Researches Trend of Korea Institute of Oriental Medicine in Sasang Constitution Field - from 2007 to 2010 (한국한의학연구원의 사상체질 연구동향 - 2007년부터 2010년까지의 논문을 중심으로 -)

  • Objective : The aim of this study is to investigate current status of Sasang constitutional medicine researches and to provide source in setting a development strategy and making a policy on the Sasang constitutional medicine. Method : This study analyzed trends in research as represented in the 142 articles of Korea Institute of Oriental Medicine in the field of Sasang constitution medicine from 2007 to 2010. Result : The main themes of studies were Sasang constitution clinical study (68, 47%) and developing instruments for measuring diagnosis (48, 33%), especially pulse diagnosis (25, 17%). Genetic researches and trends studies on Sasang constitutional medicine were respectively 17 (12%) and 8 (5%). As a result, Korea Institute of Oriental Medicine established Sasang clinical information bank, and discovered clinical character of Sasang constitution to develope various diagnosis tools include constitution survey form and pulse diagnosis instrument. Genetic researches also discover constitution related genes such as FTO and MC4R, and explore association between heredity and Sasang constitution. Conclusion : The above mentioned achievements of Korea Institute of Oriental Medicine were limited to diagnosis process, unquestionably studies for reproducibility and authenticity of constitution diagnosis were important mission of Sasang constitutional medicine. However, to develop the advance of Sasang constitutional medicine, not only constitution diagnosis but also constitutional treatment studies should be conducted.

  • This study was performed to examine the clinical discussion records where Lee Gong-yoon, participated in Royal treatment at Joseon Dynasty, was involved. Lee Gong-yoon who scouted from the outside had participated in the medical center of the Royal Family for several years as a royal doctor. But he preferred aggressive prescriptions and behaved indiscreet. Eventually, he was exiled when Kyung-Jong who was the 20th king of the Joseon Dynasty died. He was harshly criticized until the year crown prince ascended the throne following Kyung-Jong. But some officials started to defend him since March of the next year. They recognized Lee Gong-yoon's aggressive prescriptions and indiscreet behavior. But mentioned that some very aggressive prescriptions didn't come from his own idea. After they proved their insistence true, his place of exile was changed from island to outland. Different from other doctors who focused on patients' 元气 ($yu{\acute{a}}nq{\grave{i}}$), Lee Gong-yoon focused on removing patients' symptoms when he decided to cure a disease. And also he had considerable insight of "醫學入門 ($y{\bar{i}}xu{\acute{e}}r{\grave{u}}m{\acute{e}}n$)". Despite of his high degree of medical knowledge, unconventional character and behavior made the end of his life unfortunate.

Multiple Endocrinologic Complications in Thalassemia Major

  • Thalassemia major is a genetic disorder with a defective synthesis of either the alpha or the beta chain of hemoglobin A. Blood transfusion is crucial for the survival in these patients. Unfortunately, endocrine dysfunction is a very common complication in these patients and is principally due to excessive iron overload as a result of frequent blood transfusions. Although regular blood transfusion may increase life expectancy, disturbances in growth and pubertal development, abnormal gonadal functions, impaired thyroid, parathyroid and adrenal functions, diabetes, and disorderly bone growth are common side effects. We hereby present a case of a 23-year-old, unmarried woman with beta thalassemia major presenting with primary amenorrhea, poor development of secondary sexual character, and short stature. Thorough history, clinical examination, and laboratory investigation, including dynamic function test (insulin tolerance test) were conducted. These tests confirmed that she had multiple endocrinopathies, including hypogonadotropic hypogonadism, growth hormone deficiency, and subclinical adrenal insufficiency, which were caused by iron overload. She required hormone replacement therapy. Early recognition of possible deficiencies in hypothalamo-pituitary-end organ hormones caused by iron overload in thalassemia patients that undergo frequent blood transfusion procedures is essential. Appropriate treatments, including transfusion regimen and chelation therapy, as well as specific treatment of each complication are the crucial for the successful management and improvement of quality of life these patients.

