurses should be considered the most important human resources in charge of the core of family planning services in any setting. For the family planning services in the general hospital setting, nurses as a core members contribute much as change agent, motivators, counsellors, educators etc. A nurse can work with patients and their relatives when she is equipped with relevant knowledge and skills. Fur the more family planning cannot be ignored even in hospital setting where more comprehensive nursing care is needed Thus, the general objective of this study is to provide baseline data for better programming of In-service education in family planning so that effective hospital family planning nursing services can be made a part of comprehensive nursing care contributing to the national population program and human welfare. In order to meet the general objective, this study has the following specific objectives : 1. To find out the general characteristics of the clinical nurses working in Y Hospital 2. To evaluate their attitudes and practices of family planning 3. To assess their knowledge, attitudes and practices of population and family planning as professional nurses. 4. To examine and compare data collecting methods for the planning of an In-service Educational Program 5. To explore the contents to be included in this In-service Education Program. The study population randomly selected one hundred nurses working in Y Hospital A cross-sectional survey with questionnaires developed for this study was chosen for the study method. To collect reliable data, the questionnaires were distributed to and answered by the study population in a controlled situation. X²test and t-test was employed in analyzing the data. The findings of this study are as follows: 1. Y Hospital nurses had a lower ideal number of children (X=2.02) and showed no strong preference for male children, and 74% of them expressed the desire to use permanent methods of birth control 2. of this thirty Y Hospital nurses who were married 66.7% stated they were already practicing contraceptive methods. Most of them preferred male methods of contraception. 3. According to objective evaluation about knowledge of various aspects of population and family planning, respondents from collegiate programs significantly knew better the subjects on the average than did respondents from diploma programs of nursing. 4. There was a marked difference in the results of self-evaluation and objective evaluation in their family planning knowledge. It was found that the self-evaluation family planning knowledge seemed to be unreliable. Accordingly, the objective test methods appeared to be more reliable in the evaluation of knowledge levels. 5. The subject areas needed to be included in In-service education for the Hospital family planning services in Y Hospital are 1) rhythm methods, 2) tubal-legation, 3) family planning effects of contraceptives, 4) population growth, 5) demographic traction, 6) population structure and 7) infant mortality facts. In addition, 1) various oral contraceptives, 2) basal temperature method, 3) laparoscopic female sterilization, 4) interfering factors of family planning, 5) anatomy and physiology of the female reproductive organs were additional areas to be taught to respondents from 3-year diploma schools of nursing. Demographic transition was one subject area in which the four-year graduates need further study. 6. Population problems guidance and counselling in family planning instruction in the theory and practice of contraceptives should be included in future In-service Education Programs in order to provide more effective hospital Family Planning Services, stated 77.0% of the respondents.

호스피스 완화 간호사 역할규명을 위한 직무분석 (Job Analysis for Role Identification of General Hospice Palliative Nurse)

