인간의 자궁에서의 Luteinizing Hormone (LH) 유전자 발현 (Expression of Luteinizing Hormone(LH) Gene in Human Uterus)

  • Objectives: Recent studies, including our own, demonstrated that the novel expression of LH gene in rat gonads and uterus, indicating that the local production and action of the LH-like molecule. In the present study, we investigated whether human uterus also expresses the LH gene. Design: Reverse transcription-polymerase chain reaction (RT-PCR) amplified the cDNA fragments coding $LH_{\beta}$ polypeptide from human endometrium but not from myometrium. Presence of the transcripts for the ${\alpha}$-subunit in human endometrium was also confirmed by RT-PCR. Results: Transcripts for $LH_{\beta}$ subunit were detected in endometrial samples from women with endometriosis. The gene for LH/hCG receptor was expressed in both endometrium and myometrium, showing good agreement with previous studies. Increased level of $LH_{\beta}$ transcript was determined in the endometrium from follicular phase compared to that from luteal phase. Conclusion: Taken together, our findings demonstrated that 1) the genes for LH subunits and LH/hCG receptor are expressed in human uterus, 2) the uterine LH expression was changed during menstrual cycle, suggesting that the uterine LH may playa local role in the control of uterine physiology and function(s).

  • Drug transporters play an essential role in the absorption, distribution and elimination of clinical drugs, nutrients and toxicants. The importance of the transporters is exampled by therapeutic failure in cancer chemotherapy that is mainly caused by the overexpression of multidrug resistance (MDR)-related transporters. In addition, the transporters may involve in drug-drug interactions that lead to serious adverse drug responses and some transporters also contribute to inter-individual variation in drug responses. As an effort to understand the mechanism underlying the inter-individual variation of transporters activity, genetic and environmental factors influencing the expression or function of the transporters have extensively explored through last decade. Among them, genetic polymorphism of drug transporter encoding genes has generated much interest since the discovery of functional single nucleotide polymorphisms (SNP) of MDR1 gene. Besides drug transporters, xenobiotic receptors also modulate drug disposition by regulating the transcription of drug metabolizing enzymes and drug transporters. Among many xenobiotic receptors, pregnane X receptor (PXR) and constitutive androstane receptor (CAR) are two most well characterized since these receptors show wide substrate specificities and regulate the expression of various enzymes involved in drug disposition. Recently, several functional genetic polymorphisms were reported in PXR coding gene. In the present study, genetic polymorphisms of two drug transporters, MDR1 and BCRP, and two xenobiotic receptors, PXR and CAR, were investigated in Korean population.

The effects of health care programs for gestational diabetes mellitus in South Korea: a systematic review

  • Purpose: The purpose of this study was to investigate the effects and characteristics of health care programs for pregnant women with gestational diabetes mellitus (GDM) in Korea. Methods: This study was conducted according to the Cochrane Collaboration's systematic literature review handbook and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses reporting guideline. We searched eight international and domestic electronic databases for relevant studies. Two reviewers independently selected the studies and extracted data. For each study, information on the research method, participants, characteristics of the program, and results were extracted using a previously established coding table. The National Evidence-based Healthcare Collaborating Agency's risk of bias assessment tool for non-randomized studies was used to assess the risk of bias of the included articles. A qualitative review of the selected studies was performed because the interventions differed considerably and the measured outcomes varied. Results: Out of 128 initially identified papers, seven were included in the final analysis. The risk of bias was evaluated as generally low. Health care programs for pregnant women with GDM showed positive effects on blood glucose control. Anxiety and depression were reduced, and self-management and self-care behavior, self-efficacy, and maternal identity improved. Conclusion: Our study provides clinical evidence for the effectiveness of health care programs for pregnant women with GDM, and its results can be used to support the development of health care programs for GDM. More well-designed research is needed on GDM, especially studies that deal with emotional stress and apply a family-oriented approach.

A Review of Domestic Research Trends Related to the International Classification of Functioning, Disability and Health (ICF): 2015-2020

  • PURPOSE: This study was conducted as a literature review to analyze the research trends related to the International Classification of Functioning, Disability and Health (ICF) in Korea from 2015 to 2020. METHODS: Precedent studies were searched with the search term "ICF" or "international classification of functioning, disability and health" from the databases of RISS, KISS, DBpia, and Pubmed. The inclusion criteria are that the studies have been carried out in Korea from 2015 to 2020 using ICF by researchers consisting of one or more Koreans and have been peer-reviewed. RESULTS: Of the total 269 studies, 107 that met the inclusion criteria were analyzed. It was found that these studies were published at a similar frequency each year. The most common area of expertise was identified as the clinical area (n = 67), followed by special education (n = 21) and social welfare (n = 13). The study subject groups were mostly patients (n = 39), disabled people (n = 25), and related experts (n = 13). The most common research topic was functioning evaluation (n = 49) and followed by a literature review (n = 29), and the most frequently used components in all the areas of expertise were activity and participation (n = 98), body function and structure (n = 73), and environmental factors (n = 61). CONCLUSION: For the past six years, domestic ICF-related research has been conducted in a wider range of expertise areas on more subdivised subject groups. Continuous research, development of standardized curricula and contents, and development of coding tools are considered to be important in vitalizing the use of the ICF.