  • Purpose: The purpose of this study was to examine the usefulness of the Myers-Briggs Type Indicator (MBTI) through a character analysis of a novel and to improve the understanding of various types of people. This study aimed to increase nurses' understanding and empathy regarding the personality of various patients. Methods: This study categorized the characters in the novel Our Happy Time based on MBTI and analyzed their personality characteristics. Results: It is important for nurses to understand that the following types of patients can be identified using the MBTI: those with a concise explanation (ENTP), a modest attitude (ISFJ), an objective response (ENFJ), and a logical response (ENTJ). Conclusion: It is important for nurses to broaden their understanding of various personality types and backgrounds related to the formation of patients' personalities that they encounter in the clinical field via analyses using the MBTI personality types. Furthermore, understanding communication skills can be an important first step in resolving conflicts that occur during peer interactions.

  • Objectives : We studied the aspects of headache in the patients with stroke. We compared character of headache on stroke with functional one. Methods : The subject of this study was based on 63 patients with headache who were admitted to the oriental hospital after stroke. We analyzed patients into sex & age, month, severity of headache on stroke type & lesion, site & character of headache. associated symptoms & signs on headache state.Results : The age of headache with stroke is higher than general headache. There was significant differences between general headache and headache with stroke. The cerebral hemorrhage is severer than cerebral infaction in th pain of headache. The most frequently appeared site of headache is temporal area and associated symptoms is vertigo & nausea.Conclusions : The headache of stroke patient is different form functional headache. because of cerebral vascular accident influence on inducing headache. Therefore we should study different methods between the headache of stroke patient and the others in medical treatment.

Study on the Construction for Constitution Information Bank (체질정보은행 구축에 관한 연구)

  • This study aims to construct Constitution Information Bank to objectification of Sasang Constitution Diagnosis and Treatment. 422 cases clinically confirmed their Constitution by experts are collected retrospectively and 219 cases for constitution-family study are also collected in Constitution Information Bank from Nov. 2006 to Sept. 2007. The information include their physique, appearance, character, temper, symptom and treatment records. Their DNA cell lines also included in Constitution Information Bank for constitution-genome study. Constitution Information Bank is composed of constitution-clinical information -physique, appearance, character, temper, symptom and treatment records- and biological information -DNA genotyping data-. Total of 550 cases are collected in Constitution Information Bank from Nov. 2006 to Sept. 2007. Among these, 219 cases are for constitution-family study, especially, 74 cases are collected from Han's large family.

The Clinical Study on Childhood Common Cold in Four Seasons (계절별(季節別) 증상(症狀)을 중심으로 한 소아감모(小兒感冒)의 임상적 고찰)

  • Objectives: This study was performed to investigate the character of childhood common cold according to four seasons. Methods: We made an investigation into medical record of 432 children under fifteen years of age who visited pediatrics or emergency room suffering from common cold. We analyzed the principal and secondary symptoms of the children classifying four seasons by $SPSS^{\circledR}$ 12.0 for windows. Results: Male to female ratio were 1.42:1, the average of age was 3.86 years old. Distribution of principal symptoms was that nasal discharge was 231(56.1%), cough was 233(54.1%), nasal obstruction was 162(39.3%), secretion and sputum were 126(30.6%), fever was 121(29.4%), and headache was 13(3.2%). Out of these fever occurred in high frequency during spring and summer, nasal obstruction occurred in high frequency during spring and autumn. Distribution of secondary symptoms was that throat pain was 93(22.6%), otalgia was 39(9.5%), vomiting was 35(8.5%), abdominal pain was 30(7.3%), anorexia was 23(5.6%), sweating was 21(5.1%), diarrhea was 19(4.6%), and startle was 11(2.7%). Out of these otalgia occurred in high frequency during spring and summer, diarrhea occurred in high frequency during autumn and winter. Conclusions: A few symptoms can be the character according to four seasons.

