Accelerating Change in Medical Education after the Dismantlement of the Intern Training System (인턴제도 폐지 논의 이후의 의학교육 변화)

  • In light of changes in today's medical environment, whether to dismantle the 50-year-old internship training system in the Republic of Korea is under debate. Although the question remains open, discussions on such issues have drawn attention to the quality of the clinical clerkship and student career advisory programs in medical colleges. The purpose of this study is to analyze the experiential clerkship and career exploration issues. Ensuring excellence of the clerkship and career advisory sessions is an essential responsibility of educational institutions regardless of whether the intern training system is dismantled. Important objectives of the experimental clerkship include reinforcing prerequisites established by law, introducing a student practice license, developing a standardized clinical assessment and student portfolio requirement, and publishing a guidebook for clinical directors. For career exploration, it is necessary to broaden participants' experiences of specialties and to manage the variety of student career guidance programs. It is imperative for the Korean Association of Medical Colleges, in collaboration with medical colleges, to play a leading role in focusing more attention and effort on such issues.

An Analysis on Nursing Students' Clinical Situation for Development of Oriental Nursing Practice Education (한방간호 실습교육 개발을 위한 간호학생의 임상실습 실태 분석)

  • Purpose: The purpose of this study was to develop clinical education of Oriental Nursing. Methods: The subjects were one hundred and ninety-three students who have completed clinical practice. The questionnaires were composed of experience type and Oriental Nursing practices(25 items), level of satisfaction(20 items), difficulty level of practices(6 items), and preparation level of practice instructor(5 items). The reliability were Chronbach's $\alpha$=.84, .86, .74, and .93 respectively. Frequencies, percentage, t-test, and Pearson's Correlation was used in data analysis using SPSS PC+ 11.0. Results: Nursing students recognized that they had difficulties caused by a lack of knowledge of Oriental medicine, of learning the uniqueness of the nurse's role and lack of nurse's enthusiasm in teaching students. However, they were confident in preparing acupuncture and implementing Moxibustion, large Moxibustion, Bu-Hang therapy, aroma therapy and on administration of medicines and control of temperature of the medicines, exercise and rehabilitation. Therefore, instructors should train nursing students by developing a unique Oriental Nursing practice and reinforcement of nurses' independent and cooperative roles through the acquisition of Oriental Medical knowledge and terminology. It is necessary that they develop an educational program based on Oriental Nursing practice, perform consistently within the program and train its teaching faculty and instructors.

Ethical dilemma and related factors in some clinical dental hygienists (일부 임상치과위생사의 윤리적 딜레마와 관련 요인)

  • Objectives: The purpose of the study was to evaluate the level of ethical dilemma and related factors in some clinical dental hygienists. Methods: A self-reported questionnaire was filled out by 290 clinical dental hygienists in G metropolitan city from January 5 to February 27, 2015. The instruments used for this study were the modified ethical dilemma scale developed Lee, Kim and Park. The questionnaire consisted of dental hygienists & subjects(4 items), dental hygienists & professionals(12 items), dental hygienists & partners(7 items), and medical information(31 items). The questions were measured by Likert 5 points scale. The statistical analyses include descriptive statistics, t-test and stepwise multiple regression analysis using SPSS 12.0 program. Results: Dental hygienists perceived ethical dilemma scored 3.35 and 3.24 except medical information. Professionals and partners in sub-domain of ethical dilemma by experience of ethics education and ethical conflict were high and showed statistically significant difference. The strongest predictor of dental hygienists and subjects was job satisfaction. The strongest predictor of dental hygienists and partners was adequacy of ethics education in college/university. Conclusions: The results of this study suggest that it is necessary to develop the professional dental hygiene program for the prevention of ethical dilemma in dental hygienists and to make ethics education for ethical decision making.

Effects of High-fidelity Simulator and Standardized Patient on Nursing Care for Children with Fever in Nursing Students (고충실도(High-fidelity) 시뮬레이터와 표준화 환자 (Standardized Patient)를 활용한 발열환아 간호 교육의 효과)

  • Purpose: This study was conducted to investigate the effects of simulation-based education using high-fidelity simulator and standardized patient in nursing care for children with fever on nursing students. Methods: A total of 166 senior nursing students who completed pediatric nursing practicum courses participated in this study. The single group study design with pretest and posttest was used. The simulation education was provided for 200mins including orientation, simulation preparation, simulation practicum, and debriefing. Pre and post surveys were performed using questionnaires on clinical performance competency, communication skills, critical thinking disposition and self-confidence. Results: The mean scores of clinical performance competency (t=-2.56, p<.05), communication skills (t=-6.39, p<.001), critical thinking disposition (t=-3.43, p<.001), and self-confidence (t=-3.72, p<.001) in posttest were significantly higher than those in pretest. Also, clinical performance competency in nursing care for children with fever has significant relationships with communication skills, critical thinking disposition and self-confidence. Conclusion: The results indicate that simulation-based education using high-fidelity simulator and standardized patient is an effective strategy for improving clinical performance competency, communication skills, critical thinking disposition and self-confidence in nursing students. Further study is needed to verify the effects.