근육뼈대계 환자보고식 결과 측정과 KCF의 연결: 물리치료 중심으로 (Mapping between Musculoskeletal Patient-Reported Outcome Measures and KCF: Physical Therapy Perspective)

  • PURPOSE: The present study was conducted to improve the understanding of the Korean Standard Functional, Disability, and Health Classification (KCF) and its ease of use in the clinical domain of the musculoskeletal system by comparing, analyzing, and linking the KCF codes with items from patient-reported outcome measures (PROMs), which are currently mainly used to evaluate patients with neck, shoulder, waist, and knee pain. METHODS: The items of the most widely used PROMs, the Neck Disability Index (NDI), Disabilities of the Arm, Shoulder, and Hand (DASH) scores, Oswestry Disability Index (ODI), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and the codes of the KCF were linked by two experts according to the linking rules. RESULTS: The concordance between the items of each of the PROMs and the KCF code linked by the two experts was NDI,86.4%86.4%, DASH 83.3%, ODI 92.0%, and WOMAC 80.7%. The NDI, DASH, and WOMAC indexes were found to comprise items corresponding to physical function, activity, and participation areas, and were linked to 22, 43, and 31 KCF codes, respectively. In addition to these two areas, the ODI included items related to environmental factors and was linked to 25 codes (duplicated codes are treated as one). CONCLUSION: This research can be used by adding the KCF code to the questions of the currently used evaluation tool. This coding can be easily applied and will contribute to the easy understanding of the KCF.

입원환자의 영양불량위험 검색도구의 개발 및 평가 (Development and Evaluation of a Nutritional Risk Screening Tool (NRST) for Hospitalized Patients)

  • 이 연구는 다양한 병명을 가진 입원환자의 영양불량위험의 진단을 간단하고 신속하게 실시할 수 있고 재원기간 내의 임상결과를 유의하게 예측하며 NRS 2002와 비교하여 평가하였을 때 일정 수준 이상의 타당도를 가지는 영양불량 위험 검색도구를 (Nutritional Risk Screening Tool, NRST) 개발하고자 하였다. NRST의 개발 및 평가는 서울 소재 일개 종합병원에 1년여의 기간 동안 입원한 성인 환자를 대상으로 수집된 자료를 이용하여 NRST 구성요소의 선정, NRST scoring scheme의 탐색, NRST 평가의 기준점 설정, NRST의 임상결과 예측력 재확인 등의 총 4단계의 과정을 통하여 실시되었다. 아래에 본 연구의 결과 및 결론을 요약하였다. 1) 재원일수, 사망여부, 합병증 유무, 질병예후를 각각 종속변수로 하는 stepwise multiple regression을 시행한 결과 나이와 혈청 내 albumin, TLC, Hct 농도가 NRST의 구성요소로 선정되었다. 2) 선정된 각 NRST 구성요소의 수준에 따른 NRS 2002의 OR에 기준하여 NRST의 scoring scheme을 아래와 같이 설정하였다. $NRST=Albumin{\times}1+Age{\times}2.5+Hct{\times}1.5+TLC{\times}2{\ast}$ ${\ast}$ Coding:Albumin <3.5:1, ${\geqq}$ 3.5:0.Age>65:1, ${\leqq}$65:0.Hct<37:1, ${\geqq}$37:0.TLC<1800:1, ${\geqq}$1800:0. 3) NRST의 점수변화에 따른 NRS 2000에 대한 민감도와 특이도를 바탕으로 NRST에 의한 영양불량 위험의 진단 기준점수를 3.5 이상으로 정하였다. 4) NRST 결과에 따른 재원일수, 합병증여부, 질병 예후, 사망여부 등의 차이를 검토한 결과 개발된 NRST가 만족할 만한 수준의 임상결과 예측력을 가지는 것으로 나타났다. 5) NRST는 다양한 진단명을 가진 입원환자의 영양불량 위험 여부를 향후의 임상결과와 관련하여 신속하게 진단하는데 유용하리라 사료된다.

반성 열성 범저감마글로불린혈증 1가계 3환자의 Bruton's Tyrosine Kinase 유전자 변이 및 임상 양상 (Characterization of Bruton's Tyrosine Kinase Genetic Mutations in One Korean X-linked Agammaglobulinemia Family)

