Lung function tests in preschool children (5세 미만의 폐기능 검사)

  • Measurement of lung function is an integral component of respiratory physiology and of clinical assessment of lung diseases in school age children and adults. Pulmonary function test of infants and children under the age of 2 years have now been standardised and are being used both in research and as an adjunct to clinical management. By contrast, until recegntly, children of preschool age, i.e. between 2-6 years represented a major challenge for pulmonary function test assessment, this particular period commonly being referred to as the 'dark ages' of Pediatric Pulmonology. Measurement of lung function in preschool-aged children is now feasible. However, much work remains to be done in standardizing how these tests are performed, and in understanding the most appropriate role for the various tests in the study of growth and development of the respiratory system and in the clinical management of children in this age group. As the field develops and the knowledge of respiratory physiology in this age group expands, investigation of different and more appropriate algorithm use in preschool children, together with development of more appropriate reference data, may result in improved disease discrimination.

An Analysis of Requisite Knowledge Body of Physiology for Clinical Nursing Practices (임상실무에서 필요로 하는 생리학 내용에 대한 연구)

  • The purpose of this study was to define the content of physiological knowledge needed for clinical nursing practices. Subjects of physiology were classified into 15 areas, and each areas was further classified into subareas, resulting in a total of 194 subareas. The degree of importance of each subarea was measured with a 4-point scale. The subjects of this study were 179 nurses of two university hospitals located in Seoul and Inchon. The results were as follows : 1. The areas of physiology necessary for clinical nursing practice as a basic knowledge in the order of importance were : blood, respiratory system and renal physiology, function of the immune system, body fluid and cardiovascular system, body temperature, endocrine physiology and gastrointestinal physiology However, the degree of importance for reproductive physiology, neuro-physiology, energy and metabolism, cell and cell membrane physiology, muscular physiology and special sense was relatively low. 2. The most important content of physiology for all clinical areas in nursing was blood physiology. However, the degree of importance for each physiology area was different depending on clinical areas. 3. Subareas of physiology as a basic knowledge for clinical practice and education in nursing were blood transfusion, blood type, function of red blood cell, white blood cell and platelet, characteristics and function of hemoglobin, composition and function of plasma protein, and mechanism of blood coagulation and anticoagulation. In conclusion, areas of physiology necessary for clinical nursing practice were blood, respiratory system and renal physiology, function of immune, body fluid and cardiovascular system, body temperature, endocrine physiology and gastrointestinal physiology. However, the degree of importance for each physiology area was different depending on clinical areas In nursing.

A Clinical Case Study of Operation-delayed Cauda Equina Syndrome Patient Improved by Korean Traditional Treatment (한방요법으로 호전된 수술이 지연된 마미 증후군 환자의 증례보고)

  • Objectives The objective of this study is to report clinical effect of korean traditional treatments for Operation-delayed Cauda Equina Syndrome patient. Methods The patient was treated by korean traditional treatments including acupuncture, physical treatment, herbal medication. The improvement of the clinical symptoms was observed by Modified Visual analog scale (Modified VAS), SSCES (Scoring System for Cauda Equina Syndrome), function of urination and function of defecation. Results After treatments, Modified VAS, SSCES, function of urination and function of defecation were improved in this case. Conclusions On the basis of these results, we suggest that korean traditional treatment might be an effective method to improve the clinical symptoms of Operation-delayed Cauda Equina Syndrome patient.

A review of traditional Korean medical treatment for cancer-related cognitive impairment (암 관련 인지 장애의 한의학적 치료에 대한 문헌 고찰)

  • Objectives: This article aims to review clinical studies related to traditional Korean medicine (TKM) for cancer-related cognitive impairment (CRCI) up to date. Methods: EMBASE, Pubmed and eight Korean databases were searched for clinical studies about TKM for CRCI up to July 2016. We selected articles about subjects with cancer, using TKM interventions including acupuncture, moxibustion or herbal medicine, containing assessment about cognitive function. Results: Two randomized controlled trials (RCTs) and one randomized controlled feasibility study were included. Selected studies were containing assessments about cognitive function but not as primary outcomes. The three studies included one acupuncture study and two herbal-medicine studies. Only one herbal-medicine study showed improvement in cognitive function. Conclusions: Few clinical studies mainly focusing CRCI treated with TKM have been performed and have shown contradictory results until now. Future studies should be performed considering findings from pre-clinical and clinical studies altogether, and they should be controlled to minimize risk of bias thorough out overall courses of designing, conducting and reporting them.

  • Breathing is controlled by complex interactions between the central and peripheral nervous systems in conjunction with the respiratory system. Neurological diseases predispose patients to nocturnal desaturation and pneumonia due to respiratory dysfunction, which increases mortality, daytime sleepiness and fatigue, and reduces the quality of life. Respiratory function tests are required to identify respiratory function decline and to consider compensatory management. This review summarizes the characteristics of several respiratory function tests and their applications to neurological diseases.

Clinical cases of vision therapy for visual perception and attention enhancement of children with mild developmental disabilities (경도의 발달장애 아동의 시지각 및 주의력 향상을 위한 시기능 훈련의 임상사례)

  • Purpose. The purpose of this study was to investigate the effect of visual field training on visual function, visual perception and attention enhancement. Methods. We examined the changes of visual function and visual perception tests for children with mild developmental disabilities through vision therapy. Results. At the time of the initial visit of the child, the ability to control not only distant and near visual function, but also accommodation, was also very limited. Also, the visual perception score was measured at 80 points. Through vision therapy, it was confirmed that the improvement of control ability as well as the visual function and visual perception function test results were also improved to 108 points. Conclusion. It was confirmed that vision therapy results in an improvement of visual function, improved visual perception ability, as well as visual perception ability, It was concluded that visual function affected visual perception, and that vision therapy used in order to prevent visual function deterioration was found to be very helpful.

