Purpose: This study attempted to provide the basic data for the institutional improvement of the clinical practice of physical therapy department students. Methods: Research for this study included 390 participants used for the analysis of the present study. The statistical analysis was of the characteristics and satisfaction of the clinical practices of subjects, the reasons for major employment preferences before and after clinical practices, and the preferred institution and field of major employment before and after clinical practice. Results: The general characteristics of students of the physical therapy department include females in their third year of college at age 20~24 years-old. As well, no religion, usual satisfaction with major, and motivation of choosing a physical therapy employment rate were the most common. A residency type in the form where one would cook one's own food and a clinical practice with a general hospital grade were seen most often. Regarding preference of major employment before and after clinical practice, the preference of major employment was 88%, which increased by 5.2% after clinical practice from 82.8% before clinical practice. The preference for no major employment due to lack of interest and no aptitude increased 43.5% from 23.9% after clinical practice. Conclusion: The number of apprentices will increase despite practice institutions being limited; for practice of environment, content, etc., relatively changes are slow, and progress rapidly medical technology. It is a society in which expertise is required; in the present study, there is a value in providing the clinical data missing in physical therapy.
The process of drug development has seen major changes over the last two decades with the movement away from standard small molecule drug discovery programs, through computer-assisted drug design methodologies towards biotechnologically derived products. The aim of duplication of endogenously active materials to be administered exogenously has enormous impact on development practices and evaluation of safety.(omitted)
Purpose: To study the internal psychological conflicts among nursing students during an infection control protocol carried out in the hospital by measuring their observation skills and performance during clinical training. Methods: Investigation of both pre- and post- infection control was conducted using questionnaires for clinical infection practices. We identified and evaluated the students' observation skills, clinical performance, clinical perception, and internal conflict regarding clinical infection control. We also interviewed the students as part of our study. Results: Among parameters such as clinical performance, observation skills, clinical perception, and internal conflict, the average observation skills (t=5.49, p<.001) were significantly lower, while internal conflict among students (t=-7.23, p<.001) was significantly higher than expected prior to clinical training. Generally, there was a negative correlation between observation skills and internal conflict in every aspect of infection control practice (r=-.281, p=.031). Internal conflict was significantly higher than expected in the context of hand hygiene (t=-2.135, p=.037), personal hygiene (t=-3.48, p=.002), and ventilator management (t=-3.69, p<.001). Clinical performance of students in the context of hand hygiene (t=4.69, p<.001), personal hygiene (t=2.06, p=.044), and ventilator management (t=2.68, p<.001) was significantly lower than expected prior to clinical training. Conclusion: Our findings showed that internal psychological conflict is higher when infection control practices are observed or performed to a lesser degree. Therefore, reinforcing education regarding infection control among students, such as developing a systematic program, or consecutive training and monitoring, is suggested.
Purpose: The aims of this study were to assess the presence of core patient safety practices in Korean hospitals and assess the differences in reporting and learning systems of patient safety, infrastructure, and safe practices by hospital characteristics. Methods: The authors developed a questionnaire including 39 items of patient safety staffing, health information system, reporting system, and event-specific prevention practices. The survey was conducted online or e-mail with 407 tertiary, general and specialty hospitals. Results: About 90% of hospitals answered the self-reporting system of patient safety related events is established. More than 90% of hospitals applied incidence monitoring or root cause analysis on healthcare-associated infection, in-facility pressure ulcers and falls, but only 60% did on surgery/procedure related events. More than 50% of the hospitals did not adopted present on admission (POA) indicators. One hundred (80.0%) hospitals had a department of patient safety and/or quality and only 52.8% of hospitals had a patient safety officer (PSO). While 82.4% of hospitals used electronic medical records (EMRs), only 53% of these hospitals adopted clinical decision support function. Infrastructure for patient safety except EMRs was well established in training, high-level and large hospitals. Most hospitals implemented prevention practices of adverse drug events, in-facility pressure ulcers and falls (94.4-100.0%). But prevention practices of surgery/procedure related events had relatively low adoption rate (59.2-92.8%). Majority of prevention practices for patient safety events were also implemented with a relatively modest increase in resources allocated. Conclusion: The hospital-based reporting and learning system, EMRs, and core evidence-based prevention practices were implemented well in high-level and large hospitals. But POA indicator and PSO were not adopted in more than half of surveyed hospitals and implementation of prevention practices for specific event had low. To support and monitor progress in hospital's patient safety effort, national-level safety practices set is needed.
Purpose: The purpose of this study was to develop clinical education of Oriental Nursing. Methods: The subjects were one hundred and ninety-three students who have completed clinical practice. The questionnaires were composed of experience type and Oriental Nursing practices(25 items), level of satisfaction(20 items), difficulty level of practices(6 items), and preparation level of practice instructor(5 items). The reliability were Chronbach's $\alpha$=.84, .86, .74, and .93 respectively. Frequencies, percentage, t-test, and Pearson's Correlation was used in data analysis using SPSS PC+ 11.0. Results: Nursing students recognized that they had difficulties caused by a lack of knowledge of Oriental medicine, of learning the uniqueness of the nurse's role and lack of nurse's enthusiasm in teaching students. However, they were confident in preparing acupuncture and implementing Moxibustion, large Moxibustion, Bu-Hang therapy, aroma therapy and on administration of medicines and control of temperature of the medicines, exercise and rehabilitation. Therefore, instructors should train nursing students by developing a unique Oriental Nursing practice and reinforcement of nurses' independent and cooperative roles through the acquisition of Oriental Medical knowledge and terminology. It is necessary that they develop an educational program based on Oriental Nursing practice, perform consistently within the program and train its teaching faculty and instructors.
