간호대학생의 혈액 및 체액 노출실태, 감염예방 표준주의 지식, 인지도 및 수행도 (Survey of Exposure to Blood and Body Fluids, Knowledge, Awareness and Performance on Standard Precautions of Infection Control in Nursing Students)

  • 이 연구는 간호대학생들의 혈액 및 체액의 노출실태와 표준주의에 대한 지식, 인지도와 수행도를 파악하여 감염교육을 위한 기초자료로 제시하고자 시도하였다. 연구대상자는 J지역의 간호학과 3, 4학년 간호대 학생 309명이었다. 대상자의 혈액과 체액에 대한 노출경험은 71.5%이었다. 표준주의에 대한 지식 평균점수는 $20.59{\pm}2.05$점이고, 학년, 감염교육 유무 등에서 통계적으로 유의하였다. 인지도의 평균점수는 $4.67{\pm}.36$점이고 연령, 사후조치 없음의 이유 등에서 통계적으로 유의하였고, 수행도 평균점수는 $4.25{\pm}.70$점이며, 혈액 및 체액 접촉에서 유의하였다. 대상자의 표준주의에 관한 지식, 인지도와 수행도와의 관계는 각각 양의 상관관계로 나타났다. 표준주의 수행도에 영향을 미치는 요인으로는 표준주의 인지도(${\beta}=.274$)만이 유의하게 나타나, 표준주의 수행도 변인의 8.6%를 설명하였다. 표준주의의 인지도와 수행도가 낮은 점수의 영역을 중점적으로 보완한 다양한 감염관리 프로그램의 개발이 필요하며, 학생들에게는 임상실습 진입 전, 감염예방에 대한 교육이 강화되어야 할 것이다.

병동 간호활동 원가계산 프로그램 개발 :시간동인 활동기준원가계산 기반으로 (Development of Nursing Activity Cost Calculation Program Using Time-Driven Activity-Based Costing (TD-ABC))

  • 본 연구는 이현희(2017)의 연구에서 개발한 병동 간호활동원가계산 프로그램의 실무 적용 확대를 위하여 자료의 저장 및 그래프 변환 출력 기능을 보완하여 수정 개발한 후속 연구이다. 전체 프로그램 개발과정은 프로그램 요구사항 분석과 프로그램 설계 및 개발, 프로그램 타당성 검토의 3단계로 이루어졌다. 본 프로그램은 단순히 원가계산에서 그치는 것이 아니라 원가효율성 및 원가분석 추세 비교까지 결과를 산출하여 간호관리자가 병동 간호단위 관리에 필요한 원가정보를 얻을 수 있고 이를 기반으로 원가관리전략을 수립할 수 있도록 하여 그 활용도를 확장한 점에서 의의가 있다. 또한 향후 이를 기반으로 병동 단위 뿐 아니라 수술실, 중환자실과 같은 특수부서 그리고 외래와 같이 간호활동의 구성과 범위가 다른 부서별 간호활동원가계산을 수행하는 원가분석 프로그램 개발할 수 있는 토대를 마련한 점에서도 의의가 있다. 이에 본 연구를 기반으로 이를 다양한 실제 임상 간호관리 실무에 적용함으로써 임상 간호사의 원가관리 역량을 강화하고 나아가 간호단위 간호활동의 수익과 이익 분석을 통해 계량화된 간호의 기여도를 측정, 제시함으로써 간호성과 측정 연구를 확대해 나갈 것을 제안한다. 아울러 본 TD-ABC 간호활동원가계산 프로그램의 적용과 활용을 학부 수준의 간호관리학 교과과정과 접목하고, 임상간호사의 보수교육 프로그램으로도 활용하여 간호원가관리 역량 강화를 위한 교육매체로도 활용할 것을 제안한다.

