간호진단 분류체계에 근거한 간호개념틀 개발 (Development of a Conceptual Framework of Nursing from Selected Concepts of Nursing Diagnoses)

  • For the purpose of integrating nursing diagnosis into the nursing curriculum, a descriptive survey research was done using the inductive method with questionnaires and a literature review. Research subjects included nurse educators, textbooks of adult nursing published in Korea, and the course outline for adult nursing used in one college of nursing. The Results show that there was common agreement on 39 nursing diagnosis which should be in cluded in the adult nursing curriculum, textbooks of adult nursing, and patient care on the medical-surgical units. The two existing nursing diagnosis classification systems(NANDA and Gordon's Human Response Patterns) show different basic frameworks and difficulties were discovered in integration of nursing diagnosis into the curriculum. To develop a conceptual framework for a nursing diagnosis classification system, diagnosis were classified into three categories ; health promotion, high risk problem, and actual problem on the basis of the framework used in adult nursing textbooks and Gordon's 11 Functional Health Patterns. Subconcepts for actual problems were classified as ; activity and rest, nutrition and elimination, perception and coordination, stress and coping. Progress in this study supports further development of a conceptual framework of nursing based on a nursing diagnosis classification system, from which improvement in nursing education and clinical practice can be expected.

The Accuracy of Glasgow Coma Scale Knowledge and Performance among Vietnamese Nurses

  • Purpose: The purpose of this study was to investigate the accuracy of Glasgow Coma Scale (GCS) knowledge and performance among Vietnamese nurses. Methods: A cross-sectional descriptive study was conducted using a questionnaire pertaining to the nurses' knowledge of GCS and a structured evaluation tool to measure the accuracy of their GCS scores. A total of 94 Vietnamese nurses participated in this study, all from a general hospital in Ho Chi Minh City, Vietnam. Data were analyzed by conducting a t-test, a $x^2$ test, and ANOVA. Results: This study found that the vast majority of the nurses (>90%) responded correctly to questions regarding their GCS basic knowledge; however, 52.1% of the nurses answered incorrectly questions related to clinical scenarios requiring the application of the basic knowledge. Regarding the GCS performance, the nurses demonstrated acceptable accuracy rates for each component of GCS, but those who scored well in all three components accounted for only 42.6% of the subject group. These findings indicate that the Vietnamese nurses are not able to integrate their GCS knowledge into actual practice as measured by the accuracy of GCS scoring. Conclusion: This study suggests that new educational strategies should be developed for the Vietnamese nurses to improve their performance on accurate GCS scoring based on theoretical knowledge.

간호전문대학생들의 임상실습현장에서의 수행에 관한 연구 -실습의 내용 빈도를 중심으로- (A Study of the Junior Nursing College Students실 Role during Clinical Practice)

  • The role and function of nursing care nowadays, tend to acquire sophisicated technology because specialization has expended due to increase of the medical population and the improvement of national health standards. To implement nursing care independently as a professional nurse, the apprehension of specific knowledge and skill should be acquired during basic nursing education. So it is important for nursing education not only to include theory and actual techniques, but also to strengthen the practical training in the actual clinical setting. This study was carried out with the following objectives; 1. To survey the detailed content and frequency of actual nursing students display during their clinical training. 2. To investigate the detailed content and frequency of actual nursing behavior which students display in each clinical a area. 3. To identify the motive for selection of nursing as their major and to determine the degree of self confidence, extent of knowledge and recognition of nursing responsibility. 4. To observe the relationship between actual nursing behavior and each of the following; 1) Motive for selecting nursing as a major 2) Self confidence 3) Knowledge of nursing care 4) Recognition of nursing responsibility The conclusions of this study were as follows; 1. Among the detailed nursing behavior which junior nursing college students carry out in clinical training; taking respiration's showed the highest frequency, and taking body temperatures, blood pressures, and pulses and making beds were next in frequency in this order. 2. In detailed nursing behaviors according to clinical area; taking vital signs showed the highest frequency in the emergency room, pediatric ward, orthopedic ward, general surgical ward and internal medicine ward. However, in the operating room, assisting with endotracheal tube insertion and sterile techniques were showen to have the highest frequencies. In nursery, umbilical cord care and the measurement of body weight were the highest in frequency In neurosurgical ward, the measurement of vital signs, changing position and tracheostomy care were the highest in frequency. In obstetric and gynecological ward and in the delivery room, checking duration, intensity and frequency of contractions was the highest in frequency. 3. In regard to the motive for majoring in nursing, the aptitude and interest of the student had the highest percentage(32.86%), and self-confidence in nursing activities (M=3.36), knowledge in nursing activities.(M=3. 09), and the recognition of the nursing activity (M= 3.76) wire in the middle range. 4. When the detailed nursing behaviors were compared with motive, self confidence, knowledge and recognition, it was found that when the nursing behavior was difficult and regarding much endeavor although the motive was high, the frequency of the nursing behavior was rather low. But in the cases in which there was much self confidence and a high level of skill was required, nursing behavior was carried more frequently. When there was muck self confidence and skill was not required, the frequency of nursing behavior was rather low. In the cases of a high level of knowledge, the frequency of nursing behavior was low and when recognition for nursing behavior was given the frequency of nursing behavior was low.

