Purpose: Near drowning refers to immediate survival after asphyxia due to submersion or immersion in water, which is a crucial public safety problem worldwide. Acute lung injury or acute respiratory distress syndrome (ARDS) is a common complication of near drowning. The purpose of this study was to investigate the feasibility and effectiveness of noninvasive nasal positive pressure ventilation (NINPPV). Methods: This retrospective study was conducted at a tertiary emergency department. NINPPV was administered for moderate ARDS caused by submersion or immersion in patients who were older than 18 years, from January 2015 to December 2018. We collected the demographic (age, sex, length of hospital stay, and outcome), laboratory (arterial blood gas, lactate, oxygen saturation, partial pressure of oxygen divided by the fraction of inspired oxygen, complete blood count, blood urea nitrogen, and creatinine), and clinical data (acute lung injury index and ventilator failure) of the patients. A statistical analysis was performed using Statistical Package for the Social Sciences version 20.0 for Windows. Results: NINPPV treatment was provided to 57 patients for near drowning, 45 of whom (78.9%) were successfully treated without complications; in 12 (21.1%), treatment was changed to invasive mechanical ventilation within 48 hours due to ARDS or acute kidney injury. NINPPV treatment was successful in 31 (75.6%) out of 41 sea-water near drowning patients. They were more difficult to treat with NINPPV compared with the fresh-water near drowning patients (p<0.05). Conclusions: NINPPV would be useful and feasible as the initial treatment of moderate ARDS caused by near drowning.

PURPOSE: Focusing on the factors that influence the infectious disease emergency response policy (approached by dividing the factors into health policy management and economic policies), both SARS and MERS cases were based on the legal system, manpower, and budget, but there has not been enough learning from the epidemic. This study focused on infectious disease emergency governance, which various studies have neglected despite its social and academic importance. METHODS: The research is based on an analysis of SARS, MERS, and COVID-19 and compares global policies. In this study, infectious disease emergency governance was divided into health policy management and economic factors. This study focused on planning and leadership before and after the outbreak of infectious diseases and how cooperation was achieved to monitor and respond to infectious diseases successfully. RESULTS and CONCLUSION: The limit of this study was that COVID-19 is a currently ongoing infectious disease with high uncertainty. Because it is an ongoing problem, only some data and statistics are reflected, and many limitations prevent a proper comparison under the same criteria as other infectious diseases. In addition, because continuous changes are expected, there is also room for infectious diseases to develop in a completely different pattern from the current situation, and continuous research must be accompanied in the future.
Purpose: This study investigates the effect of repeated use of lithium disilicate glass-ceramics on color stability. Methods: In this study, 2 types of lithium disilicate glass-ceramics were used and they were divided into 4 groups after 1st, 2nd, 3rd, and 4th heat pressing. The L*, a*, and b* values of all specimens were measured using the VITA Easyshade Advance 4.0. These values was used to determine the ∆E value and transparency. IBM SPSS Statistics ver. 24.0, a statistical significance level of 0.05 and one-way ANOVA were used for all data. Results: In all groups, the L* and b* values were the lowest in the specimens subjected to the 4th heat pressing and the corresponding a* value was the highest. All specimens showed ∆E values of ≤3.3, indicating color changes that were not visible with the naked eye, and the color difference increased with the number of repeated heat pressing. The transparency of all specimens decreased as the number of repeated uses increased. Conclusion: According to the number of repeated heat pressing, the color difference of Rosetta SP (HASSBIO) was larger than that of IPS e.max Press (Ivoclar Vivadent), but it was clinically acceptable in all groups. Moreover, transparency decreased as the number of heat presses increased in all groups. According to the above findings, the lithium disilicate glass-ceramic has high color stability due to repeated heat pressing, implying that it can be used in clinical settings regularly.
Background: Prader-Willi syndrome (PWS) is a complex genetic disease associated with growth impairment, severe obesity and metabolic dysfunctions. High proportion of PWS patients are born small for gestational age (SGA) than normal children, which also increase the risk of growth impairment and metabolic dysfunction in PWS. We aimed to compare growth outcome and metabolic profiles between SGA and appropriate for gestational age (AGA) PWS patients. Methods: Data of 55 PWS children and adults aged more than 2 years old (32 male and 23 female, age 2-18.8 years) from single center were studied. Only patients who were treated with GH were included. The clinical characteristics and laboratory findings were reviewed retrospectively. Results: Among 55 subjects, 39 had 15q11-13 deletion and 16 had uniparental disomy (UPD). Twenty (36.3%) were born SGA. All patients received GH treatment, and 11 (20%) discontinued GH treatment. Mean age at GH treatment initiation was 2.5 (range 0.3-12.4) years, and mean duration of treatment was 6.3 (range 1.0-11.3) years. Current height-SDS (-0.36 vs -0.16) and BMI-SDS (1.44 vs 1.33) did not differ between AGA and SGA group. Two patients in SGA group, but none in AGA group had diabetes mellitus. Mean glucose level was also higher in SGA group (100.1 vs 114.4 mg/dL). Conclusion: Our report gives an overview of growth profile and metabolic dysfunctions recorded in GH treated PWS patients. Growth profile did not differ between AGA and SGA group. Glucose level was higher in SGA group, so more careful monitoring and prevention for DM will be required in SGA group.
Genetic testing in prenatal diagnosis is a precious tool providing valuable information in clinical management and parental decision-making. For the last year, cytogenetic testing methods, such as G-banding karyotype analysis, fluorescent in situ hybridization, chromosomal microarray, and gene panels have evolved to become part of routine laboratory testing. However, the limitations of each of these methods demonstrate the need for a revolutionary technology that can alleviate the need for multiple technologies. The recent introduction of new genomic technologies based on next-generation sequencing has changed the current practice of prenatal testing. The promise of these innovations lies in the fast and cost-effective generation of genome-scale sequence data with exquisite resolution and accuracy for prenatal diagnosis. Here, we review the current state of sequencing-based pediatric diagnostics, associated challenges, as well as future prospects.

