It is important to assess foot posture when investigating the relationship between lower extremity dysfunctions and foot types. Although several measurements of static foot posture have been used, there is no consensus regarding clinical measurements for foot posture. The aim of this study is to explore the differences among navicular drift (NDt), foot posture index (FPI), arch index (AI), dorsal arch height ratio (DAHR), normal navicular height truncated (NNHt) and to discover the most effective measurement. After foot types were classified by navicular drop test (NDp), clinical measurements of NDt, FPI, AI, DAHR, and NNHt were performed on 64 subjects' feet. ANOVA analysis was used for the variance of the difference between the NDp and the five kinds of clinical measurements, and the level of significance was set at ${\alpha}$=.05. The results showed that all five clinical measurements demonstrated significant differences with navicular drop. In post-hoc, FPI and NNHt showed significant differences in all foot types. The five clinical measurements are suitable the classification of foot types through the NDp. Therefore, it could be possible to assess correct and objective foot posture by using FPI and NNHt.
In plastic surgery, patient photography is a vital component of clinical, educational, legal, and research documentation. Optimal acquisition of photographic data requires a dedicated photography studio or a three-dimensional anatomic scanner, both of which are financially impractical for most clinicians. Simplified photo standardization is proposed for use in random clinical settings by using a portable device called the Mirror Stand (MirS). This model device aims to mimic a studio environment by incorporating the basic elements of producing consistent photographs. The pilot MirS is designed for facial photography. Images of 40 random subjects were obtained using the MirS with three different cameras. Real anthropometric measurements of each subject were collected, compared with the photographic measurements, and analyzed. In this study, all three cameras produced equally reliable measurements. Actual facial measurements were comparable to the photogrammetric measurements obtained from photographs taken using the MirS. A constant formula was derived; it allowed the conversion of photographic values into real anthropometric values. The MirS produced consistent photographs with respect to the measurements. The photographs obtained could be translated reliably into their real anthropometric measurements. Therefore, the MirS can be applied in daily practice, providing an efficient alternative for obtaining a standard justifiable photograph.
Purpose: Direct anthropometry is an ideal method for preoperative planning and postoperative evaluation in plastic surgery, but it requires highly skilled specialty. Indirect anthropometry, especially photogrammetry, is an alternative method. In photogrammetry, standardized clinical photography is essential. Photogrammetry-based standardized clinical photography has several advantages over direct anthropometry. It is easier to measure and has less chance to make errors during measurement. Furthermore, it is possible to repeat measurements, and available for follow up study based on permanent custody. But, it is still different from actual measurement, and inherently less accurate than anthropometry. Methods: The authors revised the standardized clinical photography and then, carried out photogrammetry using Photoshop(Adobe, U.S.A.), and compared each data with those of anthropometry. The subjects were 50 males and 50 females, undergraduate medical students in twenties. Standard head position was obtained from the wire, fixed to cephalostat which indicates the Frankfort horizontal plane. All photographs were taken at the same situation such as fixed position of light, subject and camera, etc. Results: Total 96 measurements, based on 40 landmarks, consist of linear measurements, angular measurements and inclinations include 3 measurements in head, 22 in face, 15 in orbit, 28 in nose, 16 in lip and mouth, and 12 in ear. Conclusion: Normal photogrammetric data of face of Korean in twenties was obtained. Reliable photogrammetric measurements, not significantly different from anthropometric measurement statistically, accounted for 44 in 96 measurements(45.8%). Anthropometric values different from those of photogrammetric values were obtained by multiplying coefficient by photogrammetric value.
