Brachial plexus injury developing after axillary thoracotomy is an uncommon complication. But if it occurs, it may cause annoying events. We recently experienced 2 patients who developed brachial plexus injury after wedge resection by axillary thoracotomy . The first patient was a 22 year-old man with right spontaneous pneumothorax . After wedge resection of the right upper lung by axillary thoracotomy, he complained total paralysis of the right arm. An electromyogram was obtained at 7 days after operation, with the confirmation of brachial plexus injury. He was discharged at 22days after operation and brachial plexus injury was completely recovered 4 months after discharge. The second patient was a 17 year-old man with recurrent right pneumothorax. He underwent wedge resection of the right upper lung by axillary thoracotomy. Electromyogram confirmed the diagnosis of brachial plexus injury in the immediate postoperative period. He was discharged at 15 days after operation and brachial plexus injury was recovered 2months after discharge.Brachial plexus injury after axillary thoracotomy is caused by stretching around the clavicle and tendon of pectoralis minor by fixation of the abducted arm to the frame. Thus, when we perform wedge resection by axillary thoracotomy, we must avoid over-stretching of the brachial plexus in positioning. If brachial plexus injury develops, immediate attention and management with close rapport are important to avoid possible medicolegal problems.
Objectives The purpose of this study is to know the effect of acupuncture on brachial plexus palsy due to birth injury. Methods We decided to treat one week of Vojta therapy and occupational therapy, but other were combined, and then add three weeks of acupuncture treatment as well. Results Right clavicle fracture at birth brachial plexus injury due to decreased locomotion of the right upper extremity, muscle weakness in children aged 7 months to Vojta therapy, occupational therapy and acupuncture, when performed in conjunction mobility increase of the shoulder joint, elbow support improvement, recovery of motor function of the hand grip, etc. to obtain a significant motor function recovery improvement. Conclusion Acupuncture was a good treatment of choice for the better shoulder, joint and hands movements.

Thoracic injury from restraint loading is the principle causative factor of death, which was shown to be particularly significant for older drivers. To characterize thoracic response to belt loading of older drivers, detailed finite element models of the adult and aged thorax were developed. The geometry of the 50th percentile adult male was chosen for the adult FE model. The thoracic FE model was validated against data obtained from results of PMHS pendulum impact tests. The quantified patterns of age-related shape and well-established material changes were applied to the adult model to develop the aged model. Belt force and chest deflection were applied to the developed two types of models. Rib and clavicle fracture risk obviously increased in the aged model. This finding showed that larger rib angle and reduced material properties of the ribcage produced more higher risk of injury in the older driver.

Fractures of the clavicles are extremely common, accounting for 5 to 10% of all injuries in the adult skeletal trauma. Despite this, nonunions and symptomatic malunions after clavicular fracture are uncommon. When these complications occur, however, they may be associated with 11 pattern of disability that includes not only pain but also impairment of upper extremity function. The purpose of this study was to clarify the outcome of clavicular fractures treated by conservatively, and emphasize treatment and factors that possibly predispose a clavicular fracture to poor function or nonunion. We evaluated 9] consecutive fractures of the middle-third of the clavicle in adults which had been treated conservatively. There were 59 males and 33 females ranging in age from 17 to 80 years (average 36.8 years). Nine of the 91 fractures (10%) had developed nonunion, and 4 fractures (4.4%) reported unsatisfactory results. No patients had significant impairment of range of motion or strength as a result of the injury. We found that initial shortening at the fracture of≥20mm(6 cases) had a highly significantly association with nonunion(P<0.0001). Final shortening of 20mm or more (4 cases) was associated with an unsatisfactory result, but not with nonunion(P<0.0001). Our results have led us to recommend the open reduction and internal fixation of severely displaced fractures of the middle third of the clavicle in adults patients.

PURPOSE: This study examined the incidence, location, and characteristics of sports injuries according to the position in Korean female elite handball athletes. METHODS: Thirty handball athletes, who trained at between January 2019 and December 2019 were enrolled. The event, position, and location of injury were recorded according to the IOC's Daily injury reports form. The incidence of sports injury was examined with the 95% confidence interval (CI), and the difference in the incidence of injury by position was presented as the rate ratio (RR). Moreover, differences in the location of the sports injury according to the position were examined using χ2 tests. RESULTS: A total of 148 sports injuries occurred in the handball players, with an incidence of 22.71 cases of injury per 1,000 exposures (95% CI 19.05 - 26.37) The incidence of injury was highest in the Competition group, followed by the Weights and Training groups. The most common location of sports injury was the lower extremity, followed by the upper extremity, trunk. No significant differences in the location of sports injuries were observed according to the position (p = .384). In addition, the knee, ankle, lumbar spine/lower back, shoulder/clavicle were common sites of sports injury among handball players. CONCLUSION: These results provide a baseline for predicting sports injuries occurring in athletes during games, and would provide useful information for developing performance enhancement as well as injury prevention programs.