Purpose : In recent years, essential oils have been produced using natural extracts for various uses. Their functionality is currently being tested not only for cosmetics and perfumes but also for other categories of products. Therefore, this study verified their antibacterial effects on S. mutans, P. gingivalis, and L. gasseri which are the representative strains that cause oral diseases. Methods : Eighteen types of natural essential oils were made at a concentration of 50 % (v/v) using Tween 20, and their antibacterial effects were verified by applying S. mutans, P. gingivalis, and L. gasseri. The antibacterial effects were measured with the disc diffusion method. All the experiments were repeated three times, and the mean value of three measurement values for each variable was used for data analysis. A one-way analysis of variance was conducted using these mean values. Results : Of the eighteen types of essential oils tested, sixteen types showed antibacterial effects on S. mutans, and sixteen and fifteen types exhibited antibacterial effects on P. gingivalis and L. gasseri respectively. The types of essential oils with high-level antibacterial activities were geranium, may chang, and bergamot for S. mutans, lemongrass, bergamot, and eucalyptus for P. gingivalis, and lemongrass, machan, and geranium for L. gasseri in order of antibacterial effect. This result was statistically significant (p<.001). In addition, in the case of mandarin oil, it was found that there was no antibacterial activity in all three strains. Conclusion : This study proved the antibacterial activities of essential oils, which are used for various purposes in daily life, against dental caries and periodontal diseases. The study results will likely be applied to different prevention programs for oral health and broadly used to develop products such as oral care items and dentifrices.

Accurate prediction of critical illness is significant for ensuring the lives and health of patients. The selection of indicators affects the real-time capability and accuracy of the prediction for critical illness. However, the diversity and complexity of these indicators make it difficult to find potential connections between them and critical illnesses. For the first time, this study proposes an indicator analysis model to extract key indicators from the preoperative and intraoperative clinical indicators and laboratory results of critical illnesses. In this study, preoperative and intraoperative data of heart failure and respiratory failure are used to verify the model. The proposed model processes the datum and extracts key indicators through four parts. To test the effectiveness of the proposed model, the key indicators are used to predict the two critical illnesses. The classifiers used in the prediction are light gradient boosting machine (LightGBM) and eXtreme Gradient Boosting (XGBoost). The predictive performance using key indicators is better than that using all indicators. In the prediction of heart failure, LightGBM and XGBoost have sensitivities of 0.889 and 0.892, and specificities of 0.939 and 0.937, respectively. For respiratory failure, LightGBM and XGBoost have sensitivities of 0.709 and 0.689, and specificity of 0.936 and 0.940, respectively. The proposed model can effectively analyze the correlation between indicators and postoperative critical illness. The analytical results make it possible to find the key indicators for postoperative critical illnesses. This model is meaningful to assist doctors in extracting key indicators in time and improving the reliability and efficiency of prediction.