  • 목 적 : 본 연구에서는 임상적으로 XLA로 진단받고 현재 치료 중인 한가족 3명 환아의 가계를 대상으로 말초혈액 단핵구의 Btk 단백질 발현과 Btk 유전자 변이를 분석하고자 하였다. 방 법 : 환아 말초혈액 단핵구의 Btk 발현도를 항 Btk 항체를 이용한 유세포계측을 통해 분석하고 PCR-SSCP 및 직접 염기서열 분석에 의해 Btk 유전자 변이를 분석하였다. 결 과 : 유세포 계측 및 PCR-SSCP에 의하여 가계내의 환자 3명 및 보인자 4명을 확인하였으며 환아들의 단핵구의 Btk 발현도를 조사한 결과 2.7% 미만으로 건강인에 비해 매우 유의한 감소를 나타내었다. 유전자 변이 분석 결과 Btk 유전자 exon 3에서 점돌연변이($T{\rightarrow}C$)가 발견되어 61번째 아미노산의 치환이 일어난 과오돌연변이를 확인할 수 있었다. 동일한 유전형을 지닌 3명의 환아들은 임상적으로 매우 다양한 표현형을 나타내었다. 특히 환아들의 혈청 IgG level이 낮을수록 임상 양상은 심한 소견을 보였다. 결 론 : 본 연구 결과를 종합해 볼 때 임상적으로 XLA로 진단된 환아와 가족에 대한 항 Btk 항체를 이용한 유세포계측 및 PCR-SSCP 방법은 XLA 환아 및 보인자의 진단에 매우 유용하며 신속하게 적용될 수 있는 기법으로 생각된다. 또한 분자유전학 기법을 이용하여 환아의 유전자 변이를 검색한 결과 Btk의 PH 도메인 내 한개의 과오돌연변이를 검색하여 XLA를 최종 확진할 수 있었다.

과학담화에서 과학자와 중학생의 제스처 비교 -분자운동과 물질의 상태변화를 중심으로- (The Difference of Gestures between Scientists and Middle School Students in Scientific Discourse: Focus on Molecular Movement and the Change in State of Material)

  • 과학 담화에 동반되는 제스처는 정신 모형의 구성과 모델에 기반한 추론에 중요한 역할을 한다. 체화된 인지의 관점에서 제스처는 학생의 내면에 기저한 정신모형을 유추하는 근거인 동시에 학생의 불완전한 과학적인 사고의 변화에 도움을 줄 수 있다. 이 연구는 과학 교육의 맥락에서 제스처의 역할을 탐색해보고자 과학적 담화에서 제스처의 특징을 살펴보고 과학자와 중학생의 제스처를 비교하였다. 각각 10명의 과학자와 중학생이 본 연구에 참여했으며, 일대일 면담에서는 '분자운동과 상태 변화'에 관한 세 가지 면담과제가 제시되었고 면담은 반구조화된 clinical interview의 방식으로 진행되었다. 과학자와 중학생의 제스처는 모두 비디오로 녹화하였으며, 근거 이론(grounded theory)에 기반하여 4명의 연구자가 반복적으로 비교 분석하였다. 연구 결과, 과학자와 중학생의 제스처는 4가지 측면(제스처의 특징, 제스처의 사용, 제스처의 내용, 제스처의 기능)에서 상이한 특성을 보였다. 과학자는 다양하고 정교한 제스처를 보다 빈번하게 체계적으로 사용했으며, 중학생 또한 과학적 사고와 소통의 도구로 제스처를 과학담화에서 사용했으나, 중학생의 제스처는 과학자의 제스처에 비해 과학적인 근거가 부족했으며 기능적인 특징에서도 상당한 차이점이 있었다. 이 결과는 제스처가 과학적 사고를 강화하는 데 도움이 될 수 있으며, 내면에서 일어나는 인지활동을 알아보는 수단이 될 수 있다는 것을 보여준다. 앞으로 학생들이 제스처를 과학 개념의 이해와 추론을 돕는 도구로 사용할 수 있도록 지원할 필요가 있다.

한방병원 응급실에 내원한 근골격계 질환 환자 1,485명에 대한 후향적 임상 분석 (A Retrospective Clinical Analysis of 1,485 Patients Who Visited the Emergency Room of Korean Medical Hospital with Musculoskeletal Disorders)

  • Objectives : The purpose of this study is to review the characteristics of patients who visited the emergency room of a Korean medical hospital with musculoskeletal disorders during the last 5 years and to provide a potential basis for future studies and emergency medical practice. Methods : Patient visit records between May 1st, 2010 and February 28th, 2015 were reviewed and patients with musculoskeletal disorders were included in a retrospective analysis. Results : A total of 5,218 patient visit records were initially reviewed and 1,485 patients (28.46 %) were included in the analysis. S code group was the most frequently diagnosed coding group of Korean Standard Classification of Diseases. The male to female ratio was 1.05:1 and patients in their thirties represented the highest age group. When classified by time of visit, the distribution of patients was similar from 10:00 to 22:59. The most frequently visited day and month were Sunday and September. 879 patients (59.19 %) visited the emergency room within 24 hours from onset of symptoms. The most common symptom site was the low back (45.59 %), followed by ankle (13.94 %) and neck (13.80 %). The most frequently used treatment method was acupuncture (92.32 %), followed by infrared (45.45 %), TENS (30.03 %) and herbal medication (29.02 %). The follow-up and admission rates were 30.24 % and 13.00 % respectively. 343 patients (23.10 %) received Western medical treatment within a day before visiting the emergency room of the Korean medical hospital. Conclusions : This analysis provides the latest information on the characteristics of patients who visited the emergency room of a Korean medical hospital with musculoskeletal disorders. The results can provide the basis for further studies and for other attempts to improve the clinical setting of the emergency room.
