A Case Report of Central Post-stroke Pain Improved by Gami SSanghwa-tang (가미쌍화탕으로 호전된 뇌졸중 후 중추성 통증 환자 치험 1례)

  • ■ Objectives The purpose of this case study is to report the effect of Gami SSanghwa-tang on a patient with central post-stroke pain. ■ Methods The patient was treated with herbal medicine Gami SSanghwa-tang, acupuncture, pharmaco-acupuncture, and moxibustion. The treatment effect was evaluated by Numerical Rating Scale(NRS), Neuropathic Pain Symptom Inventory(NPSI), and 36-item Short-form Health Survey(SF-36). ■ Results After the treatment, the NRS score of pain intensity was reduced from moderate to mild degree. The total NPSI score and subscores also decreased, as the various features of the pain were relieved. The SF-36 score increased, as the patient's quality of life improved. ■ Conclusion This case study suggests that Gami SSanghwa-tang, could be effective in reducing pain and improving quality of life of patients suffering from central post-stroke pain.

A Case Study of a Patient with Tentorium cerebelli Subdural hematoma Using Oreong-san (소뇌천막에 발생한 경막하출혈 환자의 오령산 치험 1례)

  • A case of a 73-year-old male patient who fell down, presented with unusual traumatic subdural hematoma localized on the lesion of tentorium cerebelli. He was treated with acupuncture, electroacupuncture and herbal medicine - 五苓散(Oreong-san). To evaluate the progress, we followed up computed tomography imaging and checked up changing of symptoms. After 17days of treatment, there were notable improvement in computed tomography imaging and symptoms. Oreong-san might be effective in treating tentorium cerebelli subdural hematoma.

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine(TKM) on a patient with insomnia occurred after onset of thalamic hemorrhage. ■ Methods A patient with insomnia diagnosed as thalamic hemorrhage was treated with herbal medication, acupuncture, electro-acupuncture, cupping and moxibustion. Then we evaluated the improvement with the number of times the patient woke up, PSQI-K(Pittsburgh Sleep Quality Index-Korean), ISI-K(Insomnia Severity Index-Korean) and KESS(Korean version of Epworth sleepiness scale). ■ Results Decrease in scores of PSQI-K, ISI-K, KESS and the number of times the patients woke up were observed after the TKM treatment. ■ Conclusion This case showed the effect of TKM treatment on poststroke insomnia.

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine(TKM) on a patient with poststroke urinary retention ■ Methods A stroke patient with urinary retention was treated with herbal medication(Paljungsan), acupuncture, electro-acupuncture and moxibustion. To evaluate the effect of TKM on post-stroke urinary retention, we observed voiding pattern and residual urine after voiding. ■ Results Maintenance of regular self voiding and decrease of residual urine after voiding were observed after the TKM treatment. ■ Conclusion This study showed the effect of TKM treatment on Urinary retention of stroke patient.

  • ■Objectives This case study is to report the effects of Korean medicine on parkinsonism patient's Gait Disturbance. ■Methods During 12 days of hospitalization, the patient was treated by acupuncture, pharmaco-acupuncture, moxibustion, herbal medicine, especially Cheongsimyeonjatang-gamibang. In order to assess the change of symptoms, we used a 3-Dimensional(3D) gait analysis system, Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait video and self-evaluation of discomfort. ■Results After treatment, The improvements of walking pattern were observed in both objective analysis results of gait analysis system and subjective video analysis. And the UPDRS score decreased, especially Part III score decreased more than minimal clinically important difference(MCID). In addition, There was improvement in self assessment of the patient. ■Conclusion This study suggests that Korean medical treatment might be effective in motor disorder of parkinsonism patient.

  • ■Objectives This study is to report the effectiveness of Korean medicine on the treatment for multiple system atropy(MSA-C) patient complaining of autonomic dysfunction. ■Methods The patient was treated with Korean medicine(mainly DokhwalJihwang-tang Gami-bang) for 39 days. The evaluations were performed using UMSARS(Unified Multiple System Atrophy Rating Scale), SCOPA-AUT(The Scale for Outcomes in Parkinson's disease-Autonomic), K-OGS(Korean version of the Orthostatic Grading Scale). ■Results After treatment, the UMSARS score decreased from 25 to 18, the SCOPA-AUT score decreased from 21 to 14, K-OGS score decrease from 15 to 8. ■Conclusion This case suggests that Korean medicine treatment may be effective for MSA-C patients with autonomic dysfuction.

  • Zoster-associated limb paresis is a relatively uncommon complication of herpes zoster that is characterized by focal motor weakness. Awareness of this disorder is important to avoid unnecessary invasive investigations and to ensure appropriate treatment. We report a case of a herpes zoster involving the femoral nerve.

  • Hashimoto's encephalopathy (HE) is a heterogeneous encephalopathy with diverse clinical presentations. Here we report on a 69-year-old woman who presented with confusion, aphasia, fever, and focal ictal discharges. Cerebrospinal fluid analysis and a workup for other fever origins revealed no abnormality and a high level of thyroperoxidase antibody was detected, which findings led to a diagnosis of HE. The symptoms subsided after treatment. This study highlights the importance of considering HE in patients presenting with fever and abnormal EEG findings.

  • In this study, a 54-year-old female woman diagnosed as Multiple System Atrophy (MSA) presented diplopia with other symptoms - gait disturbance, ataxia, sleep apnea, dysuria. She had been cared with Korean medical treatment - Herbal medicine, acupuncture, bee venom acupuncture, electroacupuncture, cupping, moxa. Notable improvement was observed in diplopia expressing time and Unified Multiple System Atrophy Rating Scale (UMSARS). For MSA patient with no typical treatment indispensable, Korean medical treatment may be effective.