The purpose of this study was to define the content of physiological knowledge needed for clinical nursing practices. Subjects of physiology were classified into 15 areas, and each areas was further classified into subareas, resulting in a total of 194 subareas. The degree of importance of each subarea was measured with a 4-point scale. The subjects of this study were 179 nurses of two university hospitals located in Seoul and Inchon. The results were as follows : 1. The areas of physiology necessary for clinical nursing practice as a basic knowledge in the order of importance were : blood, respiratory system and renal physiology, function of the immune system, body fluid and cardiovascular system, body temperature, endocrine physiology and gastrointestinal physiology However, the degree of importance for reproductive physiology, neuro-physiology, energy and metabolism, cell and cell membrane physiology, muscular physiology and special sense was relatively low. 2. The most important content of physiology for all clinical areas in nursing was blood physiology. However, the degree of importance for each physiology area was different depending on clinical areas. 3. Subareas of physiology as a basic knowledge for clinical practice and education in nursing were blood transfusion, blood type, function of red blood cell, white blood cell and platelet, characteristics and function of hemoglobin, composition and function of plasma protein, and mechanism of blood coagulation and anticoagulation. In conclusion, areas of physiology necessary for clinical nursing practice were blood, respiratory system and renal physiology, function of immune, body fluid and cardiovascular system, body temperature, endocrine physiology and gastrointestinal physiology. However, the degree of importance for each physiology area was different depending on clinical areas In nursing.
Purpose: To identify the clinical role of Advanced Practice Nurses (APN), and evaluate how other medical personnel perceive their work(difficulty, importance). Methods: A questionnaire survey was performed with 277 health care providers (APN 52, nurses 88, and medical doctors 137 [professors 51, fellows 44, & residents 42]) in a single, tertiary hospital. The questionnaire was categorized into 6 domains (total 40 tasks): 6 tasks on identifying health issues (A); 3 tasks on prescribing and conducting diagnostic tests (D); 18 tasks on disease treatment (T); 4 tasks regarding prescribing medicine (M); 3 tasks regarding medical collaboration (C); 6 tasks regarding patient education (E). The survey measured the frequency, difficulty, and importance of APN's clinical tasks, and evaluated the willingness of authorizing clinical tasks to APN. Results: The most frequent tasks for APN were A domain, lowest were T domain. The scores for perceived job difficulty were lower than those for job importance in all groups. The proportion of willingness to legally delegate clinical practices to APN was higher in A and E domains, but lower in D and T domains. However, professors, who spent the most time with APN, showed a higher willingness to legally delegate clinical practice. The participants favored medical doctors as substitutes for tasks which were not legislated for delegated job performance. Conclusion: In this study identified clinical roles that medical doctors considered possible for legal delegation to APN were identified. The results can be used as evidence for the legalization of the practice of APN.
The twenty-first century may be said to be entering into a specialized qualification age to meet the needs of new technical innovations such as environmental changes, demographical changes, changes in the constitution of diseases, changes in the needs of the national health, reforms of information and knowledge, etc., which requires the provision of competitive services that can fulfill the high level needs of consumers. In consequence, it is needed to apply a practical nursing model that can serve as a guide for healthy society and to secure the sphere that can affect nursing policy-making by keeping pace with the changing environment. Furthermore, it is also urgent to expand more the activity sphere of nurse specialists with authority and autonomy, establish their legal foundation, establish a qualification accreditation system for nurse specialists, and develop educational programs. In Korea, the law relative to organ transplant past the national assembly on February 9, 2000, legally acknowledged brain death, which indicated to us the emergence of an age of organ transplant. Therefore, it necessitates to find out those of brain death from whom organ transplant is feasible in clinical practices, with their families' consent link to those terminal organ failure patients who are in need of an organ, and mediate both parties so that smooth transplant can be accomplished. A series of these complicated procedures require systematically trained specialists with high level techniques of organic management. With this in mind, this study was conducted on 69 clinical nurse specialists for organ transplant, accredited by the hospital, who are in active service in clinical practices. The resultant findings were revealed, as follows: 1. The qualifications of clinical nurse specialists for organ transplant should be accredited by Ministry of Health and Welfare or Korea Nurses Association. 2. The validity of qualifications should be for three years, and their renewal should be based on marks of a supplemental training or an education course for more than 12 hours a year. 3. The qualification of the clinical nurse specialist necessitates theoretical lectures and practices on those nurses who have had clinical experience in the pertinent field. 4. The course of training is required to be one year in the length of training and take more than 20 credits (320 hours) and 5 credits (240 hours).
Headache is a common complaint in pediatric office practices as well as in children presenting to emergency departments. Children who complain of headache usually are brought to medical attention by their parents, who seek reassurance that the headaches are not a sign of a serious illness. The etiologies of headache range from school problems to brain tumors. A history taking, physical examination, and appropriate diagnostic testing will enable to distinguish primary headaches from those of a secondary etiology. The clinical, neuroimaging, and laboratory evaluation of the child with headache are reviewed here.