Background: Citric acid (CA) and sodium hypochlorite (NaOCl) have been used to disinfect animals to protect them against avian influenza and foot-and-mouth disease. Objectives: We performed a good laboratory practice (GLP)-compliant animal toxicity study to assess the acute toxic effects of CA and NaOCl aerosol exposure in Sprague-Dawley rats. Methods: Groups of five rats per sex were exposed for 4 h to four concentrations of the two chemicals, i.e., 0.00, 0.22, 0.67, and 2.00 mg/L, using a nose-only exposure. After a single exposure to the chemicals, clinical signs, body weight, and mortality was observed during the observation period. On day 15, an autopsy, and then gross findings, and histopathological analysis were performed. Results: After exposure to CA and NaOCl, body weight loss was observed but recovered. Two males died in the CA 2.00 mg/L group and, two males and one female died in the 2.00 mg/L NaOCl group. In the gross findings and histopathological analysis, discoloration of the lungs was observed in the CA exposed group and inflammatory lesions with discoloration of the lungs were observed in the NaOCl exposed group. These results suggest that the lethal concentration 50 (LC50) of CA is 1.73390 mg/L for males and > 1.70 mg/L for females. For NaOCl, the LC50 was 2.22222 mg/L for males and 2.39456 mg/L for females. Conclusions: The Globally Harmonized System is category 4 for both CA and NaOCl. In this study, the LC50 results were obtained through a GLP-based acute inhalation toxicity assessment. These results provide useful data to reset safety standards for CA and NaOCl use.

Purpose : This study examined the relationship between oral health management behavior and subjective health perception by using a tool that measures health-related quality of life named EQ-5D. The findings are expected to provide fundamental data for developing diverse programs, including health- and oral-health-related educational media, to ultimately enhance people's health-related quality of life. Methods : Adults over the age of 20 were extracted from the raw materials of the 8th national health and nutrition examination survey conducted in 2019. Among them, 4034 were finally chosen as the study subjects after checking the response distribution of relevant questions and excluding inadequate samples. Variables included general characteristics, health management behavior, oral health management behavior, subjective health perception, and EQ-5D level. Cross-tabulation test, independent t-test, one-way analysis of variance, and linear regression analysis were conducted using SPSS 25.0. Results : EQ-5D, which measures the level of health-related quality of life, was high when the subjects had a medical check-up experience, no experience of unsatisfactory medical service, and did not get an influenza vaccination (p<.001). The quality of life was higher when the subjects received dental inspections, did not require treatment, and used oral care goods (p<.005). Health-related quality of life was also higher when the subjective health level and subjective oral health level were high. Conclusion : Results indicate that subjective health, oral health perception level, and quality of life together were high when people had a regular check-up, did not neglect treatment, and used oral care goods. Considering the findings of this study, check-up programs that are customized for each age group are necessary. Health-related quality of life could be enhanced through the provision of diverse medical services and active efforts to prevent any medical blind spot.
Objectives: Prospective studies on vaccination status and mortality related to coronavirus disease 2019 (COVID-19) in low-resource settings are still limited. We assessed the association between vaccination status (full, partial, or none) and in-hospital mortality among COVID-19 patients at most hospitals in Jakarta, Indonesia during the Delta predomination wave. Methods: We conducted a retrospective cohort study among hospitalized COVID-19 patients who met the study criteria (>18 years old and admitted for inpatient treatment because of laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection). We linked individual-level data in the hospital admission database with vaccination records. Several socio-demographic and clinical characteristics were also analyzed. A Cox proportional hazards regression model was used to explore the association between vaccination status and in-hospital mortality in this patient group. Results: In total, 40 827 patients were included in this study. Of these, 70% were unvaccinated (n=28 543) and 19.3% (n=7882) died during hospitalization. The mean age of the patients was 49 years (range, 35-59), 53.2% were female, 22.0% had hypertension, and 14.2% were treated in the intensive care unit, and the median hospital length of stay across the group was 9 days. Our study showed that the risk of in-hospital mortality among fully and partially vaccinated patients was lower than among unvaccinated adults (adjusted hazard ratio [aHR], 0.43; 95% confidence interval [CI], 0.40 to 0.47 and aHR, 0.70; 95% CI, 0.64 to 0.77, respectively). Conclusions: Vaccinated patients had fewer severe outcomes among hospitalized adults during the Delta wave in Jakarta. These features should be carefully considered by healthcare professionals in treating adults within this patient group.