Studies on Relations between Goodness of Fit in Mother-children Temperament Interaction and Problem Behaviors of Children (모-자녀 기질 적합성과 자녀의 문제행동간의 관련성 연구)

  • Objectives The purpose of this study was to identify the importance of goodness of fit in mother-child temperamental interaction (MCTI) and the child problem behaviors. Methods The behavior problem of 180 child outpatients from traditional Korean medical clinic was measured with Korean version of Child Behavior Checklists, and the temperament of child and their mother was measured with Junior Temperament and Character Inventory and Temperament and Character Inventory-Revised-Short. The MCTI was calculated as the difference of the temperament score between mother and child. The correlation and linear regression analysis was performed to examine the effects of temperament on Child Behavior Checklists. Results The MCTI on Harm-Avoidance was the significant factor for explaining the internalizing problem (B=-.154, t=-10.130, p<.001), externalizing problem (B=-.045, t=-3.340, p=.001) and total problem (B=-.298, t=-7.574, p<.001). We also confirmed that the temperament of mother and child significantly correlated with the child behavior problems as provided previously. Conclusions These results showed that the temperament interaction between mother and child is an important factor for predicting problem behaviors in child clinical sample. The goodness of fit in MCTI and problem behavior should be considered as pivotal element in traditional Korean pediatrics.

A Study of Clinical Characteristics of Female Patients with Cold Hypersensitivity on Hands and Feet (여성 수족냉증 환자의 임상 특성에 관한 연구)

  • Objectives: This study was aimed to research clinical characteristics of female patients with cold hypersensitivity on the hands and feet compared with non-cold hypersensitivity group. Methods: 134 women patients were collected from 6 Korean medicine hospitals, and divided into 2 groups(cold hypersensitivity group and non-cold hypersensitivity group). This survey was composed of 19 questions related to clinical symptoms of cold hypersensitivity. Results: We found out the differences between 2 groups in 10 questions among 19 questions in the survey results. (Body Type, Energy, Digestibility, Skin Type, Facial Complexion, Chill, Dry Mouth and Thirst, Preference between Coolness and Warmth, Thickness of Tongue Fur, Floating and Sinking of Pulse) Conclusions: From these results, Patients with cold hypersensitivity on hands and feet have relative clinical characteristics as follows. They are relatively weak and lethargic. They have dry skin and look more pale. They feel chill easily and have dry mouth and thirst. They prefer warmth and have indigestion more. They had more various infirm and ill clinical characteristics compared with the other group from the view of Korean medicine.

A Study on the Correlation among the Patterns of the Zone 1, 2, 3 of Factor AA in 7-Zone-Diagnostic System and the Clinical Parameters (7구역진단기의 Factor AA 제1, 2, 3구역 유형과 임상지표와의 상관성 연구)

  • Objectives : The 7-zone-diagnostic system is a diagnostic device to predetermine bodily locations by measuring the energy of body. This study was to investigate the relation between the different patterns of Zone 1, 2, 3 of Factor AA in CP-6000A(VEGA, Germany), 7-zone-diagnostic system and clinical parameters. The purpose of this study was relation Korean traditional medicine and western medicine with the data from 7-zone-diagnostic system and the clinical parameters. Methods : This study was carried out with the data from some clinical parameters. We made three groups according to the Factor AA patterns of CP-6000A. The Factor AA pattern of Group A is that the red bar graph of zone 1, 2, 3 were higher than the normal range and the others were the normal range. The Factor AA pattern of Group B was that the red bar graph of zone 1, 2, 3 was the normal range and the others were the normal range. The Factor AA pattern of Group C was that the red bar graph of zone 1, 2, 3 was lower than the normal range and the others were the normal range. After the data from clinical parameters to correspond with conditions of each group were selected, the data from clinical parameters among each groups analyzed statistically. Results : The values of GOT, GPT, r-GPT, Triglyceride, BUN, Uric acid of group A was higher than group C. Gastroscope of group A and B was higher than group C. Conclusions : It is thought that the red bar graph of zone 1, 2, 3 is higher, the group has the higher energy and the energy has a character of fire(熱). Those patterns have a high risk of hyperlipermia and liver, stomach disease.