간호윤리 상황극을 통한 병원간호사의 긍정윤리관 변화 (Changing to Positive Ethics in Hospital Nurses through 'Theatre of Situations')

  • Purpose: This study was done to examine changes towards positive ethics in registered nurses working in hospitals who took part in performing 'theatre of situations' in terms of issues of nursing ethics. Methods: This research was conducted with one-group pretest-posttest design. The sample, 232 registered nurses, voluntarily participated in a contest of 'Theatre of Situations' with themes of nursing ethics hosted by Hospital Nurses Association of Korea on September 8, 2011. For this contest, eight nursing situational scenario regarding ethical issues or dilemma were created. The theatre of situation was performed by volunteer nurses over a period of 160 minutes. Data were collected before and after the performance using 10 items to measure Positive Ethics. Item internal consistency had a Cronbach's alpha in this study of .716. Results: Before the theatre of situations was performed, the mean (${\pm}$standard deviation) level of positive ethics was 2.34 (${\pm}.37$). For the nurses who participated, there was a significant increase in the degree of positive ethics after the theatre of situations ($2.62{\pm}.36$, p<.001). Conclusion: This result implies that indirect experiences such as 'theatre of situations' in terms of ethical issues, created as well as performed, is effective for registered nurses in changing their ethical perspective positively and in realizing ethical ideals.

Relationships of Fear of Breast Cancer and Fatalism with Screening Behavior in Women Referred to Health Centers of Tabriz in Iran

  • Background: Fear and fatalism have been proposed as factors affecting breast cancer screening, but the evidence is not strong. This study aimed to determine relationships of fear and fatalism with breast cancer screening behavior among Tabriz women in Iran. Materials and Methods: In a cross- sectional study, 370 women referred to 12 health centers in Tabriz were selected with two-stage cluster sampling and data regarding breast cancer screening, fatalism and fear of breast cancer were collected respectively with a checklist for screening performance, Champions Fear and Pow Fatalism Questionnaires. Data were analyzed by logistic regression with SPSS software version 16. Results: Only 43% and 23% of participants had undergone breast self- examination and clinical breast examination. Among women older than 40 years, 38.2% had mammography history and only 2.7% of them had done it annually. Although fatalism and fear had a stimulating effects on breast cancer screening performance th relationships were not significant (P>0.05). There was a negative significant correlation between fear and fatalism (r= -0.24, p=0.000). On logistic regression analysis, age (OR=1.037, p<0.01) and income status (OR= 0.411, p<0.05) significantly explained BSE and age (OR=1.051, p<0.01) and body mass index (OR= 0.879, p<0.01) explained CBE. Also BMI (OR= 0.074, p<0.05) and income status (OR=0.155, p<0.01) was significantly effective for mammography following. Conclusions: Breast cancer screening behavior is inappropriate and affected by family livelihood status and lifestyle leads to weight gain, so that for promoting of screening behaviors, economic support to families, lifestyle modification and public education are suggested.

침상각도 상승 교육에 대한 중환자실 간호사의 지식, 인식 및 수행 정도 (The Effect of Backrest Elevation Education on ICU Nurse's Knowledge, Perception and Performance)

  • Purpose: This study was conducted to examine the differences of knowledge, perception, and performance between the points of time before and after ICU nurses had the backrest elevation education. Method: The study subjects were 58 nurses at a medical and surgical ICU of one general Y hospital located in Seoul. They received the education, including backrest elevation guideline and related education materials. Data were collected from May 11 throughout August 12, 2007 with a structured questionnaire. Results: 1) There were significant differences in the mean scores of knowledge(2.21 at pre-education, 5.24 at post-education), perception(36.96 at pre-education, 53.36 at post-education), and performance(32.08 at pre-education, 43.51 at post-education), 2) There was a significant correlation between nurse's perception and performance (p=.000). 3) The nurse's perception regarding the importance of the back rest elevation education was significantly effective on their performance (p=.000). Conclusion: The backrest elevation education would contribute to improve ICU nurse's knowledge, perception and performance. The more the nurses would consider the importance of this education, the better they would perform the nursing intervention of backrest elevation.