Content Analysis of the Experience of Preceptors in Clinical Education for Senior Student Nurses (학생 임상실습지도자(Preceptor)의 역할수행경험에 대한 내용분석)

  • Purpose: The purpose of this study was to identify experiences of nurses who served as preceptors in clinical education for senior student nurses in a college of medicine in Wonju city. Method: Data was collected from 20 preceptors instructing senior student nurses in 2001 using a self-completion questionnaire. To analyze data, content analysis was done using an analysis scheme developed by the investigators. Result: The analysis scheme consisted of 7 categories and 25 subcategories. 135 significant statements were analyzed and categorized. Preceptors indicated that they were role models, socialization facilitators and educators while instructing students in the clinical practicum. In performing the preceptors' role, preceptors reported that their most important change was self-enhancement, and positive experience was a constructive work atmosphere. The most important factor facilitating the preceptors' role performance was support from head nurses, and the most discouraging factor was work loads. Conclusion: This study suggests that interventions for encouragement and socialization of preceptors should be developed to promote clinical education for senior student nurses.

Relationship among Essentials of Fundamental Nursing Skills Performance, Stress from Work and Work Capability of New Clinical Nurses (신규간호사의 핵심기본간호술 수행과 업무스트레스 및 업무수행능력과의 관계)

  • Purpose: This study identifies the relation among the essentials of fundamental nursing skills performance (EFNSP), work capability and stress from work experienced by new clinical nurses. Method: This is a cross-sectional descriptive study with a self-reported questionnaire. The subjects were 224 new clinical nurses employed by general hospitals having more than 400 beds. The data, collected from February 10 to March 7, 2014, were analyzed using descriptive statistics, t-tests, ANCOVA, Pearson's correlation analysis and hierarchical linear regression. Results: The highest frequency of EFNSP of new clinical nurses was vital sign measurement (4.74, ${\pm}.89$) and the lowest frequency of performance was Basic CPR & defibrillator application (1.81, ${\pm}.94$). There were significant positive correlations between frequency and confidence of EFNSP (r=.64, p<.001), frequency and work capability (r=.34, p<.001), and confidence of EFNSP and work capability (r=.48, p<.001), but negative correlation stress from work and work capability (r=-.17, p=.009). General characteristics, stress from work, frequency and confidence in EFNSP and stress from work explain 25.1% of work capability. Conclusion: This result suggests the importance of EFNSP education in nursing schools and availability of opportunities to practice EFNSP during the post-graduation waiting period until assignment to a hospital.

A Study on Introduction and Application of Preceptor System in Clinical Practice Process of Dental Hygiene Students (치위생과 학생들의 임상 실습과정에서 프리셉터(가칭: 임상실무지도자)의 도입과 활용에 관한 연구)

  • The realistic problem about the dental hygiene education in our country is what the correlation between theoretical education and clinical education is short, and what the systematically clinical-practice education is failing to be realized. In order to solve this problem, the aim is to introduce preceptor and to present necessity in order to apply preceptor to the practice education for dental hygiene department. 1. What is the largest in the difference between the observation experience and the performance experience by element in clinical-performance activity is the patience-care service sphere(P < .001). 2. A hospital with the highest observation-practice experience level in children with the observation experience by hospital was in order of K hospital, Y hospital, H hospital, B hospital, and A hospital(P < .001, P < .01). 3. A hospital was the highest in the difference of the performance-practice experience level by hospital. And, the next was in order of H hospital, Y hospital, K hospital, and B hospital(P < .001, P < .01). 4. As for recognition on importance of clinical-practice activity in dental hygienists, the dental hygienists with more than the third year were indicated to be the highest than the dental hygienists with less than the 3rd year among 58 dental hygienists. Given seeing the results of this study, the more systematic and educational clinical-practice environment will need to be improved by suggesting a role of preceptor and to seek for a plan for application in the clinical-practice education.