전문간호사의 수급 현황과 건강보험 급여화 방안 (Demand-supply of Advanced Practice Nurse (APN) and Alternative Benefit Strategies in the National Health Insurance)

  • Purpose: The purpose of this paper is to analyze the demand and supply of advanced practice nurses and suggest alternative benefit strategies in the Korean national health insurance. Methods: A revised demand & supply model was used to estimate the excess supply of APNs, and policy making process and key actors in the Korean health insurance were considered to develop a political approach to the APN issue. Results: The social demand for APNs is currently estimated to be less than 50% of its supply and the APN education program fell into difficulties in recruits. No reimbursement mechanism for APN's services in the national health insurance has given no economic incentive to hospital managers who have monopsony power in nursing labor market, which has caused the demand shortage of APNs in hospital industry. Payment for APN's services recognized as one of the most significant strategies to booster the social demand for APN's services should be carefully designed and implemented in the national health insurance. In line with this, key actors in health insurance policy decision-making include government, national assembly, labor unions, NGOs, civic groups, medical associations, and academia. Conclusion: The basic researches for APN's activities and cost-effectiveness analysis in clinical settings are required to support the strategies aforementioned. Constructing a policy network among key actors is able to make the payment strategy feasible, which will increase the socal demand for APNs.

국내 아동간호단위 간호중재 분석 (A Survey of the Nursing Interventions Performed by Pediatric Care Unit Nurses)

  • The purpose of this study was to identify nursing interventions performed by pediatric care unit nurses. For data collection this study used the taxonomy of Nursing Intervention Classification (NIC : 486 nursing intervention) which was modified by McCloskey & Bulecheck(2000). The 419 nursing interventions were selected by panel group, which consist of pediatric clinical experts and nursing scholars. The data were collected 104 nurses of pediatric care unit. There were 158 nursing interventions identified as being used at least monthly 50% or more of the nurses. The 32 nursing interventions were used at least daily, indication a set of core interventions unique to pediatric care unit practice. The most frequently used nursing interventions were 'Medication administration: intravenous' & 'Medication administration: oral'. The 27 nursing interventions were rarely performed by 90% or more of the nurses. The rarely used interventions were 'Ostomy care' & 'Rectal prolaps management'. The domain received the highest mean score was Health System, followed by Physiolocal: complex, Physiolocal: Basic, Safty, Community, Family, Behavior domain. These findings will help in building of a standardized language for the pediatric care units and enhance the quality of nursing care.

전국 의료기관의 냉 요법 실무지침 및 임상간호사의 냉 요법 실무 현황 (Nursing Protocols of Cold Application in Different Medical Institutions in Korea)

  • Purpose: The purpose of this study was to evaluate nursing protocols for superficial cryotherapy in different medical institutions. Methods: The study was conducted with a cross-sectional descriptive design. The medical institutions including general hospitals, hospitals, and geriatric hospitals were randomly selected. A total of 435 nurses from 126 institutions completed a questionnaire. Data were collected from December 2014 to June 2015. Results: Forty-two institutions (39.5%) had nursing protocol for cryotherapy. The nurses reported that durations of cold application were 2 minutes to 5 hours. Frequently used cold therapy devices in order of frequency were frozen gel packs, ice packs, and frozen IV fluid bags. There were variances in the duration of cold application according to the types of institution ($x^2=7.78$, p=.020) and nursing units ($x^2=26.42$, p<.001). In addition, intervals of cold application were different according to the nursing units (x=12.23, p=.032). There were differences in cold application instruments by regional groups (x=70.38, p<.001). Most of the nurses (95.6%) responded that national nursing protocol for superficial cryotherapy were needed. Conclusion: There were difficulties in providing consistent nursing interventions because of the practical differences and absence of evidence-based guidelines for cryotherapy. The researchers recommend that basic studies with various instruments be conducted and proper nursing protocols be developed for cryotherapy.