PURPOSE: This study examined the incidence, location, and characteristics of sports injuries according to the position in Korean national Kabaddi athletes. METHODS: Twenty-four national Kabaddi athletes (12 males and 12 females) who trained at the national training center between January 2019 and December 2019 were enrolled in this study. The incidence of sports injury was examined with the 95% confidence interval (CI), and the difference in the incidence of injury by position was presented as the rate ratio (RR). Moreover, the differences in the location of sports injury according to position were examined using χ2 tests. RESULTS: One hundred and nine sports injuries occurred among Kabaddi players, with an incidence of 20.28 cases of injuries per 1,000 exposures (95% CI 16.47 - 24.08), and the incidence of injury was the highest among the competition, followed by training and weights. The most common location of sports injury was the lower extremity, followed by the upper extremity, trunk, head, and neck. In addition, no significant differences in the locations of the sports injuries were noted according to gender (p = .689). Furthermore, knee, ankle, lumbar spine/lower back, shoulder/clavicle were common sites of sports injury among Kabaddi players. CONCLUSION: These results provide a baseline for predicting sports injuries occurring in athletes during games, and would provide useful information for developing performance enhancement as well as injury prevention programs.
A clinical evaluation was performed on 76 cases of chest injury experienced at department of Chest Surgery, Capital Armed Forces General Hospital during the past 3 years period from January 1981 to August 1983. 1.The most common cause of the chest trauma was gun shot by which 26 cases were injured among 44 cases [57.9%] of penetrating injury. Remaining 32 cases [42.1%] were injured by non-penetrating blunt trauma. 2.Hemopneumothorax was observed in 60 cases [78.9%], those were caused by both penetrating [65%] and non-penetrating [35%] injuries. 3.Rib fracture was found in 58.7% of total cases and with rib fracture, clavicle fracture was combined at 19.6% and sternal fracture, at 8.7%. 4.Most common symptoms were chest pain and dyspnea, and most common signs were breath sound diminution and subcutaneous emphysema. 5.Common site of rib fracture was from 4th rib to 8th rib [69.4%]. 6.In 58 cases [76.3%], patients were treated with operation including open thoracotomy [25 cases]. 7.Overall mortality was 5.3%[4 cases] and causes of death were septic shock and respiratory failure.
Acquired, traumatic tracheoesophageal fistula [TEF] is rare and difficult problem to manage. This 55 years old man met with a roller accident of a tractor. During accident, he received a penetrating injury on the left upper sternal border. At local clinic, he received closed thoracotomy drainage [CTD]for relief of pneumothorax[left]. Three days after CTD, he complained abdominal pain and hematemesis. The endoscopy revealed large ulcer at the stomach, so he received subtotal gastrectomy. On 10th day post subtotal gastrectomy, he developed aspiration and coughing from a TEF. The esophagogram showed large TEF at the mid-trachea level. So he transfered to our hospital for operation. This patient was operated on for late TEF three weeks after injury. We have used absorble 4-0 Vicryl sutures to repair trachea. We repair all esophageal injuries with two layers of nonabsorbable silk suture. Where suture line on the esophagus, the strap muscle was interposed for reinforcement. And for feeding, the feeding jejunostomy was performed. Postoperatively the osteomyelitis of the manubrium site was developed, so on the 30th postoperative day, an ostectomy of manubrium, both clavicle and fight 1st, 2nd ribs, and the pectoralis major musculo-cutaneous flap coverage were performed.
190 cases of the thoracic injuries experienced at the department of chest surgery, Chungnam National University Hospital, for 5 years from Jan. 1976 till Dec 1980, were analysed. The results are as follows; 1. The incidence rate of male to female was 5.1:1. The common age groups were 3rd, 4th and 5th decades, and the most common age group in the penetrating injury was 3rd decade. 2. The most common mode of the nonpenetrating injuries was a traffic accident [63.0%], and the most commonly used tool in the penetrating injuries is a knife [66.7%]. 3. The most common nonpenetrating injury was rib fracture [73.9%], and the common fracture sites were 5th, 6th, 7th and 8th ribs [especially, 7th rib]. The incidence rate of flail chest was 15% of the cases of the rib fractures. 4. The common associated injuries of the nonpenetrating were long bone fracture [18.3%], brain contusion [15.9%], and clavicle fracture etc.. 5.