Clinical examination, photometric evaluation, & radiographic analysis have been used to evaluate the asymmetry of the face. Commonly used skull radiographs to assess skeletal asymmetry include poatero-anterior cephalometries, submentovertex view, & panoramic view. The purpose of this study is the comparison of the reliance of the postero-anterior cephalometric view, submentovertex view, & panoramic view in the asymmetry evaluation. All measurements were performed on the each radiographs of 31 control group & 30 asymmetric group. The measurements are MSL(midsagittal plane)-Co, MSL-Go, MSL-Me, MSL-Al, MSL-Bl, Mn. Ramus Height(Co-Go), Mn. Body Length(Go-Me), and Total Mn. Length(Co-Me). The results were as follows: 1. The lack of either a right-sided or left-sided asymmetric dominant was found. 2. The postero-anterior cephalometric view & submentovertex view relatively agreed with each other in the result. The postero-anterior cephalometric view & submentovertex view had the clinical reliability & effectiveness in the diagnosis of the skeletal asymmetry(p<0.05). 3. The panoramic view showed more magnification compared to the other radiographs. In the vertical measurements the panoramic view had clinical reliability relatively(p<0.05). But we cannot rely on the horizontal measurements in the panoramic view(p>0.05).
Purpose: The objective of this study was to investigate the effect of varying resolutions of cone-beam computed tomography images on the accuracy of linear measurements of edentulous areas in human cadaver heads. Intact cadaver heads were used to simulate a clinical situation. Materials and Methods: Fiduciary markers were placed in the edentulous areas of 4 intact embalmed cadaver heads. The heads were scanned with two different CBCT units using a large field of view ($13cm{\times}16cm$) and small field of view ($5cm{\times}8cm$) at varying voxel sizes (0.3 mm, 0.2 mm, and 0.16 mm). The ground truth was established with digital caliper measurements. The imaging measurements were then compared with caliper measurements to determine accuracy. Results: The Wilcoxon signed rank test revealed no statistically significant difference between the medians of the physical measurements obtained with calipers and the medians of the CBCT measurements. A comparison of accuracy among the different imaging protocols revealed no significant differences as determined by the Friedman test. The intraclass correlation coefficient was 0.961, indicating excellent reproducibility. Inter-observer variability was determined graphically with a Bland-Altman plot and by calculating the intraclass correlation coefficient. The Bland-Altman plot indicated very good reproducibility for smaller measurements but larger discrepancies with larger measurements. Conclusion: The CBCT-based linear measurements in the edentulous sites using different voxel sizes and FOVs are accurate compared with the direct caliper measurements of these sites. Higher resolution CBCT images with smaller voxel size did not result in greater accuracy of the linear measurements.

Background: There are many methods of evaluating diaphragmatic function, including trans-diaphragmatic pressure measurements, which are considered the key rule of diagnosis. We studied the clinical usefulness of chest ultrasonography in evaluating stable chronic obstructive pulmonary disease (COPD) patients and those in exacerbation, focusing on diaphragmatic measurements and their correlation with spirometry and other clinical parameters. Methods: In a prospective case-control study, we enrolled 100 COPD patients divided into 40 stable COPD patients and 60 patients with exacerbation. The analysis included 20 age-matched controls. In addition to the clinical assessment of the study population, radiological evaluation included chest radiographs and chest computed tomography. Transthoracic ultrasonography (TUS) was performed for all included subjects. Results: Multiple A lines (more than 3) were more frequent in COPD exacerbation than in stable patients, as was the case for B-lines. TUS significantly showed high specificity, negative predictive value, positive predictive value, and accuracy in detecting pleural effusion, consolidation, pneumothorax, and lung mass. Diaphragmatic measurements were significantly lower among stable COPD subjects than healthy controls. Diaphragmatic thickness and excursion displayed a significant negative correlation with body mass index and the dyspnea scale, and a positive correlation with spirometry measures. Patients in Global Initiative for Chronic Obstructive Lung Disease (GOLD) group D showed lower diaphragmatic measurements (thickness and excursion). Conclusion: The TUS of COPD patients both in stable and exacerbated conditions and the assessment of diaphragm excursion and thickness by TUS in COPD patients and their correlations to disease-related factors proved informative and paved the way for the better management of COPD patients.