Effect of Integrated-Simulation Practice Education on Nursing Students' Communication Ability, Learning Confidence, Clinical Reasoning Competence, and Clinical Performance (통합시뮬레이션 실습 교육이 간호대학생의 의사소통능력, 학습자신감, 임상추론역량, 임상수행능력에 미치는 효과)

  • This study aimed to examine the effects of integrated simulation practice education for nursing college students on their communication skills, self-efficacy in learning, clinical reasoning ability, and clinical performance through a pre-post quasi-experimental design with a single group. The participants were 41 fourth-year nursing college students from a university located in G City, and the intervention took place from September to November 2023 over a period of nine weeks. The collected data were analyzed using SPSS 23.0, and the research findings are as follows. First, after simulation practice education, significant effects were observed in self-efficacy in learning (t=-2.21, p=.033), clinical reasoning ability (t=-2.97, p=.005), and clinical performance (t=-2.84, p=.007), but no significant difference was found in communication skills (t=-.224, p=.824). Second, after simulation classes for nursing students, clinical performance showed significant positive correlations with communication skills (r=.650, p<.001), self-efficacy in learning (r=.440, p<.001), and clinical reasoning ability (r=.824, p<.001). Based on these results, integrated simulation practice education for nursing students appears to be an effective educational method for enhancing their self-efficacy in learning, clinical reasoning ability, and clinical performance. To improve nursing students' clinical performance, it is necessary to operate simulation practices with various scenarios.

A Study on the Relationship between Self-esteem, Self-efficacy and Clinical Practice Performance, Academic Achievement, Decision Making of Major Field in Clinical Practice (간호학생의 자아존중감 및 자기효능감과 임상실습수행, 학업성취, 전공실습결정과의 관계)

  • The purpose of this study was to identify the relationship between self-esteem, self- efficacy and clinical practice performance, academic achievement, decision making of major field in clinical practice . The study subjects consisted with 244 students from G. junior nursing college that will adapt a new majoring program of clinical nursing practice funded by Ministry of human and resources development. Data were analyzed using descriptive statistics, and t-test, Pearson correlation, and multiple regression. The results of this study were summarized as follows: 1. The students were relatively performed well in clinical practice setting (total M :3.78). The item of student's personal relationship between patients, patient's family, and professional staff showed high score (M=3.96). However students did not feel comfortable to communicate with patients, patient's family and professional staffs in clinical setting(mean=3.56). 2. Students who had experiences of temporal absence from school showed significant difference in the variable of academic achievement and decision making of major field in clinical practice Students who entered to nursing school with self-motive revealed statistically significant difference in the variable of clinical practice performance, academic achievement, self-esteem, self-efficacy, and decision making of major field in clinical practice. 3. There was a significant correlation between self-esteem and clinical practice performance, (r=.512, p<.000). And the relation between self-esteem and decision making of major field in clinical practice was significantly correlated(r=.377, p<.000). Self-efficacy and clinical practice performance(r=.567, p<.000), and decision making of major field in clinical practice (r=.441, p<.000) showed significant correlations. Also relation between academic achievement and self-esteem reveled a significant correlation(r=.129, p<.05) but correlation between self-efficacy and academic achievement was not significant. 4. Correlation between clinical practice performance and decision making of major field in clinical practice also identified a significant relation(r=.498, p<.000). 5. Self-esteem and self-efficacy variables predicted clinical practice performance and explained 37.7% of clinical practice performance. 6. Aptitude and personal interests(m=4.07) reveled important factor affecting to select majoring area in clinical practice, and advice from parents reflected low percentage on selecting places(m=2.42). In conclusion, the variables of self-esteem and self-efficacy showed significant correlations with the variables of student's clinical practice performance, academic achievement, and decision making of major field in clinical practice. Thus it is recommended that student's affective domain of self-esteem and self-efficacy should be developed by nursing intervention before clinical nursing education.

The Effect of Ego-resilience, Stress Coping Styles, Teaching, Effectiveness, and Family Support on Satisfaction of Clinical Practice in Nursing Students by AMOS Structural Equation Model (간호대학생의 자아탄력성, 스트레스 대처방식, 교수효율성, 가족지지가 임상실습만족도에 미치는 효과: 구조방정식 모형구축)

  • Purpose: This study aimedto develop a structural equation model on the relationship among ego-resilience, and teaching effectiveness on clinical education, stress coping style, perceived family support, and satisfaction of clinical practice in nursing students in order to increase the satisfaction of clinical practice. Methods: The subjects were 399 undergraduate nursing students enrolled in 7 universities and participating in 10 clinical practice setting areas in 5 cities in South Korea. The research was conducted from December 5th, 2009 to February 20th, 2010. The structural equation model was used to perform the analysis with the statistics program of SPSS win 17.0 and AMOS 5.0. Results: Teaching effectiveness and family support showed a direct effect on satisfaction of clinical practice, while ego-resilience and stress coping styles showed an indirect effect. The biggest total effect on satisfaction of clinical practice was teaching effectiveness followed by family support, ego-resilience, and stress coping styles respectively, which accounted for 50.9%. Conclusion: Based on outcomes of this study, the proposed model allows better understanding of the satisfaction of clinical practice. This result implies that strategies or intervention programs enhancing extrinsic protection factors, motivation factors, and intrinsic protection factors should be considered. In addition, a structural support system for increasing the satisfaction level of clinical practice in nursing students should also be considered.