전문대학 치위생과 학생의 학과선택 및 실습실에 대한 만족도 (A Study on Post-Admission Satisfaction Level among Dental Hygiene Students in Colleges)

  • The purpose of this study was to help improve the quality of education to keep up with fast-changing environments in educating sector. Relevant literature and data were reviewed, and the subjects in this study were dental hygiene juniors at seven two-year colleges in the Seoul metropolitan area and other regions, on whom a survey was conducted for about three months from August through October, 2004, to assess their satisfaction level with their major and educational environments. And it's concluded that in order to attract more new students and foster qualified students, dental hygiene practice labs should be improved on a large scale, and that there should be a great deal of investment in teaching facilities. The major findings of the study were as follows: 1. As for satisfaction with major choice, 57.5% of the students investigated, the largest percentage, expressed satisfaction with their major. 27.8%, the second largest group, found their choice not to be bad. 2. Regarding satisfaction with the state of practice labs, 41.9% were pleased with practice labs. This rate was quite lower than that of nursing students, as 64.2% of nursing students were pleased with their practice labs.7) 3. As to satisfaction with individual practice labs, oral prophylaxis labs were considered most satisfactory, as 53.9% were satisfied with them. Digital radiation labs appeared to be satisfactory the least, which made 77.0% dissatisfied. 4. Whether there were any connections between their working experience as a clinical worker and satisfaction level was investigated. In terms of their satisfaction with major choice and the state of practice labs, the students who had ever worked as a clinical worker were relatively pleased with the state of practice labs. As for satisfaction with each practice lab, those who had ever served as a clinical worker before being admitted into college expressed more satisfaction with radiation practice labs(p<0.05) and digital radiation labs(p<.05) than the others who hadn't. The gap between the two was statistically significant. 5. In regard to the causes of dissatisfaction with practice labs, 93.7 and 80.0% were respectively dissatisfied with digital radiation labs and basic dental hygiene labs because their colleges were devoid of those labs. 51.6%, the largest group, found oral prophylaxis labs unsatisfactory because of their frequent breakdown. In the event of most of the other practice labs, the greatest percentage were discontented due to a shortage of equipment.

의료기관 가정전문간호사의 직무분석 (Job Descriptions of Hospital Based Home Care Nurse Practitioners in Korea by DACUM Technique)

  • Purpose: The aim of this study was to develop and to analyze the task of hospital based home care nurse practitioners in Korea. Method: The definition of home care nurse practitioners and job description was developed based on developing a curriculum(DACUM) by 7 panels who have experienced in home care nursing. One hundred fifty four nurses who were working at hospital based on home care were participated. Result: Fourteen kinds of duties were identified : the selection of home care patients; basic home care nursing; advanced home care nursing; patient/family education and counseling; medical decision making and coordination of patient service; management of home care supplies and drugs for patients; management of medical records; management of home care the agency; management of home care personnel; management of the home care supplies for agency; home care public relations; improvement of home care quality; management of long-term care service; and self-improvement. Ninety-six tasks were classified. Conclusion: The abilities for quality improvement and the advanced nursing practice of home care nurses should be empowered.