A Study on Factors for Improving CPR based on Health Care Professionals Treating Cardiac Arrests

  • This study aimed to help build a quality control program to improve cardiac arrest treatment via analysis of medical records in a local tertiary general hospital to evaluate factors that influence clinical outcomes of in-hospital cardiac arrest. At first, the medical records of in-hospital cardiac arrest were analyzed, and targeted surveys about functional and structural factors associated with cardiopulmonary resuscitation (CPR) were conducted amongst the workforce in charge of cardiac arrest treatment. From January 2012 through June 2013, a total of 486 adult cases of in-hospital cardiac arrests, except for those occurring in the emergency room, were enrolled in this study. Among the patients, those of recovery of spontaneous circulation were 57.8%; 13.8% of patients were discharged alive; 8.9% of patients were discharged without significant neurologic sequela. Despite CPR is usually successful when administered as quickly as possible, in this analysis showed that prompt reaction after initial recognition was significantly lower in nurses compared with doctors. Analysis of survey results showed that confidence in performing CPR was significantly associated with the experience of CPR in doctors, while in nurses educational experience showed a correlation. In order to improve quality of in-hospital CPR system maintaining and increasing confidence of CPR performance is the most important factor. Therefore it can be helpful to develop and apply a phased, customized education program using training simulators as well as personalizing them to increase the personnel's confidence in CPR performance.

간호대학생의 신생아 시뮬레이션 실습교육 연구동향(2011년~2023년) (Research Trends in Neonatal Simulation Practice Education of Nursing Students)

  • 본 연구는 국내 간호대학생의 신생아 시뮬레이션 실습교육 관련 연구동향을 살펴보기 위한 서술적 조사연구이다. 문헌수집은 국내 6개 전자 데이터베이스에서 ('신생아 시뮬레이션')과 ('간호학생' OR '간호대학생' OR '학생간호사')로 조합하여 검색한 결과 총 17편을 선정하였다. 연구결과는 2011년~2015년 7편으로 시작하여 2016년~2020년 5편과 2021년~2023년 5편으로 다소 감소하는 것으로 나타났다. 연구설계는 양적연구 총 16편(94%)으로 중재연구 15편(88%), 서술적 조사연구 1편(6%)이고, 혼합연구는 1편(6%)이었다. 시뮬레이션 주요 사례 주제는 고위험 신생아 간호 9편(52%), 호흡곤란증후군 중환아 간호 3편(18%), 신생아 간호 3편(18%) 정상 신생아 간호 1편(6%), 신생아 응급 기도 간호 1편(6%)으로 나타났다. 주요 결과 변수 및 적용효과는 임상수행능력 5편(19.2%)이 가장 많았고 그 다음 순으로 실습만족도 3편(11.5%)이었고 적용 효과가 있는 것으로 나타났다. 결론적으로 간호대학생의 신생아 시뮬레이션 실습교육의 확대, 반복연구, 질적연구 등 다양한 연구방법이 요구된다.

혈액종양내과 병동간호사의 긍정심리자본과 죽음인식이 임종간호 수행에 미치는 영향 (Influence of Positive Psychological Capital and Death Awareness on Terminal Care Performance of Hematooncology Unit Nurses)

  • 목적: 본 연구는 혈액종양내과 병동에 근무하는 간호사를 대상으로 긍정심리자본과 죽음인식을 확인하고 임종간호 수행에 미치는 영향을 파악하여 임종간호 수행의 질적 수준 향상을 위한 근거자료를 제공하고자 하였다. 방법: 서울시에 소재한 일 상급종합병원에서 근무하는 혈액종양내과 병동에 근무하는 간호사 127명을 대상으로 자가보고 설문지를 통해 긍정심리자본, 죽음인식, 임종간호수행을 조사하였고, 기술 통계, 피어슨 상관관계 및 다중회귀분석을 이용하여 분석하였다. 결과: 긍정심리자본과 죽음인식, 임종간호 수행 간에는 모두 양의 상관관계가 나타났다. 임종간호 수행에 미치는 영향요인은 긍정심리자본의 하위 영역 중 희망, 회복탄력성, 지난 1년 이내 가족, 친척 또는 친구의 죽음 경험이었으며 임종간호 수행에 대한 설명력은 32.1%로 나타났다. 결론: 혈액종양내과 병동에 근무하는 간호사의 임종간호 수행의 질적 수준 향상을 위하여 희망과 회복탄력성을 포함한 교육 프로그램 개발이 필요하다.

임상간호사의 조직몰입과 관련변인 분석 (Analysis of the Clinical Nurses' Organizational Commitment and Relating Variables.)