Purpose : The purpose of this study was to investigate the correlations between clinical balance scales and Balance Performance Monitor parameters in patient with adolescent idiopathic scoliosis (AIS). Methods : Twenty AIS subjects (age, $14.26{\pm}1.93yrs$; height, $160.56{\pm}7.98cm$; weight, $47.54{\pm}6.94kg$)were participated in this study. Postural sway(mean balance, sway angle, sway area, sway path, maximal sway velocity) were were evaluated by balance performance monitor. Measurements for clinical balance scales were Functional reach test (both side), the Lateral reach test (both side) and One leg standing test (both legs). Results : The results were as follows. There were positive strong correlation between major curve direction and left-right sway angle, sway path, maximal sway velocity. There were negative strong correlation between the functional reach and left-right sway angle, sway area, sway path, maximal sway velocity. And the lateral reach were also showed negative strong correlation parameters of balance performance monitor. One leg standing were negatively correlated with left-right sway angle, sway path, maximal sway velocity. Conclusion : The clinical balance scales will be useful tools for balance measurements, and basic tools for clinical setting for patient with AIS.

In clinical trials with repeated measurements, the time-averaged difference (TAD) may provide a more powerful evaluation of treatment efficacy than the rate of changes over time when the treatment effect has rapid onset and repeated measurements continue across an extended period after a maximum effect is achieved (Overall and Doyle, Controlled Clinical Trials, 15, 100-123, 1994). The sample size formula has been investigated by many researchers for the evaluation of TAD in two treatment groups. For the evaluation of TAD in multi-arm trials, Zhang and Ahn (Computational Statistics & Data Analysis, 58, 283-291, 2013) and Lou et al. (Communications in Statistics-Theory and Methods, 46, 11204-11213, 2017b) developed the sample size formulas for continuous outcomes and count outcomes, respectively. In this paper, we derive a sample size formula to evaluate the TAD of the repeated binary outcomes in multi-arm trials using the generalized estimating equation approach. This proposed sample size formula accounts for various correlation structures and missing patterns (including a mixture of independent missing and monotone missing patterns) that are frequently encountered by practitioners in clinical trials. We conduct simulation studies to assess the performance of the proposed sample size formula under a wide range of design parameters. The results show that the empirical powers and the empirical Type I errors are close to nominal levels. We illustrate our proposed method using a clinical trial example.
Exhaled nitric oxide (NO) has been extensively investigated as a noninvasive marker of airway inflammation in asthma. The increased NO expression induced by inflammatory mediators in airways can be monitored easily in exhaled air from asthmatic children. Based on the relationship between the increased NO expression and eosinophilic airway inflammation, fractional exhaled nitric oxide (FeNO) measurements become an important adjunct for the evaluation of asthma. In addition, the availability of portable devices makes it possible to measure FeNO more easily and frequently in the routine pediatric practice. Despite various confounding factors affecting its levels, FeNO can be applicable in diagnosing asthma, monitoring treatment response, evaluating asthma control, and predicting asthma exacerbations. Thus, although pulmonary function tests are the standard tools for objective measurements of asthmatic control, FeNO can broaden the way of asthma monitoring and supplement standard clinical asthma care guidelines.
Purpose: Dental diagnostic records derived from study models are a popular method of obtaining reliable and vital information. Conventional plaster models are the most common method, however, they are being gradually replaced by digital impressions as technology advances. Moreover, three-dimensional dental models are becoming increasingly common in dental offices, and various methods are available for obtaining them. This study aimed to evaluate the accuracy of the measurement of dental digital models by comparing them with conventional plaster and to determine their clinical validity. Materials and Methods: The study was conducted on 16 patients' maxillary and mandibular dental models. Tooth size (TS), intercanine width (ICW), intermolar width (IMW), and Bolton analysis were taken by using a digital caliper on a plaster model obtained from each patient, while intraoral scans were manually measured using two digital analysis software. A one-way analysis of variance test was used to compare the dental measurements of the three methods. Result: No significant differences were reported between the TS, the ICW and IMW, and the Bolton analysis through the conventional and two digital groups. Conclusion: Measurements of TS, arch width, and Bolton analysis produced from digital models have shown acceptable clinical validity. No significant differences were observed between the three dental measurement techniques.