  • The home health care industry has grown rapidly and can be expected to continue to grow in the foreseeable future. Home health care refers to the practice of nursing applied to clients with a health condition in the clients place of residence. clients and their designated care givers are the focus at home health nursing practice. The goal of care is to initiate. manage and evaluate the resources needed to promote the clients optimal level of well-being and function. Nursing activities necessary to achieve this goal may warrant preventive maintenance and restorative emphases to prevent potential problems from developing. Many project program were suggested home health care model for Korea's health care system and policy direction for expansion and establishment of home health care .But the aim of this paper is to provide on overview for theoretical frame work in home health care. Theories and conceptual frameworks or models are important nursing because they define and guide the boundaries of professional practice and identify key nurse-patient-caregiver relationships that emerge with caring. Following is the research with an investigation of the literature review in the University of Arizona international medline database, In conclusion, are as followers: First, many nursing theorists have had a tremendous impact on nursing practice. the following highlights those nursing theorists that are particularly helpful in understanding home health care. 1. Florence Nightingale : Our earliest theoretical legacy. Nightingale's believes are reflected in basic infection control practice such as hand washing and infectious waste disposal and are key nursing interventions in home care. 2. Martha Roger's :Science of unitary human beings theory. Rorger's believed that the focus of shared. non invasive healing modelities is the human environmental field rather than direct physical care. These modelities continue to evolve as our awareness (reflecting greater diversity, faster rhythms, motions, and ways of knowing) transcends time and space, allowing individuals to get in touch with their integral nature of unbroken wholeness. On people as ever changing energy fields have special relevance in home care especially with hospice and palliative care applications. 3. Madeline Leininger's; Transcultural nursing theory. Home care nurses move through a variety of communities and often care for patients from different cultural back grounds. Therefore Leininger's work has a good that with home care because home care nursing practice is very culturally focused. 4. Dorothea Orem's : Self care deficit theory. Orem's theory views care as something to be performed by both nurses and patients. The role of the nurse is to provide education and support that help patients acquire the necessary activities to perform self-care. Orem's theory is foundational to have care because it begins to truly acknowledge the role of the patient in managing his or her own health. which is referred to as self-care. 5. Margaret Neuman's; Health as expending consciousness theory. Neuman believes that health compasses disease and reflects an underlying pattern of person-environment interaction. A key application of 'Neuman's work to home care is for nurses to understand that health and illness do not necessarily exist at opposite ends of a continuum. 6. Jean Watson's: Theory of human caring. Watson's theory of human caring in nursing proposes human caring as the moral ideal of nursing. Nurses participate human caring to protect, enhance and preserve humanity by assisting individuals to fing meaning in illness. pain and existence and to help others gain self knowledge. self control. and self healing such thinking lends richness to theory development. as well as clinical practice in home care. Second, Robin Rice : Dynamic self determination for self care. (A theoretical framework for home care) Dynamical self determination for self care can be useful to home care nurses in a variety of ways. As research tool it can be reflected in the interview process when the home visit. The home care nurse's role is that of facilitator of patient self-determination for self care through numerous strategies. including patient education and case management.

간호대학생의 환자안전 역량과 환자안전관리 행위 (Nursing Students' Patient Safety Competency and Patient Safety Management Practice)

  • 본 연구는 간호대학생의 환자안전 역량이 환자안전관리 행위에 미치는 영향을 확인하여 환자안전관리 행위 향상을 위한 프로그램 개발의 기초자료를 제공하기 위하여 시도되었다. 참여자는 1년 이상 학생 간호사로 임상 실습을 받은 293명의 4학년 간호대학생이다. 데이터는 2018년 4월 26일부터 5월 9일까지 수집되었다. 수집된 자료는 SPSS/WIN 24.0 컴퓨터 프로그램으로 빈도, 백분율, 평균, 표준 편차, t-test, ANOVA, Pearson's Correlation Coefficient, Multiple regression으로 분석하였다. 간호대학생의 환자안전 역량은 평균 $2.90{\pm}0.38$점 (환자안전 지식 $2.68{\pm}0.65$, 환자안전 기술 $3.26{\pm}0.56$, 환자안전 태도 $2.75{\pm}0.40$)이었다. 환자안전관리 행위 점수는 평균 $4.13{\pm}0.57$점이었다. 간호대학생의 환자안전 지식, 기술, 태도 및 환자안전관리 행위 간의 상관관계에서, 기술 및 관리 행위는 r=.337, p<.001, 태도 및 관리 행위는 r=-.150, p=.010, 지식 및 관리 행위는 r=.171, p=.003이었다. 대학생의 환자안전 관리 행위에 영향을 미치는 요인은 환자안전 기술(${\beta}=.307$, p<.001), 화재에 대한 안전사고 경험(${\beta}=-.127$, p=.026), 환자안전 태도(${\beta}=-.121$, p=.026), 환자교육에 대한 안전사고 경험(${\beta}=-.119$, p=.034)으로 나타났고, 설명력은 15.7%였다. 본 연구결과를 통하여 간호대학생의 환자안전관리 행위에 영향을 미치는 다양한 요인을 확인하기 위한 반복연구가 필요하고, 환자안전관리 행위를 높일 수 있는 교육프로그램 개발이 요구된다.