  • 목적: 말기 환자와 가족을 돌보는 호스피스 완화 간호사의 정의와 직무내용을 파악하고, 직무기술서를 작성하여, 호스피스 완화 간호사의 역할을 제시하는 데 있다. 방법: 호스피스 완화 간호사의 직무를 분석하기 위해 데이컴 직무분석 기법을 적용하였으며 직무분석의 빈도, 중요도, 난이도 조사를 위한 대상은 ELNEC project Korea에 참석한 간호사 136명의 설문자료를 대상으로 하였다. 직무분석을 위한 설문지는 8개의 임무와 36개의 일 내용으로 구성되었다. 36개의 일 내용에 해당되는 일의 요소는 137개 문항으로서 각 일의 요소에 대한 중요도, 빈도, 난이도는 각각 4점 척도로 측정하였다. 수집된 자료는 일반적 특성과 직무내용에 대한 빈도, 중요도, 난이도를 백분율, 평균, 표준편차로 분석하였다. 결과: 호스피스 완화 간호사는 '간호사 면허 소지자로서 호스피스 완화의료 영역에서 신체적(신체사정, 통증 및 증상관리 포함), 심리사회적, 영적 간호 돌봄에 대한 임상경험과 실무능력을 갖춘 간호사'로 정의하며, 호스피스 완화 간호사 직무 기술서 임무 8개, 일 36개, 일의 요소 137개로 구성되었다. 호스피스 완화 간호사의 임무에 대한 빈도, 중요도, 난이도 평균은 각각 2.94, 3.66, 2.80점이었고, 가장 빈도가 높은 임무는 자료수집(평균 3.23), 빈도가 가장 낮은 임무는 환경 및 자원관리(평균 2.74)이었다. 각 임무에 대한 중요도는 모두 평균 3.50점 이상(평균범위 3.53~3.72)으로 모두 중요한 임무라고 응답하였다. 난이도는 의뢰/조정/협동(평균 2.63)과 환경 및 자원관리(평균 2.65)를 제외한 모든 임무 영역에서 비슷한 난이도(2.73~2.77)를 보였다. 호스피스 완화 간호사 직무의 일에서 가장 빈도가 높은 역할은 통증 사정(평균 3.64)이었고 빈도가 가장 낮은 역할은 재정관리하기 (평균 2.36)이었다. 중요도 수준에서 가장 높은 점수를 보인 일은 통증 사정(평균 3.92)이었고 가장 점수가 낮은 일은 진단 검사를 통한 자료수집하기(평균 3.43)이었다. 난이도 항목에서 가장 점수가 높은 일은 말기환자와 가족의 영적 상태 사정하기(평균 3.11)였고 난이도가 가장 낮은 일은 문서/정보관리하기(평균 2.31)이었다. 결론: 이상의 호스피스 완화 간호사의 역할은 호스피스 전문 간호사에 비해 직접간호 실무 영역에서 더욱 활발한 역할을 수행하고 있으나 보다 체계적인 통증 및 신체증상 관리에 대한 계속교육이 요구되며 치료적 관계를 형성하기 위한 심리사회적, 영적 요구사정 및 중재, 의사소통 및 상담, 사별관리 영역의 실무지침 및 심화교육이 절실히 필요한 것으로 파악되었다. 이를 통해 호스피스 완화의료 대상자(환자와 가족)의 요구에 맞는 변화되고 진보된 호스피스 완화 간호사의 역할을 수행할 수 있다고 본다. 또한 호스피스 완화 간호사의 직무분석 결과를 토대로 이들도 호스피스 관련 자격인증을 받을 수 있는 호스피스 완화의료학회와 호스피스 완화간호사회 및 대한간호협회 차원의 노력이 조속히 진행되어야겠다.

Toxicity of Eun-bi San Pharmacopuncture Injection inthe Muscle of Rats

  • Objectives: This study was carried out to analyze the single-dose toxicity of Eun-Bi San pharmacopuncture injected into the muscle of Sprague-Dawley (SD) rats. Methods: All experiments were performed at Biotoxtech, an institution certified to conduct non-clinical studies under the Good Laboratory Practice (GLP) regulations. Six week old SD rats reared by ORIENTBIO were chosen for this pilot study. The reason SD rats were chosen is that they have been widely used in safety tests in the field of medicine, so the results can be easily compared with many other databases. The Eun-Bi San pharmacopuncture was made in a clean room at the Korean Pharmacopuncture Institute (KPI, K-GMP). The constituents of the Eun-Bi San pharmacopuncture are Angelicae gigantis radix, Strychni semen and Glycyrrhizae radix. These were extracted at low temperature and low pressure in an aseptic room at the KPI. Doses of Eun-Bi San pharmacopuncture, 0.25, 0.5 and 1.0 mL, were administered to the experimental groups, and a dose of normal saline solution, 1.0 mL, was administered to the control group. This study was performed under the approval of the Institutional Animal Ethics Committee of Biotoxtech Co., Ltd. Results: No deaths or abnormalities occurred in any of the four groups. No significant changes in weight, hematological parameters or clinical chemistry between the control group and the experimental groups were observed. To determine if abnormalities existed in any organs and tissues, we used microscopy to examine representative histological sections of each specified organ; the results showed no significant differences in any of the organs or tissues. Conclusion: The results of this study suggest that treatment with Eun-Bi San pharmacopuncture is relatively safe and that its clinical use may be beneficial. Further evaluations and studies on this subject will be needed to provide more concrete evidence in support of these conclusions.