A Survey on the Job Importance of Occupational Therapists According to Length of Clinical Career (작업치료사의 임상 경력에 따른 작업치료직무 중요도에 관한 조사)

  • Purpose : This study examined the differences in occupational therapists' jobs depending on their past clinical careers using a survey of the importance of occupational therapy tasks in South Korea. This survey sought to provide fundamental data for strengthening occupational therapists' capacities and developing clinical training programs for therapists' future careers. Methods : An online survey was used to collect data from 93 South Korean occupational therapists. The subjects were divided into three groups for the analysis of the importance of occupational therapy tasks, depending on the length of their clinical careers: less than 3 years, from 4 to 6 years, and over 7 years. Results : A within-groups comparison of task importance found that "occupational therapy evaluation" was most important in all three groups. In between-groups comparisons, "patient information check", "assessment planning and establishment", "sensory function assessment", "social skills assessment", "developmental function improvement", "basic daily activities training", "social skills improvement", and "patient education" demonstrated statistically significant differences (p<.05). Each group showed different priorities. In the less-than-3-years group, "understanding of patient's needs" was most important, followed by "arm function improvement", "arm function assessment", "nerve, musculoskeletal, and motor-related functional improvement", and "basic daily activities training". In the 4-to-6-years group, "arm function assessment" was most important, followed by "basic daily activities assessment", "patient's needs", "arm function improvement", and "patient information check". In the over-7-years group, "understanding of patient's needs" was most important, followed by "patient information check", "basic daily activities assessment", "basic daily activities training", and "sensory function assessment". Conclusion : This study found that what occupational therapists consider important in their jobs varies according to their past clinical career and that occupational therapists with more than 7 years' experience are more aware of the importance of diverse fields of occupational therapy. Differentiated education that considers length of career and a range of training programs for more experienced occupational therapists will be needed in the future.

Clinical Effect of Albumin Addition to the Prime solution in Cardiopulmonary Bypass (체외순환시 산화기 충전액에 첨가된 알부민의 임상적 효과)

  • Colloid solution is commonly used to increase the oncotic pressures of priming solutions used in the cardiopulmonary bypass circuit. To study the effectiveness of this practice, we retrospectively evaluated 24 patients undergoing cardiac operations to receive isoelectric solution plus 50 gm of albumin[group A], isoelectric solution plus 25 gm of albumin[group B] and isoelectric solution without albumin[group C] as the prime solution for the bypass circuit. Various clinical parameters related to the perioperative fluid balance, cardiopulmonary function, and renal function were studied. There were no differences between the three groups with regard to postoperative clinical parameter of cardiopulmonary, liver function and renal function. But at the previous report by Cho et al. [38], there was a significant difference between groups with regard to hematologic property, especially in crenated red blood cells. These hematologic events did not affect the clinical outcome but thorough evaluations are needed in order to prove the clinical effect of crenated red blood cell. We conclude that there is no clinically detectable advantage for the routinely supplementing of albumin to the priming solution of bypass circuit in cardiac operations. Whether this practice can be of value in selected cases needs to be further studied.

A study on the effect of vision therapy after surgery for intermittent exotropia under 12 years of age (12세 미만 간헐성 외사시안의 수술 이후 시각훈련효과에 대한 임상연구)

  • Purpose. This study conducted visual function training for children under 12 years of age who relapsed after surgery for intermittent exotropia. We are trying to find out whether the visual function has been improved by visual function training. Methods. After surgery, the subject with recurrent exotropia was given a prescription for refractive error, followed by visual function training and vision therapy with visual sence using prisms and lenses. Results. The subjects' positive relative convergence improved to 19.69𝚫, corrected visual acuity improved to 0.88, and stereoscopic vision function improved to 53.08 arc second. It was found that the smaller the angle of deviation at the time of recurrence after surgery, the better the vision therapy effect. Conclusions. It can be seen that visual function training is helpful in improving visual function, and the importance of visual function training can be known.

Clinical Use of PFA®-100 in Pre-surgical Screening for Platelet Function Test (수술 전 혈소판 기능 검사를 위한 PFA®-100의 임상적 이용)

  • The Platelet Function Analyzer (PFA)$^{(R)}$-100 measures the ability of platelets activated in a high-shear environment to occlude an aperture in a membrane treated with collagen and epinephrine (CEPI) or collagen and ADP (CADP). The time taken for the flow across the membrane to stop (closure time, CT) is recorded. The aim of this study was to assess the potential of the PFA$^{(R)}$-100 as a primary clinical screening tool using the wide spectrum of clinical samples assessed for platelet function as well as to perform the optimal algorithm for the use of PFA$^{(R)}$-100. We established the reference interval in 460 hospital inpatients defined as having normal platelet function based on classical laboratory tests. The reference interval by using the range $5^{th}$ and $95^{th}$ percentile was 84~251 seconds for males CEPI-CT and 85~249 seconds for females CEPI-CT. A total of 1,200 inpatients were enrolled to identify impaired hemostasis before surgical interventions. The abnormal group showing prolonged CEPI-CT was 303 cases (18.9%). Only 3 cases had both abnormal CEPI-CT and CADP-CT. Several factors including sample errors, drugs, hematologic abnoralities were contributed to unexpected prolonged CEPI-CT for screening test. The von Willebrand factor (vWF:Ag) assay was performed only in one patient to verify the algorithm for the use of PFA$^{(R)}$-100. The PFA$^{(R)}$-100 was sensitive and rapid method for primary screening test of platelet dysfunction, so we can substitute it for the bleeding time in routine clinical practice.