  • Organizational commitment is characterized by a strong belief in and acceptance of the organization's goal and values ; a willingness to exert considerable effort on behalf of the organization ; a strong desire to maintain membership in the organization. The purpose of this study was to identify the variables which influence the organizational commitment and to test the relationship between the clinical nurses' organizational commitment and turnover intention. The subjects were 412 nurses who were working on general wards at three General Hospitals in Chounbuk. The data were collected by self-reporting questionnaire from Sept. 20 to Sept. 31, 1995. The instruments used in this study were Likert-type scale which were Organizational Commitment Scale by Mowday et al, and Turnover Intention Scale by Mobley et al. The questionnaire of organizational characteristics was made through reviewing literature. The data were analyzed by frequency, percentage, t-test, one-way ANOVA, Scheffe test, Pearson's correlation coefficient, and stepwise multiple regression with $SPSS-PC^+$ program. Major findings are as follows : 1. Mean scores for organizational commitment were 3.008 and turnover intention 3.167 on a 5 point scale. 2. Age and clinical experience were positively related to organizational commitment(r=.4806, p=.000; r=.4792, p=.000). 3. In the personal variables, hospital type(F=21.374, p=.000), education level(F=5.481, p=.001), position(F=30.867, P=.000), and marital status(t=-5.63, p=.000) of the nurses showed significant differences in organizational commitment. 4. The variables to the organizational characteristic were statistically significant in organizational commitment : formalization(r=.3458, p=.000), human-centered organizational characteristic(r=.4302, p=.000), performance- centered organizational characteristic(r=-.1502, p=.000), payment(r=.2234, p=.000), promotion(r=.2710, p=.000), benefit & service(r=.2325, p=.000), and resource inadequacy(r=-.2172, p=.000). 5. For the purpose of identifying the predicting variables in organizational commitment, stepwise multiple regression is conducted. The results show that age, human-centered organizational characteristic, formalization, hospital type, resource inadequacy, promotion, and benefit & service were significant variables and explained 46% of the variance. 6. Organizational commitment was significantly negatively correlated with turnover intention(r=-.6442, p=.000). As a result of stepwise multiple regression analysis, organizational commitment is the most powerful variable predicting turnover intention and explained 41.5% of the variance. In conclusion, this study shows that the higher the organizational commitment level, the lower the turnover intention. Therefore, for effective turnover management, it is important to improve the organizational commitment among clinical nurses. In order to enhance the organizational commitment by the clinical nurses, it will be useful to consider significant organizational characteristics variables identified in this study.

보건관리자와 임상간호사의 직무특성, 지식공유 및 조직문화가 조직몰입에 미치는 영향 (Relationships among Job Characteristics, Knowledge Sharing, Organizational Culture, and Organizational Commitment in Occupational Health Nurses and Clinical Nurses)

  • Purpose: This study aimed to investigate the relationships among job characteristics, knowledge sharing, organizational culture, and organizational commitment in occupational health nurses (OHN) and clinical nurses (CN) to identify factors that might influence organizational commitment. It aimed to contribute to the improvement of knowledge management in health care organizations, including industrial workplaces and hospitals. Methods: A convenience sample of 188 nurses, specifically 100 OHN and 88 CN, was selected. Data were analyzed with descriptive statistics, t-tests, correlation coefficients, and multiple regression analysis. Results: OHNs had a significantly higher education level and current work experience; higher proportion of participants with contract employment status, day work shift, less turnover experience, good lifestyle; and higher scores on organizational culture ($3.3{\pm}0.61$) and organizational commitment ($3.1{\pm}0.60$). Additionally, the correlations among organizational commitment, job characteristics, knowledge sharing, and organizational culture ware higher (r=.38~.57) among OHNs as compared to those among CNs. Further, the organizational commitment of OHNs was significantly influenced by knowledge sharing, organizational culture, working hours, and salary ($R^2=.32$), while that of CNs was influenced by organizational culture and life event stress ($R^2=.11$). Conclusion: There is a need to develop measures to enable nurses to contribute to the improvement of knowledge management in health care organizations as major human resources, by including interventions for the major factors leading to between-group differences in organizational performance.