Medical Students Understanding of The Scope of Plastic Surgery

  • Background Plastic surgery has developed to benefit in a variety of challenging areas formerly handled by other disciplines. Medical students do not have a clear picture of plastic surgery as a career due to lacking scope, clinical practice, and understanding of plastic surgery as a clinical area of expertise, including general practitioners, nursing staff, medical trainees, and the general public, and misconceptions about the extent of reconstructive and plastic surgery. Methods A cross-sectional observational study was conducted on Kuwait University Medical students (2nd-7th Years) over a period of 1 month. A questionnaire and a consent form were provided to eligible students. The inclusion criteria were Kuwait University Medical students from 2nd to 7th Years with signed consent form. The response was collected via email sent in coordination with the Vice Dean of Student Affairs in the Faculty of Medicine. Using statistical package for the social sciences, responses were statistically analyzed. Pearson's chi-square test was used to calculate p-values, where p < 0.05 was considered statistically significant. Results A total of 244 eligible medical students, 121 males and 123 females, were included in the study, with a mean age of 21 (±2) years. Similarly, 126 (51.6%) were preclinical students (2nd-4th-year students), while 118 (48.4%) were clinical students (5th-7th-year students). About 79.8% of medical students believed that plastic surgery plays an essential role in trauma management, whereas 9.2% did not consider plastic surgery significant for trauma management. This study found that only 15.5% of medical students were interested in enrolling in plastic surgery residency after graduation, while 47.1% of students did not consider plastic surgery residency after graduation. However, 37.4% were uncertain. The two most driving factors in deciding on plastic surgery residency were expected income (61.8%) and lifestyle (14.3%). Conclusion Improving medical students' education quality can enhance their perception and awareness of plastic surgery. Students should be taught the broader scope of plastic surgery. The inclusion of formal training during undergraduation is the essence of time and should be added to or improved during plastic surgery rotations with more emphasis on reconstructive and hand/peripheral nerve surgery. Student-led interest groups can be a useful tool for educating students about their specialty.

말기 암환자의 체험에 관한 현상학적 연구 (Lived Experience of patients with Terminal Cancer : Parses Human Becoming Methodology)

  • Human health is an integral part of experience in the process of Human Becoming. Through continual interaction with the environment human beings freely choose experience and develop as responsible beings. The process of the health experience of patient with terminal cancer is a unique. he objective of this study is to understand the lived experience of patients with terminal cancer in order to provide basic information for nursing care in the clinical setting and to develop a theoretical background for clinical practice. This study is to de-scribe and define the lived experience of patients with terminal cancer in order to provide a foundation for nursing research and education. Data collection has been done between December 1993 and November 1994. The subjects included five persons -four females and one male : one who was in her sixties, one in his fifties, two in their forties, and one who was in her thirties. The researcher has met with these patients 35 times, but at eight times the patient was in a stuporous condition and not able to participate, so these were not included in the data analysis. Parse's "Human Becoming Methodology", an existential phenomenological research methodology is used for this study. Data has been collected using he dialogical engagement process of "I and You", the participant researcher and the participant subject. Dialogical engagement was discontinued when the data was theoretically saturated. Data was analyzed using the extraction - synthesis and heuristic interpretation. The criteria of Guba and Lincoln(1985). and Sandelo wski(1986) : credibility, auditability, fitness and objectivity were used to test the validity and reliability of the data. The following is a description of the structure of the lived experience of patients with terminal cancer as defined by this study : 1. Structure : 1) Suffering through the reminiscence of past experience 2) The appearance of complex emotions related to life and connectedness 3) The increasing importance of significant people and of the Absolute Being 4) The increasing realization of the importance of health and belief 5) Desire for a return to health and a peaceful life or for acceptance of dying and a comfortable death In summary the structure of the lived experience of these patients can be said to be : suffering comes through reminiscence of past experience, and there are complex emotions related to life and connectedness. Significant people and the Absolute Being become increasingly important along with a realization of the importance of health and faith. The lived experience of patients with terminal cancer involves the solving of past conflicts, and the experience of the healing and valuing of sorrow and pain. Through the relation of life and health, and the complex emotions that arise, the lived experience of revealing - concealing is of paradoxical emotions. The increasing importance of significant others and of the Absolute Being shows Connecting and Separating an on- going process of nearness and farness. Revision of thoughts about health and faith is interpreted as transforming and desire for restoration to health and a peaceful life or acceptance of dying and a cowfortable death, as powering. In summary, it is possible to see, in the lived experience of patients with terminal cancer, the relationship of the five concepts of Parse's theory : valuing, revealing -concealing, connecting-separating, transforming, and powering. From Parse's theory, the results of this study show that meaning is related to valuing, rhythmicity to revealing-concealing and connect-ing-separating, and cotranscendence to transforming and powering.
